2 Overview, Target Audience and Learner Outcome Infants, children and youth with increasingly complex health needs continue to transition back to their communities. Pediatric nurses, advanced practice providers, therapists, social workers, mental health counselors, care coordinators, case managers, child care providers, teachers and others in hospital, ambulatory, telephone triage, community health, public health, school, child care and home settings are invited. Emphasizing an integrated approach to care among family, community and healthcare teams through case-based presentations, thought provoking discussions and an optional Lunch n Learn, the learner outcome is to self-report expanded knowledge of complex health conditions, use available resources and implement evidencebased practice improvements for the care provided. Posters and exhibits at will offer a variety of information and resources. Choose to attend the live conference at or at one of the video conference sites in ( Springs), Montana (Billings, two sites), Nebraska (Scottsbluff) or Wyoming (Gillette and Jackson). Objectives Upon completion of this conference, participants should be able to: Explore health equity, care coordination and social justice in the provision of culturally sensitive care for immigrant and refugee populations. Discuss several methods of improving communication between school nurses and community providers. Explain immunization trends, new guidelines and ways to engage parents to support vaccines. Optional Lunch n Learn Discuss current issues and hot topics in child care programs. Identify the co-morbid nature of anxiety and depression and best practices for treatment. Describe the depth of youth suicide in, available resources and current initiatives. Demonstrate the importance of employing culturally appropriate communication for community partnerships in outbreak prevention and control. State two essential components of telephone triage as illustrated through a case presentation. Describe the urgent management of adrenal insufficiency in the nonmedical setting and the available resources to guide the delegation practice of nursing tasks to unlicensed personnel. Describe unique communication needs and effective adaptive strategies for decreasing anxiety and related behaviors in children with autism and other neurodevelopmental delays. Explain common ENT problems. Examine Medicaid s care coordination focus within its Accountable Care Collaborative, as well as its impacts in various health care settings. Review safety and efficacy data for common alternative remedies available to children using a decision making process and scenarios to determine when and where administration would be appropriate. Continuing Education Credit Registration, attendance, sign-in and submission of the evaluation, including a written response to questions related to any change in practice that you may make as a result of learning that took place at this activity, are required for successful completion and receipt of the certificate of attendance. Claim only those hours you attend. Nursing is approved as a provider of continuing nursing education by the Western Multi-State Division, an accredited approver by the American Nurses Credentialing Center s Commission on Accreditation. This educational activity for 13 nursing contact hours is provided by (6.5 hours, including optional Lunch n Learn on 6/14; 6.5 hours on 6/15).
