Cmmunity health wrkers (CHWs), trained laypersns wh share life experience with lw-ses patients, are uniquely qualified t prvide this supprt. When jined tgether gal these strategies are prving t have a prfund impact in the crdinatin f care, system navigatin and health utcmes f individuals living in pverty. PPMC is a key partner in the IMPaCT prgram - Individualized Management twards Patient-Centered Targets - an interventin in which CHWs help lw-ses patients achieve their health gals. IMPaCT care teams prvide individualized transitin supprt pst-acute stay and cntinued supprt in a primary care setting fr patients with multiple chrnic cnditins. In the FY2014 year, PPMC has made targeted specialty care recruitments in the Neursciences and has secured new faculty in general neurlgy as well as strke care. These strategic recruitments will frm the fundatin fr establishing greater access t Neursciences services n the PPMC campus. In the FY2014 year, PPMC is planning address specialty care in urlgy by prmting a PSA screening targeted t men ages 45 years and ver t prmte access t imprved prstate health. (PSA is a prtein made by prstate tissue. Men with prstate cancer ften have elevated PSA levels because the cancer cells make excessive amunts f this prtein. The PSA test is a bld test which can detect the early signs f prstate cancer). Additinal urlgy faculty has als been hired t prvide mre appintment access and care. - Describe anticipated impact f cmmitted prgrams and resurces n the health need The IMPaCT prgram is aimed at addressing access, chrnic disease management, patient satisfactin and trust as well as navigatin f the cmplex health system. PPMC currently funds tw transitin partners fr the IMPaCT prgram and plans t increase supprt ver the next year as admissins increase and the need expands. Gals and impact f the prgram is aimed at having patients achieving their health gals feeling healthy (bth emtinally and physically), trusting their primary care dctr, and feeling like they were getting primary care that was fcused n their needs, and nt having t g t the hspital as frequently. In additin t aiding the health needs f these patients the IMPaCT prgram is targeted at reducing health disparities based n sciecnmic factrs such as incme, race/ethnicity, insurance and educatinal attainment, which have been cnsistently dcumented acrss a wide range f chrnic diseases and utcmes. The prgram has been successful in breaking dwn ften unaddressed barriers t chrnic disease management that clinicians ften cannt address that detracted frm patients ability t achieve gals fr chrnic disease management. These were cmpeting sciecnmic issues (e.g. fd insecurity, hmelessness); difficulty navigating the health system and crdinating care fr multiple chrnic cnditins; and lack f mtivatin r ability t perfrm recmmended health behavirs. 10 Increased specialty care appintment availability fr Neurlgy appintments was cited as being particularly difficult t secure fr residents. PPMC s new recruitment as well as a new referral line prgram is aimed at reducing this waiting time. Mre than fur in ten (45.8%) men ages 45 years and ver in the service area did nt have a PSA r rectal exam fr prstate cancer in the past year. These prescriptins are generally a 5 day supply f essential medicatins but exceptins can be made, such as antibitics and tapers. Our Primary Care practitiners wrk clsely with the resettlement agency HIAS (Hebrew Internatinal Aid Sciety) and multiple ther entities. We prvide medical care, vaccinatins, and n-ging referrals nce their medical needs have been identified. These are highlighted prgrams which the rganizatin plans t cntinue t supprt t address educatin related t chrnic disease: PPMC is cmmitted t ur Annual Spring int Health Cmmunity Day, prviding health infrmatin, bld pressure screenings, and CPR demnstratins t children, adults and senirs frm the West Philadelphia cmmunity. Fr this event, mre than 50 PPMC nurses, nursing assistants, and physicians prvide screenings and educatin. Annual ver 300 participants receive bld pressure and diabetes screenings with educatin, HIV testing and educatin, nutritinal cunseling, pharmaceutical medicatin reviews, physical exercise tips, and mre. PPMC s is a Partner in the Annual West Philadelphia Cmmunity Wellness Day and spnsred. Mre than 500 peple in the cmmunity attended t learn hw t imprve their health. Presented by Cuncilwman Jannie L. Blackwell, the University f Pennsylvania Schl f Nursing, Healthy in Philadelphia and LIFE (Living Independently Fr Elders), and in cllabratin with University f Pennsylvania Office f Gvernment & Cmmunity Affairs and PPMC, the event ffered an afternn f health screenings, exercise wrkshps and mre. Penn Presbyterian assisted with prducing a health fair at Temple University s Liacuras Center. Hsted by Philadelphia radi persnality, Tm Jyner, Take a Lved One t the Dctr which invited many health care partners t address primary health cncerns. On this day, Penn Presbyterian assisted in bld pressure and BMI readings and educated