The Aboriginal Health & Liaison Worker will have the capability to respond one to one, to the unique issues that Aboriginal young people face, including consideration of intergenerational trauma, alcohol and drug issues and engagement in education or employment. The position seeks to reduce stigma, improve the health outcomes and enhance Aboriginal young people s engagement in community, education and employment. A component of this position will be around increasing understanding of the impact of mental and physical health issues and to providing young people with a positive experience of help-seeking. 1
2 Position Summary Background: headspace Frankston is committed to providing access for young people to a safe and youth friendly environment which is responsive, of high quality and appropriately coordinates care and wrap around support within a primary early intervention and prevention focus. The importance of a holistic approach to the wellbeing of our Aboriginal young people, their families and community is recognised, with an understanding that culture, language and connection to land are at the core of social and emotional wellbeing. This newly created position aligns with the Koolin Balit strategic direction and aims to make a significant and measurable impact on improving the length and quality of the lives of our Aboriginal and Torres Strait Islander community. By providing an integrated approach informed by the social determinants model of health, this position will focus on key health outcomes of Aboriginal young people aged in support of a healthy transition to adulthood. The position aims to support and guide the health, wellbeing and healing of our Aboriginal young people. As a course of standard practice it is intended the Aboriginal Health & Liaison Worker will enhance headspace Frankston s capability to provide an environment that respectfully supports equitable access to and utilisation of our services and allay fears and mistrust of a mainstream health service. The Aboriginal Health & Liaison Worker role will provide an opportunity for a health professional working competently within their scope of practice to work as a part of a multidisciplinary primary health care team linking with medical practitioners, nurses, allied health professionals, youth workers, other co-located service providers and key headspace Frankston stakeholders. Partnerships with local Aboriginal workers, services and Gathering Places will be essential in further developing support and key entry points to culturally appropriate pathways of care for young people and their families. headspace Frankston model brings together key organisations experienced in the delivery of primary health care, mental health, alcohol and drug services, social welfare, vocation and education for young people. headspace Frankston is governed by a consortium of 10 local agencies, with YSAS (as the lead agent) being contracted by the National Youth Mental Health Foundation (headspace) to deliver headspace Frankston services. This strategic alliance aims to establish a community of collaborative youth services, supporting access to and the delivery of comprehensive coordinated early intervention services for young people aged years, in the Frankston Mornington Peninsula region. headspace is designed to make it as easy as possible for a young person and their family to get the help they need for problems affecting their wellbeing. The headspace model is a one stop shop offering young people the opportunity to seek assistance for a broad range of health and vocational issues through the provision of accessible information and youth friendly multidisciplinary and multi-sectorial teams onsite. 2
3 Key Responsibilities Key Area Key Responsibilities Agreed Achievement Direct Service Activities Provision of primary health care support, brief interventions and information to Aboriginal young people Work collaboratively within our multidisciplinary team to support decision making processes with our Aboriginal young people and their families. Facilitate the development of basic life skills for Aboriginal young people Facilitate a range of responses that meet the individual s assessed primary health care needs Support Aboriginal young people to feel comfortable and safe to engage with our health professionals, youth workers and the environment in which we deliver services. Where indicated, support and encourage Aboriginal young people to develop links with other services and/or communities Facilitate targeted health promotion activities, community engagement and cultural connectedness. Work cooperatively and support the facilitation of the Kooriez Space Youth Group program at headspace Frankston in consultation with the key youth worker. Ability to work flexibly to respond to the needs of an Aboriginal young person spontaneously. This may include, drop in presentations, secondary consultation with other service providers or as required in a GP or other health professional consultation. Capacity to work outside of a structured environment for the engagement of Aboriginal young people or key stakeholders i.e. outreach to local organisations or schools Model appropriate behaviour and facilitate positive communication between and with Aboriginal young people As required advocate on behalf of Aboriginal young people Screening, assessment and care plan development of Aboriginal young people Facilitate multidisciplinary care through care coordination Attendance at handover & clinical case review Culturally safe and appropriate practice at all times Maintain Electronic Medical Record ( EMR) & data collection Increased number of Aboriginal young people accessing preventative health checks, immunisation and primary care services offered at hsf Delivery of at least one health promotion activity per term Minimum of two key stakeholder contacts weekly Increased number of young people identifying as Aboriginal or Torres Strait Islander at hsf 3
