2 NC/SC OHNS 2014 Assembly July 25-27, Omni Grove Park Inn Resort - Asheville, NC Target Audience and Purpose The purpose of this educational activity is to provide practical, case-based knowledge and skills to active and academic otolaryngologists, fellows and residents in training. Objectives At the conclusion of this educational activity participants should be able to: Explain current techniques in management of the post-radiated neck. Explain the indications and contraindications for revision tympanoplasty. Appropriately manage children with chronic cough. Identify indications for functional rhinoplasty in the setting of chronic sinusitis. Manage sequelae related to botulinum toxin injections. Discuss management of vascular anomalies. Discuss options for management of tympanic membrane perforations and ways to minimize risk of recurrence. Review the guidelines and discuss postoperative pain control as well as indications for outpatient tonsillectomy. AMA PRA Category 1 Credit TM hours Credit Statement The Southern Regional AHEC designates this live activity for a maximum of 9.5 AMA PRA Category 1 Credit(s) TM. Physicians should claim credit commensurate with the extent of their participation in the activity. Accreditation This activity has been planned and implemented in accordance with the Essentials and Standards of the North Carolina Medical Society through the joint providership of the Southern Regional AHEC and the North Carolina Society of Otolaryngology and Head & Neck Surgery. The Southern Regional AHEC is accredited by the NCMS to provide continuing medical education for physicians. Additional Credit Other health professionals will receive Southern Regional AHEC CEU and/or contact hours and a certificate of attendance from an AMA PRA Category 1 TM activity. These certificates are accepted by the NC boards for physician assistants, nurse practitioners, and nurses. License requirements are subject to change. Southern Regional AHEC recommends that participants contact their licensing board with specific questions. Southern Regional AHEC will provide 1.0 Continuing Education Units (9.5 contact hours) to participants upon completion of this activity. Disclosure Statement The Southern Regional AHEC adheres to ACCME Essential Areas and Policies regarding industry support of continuing medical education. Disclosure of faculty/planning committee members and commercial relationships will be made known at the activity. Speakers are also expected to openly disclose a discussion of any off-label, experimental, or investigational use of drugs or devices in their presentations.

3 Featured Speaker David Nielsen, MD, FACS, is the Executive Vice President and CEO of the American Academy of Otolaryngology - Head and Neck Surgery, and lives in Alexandria, Virginia. Dr. Nielsen earned his MD from the University of Utah School of Medicine and completed his internship at the Providence Hospital. Dr. Nielsen will give an AAO-HNS update on Saturday, July 26. Meeting Features This year's Assembly will include several activities beyond the scientific sessions for your enjoyment. Friday's session begins with head and neck and otology sessions and panel discussions. NC and SC business meetings and state legislative and advocacy updates will take place Friday afternoon, followed by the welcome reception in the exhibit hall. Resident and student presentations are scheduled for Saturday morning, followed by Dr. Nielsen's presentation and a pediatrics session and panel. Saturday afternoon is free; we are offering several recreation options (minimum registration numbers apply). The family-friendly Saturday buffet dinner will feature musical entertainment. Sunday includes a rhinology session and panel, and the meeting concludes with your choice of breakout sessions. Attire for all functions is business casual. Accommodations - Reserve Your Room Prior to June 24 The historic, four-diamond rated Omni Grove Park Inn Resort celebrated its 100th anniversary in 2013 with the completion of a $25 million renovation. The resort features a world-class spa, golf and tennis, several dining outlets, and new shops featuring local mountain artisan crafts, food and gifts. A deposit for the first night s rate plus taxes is required; the group rates are $229 to $299, depending on view, per night, plus taxes, single or double occupancy. Kids Club Families are welcome at the Assembly. The Omni Grove Park Inn Resort provides full-day, half-day, and evening children's programs, as well as baby-sitting services. Fees include all meeting sessions and materials, the Friday evening welcome reception, the Saturday evening dinner and entertainment for the participant, daily breaks and two continental breakfasts. There is a $75 fee for spouses/guests planning to attend the Saturday reception and dinner; the fee for children 3 to 12 years of age is $20. There is no registration fee for students; however, the Saturday dinner fees will apply. Fees include green fees, cart, boxed lunch, beverages and prizes. $120 per person. - Note: Register for golf outing on Meeting Registration Form; fees to be paid to NCSOHNS Biltmore Estate and Winery Tour: Visit America s largest home for a self-paced, self-guided tour of the restored rooms of the Biltmore House, acres of beautiful gardens, and a tour of Biltmore's award-winning Winery followed by a wine tasting. George Vanderbilt, the youngest son of the famous American family, opened his country retreat in The residence contains 34 bedrooms, 43 bathrooms, and 65 fireplaces covering four acres of floor space. Vanderbilt traveled the world to furnish his home: art by Renoir, Whistler, and Sargent; exquisite 16th-century tapestries; and 24,000 books in seven languages. The self-guided tour covers more than 60 rooms carefully preserved to appear as they did a century ago. Fees include transportation, Estate admission, self-guided audio tour of the Biltmore House, all inclusive lunch at the Stable Cafe Biltmore, professional guide, winery tour and tasting. $129 per person. Whitewater Rafting on the French Broad or Pigeon River: Embark on a challenging and exciting raft trip with professional guides through Class I to III rapids on the French Broad River or Class I to IV rapids on the Pigeon River, depending on conditions. Children must be at least eight years of age to participate, and parents or guardians will be required to sign waivers for children under 18. Dry clothes and towels are required for the return trip. Fees include transportation, professional guide, all-inclusive lunch, and all equipment. $99 per person. Canopy Zip Line Adventure: Located in a stunning area of forested mountain property just minutes north of Asheville, this course features over one mile of suspended cable with ten exhilarating zip lines (including one that soars 200 feet above the forest floor and another that is 1100 feet in length), artfully-crafted platforms nested in large native trees, two rappelling experiences, two suspended sky bridges, and three short hikes. At least two professional canopy rangers, who participate in ongoing, rigorous training and are certified in First Aid, CPR, and AED use, accompany small groups. They are members of the American National Standards Institute-accredited Association for Challenge Course Technologies. Participants should be in moderate to good physical condition, between 90 and 250 pounds, 10 years old and over, able to walk one mile, able to reach over their head for the zip line speed control and braking, have the strength to pull their weight along the cable, and must sign a Participation Agreement and release of liability. Legal guardians must sign waivers for youth under 18 years of age. Fees include transportation, all necessary equipment including helmets, gloves, full-body harnesses, thorough training and practice sessions, boxed lunch and beverages. $159 per person.
