The UPHS has develped a primary care plan t increase primary care access UPHS partners with health centers in West Philadelphia t crdinate the delivery f care and help prvide inpatient and emergency care services fr peple in UPHS service area. There is an agreement fr Penn Medicine attending physicians t prvide services at The Crpral Michael J. Crescenz VA Medical Center in Philadelphia in several specialties. UPHS Nurse Primary Care Cnnectrs supprt patients after discharge by facilitating access t n-ging care services and cmmunity service prviders. UPHS supprts expanded residency slts that increase access t care and increase the pl f prviders in the cmmunity particularly at FQHCs. The PPMC Divisin f Infectius prvide nursing care, case management and care crdinatin, nutritinist, linkages t cmmunity service prviders, n-site utpatient pharmacy services, wellness events, and supprt grups. Thrugh the Department f Clinical Resurce Management and Scial Wrk, and Financial Cunselrs, UPHS assists uninsured patients t apply fr Medicaid and Medical Assistance fr Wrkers with Disabilities with in-huse Financial Cunselrs and Patient Financial Advcates. UPHS ffers eligible patients the Prescriptin Assistance Cntract fr the Elderly (PACE) and Prescriptin Assistance Cntract fr the Elderly Needs Enhancement Tier (PACENET) prgram thrugh eligibility identificatin during hspitalizatin. PACE and PACENET ffer lw-cst prescriptin medicatin t qualified residents, age 65 and lder. The Department f Scial Wrk and Clinical Resurce Management screens all patients fr PACE and PACENET eligibility and prvides assistance t apply t the prgram and refers t the PA Department f Aging, Benefits Data Trust, and Pennsylvania Health Law Prject. UPHS has established specific purpse funds t cver the cst f medicatins fr any hspitalized r bservatin patient withut prescriptin cverage fr their discharge medicatins, withut regard t the cst f medicatins fr thse eligible based n the incme and ther sciecnmic factrs, as determined by the Scial Services Department. The Gift f Life Unifrm Organ Dnatin Cuncil is held n site at HUP and prvides access t transplantatin services. The multicultural ppulatin served by this facility is vast. UPHS is cmmitted t addressing the cultural needs f the patients and cmmunity that we serve. Sme f the many ppulatins that we serve include Latin, Chinese, Vietnamese, and West African. Prgrams and resurces that are currently available at the UPHS include: All patients will cntinue t have access t live interpreters; All patients will cntinue t have direct cntact with a certified medical interpreter via a telephne service (each nursing units has access t mre than ne telephne device and additinal are available as needed); All patients will cntinue t be prvided availability t have dcuments translated int written dcuments within a few hurs t ne business day; A UPHS Cultural Cmpetence Cmmittee (HCCC) creates envirnments that respect diverse perspectives, generate multiple alternatives, and develp new pssibilities fr delivering culturally apprpriate care; The Patient and Guest Services Department address specific interventins fr patients and their families; The Department f Pastral Care meets the spiritual needs f the multicultural ppulatin that we serve; Hspital signage is available in cmmn languages identified by ur cnsumers. Every autumn, Penn Asian Health Initiatives administers free influenza vaccines t uninsured and underinsured members f the Vietnamese American cmmunity and prvides health educatin n tpics such as cancer screening and preventin, diabetes, and hypertensin. Every autumn, UPHS administers free influenza vaccines t members f the cmmunity.
