1 Cmmunity Health Needs Assessment Implementatin Strategy Adpted by St. Jhn Medical Center s Gverning Bard in December Intrductin St. Jhn Medical Center (the Hspital ) cnducted a cmmunity health needs assessment (a CHNA ) f the gegraphic areas served by the Hspital pursuant t the requirements f Sectin 501(r) f the Internal Revenue Cde ( Sectin 501(r) ).1 The CHNA findings were published n the Hspital s website in December 2012 (the 2012 CHNA ).2 This implementatin strategy (the Strategy ), als required by Sectin 501(r), dcuments the Hspital s effrts t address the cmmunity health needs identified in the 2012 CHNA.3 The Strategy identifies the means thrugh which the Hspital plans t address a number f the needs that are cnsistent with the Hspital s charitable missin during 2013 thrugh 2015 as part f its cmmunity benefit prgrams. Beynd these prgrams, the Hspital is addressing sme f these needs simply by prviding care t all, regardless f their ability t pay, every day. The Hspital anticipates the strategies may change and therefre, a flexible apprach is best suited fr the develpment f its respnse t the 2012 CHNA. Fr example, certain cmmunity health needs may becme mre prnunced and require changes t the initiatives identified by the Hspital in the Strategy. During 2013 thrugh 2015, ther cmmunity rganizatins may address certain needs, indicating that the Hspital s strategies shuld be refcused n alternative cmmunity health needs r assume a different fcus n the needs identified in the 2012 CHNA. In additin, changes may be warranted by the publicatin f final regulatins. 1
