To work effectively as a member of the multidisciplinary team, ensuring optimal patient care in the patients home environment. The post holder will utilise clinical skills and knowledge incorporating a high level of critical thinking and clinical judgement skills. They lead on developing an impact evaluation on this service. This will involve working autonomously with high level of decision making and problem solving. They will lead this service improving practice underpinned with values based care. To provide clinical support and advice to other members of the team whilst also acting as a first point of contact and making referrals to or co-ordinating with the other members of the multi-disciplinary team. Possess the full range of skills and competences to meet and priorities emergency and urgent care needs of patients who are within the care of the IOPS team to cover a wide range of interventions and have the capacity to call in specialist expertise when a patient requires it. Ensure the highest standard of personalised care is delivered to patients, families and work in partnership with the multidisciplinary team responsible within IOPS.

DIMENSIONS The post holder contributes to the clinical and management responsibility of the integrated older peoples service, within the multi-disciplinary team (medical, nursing and AHP staff) for NHS Lothian. Key staff with whom the post holder will interact with include the Clinical Lead/Clinical nurse manager/consultants for the specific service, allied health professionals, clinical and non clinical staff. 4. 9 5. ROLE OF DEPARTMENT The IOPS service provides a safe and equitable service with high standards of care for elderly/acute stroke patients.. The Service also has a role in the education and training as well as effecting clinical supervision. Leading on the development of managing patients in their own surrounds as identified by local policies and national HEAT targets 6. KEY RESULT AREAS Professional Practice at all times within the Nursing and Midwifery Council Code: Standards of conduct, performance and ethics Using expertise in advanced communication strategies to develop and enhance therapeutic relationships with service users within practice Record personally generated information and update patient/client records in accordance with Nursing and Midwifery council standards, and local and national guidelines Undertake clinical practice in accordance with national advanced practice Guidelines and standards. Participation and development of the multi-professional team through the development of collaborative and innovative practice, ensuring patient is at the centre of care. Monitor standards of care within defined policies, procedures and protocols of the Wards, Directorate and Division to ensure adherence to, and delivery of, a high quality service. Act as an exemplary advanced professional role model and an ambassador for the Advanced practitioner nurse service in professional and public settings. Clinical Provide advanced autonomous clinical care, incorporating high level of decision making and problem solving. They will lead on improving care of in-patients and selected newly admitted patients. Utilise skills essential for assessing and interpreting complex conditions through critical thinking and critical judgement skill and thus analysing the interpretation and comparison of a range of options Identify and prioritise acutely ill patients and facilitate early medical intervention. Undertake investigation of differential diagnosis and prioritise medical and nursing care accordingly, following training at BSc hons/msc level. Under the auspices of agreed protocols, assess the need to order and interpret advanced appropriate clinical investigations. Demonstrate high level of accountability in own practice including the areas of assessment & management of risk Utilising skills acquired through advanced training and competence achievement, establish a patient s management plan making autonomous admission decisions to the appropriate Medicine of

10 the Elderly unit. Provide advanced clinical expertise within training framework to wards, departments and multidisciplinary teams and support junior staff in clinical practice. Develop and Implement protocols within boundaries of NMC code of conduct in order to optimise patient care and be aware of boundaries of responsibilities and limitations of clinical skills. Provide support to patients and families during the hospital stay, underpinned by values based care. Administering medication following completion of non-medical prescribing at MSc level. Promote equality, diversity and patient/client rights and lead the improvement of care. Provide advanced clinical care, telephone triage of patients liaising with GPs and other members of the Primary Health care team. Offer advanced advice/guidance to other medical specialities in relation to appropriate investigation. Provide initial support in bereavement situations to the bereaved and their families, addressing practical, spiritual and cultural issues, contacting bereavement counsellor for the continuation of family support. Respond to clinical emergencies inclusive of Cardiac Arrest and Major Incidents and actively participate within the clinical team. Educational As part of NHS Lothian s Advanced practice framework - completed BSc/Hons level programme working towards or holds advanced practitioner qualification. Maintain a portfolio of evidence outlining development in the national Scottish advanced practice attributes, which include, autonomous practice, critical thinking & clinical judegement skills, high levels of decision making and problem solving, Improving practice and values based care. Responsible for identifying where individual additional education support/development is required and establish an action plan utilising Scottish Advanced Development Toolkit to meet individual personal training and development needs. Identify the educational needs of the specialist groups of patients and their carers and ensure that these are met through the service provided. Act as a resource of knowledge and clinical expertise to ensure care delivered to the patient and family is appropriate and optimum. Assist with the supervised practice of nursing staff within the Division. Delivering education on a core range of subjects appropriate to ward based staff. Ensure that the on-going personal development needs and professional education and research are identified and met. ASSIGNMENT AND REVIEW OF WORK The post holder will required to work autonomously in clinical practice, employing high levels of decision making and problem solving. Work will be assigned by medical and nursing lead and supervisor in response to needs of patients within the service. The clinical workload is generated from the hospitals general patient activity, the services within the division, legislation and performance indicators set by the Scottish Executive Health Department The potholder will have a KSF/Professional Personal Development Plan, linked to the Scottish Advanced Practice Development toolkit which will be reviewed annually by the delegated line manager and clinical supervisor. This includes an assessment of clinical competence. The Clinical Nurse Managers and Chief Nurse will provide professional nursing advice. Speciality consultant or designee will manage assessment of clinical competence. The outcome of this will benefit the quality of the service to the patient. 9. DECISIONS AND JUDGEMENTS The post holder is expected to deal with competing and complex demands from the patient, nursing and medical staff. Clinically and professionally expected to make autonomous decisions on a daily basis, including provision of advice to junior staff in clinical decision making, multidisciplinary team (including medical staff) Freedom to act, and accountable for own professional actions as guided by precedent and clearly defined Divisional policies, protocol/procedures and codes of conduct in accordance with NMC regulations, Clinical and Staff Governance Framework and the EU Clinical Trials Directive.