4 Key Area Key Responsibilities Agreed Achievement Service Development & Promotion Referral Networks & Pathways Actively support and participate in the building the cultural competence, responsiveness and capability of our existing workforce team, with a particular emphasis on the youth health clinic and counselling services. Participate in the development of the Aboriginal Youth Health Clinic services and model of care Support pathway development and uptake of MBS funded primary healthcare services offered by GPs, Nurses and AHW for Aboriginal young people. Actively support and contribute to health promotion & wellbeing program planning and delivery alongside the Kooriez Space Youth Group program. Key worker collaboration, creating cohesive pathways and wrap around support by a no wrong door and warm referral links between our social and groups program with the onsite multidisciplinary services to better address the mental, physical and sexual health, AOD, education and vocational needs for Aboriginal young people as they arise. Facilitate relationship building between headspace Frankston, organisations, community and Aboriginal young people. Increasing our reach, relevance and responsiveness within the local community. Provide one-on-one support to Aboriginal young people as per referrals from the hsf clinical services program, youth health clinic & social programs. Provide referral and linkages to a range of youth services within headspace Frankston and partner agencies. Develop and establish collaborative relationships with key stakeholders to enhance service co-ordination Provide secondary consultation to internal and external services Attendance at regular stakeholder meetings as required Attendance at Youth Health Clinic & hsf team meetings Development of a health promotion plan for Aboriginal young people Ongoing review and development of clinical and referral pathways for Aboriginal young people engaged with hsf Monthly contact with key Aboriginal organisations and workers i.e. DDACL, KESO s, Gathering Places Develop collaborative partnerships with relevant stakeholders Develop and maintain formal & informal networks in order to maintain positive relationships 4

Foster and facilitate a co-operative team environment which values collaboration to ensure the focus on efficient systems and needs of young people and the wider headspace Frankston team Participate in ongoing planning, development and implementation of clinical services and integration at headspace Frankston Participate in community awareness and engagement activities, including clinical and non-clinical groups as directed Contribute to continuous quality improvement in relation to service delivery; Young people individually supported in links to group programs, headspace supports and other community services Participation in fortnightly supervision with delegated supervisor Contribute to individual & team work plans including participation in yearly performance reviews Contribute to evaluation practices for service delivery and groups as required Developing linkages with referral services; Participation in evaluation and ongoing monitoring of the program; Include clients in the evaluation of the service where appropriate; Collaborate with other support services and community based activities to integrate support and provide optimal service provision. 5

Key Selection Criteria Identify as Aboriginal and/or Torres Strait Islander Recent experience and demonstrated understanding of Aboriginal culture and unique issues young people face Demonstrated experience of health screening, assessment and care co-ordination of clients in order to plan, implement and evaluate care interventions Demonstrated ability to work independently and as a part of a multidisciplinary team Demonstrated ability to communicate and liaise effectively with a wide range of stakeholders, both mainstream and Aboriginal community to provide advocacy, advice, develop appropriate referral pathways and protocols Understanding of the Medicare Benefit Schedule and other funding initiatives to support the development and sustainability of an Aboriginal Youth Clinic Knowledge, demonstrated understanding and commitment to the principles of equity, diversity and OHS Knowledge and understanding of relevant legislation, policies and issues informing health services for young people and their families. Highly developed writing skills, electronic record keeping and data management Capacity to be flexible and open to change and contribute positively to the workplace and team To adhere to relevant professional registration requirements (Nursing, AHP or AHW) I accept the PD as acknowledged above and understand that the PD will be reviewed by the year.