nearly 300 fair attendees n heart health and healthy life practices. Healthy in Philadelphia Cmmittee, which versees utreach pprtunities in the lcal area. This grup is a partnership between the health system s nursing schl and the lcal cmmunity that aims t leverage cllective resurces t imprve the health f residents. Since 2011, PPMC has been a standing member f this rganizatin s First Thursday initiative, which enables nurses t meet regularly with blck captains and determine hw they can better serve and educate the cmmunity. In the upcming fiscal year, PPMC is planning a targeted direct mailing, scial netwrking and a cmmunity engagement strategy specifically fcused n smking preventin, interventin and cessatin. This direct utreach will highlight the resurces in smking cessatin and COPD that are ffered in clinical settings and will identify supprting cmmunity resurces t aid in this preventin effrt. Our faculty/prviders lecture fr cmmunity based clinicians thrugh the AIDS Educatin and Training Center. - Describe anticipated impact f cmmitted prgrams and resurces n the health need Heart disease is the leading cause f death in the service area fllwed by all frms f cancer, lung disease and strke. An imprtant part f PPMC s missin is reaching ut t the cmmunity. PPMC and the University f Pennsylvania Health System jintly spnsr a speaker s bureau and can arrange t have a health prfessinal speak at ff-campus cmmunity meetings withut charge, staff members can als be scheduled t represent the hspital at health fairs, bringing infrmatin n hspital services, screenings and preventin. Educatinal prgrams t address behaviral health cnditins and treatment ptins fr adults, teens, lw-incme and uninsured individuals - Identify facility's specific prgrams and resurces the facility plans t cmmit t meeting the need; The PPMC Behaviral Health Prgram prvides inpatient and utpatient services fr mental health and addictin treatments as well as mental health prgrams fr lder adults. Wright-5 is a 22-bed inpatient adult shrt-term psychiatric unit, which accepts all levels f diagnsis. We specialize in the treatment f patients with a c-ccurring addictin prblem secndary t the primary psychiatric prblem. The unit is staffed by physicians, registered nurses, mental health technicians, addictin cunselrs and scial wrkers. Wright- 4 and the dedicated staff wrking in the Addictin Treatment Center at the Penn-Presbyterian Medical Center are here t serve thse in the cmmunity and surrunding area suffering frm the disease f addictin. We ffer a cntinuum f care that has received State & Natinal recgnitin. Recvery is a day-by-day, life lng prcess. In FY2014, PPMC is planning t fcus n behaviral health educatin prgrams targeted at increasing awareness f behaviral health needs amng primary care prviders and lcal cmmunity rganizatins with the intent f creating imprved pathways fr early interventin. In additin, PPMC alng with ur peers acrss the health system plan t address system cllabratin and access fr patients t ensure the mst beneficial treatment envirnment is prvided in a mre crdinated fashin acrss the system. PPMC Infectius Disease practice was chsen as a site fr a Behaviral Health Services Initiative. As part f this grant, which is crdinated thrugh anther university in the city, the ID Practice has a fulltime behaviral health specialist n-site t prvide nging cunseling and mental health services. Examples f the services prvided by ur behaviral health specialist include: Cunseling fr acute mental health issues Cunseling and behaviral therapy fr lng-term issues like depressin, anxiety, and mre Cunseling fr smking cessatin, insmnia, issues with medicatin adherence - Describe anticipated impact f cmmitted prgrams and resurces n the health need Accrding t the cmmunity needs assessment ne in five adults in the service area has been diagnsed with a mental health cnditin and f thse with a mental health cnditin, mre than fur in ten are nt receiving treatment fr the cnditin. PPMC aims t impact this health need thrugh the prmtin f prgrams ffered. Accrding t the cmmunity needs assessment, there is a shrtage f drug and alchl treatment prviders fr lw-incme ppulatins. There is an especially high need fr treatment prgrams fr individuals with bth mental health and substance use disrder. PPMC is a member f Friends f 40 th Street a grup charged with enhancing the business climate n 40 th street between Lancaster and Baltimre Avenues. PPMC is a participant in Make yur Mark Lwer Lancaster Avenue Steering Cmmittee, a study spearheaded by Peple s Emergency Center as a cmprehensive planning initiative lking at jbs, public spaces, husing, entertainment, transprtatin and verall quality f life issues. - Describe anticipated impact f cmmitted prgrams and resurces n the health need The Cmmunity Safety Plan s missin is t impact the West Philadelphia cmmunity neighbrhd t reduce vilence and aid residence in leaving their hmes when in need f medical help. Specific prgrams include safety imprvements t and maintenance f Saunders Park and