In collaboration with the Senior Charge Nurses within Medicine, the post holder will have delegated responsibility for the management of the Unit, including supervision and allocation of staff. The post holder has responsibility and accountability for maintaining both clinical and staff governance within the unit. 3. DIMENSIONS The post holder contributes to the clinical and management responsibility for medical patients within the hospital setting. The post holder will assist with providing acute and ongoing of pre-admission clinics within the overall service provision for patients within the paediatric specialty and will be actively involved in developing nurse led clinics within the surgical setting. The post holder is expected to work in partnership with Medical Teams (Consultants both general and specialist and medical trainees) and specialist nurse teams to develop individualised management packages for children requiring treatment.

6 The post holder is not responsible for managing the budget but needs to be aware of the resources available and the need to remain within the financial envelope. ROLE OF THE SERVICE: Children s Medical Services cover all aspects of acute general care to highly specialised short or long term medical conditions. To provide a high quality, safe and supportive environment in order to care for patients meeting the identified physical, spiritual and psycho-social needs, providing an holistic and family orientated approach to care, and provide support for families and carers of children within Children s Services. To provide specialist care for children requiring medical intervention and/ or referral

7 To provide a 24 hour specialist resource for families and carers of children with specific needs undergoing long term treatment as well as to other staff within the Division, as appropriate. This encompasses: - Direct clinical care Advisory role Provision of multi-disciplinary and nurse-led clinics Education of medical, nursing and allied professionals Audit and research Empowerment of children, parents and staff in managing symptoms/ ongoing care in hospital and at home Discharge planning for complex cases utilising the support of the discharge planner as appropriate NB Throughout this document, the use of the term children refers to pre-term, neonates, infants, children and young people (up to 18 years of age). 6. KEY RESULT AREAS Professional 1. Practice at all times within the Nursing and Midwifery Council Code of Professional Conduct and acting as an exemplary professional role model for leading specialist nursing services. 2. Develop the role by use of evidence-based practice and continuous updating of own knowledge, in line with PREP guidelines ensuring that on-going personal development needs and professional education and research needs, are identified at annual appraisal system and met through attending and participating in appropriate seminars, study days and conferences. 3. Conduct clinical evidence based practice in accordance with national SIGN/Nice Guidelines and NHS Quality Improvement Scotland Leadership and Management 15% 4. Ensure patient needs, are assessed, care planned, implemented and evaluated in collaboration with other team members, and that there is consultation and involvement of carers. 5. Provide leadership, education and direction to multi-disciplinary professionals within the clinical setting and act as a resource for hospital staff, community staff, carers and other professionals, ensuring that medical and nursing staff along with members of the multidisciplinary team have appropriate skills and competencies and are aware of, and work within, local, directorate and divisional policies and procedures to ensure that safe working practices are maintained for both patients and staff

The post holder can frequently be required for consultation, specialist opinion and guidance in regards to care in complex cases involving sensitive and emotional issues e.g. complex respiratory patients who require lonf term assisted ventillation. This may require the allocation and reallocation of duties in response to changing patient need. 7. Act timeously in resolving critical incidents effectively through investigating and responding at local level, and escalate as appropriate. 8. The post holder is required to assume a lead role in developing implementing and monitoring effectiveness of Patient Group Directives (PGD s) relevant to the specialty area of practice Clinical 70% 9. Under the direction of the Senior Charge Nurse and Specialist Nursing teams, provide professional advice and clinical management of symptoms within the paediatric acute and non-acute medical setting. This includes providing support and telephone advice for families when at home. 10. As part of the Clinical team, the post holder will be able to act independently to, monitor, and give specialised advice on, the management of condition/ treatment sequelae for Paediatric patients. 11. Work as a member of the senior medical nursing team delivering direct clinical care to patients, carrying out clinical assessment and review of children and young people and ensuring that all management advice reflects current research and evidence-based practice particularly in relation to extended practice roles. 12. Maintain high standards of symptom and pain management by working closely with the Consultants and other medical and nursing staff, nurse specialists and the multi-disciplinary team. 13. Key clinical roles include, management of Non invasive ventillation processes and equipment, venous access devices such as central lines, Total Parenteral Nutrition (TPN), Patient Controlled Analgesia ( PCA), ordering of investigations ( eg bloods, urine/ stool screening and xray) developing skills to interpret and action on basis of results 14. Work in partnership with the multi-disciplinary team to initiate, develop, maintain and implement clinical guidelines policies and nursing protocols relating to the specialty within NHS Lothian. 15. In partnership with the multi-disciplinary team, identify and develop and implement highly specialist pathways of care for children requiring the expertise of the service. Research 5% 16. Promote and participate in research and clinical audit programmes to support best practice for the specialty service which is evidence-based and leads to continuous improvements in care and ensure evidence-based practice. e.g., national benchmarking and audits

