4 Job Advert Heart Manual Post The Heart Manual Programme (NHS Lothian) is the UK's leading home based supported self management programme for individuals recovering from acute Myocardial Infarction and/or Revascularisation. Since its implementation, numerous published studies have accumulated evidence on its effectiveness. The Heart Manual is the most widely investigated, validated and recommended programme for an increasing number of patients and health authorities UK wide and further afield This NHS Lothian service has a UK wide remit to develop, deliver and maintain patient programmes and associated training for health professionals, particularly in the area of cardiac rehabilitation and long term condition management, and as such we are seeking an enthusiastic RGN or AHP to join our team. We anticipate the post holder will have excellent communication skills and the flexibility required to work within a developing/changing environment. While based in NHS Lothian, the successful applicant will have an opportunity to expand their knowledge base in the job, also collaborating with external partners such as Universities and charities UK wide in the development and sustainability of new initiatives. JOB PURPOSE The purpose of this post is to lead independently and in partnership with team members and external collaborators on the upkeep, refinement and currency of The Heart Manual (and associated) Programme/s The post holder will provide a lead role in the development of patient programmes for a range of cardiac conditions and the relevant clinician training surrounding issues on patho-physiology, cardiac rehabilitation, long term condition management, and facilitation, all essential to the programmes effective delivery with patients. The post-holder will also co-lead on new initiatives in the Heart Manual department and as such may work with university academics; cardiologists etc UK wide and further afield to enable such. 3. DIMENSIONS The Heart Manual currently reaches 14,000 appropriate patients in the UK pa ( ) 1,000 Heart Manual facilitators / health professionals are trained to facilitate the programme Approximately 100 Healthcare staff trained annually ( ) Currently 6 trainers now trained to support with training nationally. Trainers receive guidance/support from this post/department to ensure quality and standards are maintained Heart Manual/s currently facilitated internationally in Canada as part of cardiac rehabilitation. The Heart Manual Management Group consists of clinicians, administration, management and finance personnel.( and legal representation as required)

8 A Heart Failure manual, funded by the National Institute of Healthcare Research (NIHR) involving peers from across the UK has completed a large trial with outcomes to be published early in 4 people in the UK could develop heart failure at some point in their lives. This condition uses up 1%- 2% of the total NHS budget= 725m per annum. There is now an urgent need to develop, evaluate, and introduce a home-based rehabilitation programme for individuals with heart failure, which also supports caregivers. The heart manual department were approached to be a key collaborator as the only service in the UK providing evidenced home- based programmes. Stroke workbook-(based on Heart Manual), is now in circulation Heart Manual Programme has been adapted into a Cancer Programme with the department clinicians providing consultancy and support to enable such. 4. ROLE OF DEPARTMENT The Rehabilitation Service The purpose of the Rehabilitation Medicine Service is to provide an expert rehabilitation service designed to meet the individual needs of people who suffer physical disability as a result of trauma or disease. These services are aimed at assisting the recovery and resettlement in the community of people who have suffered an episode of illness or injury and at helping those with persisting disabilities reach and maintain their maximum potential in physical, psychological and social terms

9 The service has a multi-disciplinary approach and includes medicine, nursing, physiotherapy, occupational therapy, dietetics, clinical psychology, speech and language therapy and social work. The Heart Manual Department The Heart Manual Department provides patient programmes and training, (concurrent with evidence based practice and educational processes), to health professionals, nationally and internationally so that the programme can be facilitated with patients as appropriate. Another associated purpose is to lead on modern delivery of this area of clinical care; high quality research programmes in cardiac rehabilitation developments and other chronic conditions acknowledging the principles of clinical governance and new delivery methods such as digital technology and telehealth /care. With digital formats of the Heart Manual Programmes now in use in UK health boards, the department are keen to promote this aspect of delivery. The organisation provides a service within the Cardiac Rehabilitation framework UK wide which covers the patient pathway from in-patient, to community and specialist rehabilitation settings. The Heart Manual Department s remit covers a local, national and international service. 6. KEY RESULT AREAS To lead on the design, development and delivery of specialist training for the Heart Manual Programme/s, in the area of patho-physiology, diagnosis,treatment and facilitation in line with international guidelines, policy recommendations, educational benchmarks, modern media formats, and clinical competencies of health professionals to enable an optimum outcome. To co-lead ((in association with the Health Psychologist) on the overall design, development and evaluation of all patient programmes so that comprehensive and supported self management resources are available and current to this population group. To ensure (in association with the Health Psychologist) that any new intellectual property developed by Heart Manual clinical staff during projects is tabled accurately in order to support and protect the intellectual property which Lothian Health Board owns. To write content on specific aspects of the programme e.g. medication; clinical manifestations, self-management etc after interpreting guidelines from UK, Europe and USA. To train support, and monitor newly trained Heart Manual facilitators UK wide as required To compile reports for internal and external use, plus design and deliver national and international presentations as required. To lead on the delivery of facilitation of the patient programmes ensuring quality and currency is prioritised.(these are 2 day courses of which this post holder will deliver 1 day)

