2 COURSE DESCRIPTION This three day course is designed to improve and update the knowledge of primary care providers in evidenced-based management of patients with a variety of cardiac or cancer diseases. Cardiology topics will include cardiovascular risk reduction, selecting stress tests, sleep disorders risk and preoperative clearance. Cancer related topics will include warning signs of skin cancer, screening tests and tumor markers, and management of elevated PSA. COURSE LEARNING OBJECTIVES Upon conclusion of this program, participants should be able to: Evaluate the athlete with possible cardiac disease Prepare a follow-up care plan for breast cancer survivors Identify appropriate cancer screening tests Select a cardiac stress test Appropriately prescribe injectible anticoagulants Recognize EKGs that should prompt referral Use billing modifiers more accurately Describe the cardiovascular risks associated with sleep disorders Design a treatment plan for metabolic syndrome Attendance at this Mayo Clinic course does not indicate nor guarantee competence or proficiency in the performance of any procedures which may be discussed or taught in this course. INTENDED AUDIENCE This course is designed for family physicians, internal medicine physicians, physician s assistants, nurses and other primary care providers. CREDIT Mayo Clinic College of Medicine is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians. AMA Mayo Clinic College of Medicine designates this educational activity for a maximum of 17 AMA PRA Category 1 Credit(s). Physicians should only claim credit commensurate with the extent of their participation in the activity. AAFP This activity has been reviewed and is acceptable for up to Prescribed credits by the American Academy of Family Physicians. AOA This program has been approved by the American Osteopathic Association for 17 hours of 2A accreditation. Other Health Care Professionals A certificate of attendance will be provided to other health care professionals for requesting credits in accordance with state nursing boards, specialty societies, or other professional associations.

3 FACULTY INTERACTION There will be opportunities for increased interaction with faculty during this course. First, the faculty will participate in roundtable discussions during lunch on Friday, October 12. Additionally, selected faculty will be available at The Doctor is In table. After the Question & Answer sessions, the faculty will move to a table outside of the meeting space and will be available for additional dialogue with attendees during the break periods. We hope that you will find these opportunities for interaction a positive and informative experience. DATE AND LOCATION The Cardiology and Cancer Care for the Primary Care Physician course will be held October 11-13, Course headquarters will be located at the Grove Park Inn in Asheville, North Carolina. REGISTRATION To register online, visit or complete the attached registration form and return by mail or fax. The registration fee includes tuition, comprehensive course syllabus, continental breakfasts, break refreshments and lunch on Friday, October 12th. Although it is not Mayo School of CME policy to limit the number of registrants for a course, conference room facilities may necessitate closing of enrollment; therefore, early registration is advised. A letter of confirmation will be sent upon receipt of payment and completed registration form. Please present the confirmation letter when checking in at the meeting registration desk. CANCELLATION POLICY If you cancel your participation in this course, your registration fee, less a $75 administrative fee, will be refunded when written notification is received by Mayo School of CME on/before September 28, 2007 (fax#: ). No refunds will be made after September 29, Mayo School of CME reserves the right to cancel or postpone any course due to unforeseen circumstances. In the unlikely event Mayo School of CME must cancel or postpone this course, Mayo School of CME will refund the registration fee, but is not responsible for any related costs, charges, or expenses to participants, including fees assessed by airline/travel/lodging agencies.

4 LODGING ACCOMMODATIONS The Grove Park Inn Resort & Spa, one of the regions oldest and most famous grand hotels, was built in 1913 overlooking Asheville s skyline and the Blue Ridge Mountains. Its architecture is unique, with the historic Main Inn constructed of native granite boulders quarried from nearby Sunset Mountain. On the National Register of Historic Places and a member of Historic Hotels of America, the Inn houses the world s largest public collection of Arts & Crafts antiques under its red-tiled roof. The Inn is famous for its splendid views, Old World charm, massive stone fireplaces and a long tradition of exceptional service and hospitality. Amenities at the 510-room resort include a restored, century-old Donald Ross golf course, indoor and outdoor tennis, two pools, sports complex, shops, fine dining, Children s Programs and family activities. In 2001, the resort debuted a 40,000 square foot spa; The Spa was subsequently rated one of the world s top ten by USA Today. The resort also boasts a club floor with specialty themed rooms, seven-jet hydro showers and Bose Wave radio/cd music systems. A block of sleeping rooms is reserved for attendees of the Cardiology and Cancer Care for the Primary Care Physician course. The discounted hotel room is $ sgl/dbl occupancy plus tax. There is a $40.00/person charge for each third and fourth person sharing a room.