9 17. Act as the patient s advocate to ensure their rights are upheld, when identifying, screening and recruiting subjects into clinical research studies. Education 10% 18. Contribute to the provision of appropriate multidisciplinary specialist education and training programmes to promote a wider understanding of the specialty in the healthcare setting, including supporting the provision of clinical practice placements and mentorship for pre and post registration nurses, all grades of medical staff, Allied Health Professionals, and Play Specialists 19. Contribute to delivery of elements of specialist education and training programmes and activities which form part of vocational qualifications e.g. Clinical Decision Making Module (CDM) and nurse prescribing. 20. ASSIGNMENT AND REVIEW OF WORK The post holder will make autonomous decisions using high level of knowledge and skills while recognising limitations and seeking advice from senior colleagues when appropriate. The post holder will be expected to screen, assess and manage an unpredictable and potentially highly complex caseload of patients. Where appropriate they will initiate, and follow up investigations, treatments, and refer on to other services The post holder is required to rapidly respond to unpredictable events, adapting planned work schedules in response to these events. The post holder is required to prioritise activities in reaction to the complexity of the patient caseload e.g. palliative care/admission of seriously ill patients through accident and emergency/aacutely unwell inpatients. Work is generated depending on in-patient and day-care activity, outpatient clinics and a small proportion of community care. The postholder will have a Professional Personal Development Plan, which is reviewed annually by the delegated line manager

DECISIONS AND JUDGEMENTS The post holder will have sufficient knowledge and expertise to make complex clinical and professional autonomous decisions on a daily basis regarding the nursing and medical management of patients. The MDT will support this. Informing clinical decision making with regard to clinical management, through stringent monitoring of the patients condition and acting on clinical judgements as well as information from the nurse/ team caring for the patient The post holder is expected to make clinical and professional decisions on a daily basis to ensure continuity of optimal care, using expert listening, verbal and written communication skills this will be supported by the MDT. Justify clinical decision making with regard to patient s health care, through stringent monitoring of the patient s condition and acting on clinical judgements. Freedom to act is guided by clearly defined Divisional policies, protocols, procedures and codes of conduct in accordance with NMC regulations, and the Clinical and Staff Governance Framework Advise health care professionals on appropriate range of medications and delivery routes for children of all age groups. Assist with and recognise staff competence issues and appraise accordingly, referring issues to Senior Charge Nurse/ Lead Consultant for information and/or further action. 10. MOST CHALLENGING/DIFFICULT PARTS OF THE JOB Time and Risk management - requires high levels of concentration and accuracy e.g. when setting up TPN, PCA, and when reviewing and advising changes to medication regimes / prescriptions. Diplomatic management of patients and parents under pressure -A high level of emotional effort is required when dealing with palliative care and bereavement; also when dealing with children and families affected by a new diagnosis or complication post surgery. Improving and streamlining the process of care and communication for children requiring surgical management in hospital, out-patient and community settings. Dealing with multi-faceted organisations e.g. hospital multi-disciplinary team, GPs, children s community nurses, learning disability nurses and on occasion children s hospice and respite / residential care facilities. Unpredictability of patient activity and demand, including acute oncology and palliative care

12 11. COMMUNICATIONS AND RELATIONSHIPS Communicate with the child, parent and carers regarding the delivery of patient care on a daily basis, face to face and by telephone. Liaise with the multi-disciplinary team regarding service needs and requirements daily, face to face, by telephone and Communicate daily, face-to-face, by telephone, by or scanner with hospital and community nursing and community medical staff to discuss children under our care. The post holder is responsible for providing and receiving highly complex and sensitive treatment and condition information to families where there can be significant barriers to understanding or acceptance; This may be due to learning difficulties, mental health issues, where English is spoken as a foreign language or in breaking bad news to families. Liaise, and share knowledge and expertise, with haematology/ oncology nurses / specialists within the division and nationally. PHYSICAL, MENTAL, EMOTIONAL AND ENVIRONMENTAL DEMANDS OF THE JOB Physical Skills: Frequent Administration intravenous and sub-cutaneous injections. Setting of syringe pumps and infusion devices. Intravenous additives. Insert sub-cutaneous cannula and central line access devices. Advanced life support. Advanced maintenance of patient s airway (ambu-bagging). Application of topical local anaesthetic. Physical Demands: Frequent Stand/walking for the majority of shift. Patient movement with use of mechanical aides, manoeuvre patients.
