During this time, yur physician will cnduct a thrugh medical and physical examinatin, review yur medicatins and rder lab tests. The purpse f this visit is t make sure that yu are ready t have surgery. Yur PCP will review and frward all infrmatin t yur surgen. Tgether, they will decide if yu are ready t have yur surgery. Please ntify yur surgen if yu have any change in medical cnditin r medicatins since yur last visit. Medicatins and Herbal Supplements Please bring all f yur medicatins, in the riginal cntainer with yu t yur dctr visit. Medicatins include all prescriptin medicatins, ver-the-cunter medicatins and vitamin r herbal supplements. There are sme medicatins such as aspirin, ibuprfen and herbal supplements that can cause increased bleeding during surgery and prevent yur fusin frm healing prperly. Yu shuld stp taking these frms f medicatin and all herbal supplements fr tw weeks prir t surgery. If yu are taking Cumadin, Warfarin r Plavix please talk with yur physician abut when yu shuld discntinue use and restart this medicatin. Make sure yur surgen is als aware that yu are n bld thinners. 3 Being well infrmed abut yur surgery can actually imprve yur recvery and utcme. It is ur gal t prepare yu fr surgery and assist yu n the rad t recvery. We feel that sharing the infrmatin in this handbk is the first step f the jurney. Bld Management Sme spine surgeries may require yu t get a bld transfusin. Our bld is screened and tested t prevent transmissin f any disease. Yu will be asked t sign a cnsent frm fr bld transfusin. 4 Back Brace Yur surgen may require yu t wear a brace after surgery t supprt yur spine. Yu will be taught hw t put n and take ff yur brace. Spine Precautins Yu will be instructed t fllw spine precautins after yur surgery t ensure prper functining f yur spine. Please review the spine precautins sectin f this handbk lcated n page 14 befre yur surgery s that yu are familiar with them. A vide demnstratin is available at penrsestfrancis.rg/spinehandbk. Preparing Yur Hme We suggest that yu take a few steps t prepare yur hme prir t yur surgery. Preparing early will help make things easier fr yu upn yur return frm the hspital. ~ Prepare meals in advance and freeze them. This will make meal time much easier when yu return hme. ~ Be sure there is plenty f fd, supplies and medicatins. ~ Stre yur imprtant items at waist level s yu d nt have t bend ver t reach them. ~ Remve rugs, tape dwn electrical crds and arrange furniture t allw easy access fr walking. ~ Make sure yur stairs have a sturdy hand rail. ~ Wear sturdy walking shes when at hme. Avid sandals r slippers. ~ Place a nn-slip mat n the flr f yur tub/shwer. We suggest use f a shwer caddy t stre items such as sap and shamp. ~ Yur pets are imprtant. Please arrange fr pet care bth while yu are in the hspital and when yu return hme. ~ Arrange fr yur transprtatin t hme, fllw-up appintments and any errands. Smking Smking prduces mucus and makes it harder fr yur lungs t wrk. This increases yur chance f cmplicatins after surgery. Anther cnsequence f smking and use f nictine is the affect it has n the healing f tissue and bnes. Nictine in cigarettes and chewing tbacc cnstricts bld vessels and can clg arteries that feed xygen t new bne grwth. It is strngly encuraged that yu quit using tbacc. Please discuss any cncerns with yur physician prir t admissin t the hspital if yu feel this may be an issue fr yu. Additinally, please nte medical marijuana use is nt permitted fr patients being treated at any Penrse-St. Francis Health Services facility. All Penrse-St. Francis facilities are tbacc and smke free. There are hspital chaplains available and f curse yur pastr, bishp r rabbi is welcme t visit as well. Chaplains are available t assist yu with questins regarding Advanced Medical Directives. Presurgical Prcess Please arrive at the hspital tw hurs befre surgery r as directed by yur surgen. After registratin, when yu are waiting in the surgical waiting area, yur nurse will greet yu, will take yu back t the preperative area, and will ask yu t change int a gwn and will review medical infrmatin. Yur nurse will insert an intravenus (IV) catheter int yur arm t keep yu hydrated with fluids. Once yur nurse has cmpleted yur preparatin 1-2 family members can sit with yu until it is time fr surgery. Yur anesthesilgist will see yu befre surgery. He r she will review yur medical histry and d a brief examinatin. The anesthesilgist will explain the type f anesthesia yu will receive, the risks invlved and will answer any questins. Mst spine surgeries are