10 To play a lead role in overall service development and implementation of change, hence promoting processes nationally where patients have equitable access and choice. To interpret relevant NHS policies and external recommendations in relation to cardiac rehabilitation; acute care and heart failure in order to apply to the programme for patients and the training for clinicians. To collaborate and contribute to research, audit and evaluation activities within or external to the department which may be local, national or international in nature. To lead on the Patient Involvement Group for the Heart Manual programmes (PIHMs) so that their voices help to inform future service directions. To maintain and develop own professional competencies in clinical and education fields. To support NHS Lothian s values of quality, teamwork, care and compassion, dignity and respect and openness, honesty and responsibility through the application of appropriate behaviours and attitudes. 7a. EQUIPMENT AND MACHINERY IT equipment: computer/laptop, and flipcharts used frequently for training/presentation purposes. Use of telephone and department alarm system Use of tele/video-conferencing for training of clinicians. 7b. SYSTEMS IT responsibilities To be able to use word processing packages, , internet, and databases in the course of daily work, presentations to stakeholder groups and external collaborators. Ability to search for relevant articles as appropriate General Oversee that attendees of heart manual courses have communicated correct details on actual days of courses and have successfully completed the course attended. 8. ASSIGNMENT AND REVIEW OF WORK The post-holder is responsible for planning, prioritising and allocating his/her own work and has freedom to initiate, implement and deliver outcomes within an agreed department framework. Workload is determined by the needs of the department and external demands. The line manager will be the Heart Manual Department Lead The post-holder will be responsible for the management and prioritisation of own workload, work without direct daily supervision. Regular supervision & management will be formally established with the Departmental Lead on appointment.

11 The post holder will be responsible for developing and setting targets within the noted budget, so that project outcomes are achieved; regularly self- appraising these to ensure timescales are met. The post-holder will be expected to work autonomously and as part of internal and UK teams. The post-holder will be responsible to the Heart Manual Department Lead for line management, professional /clinical guidance, including work review and supervision The post holder will represent NHS Lothian at occasional UK meetings with regard to department developments. 9. DECISIONS AND JUDGEMENTS The post-holder will: Anticipate problems or needs and apply the best plan to resolute locally/nation-wide where appropriate Lead on decisions and judgements regarding what to include/exclude in patient information and clinical training for all patient programmes. This is paramount to the overall outcomes of these national developments. Delegate duties to team members (eg administration) as appropriate in order to deliver key result areas and achieve project objectives. Interpret complex information to inform development, design and delivery of the programme 10. MOST CHALLENGING/DIFFICULT PARTS OF THE JOB Ensuring own competencies are achieved and maintained in the global advances of cardiac care, rehabilitation and chronic conditions including the use of multi-media technology and educational principles/processes Manage and prioritise a complex, wide-ranging workload frequently within restricted deadlines. This is intensified as the job is part-time Marrying clinical expertise and educational judgements Working across traditional professional boundaries UK wide, regularly negotiating with project collaborators as to priorities 11. COMMUNICATIONS AND RELATIONSHIPS The post-holder will communicate with the Heart Manual team and the management/directorate team associated with the service, as advised by the Department Lead The post-holder will be expected to share in good practice with all relevant clinicians plus provide education for specialist staff nationwide as appropriate The post holder must have excellent articulation skills, both verbally and written with an ability to discuss and present complex information effectively to a wide range of staff nationally The types of communication required will include conducting training, contributing to meetings, external forums, negotiations and formal/informal presentations.

12 The provision of advice nationally and internationally, plus interpretation,analysis and editing of complex information will be a key requirement of this role The post-holder will communicate on an agreed regular basis with the Department Lead re issues pertaining to this post and the service. The post holder will communicate and negotiate with affiliated external institutions, QMU, NES and related MCNs with specific regard to education for professionals facilitating the programmes. The post holder will lead on the Patient Information Group for the Heart Manual Department. 12.