physical enhancements f the surrunding cmmunity including lighting enhancements. The Friends f 40 th Street and Make Yur Mark prjects hpe t target and create a safe and healthy cmmunity t fster and strengthen the cmmunity and seed a safe fundatin and cmmunity netwrk aimed at deterring vilence. - Describe facility's pririties in meeting these needs This grup meets quarterly and addresses issues f campus/cmmunity safety, hmelessness, crime reductin and quality f life issues. In additin, Penn Medicine is wrking n expanding pilt prgrams fr creating same day and next day appintments t fill a grwing need fr immediate care respnse in an utpatient setting. - Describe anticipated impact f cmmitted prgrams and resurces n the health need Center City cmmunity members reprted that it can take up t ten weeks fr psychiatric intake, which ultimately discurages peple frm getting help. Members f the West cmmunity grup said that there are lng wait lists at the city s health centers fr lw incme adults seeking primary care. Many adults d even try t get an appintment because they perceive the wait times t be t lng. Regular health screenings can help identify health prblems befre they start. Early detectin can imprve chances fr treatment and cure and help individuals t live lnger, healthier lives. In the Penn Presbyterian Medical Center service area, ne in six (16.2%) adults did nt visit a health care prvider in the past year; this percentage represents 48,200 adults - Describe facility's pririties in meeting these needs Penn Medicine and PPMC have active task frces fcused n access and increasing the pprtunities and availability f creating mre appintments and mre flexibility fr patients t access the care needed in the mst timely manner pssible. Cmmunity mailings advertising screening services and care pathways can als be used t target this need in the cmmunity alng with ur cmmunity health wrkers The PPMC Divisin f Infectius Disease has maintained grant funding fr several years frm the Ryan White HIV/AIDS Prgram, which is a federally funded grants prgram that cvers the csts f medical care fr uninsured patients. Thrugh ur Ryan White funding we have been able t build ur practice and prvide several services necessary t the care and well-being f ur HIV-infected patients. Examples f these services include: Nursing care Case management services Nutritinist services Linkage care crdinatr Cverage f the csts f certain expensive medical tests fr patients withut insurance cverage fr such tests Linkages t surrunding cmmunity service rganizatins that prvide case management services fr peple living with HIV/AIDS On-site utpatient pharmacy Wellness events Mnthly grup supprt meetings lead by ur linkage crdinatr fr patients living with HIV/AIDS The ID Divisin als has a federally funded grant fr early interventin fr HIV. This grant has allwed us t supprt a Rapid HIV Testing Prgram. This prgram includes the fllwing: Testing crdinatrs wh prvide rapid HIV testing in the Emergency Department, inpatient units, in ur utpatient practice, and at cmmunity events. A prgram dedicated t quickly and efficiently linking newly diagnsed patients frm the hspital, Emergency Department, r cmmunity t ur practice fr initial and subsequent HIV care The Infectius Disease practice is part f the Viral Hepatitis Center f the Perelman Schl f Medicine, a cmbined effrt f the Infectius Diseases and Gastrenterlgy Divisins within the Department f Medicine. We prvide diagnstic, treatment, and preventin services fr thse with chrnic hepatitis B and C, including availability f clinical trials with new drugs. This is an nging and unmet cmmunity need. It is estimated that ne third f thse infected with hepatitis C in the United States are unaware f their diagnsis and a significant prprtin f thse are uninsured. There is n system in place t help thse patients, as there is with HIV infectin. We wrk clsely with ur scial wrker and with prgrams that can prvide medicatins fr patients thrugh patient assistance prgrams. 19 We are partnering with cmmunity based testing campaigns fr HCV in rder t facilitate linkage t care fr thse newly diagnsed with HCV infectin. - Describe anticipated impact f cmmitted prgrams and resurces n the health need The cmmunicable disease rates fr chrnic Hepatitis B, Tuberculsis, Varicella (chicken px), Chlamydia, Gnrrhea, and Syphilis (primary and secndary) in Philadelphia Cunty are higher than state rates. Fur in ten (39.6%) wmen in the service area did nt receive a Pap Smear test in the past year. Mre than ne-third (35.9%) f wmen ages 40 r lder in the service area is nt receiving this screening annually In Philadelphia Cunty, there are 16,234 individuals wh are living with HIV, including AIDS, representing a prevalence rate f 1,121.6 cases per 100,000 ppulatin. This prevalence rate is five times higher than the state s rate (244.9) The area in West Philadelphia arund PPMC is ne f the highest incident and prevalent areas fr HIV infectin in Philadelphia. The ID practice at PPMC has grwn frm 50 t 750 patients in the last 15 years.