perfrmed with general anesthesia (where medicatins will help yu sleep thrugh the prcedure). Befre surgery yur surgen will meet with yu t answer last minute questins, cnfirm the surgical prcedure and mark the area. We want yu t feel as cmfrtable as pssible. Remember yu can ask questins at any time! The nurse wh will be assisting yur surgen will take yu t the perating rm (OR). Yur family will be asked t g t the surgery waiting area. Surgery Suite The spine surgical suite will have bright lights, a lt f equipment and many nurses, technicians and ther staff members t assist in caring fr yu. The rm may seem cl but dn t wrry, they have nice warm blankets fr yu. After Surgery Pst Anesthesia Care Unit (PACU) After surgery is finished, the surgen will g t the waiting area t let yur family knw the prcedure is cmpleted. Yu will be transferred t the Pst Anesthesia Care Unit (PACU) where yu will wake-up after anesthesia. Yur family is nt allwed in the PACU, but can see yu as sn as yu get t yur rm. The average stay in the PACU is arund 60 minutes, but time culd be lnger fr several reasns: additinal mnitring, giving medicatins, yur ability t wake up, if yu have a histry f sleep apnea r bstructive sleep apnea r waiting fr a rm assignment. While yu are in the PACU, yur nurse will: ~ Clsely mnitr yur vital signs ~ Wrk with yu n managing yur pain ~ Mnitr yur wund, dressing and drain (if present) ~ Manage any nausea We want t partner with ur patients t prvide the ptimal pain management prgram. It is imprtant fr yu t let the nurse knw hw yu are feeling. Let them knw if yu are feeling nauseated and hw well yur pain is cntrlled. Our pain scale is frm Yu will be asked t rate yur pain frm 0 = N Pain t 10= wrst pain ever. 7 N Pain Least Pain Mild Pain Mderate Pain Severe Pain Excruciating Pain What t Expect After Surgery ~ Yur nurse will check yur vital signs frequently. The nurse will als test the strength in yur arms and legs and will ask yu if yu have any numbness r tingling. ~ Yu will have fluids given t yu thrugh an IV t keep yu hydrated. ~ Yu may have a drain t remve fluid frm yur surgery site. ~ Yu will have a urinary catheter t drain yur bladder. 6 8 ~ Yu will have xygen given thrugh little prngs under yur nse. ~ Yu will have wraps arund yur lwer legs t help prevent bld clts. ~ Yur nurse will instruct yu n the regular exercise f mving ankles up and dwn and wiggling yur tes while yu recver. This will start the day f surgery. ~ Yur nurse will help yu change psitins (lgrll) every tw hurs. ~ It is imprtant t start mving even when yu still have discmfrt frm the surgery. ~ Smetimes yu will be able t sit up at the side f yur bed r even start t walk in yur rm the day f surgery. Yur surgen will decide and it will nly be allwed with help f ur hspital staff. ~ Yur nurse will let yu knw when yu are allwed t drink r chew ice. ~ Yu will be instructed t cugh and breathe deeply t prevent breathing prblems after surgery. ~ Yu will be taught hw t use an incentive spirmeter. This is a small plastic piece f equipment that helps yu expand yur lungs. Yu will need t use this piece f equipment abut ten times an hur while awake. Pain Management At Penrse-St. Francis, we are partners in yur pain management. We treat each patient as an individual with pain management needs specific t them. Pain management is apprached in a variety f ways. Yu may receive yur pain medicatins by IV and/r by patient cntrlled analgesia (PCA), and later by muth in pill frm. Additinal methds f pain relief will be encuraged and may include: ice, repsitining, music therapy and relaxatin. Simple exercises such as slw deep breathing t relax yur muscles and reduce tensin will als help t cntrl yur pain. It is better t use medicatin rutinely than t let the pain get unbearable and try t cntrl it at that pint. Keep yur nurse infrmed n yur level f pain cntrl. Yur Passprt T Discharge Yu are an imprtant member f yur health care team! We need yu t take part in yur wn healing s that yu can achieve the level f health and functin required t safely g hme. Listed belw are daily guidelines yu may expect fr each day. This is nly a guide t yur stay. Sme patients will mve thrugh the guidelines mre quickly and thers mre slwly. Each spine surgery is different. Yur surgen can give yu mre specifics t yur persnal surgery and recvery. Gals which will need t be accmplished befre discharge: ~ Medically stable (n fever, uncntrlled bld sugars, bld pressure, etc.) ~ Walk in hall 250 ft. r greater ~ Get in and ut f bed independently ~ Cmplete grming/dressing independently ~ Eat and drink withut nausea ~ Urinate withut prblems ~ Have a bwel mvement ~ Manage pain with ral pain medicatins 9

Yur therapist and nurse will teach yu the prper way t rll, get in and ut f bed and hw t prmte gd psture t prtect yur back. Out f bed walking tw times per day with assistance f hspital staff in additin t yur therapy sessins. Chew Gum: If yu are nt drwsy, chewing gum may help wake up yur stmach and bwels t wrk nrmally. Make sure yu are awake and the head f the bed is up. Up walking with assistance nly. Yu may r may nt have a brace t use during this activity. Therapy will cntinue until yu are independent and safely walking n yur wn. They will make sure yu can safely g up and dwn stairs Yu will als see an ccupatinal therapist t g ver any needs yu have at hme t help with daily functins such as dressing, bathing, hygiene, and taking care f yurself at hme. 10 Yu may put yur brace n r take it ff while sitting r standing. Yu may be allwed t have ice chips. This must be apprved by yur surgen. Yu are encuraged t sit up in the chair fr meals fr a maximum f minutes r as tlerated. D nt exceed a tlerable cmfrt level. Yu culd try sitting up mre ften and fr shrter perids f time. If yu want t stay up lnger take a standing rest with nursing staff. 11 Use yur incentive spirmeter every hur yu are awake t exercise yur lungs. Yur diet may be advanced t clear liquids if yur bwel sunds are present (this must be apprved by yur surgen). When yu are able t eat and drink yur IV fluids will be stpped. Change yur psitin in bed every tw hurs (lgrlling) with assistance f hspital staff. The catheter emptying yur bladder will als be remved. Pain medicatin is given thrugh yur IV and PCA pump. Tell yur nurse if yur pain is nt under cntrl. We want yu t be as cmfrtable as pssible. Yur nurse will instruct yu that nly yu can push yur pain buttn, nt family r friends. If yu are taking liquids, start taking a stl sftener. Cnstipatin is ften an issue with patients after surgery. Cntinue using yur spirmeter. Dn t wait fr the nurse t remind yu. Yu may have an epidural catheter in place fr pain cntrl. Yu may have a catheter in place t empty yur bladder. This will be remved as sn as pssible. Yur nurse case manager (discharge nurse) will cntinue planning fr yur discharge. He r she will assess yur needs and will cmmunicate with all team members wrking with yu t verify yur safety at hme befre discharge.

POST-OP Checklist Pst-p Day 3 r Day f Discharge (please make a check in the bx as each sectin is cmpleted) Safety is ur cncern. Yu will need t meet the fllwing criteria in rder t be able t g hme: Put n brace independently (if rdered by yur surgen). Yur family may help yu if yu need assistance. Cntinue advancing t regular diet if nt dne yet. Yu will need t be able t safely walk and get in and ut f bed and a chair. Yu are able t eat and drink withut any prblem. Yu can urinate withut a catheter. Remval f pain pumps/iv s and all ther tubes if eating and drinking well (if nt dne by this time). Yu have had a bwel mvement. Yur pain is managed well with ral pain medicatins r ther alternatives. Catheter emptying bladder will als be remved if still in place. Ensure yu have needed equipment r aid fr walking prir t discharge. 12 Shwer befre discharge. Prir t leaving, the nurse will review any discharge instructins such as medicatins and incisin care. 13 If n bwel mvement yet, please discuss with yur nurse tday. Things t d when yu are at hme: Cntinue with therapy as instructed. Our physical therapist and ccupatinal therapist will cntinue t prepare yu fr yur return t hme. Try t be ut f bed and independent as much as pssible, BUT never cmprmise yur safety. Always call fr assistance when getting ut f bed. Inspect yur incisin every day fr redness, swelling r drainage. Fllw yur physician s instructins fr care f incisin when shwering. D nt take a bath, use a ht tub r swim until cleared by yur physician. Discharge plans are finalized. Remember t use yur spine precautins at all times. Yu will have the pprtunity t cmplete a satisfactin questinnaire befre discharge. Yu may als get a call at hme after discharge t express yur pinin f yur care while at Penrse-St Francis Health Services. Yur pinin is valuable t us as we strive t give excellent care fr ur spine patients. Take deep breaths and cugh thrughut the day. Take rest breaks thrughut the day. Eat three meals a day with plenty f fruits and vegetables. Drink plenty f water. We suggest taking stl sfteners while yu are taking yur prescriptin pain medicatins. Maintain walking rutine at least 4-6 times a week.
9 14 Check With Yur Dctr Regarding: ~ Driving restrictins ~ Sitting limitatins ~ Lifting limitatins ~ Sexual activity Call Yur Dctr: ~ If yur temperature is greater than 101 degrees ~ If yur pain is severe and nt being cntrlled by yur pain medicatins ~ If yu ntice an increase in redness, tenderness, swelling r cntinued drainage at yur incisin area ~ If yu have any bleeding Exercise: ~ The nly exercise that has been prescribed by yur spine surgen immediately after yur surgery is walking. ~ Begin walking fr minutes per day and increase t minutes, as tlerated. ~ Walk 4-6 times per week. The gal is t increase yur time walking n a weekly basis. Take yur time walking and avid hills r inclines. Shrt, frequent walks may be mre beneficial than ne lng walking sessin. ~ Regular walking exercise restres the strength f yur back and a gradual return t every day activities are imprtant fr yur full recvery. Please get clearance frm yur surgen n any exercises besides walking. Spine Precautins (Vide demnstratin available at Perfrming Daily Activities Sitting Dwn/Standing Up ~ Sit in a firm sturdy chair with armrests, if pssible. ~ Avid sitting in a lw chair. Yu can place a pillw r blanket in the chair fr additinal height and cmfrt. ~ Keep yur back straight while sitting. D nt sluch. ~ Use lumbar supprt r pillw when needed. ~ When standing and sitting, bend frward at the hips, while keeping yur back straight. Getting int/ut f a car: ~ Yu will need t sit in the frnt passenger seat. ~ The driver shuld psitin the frnt seat as far back as pssible. ~ Back up t the seat until the back f yur legs tuch the seat. ~ T sit dwn, have yur back t the seat and slwly lwer yurself t the seat, hlding n t a stable surface (back f the seat, dashbard r car frame). ~ Bring yur legs int the vehicle ne at a time. As yu turn, keep yur shulders, hips and knees in line t avid twisting. ~ Yu may be mre cmfrtable with the seat slightly reclined. ~ Reverse the prcedure t get ut f the car. ~ On lng trips, get ut f the car and walk arund at least every hur. ~ Change yur seat psitin every minutes (reclined, partially reclined, upright). 15 Precautins: N BLT ~ N Bending-d nt bend frward at the waist. Yu may need a reacher t get items ff f the flr. ~ N Lifting-push rather than pull. ~ N Twisting-turn yur whle bdy as a unit. Keep yur back straight. Grming/Eating: ~ D nt bend at the waist t lk in the mirrr r t use the sink. ~ Bring items such as plates, cups and a wash clth t yur face t avid bending frward. ~ If yur back is tired, yu may need t sit in a chair t brush yur teeth, wash yur face, r shave. Bed Mbility And Transfers: ~ Lg rll t yur side t get in and ut f bed ~ Mve as ne unit t avid twisting ~ D Nt Bridge (lift up yur buttcks) t sct up r shift in bed
10 Dressing: ~ Yu will receive instructin frm an Occupatinal Therapist n dressing techniques. These appraches are designed t fllw spine precautins, which help prtect yur back during the healing prcess. ~ Yu may need t put yur pants, shes and scks n while sitting in a chair with back supprt. ~ Yur Occupatinal Therapist may recmmend equipment fr hme use. Yu need t use the equipment until yur dctr discntinues yur spine precautins.
