Hope everyone is doing well, professionally and personally. It s been a busy month for the Board members and committees. We are anticipating a hectic, but fulfilling year. Thank you to the Board and committees for all their hard work now and yet to come! Members, I hope it is evident to you how much effort everyone is making for a successful year! The Conference Committee is excitedly completing the finishing touches for the April conference. Attendees are guaranteed to enjoy the speakers, vendors and networking! The Education & Planning Committee have almost all the slots for speakers filled throughout the year and will be contacting the potential speaker for the October mini-conference. Case managers play a vital role in the care management of patients that are being discharged from a hospital. They work with patients and their families to formulate a plan when a patient requires special arrangements such as physical and mental rehabilitation services, hospice care, skilled nursing, or other post-acute needs. Many care management personnel are registered nurses or social workers. Sue Gatchel has worked at Cleveland Clinic as a case manager for 13 years. She currently helps patients who are leaving the surgical or medical intensive care unit at the Clinic s main campus. She primarily works with patients who have had tracheotomies and are ventilatordependent to find long-term acute care hospitals to continue respiratory and physical rehabilitation. As a registered nurse, Gatchel first moved into case management after losing a previous job as a nurse manager at another hospital. In addition to a Bachelor s of Science degree in registered nursing from St. John College of Cleveland, Gatchel holds a master s degree in Public Administration from Cleveland State University. In April, she will sit for the Certified Case Manager exam, which is a requirement for her job. Studying for the national exam really helps us evaluate our role by allowing us to understand what case management is and how it can support patients and their families, Gatchel explained. The job entails working with insurance companies for precertification and arranging for transportation and admission to follow-up facilities for patients. She spends time discussing each case with the family and medical team, including physicians, nurses, other case managers, and social workers. You really have to be able to multitask because a lot of things and cases are happening at the same time. You have to be organized and present a clear picture regarding the patient, Gatchel said. You also need empathy to handle people who are very sick and deal with families who are experiencing a lot of stress. Gatchel said her job as a case manager is one of the most satisfying of her career, particularly because she learns a tremendous amount. Physicians pull her in for consultations and are always ready to answer a question, which helps her to learn and grow in the position, she said. Social work s role in care management Marlene Silbermann, a licensed social worker at South Pointe Hospital, handles care management for intensive care and emergency department patients. She has been a social worker for 19 years, with 17 of those years being in a hospital setting. Many of Silbermann s patients and their families are dealing with endof-life issues. My job requires someone who is able to formulate a plan at the beginning of a day, knowing that the plan can CM continued on page 3.

President Karen Hooven called the meeting to order at 6:05 PM. Karen introduced the Board members. She also announced that the drawing for the winner to accompany her to the Annual CMSA Conference in San Antonio, Texas in June would be drawn at the March General meeting. All paid members as of the Monday before the meeting (March 14, 2011) will be entered into the drawing. Treasurer's Report Bob Larocca stated that a CPA Review of 2010 will be conducted later this year. He did note that there was an increase in golf outing revenue and that there was a decrease in revenue from membership, conference, general meeting and interest income areas. Our general expenses increased as did our conference expenses. Education & Planning Cheryl Waters asked for suggestions for speakers and to please let her know if anyone knows of vendors that would like to exhibit at a general meeting. Nominating Committee Gale Franko announced which offices would be up for reelection in November. A summary was provided of the benefits of becoming an officer of. Conference Committee Carol Dahl reported that all conference speaker slots were filled. If anyone would like to sponsor or exhibit at the conference, please contact her.. Special Events Peggy Greenlee announced that the golf outing is tentatively scheduled for July 29, 2011 at Astorhurst Country Club. A review of other venues is being conducted. CCM Review Class Anne Llewellyn will conduct a CCM Review Class at the Sheraton Cleveland Airport hotel on August 6, Please save the date. Legislative Committee Jane Perkins reported on Nurses Day at the Statehouse which is March 23. Membership Committee Charlene Szunyog reported that membership prior to the general meeting was at 189. At the meeting, 53 individuals either renewed or joined for a total membership of 241 members. I have to be flexible to deal with family meetings and crises and still keep an optimistic attitude, said Silbermann, who graduated from Cleveland State University with a bachelor s degree in social work. Silbermann was drawn to a career in social work through her parents and grandfather s influence. She also credits a strong faith and the desire to help others in difficult times. She said it is important for those who are considering a career in social work to explore what type of setting is best for them. One of the things that Silbermann enjoys best about her job is getting to know the life history of each patient, as well as who they are and how they got to where they are now. Also, there isn t a day that goes by that she doesn t learn something new, she said, whether from patients, their families, and staff, including physicians, nurses or housekeeping and dietary personnel. I ve been very fortunate, too, that I ve had veteran social workers that gave me good advice. I ve been able to grow as a social worker, taking on larger and more complex case loads because of the encouragement I ve received from them over the years, Silbermann said. Certifications help define case management workers Case management is a collaborative process for formulating and implementing plans that are associated with a patient s health and human service needs. It involves evaluation, advocacy, communication, coordination, and resource management skills. A master s degree is standard for many medical and health service case management positions. Many have advanced degrees in health services administration, long-term care administration, health sciences, public health, or public administration. Others are registered nurses or social workers who specialize in case management. Certified Case Manager is a nationallyaccredited case manager credential that underscores expertise, knowledge and professional experience in handling VocWorks Seeks Field Case Managers VocWorks is seeking Field Case Managers in the North Central Ohio region. Experience with the Ohio workers compensation system and a licensure as a CCM, CRC, CDMS, CVE or COHN required. Our Field Case Managers use field-based case management to help injured Ohioans by developing and monitoring rehabilitation plans for prompt, safe recovery and return to work. They must also have completed 1,500 hours of paid, supervised post-graduate work experience. The median average salary in the U.S. for case managers is $67,782, according to national salary surveys. 6 Care Management Update One Passion One Goal Many Talents By Sue Collier. Article reprinted. Medicare Question: An 86 year old female was taken to the hospital after she fell out of bed at the nursing home and fractured her left humerus. The patient has a history of a previous left shoulder replacement 15 years ago. An orthopedic surgeon is consulted and recommends surgery to maintain reasonable function of the shoulder; however, the family chose not to pursue surgical intervention. The patient was started on oral medications and was stable. The ED documentation indicated that this was a social admission pending rehabilitation placement as the patient did not want to go back to the same facility. This one day stay should be provided as: A. Inpatient admission B. Outpatient observation services C. Extended recovery D. Patient should have stayed in ED for monitoring Medicare Answer: B Outpatient observation services. The documentation does not support that this inpatient admission was medically reasonable and necessary. CMS does not allow payment for services for the convenience of the patient, provider or family. Inpatient care would only be required if the patient s medical condition, health or safety would be significantly and directly jeopardized if care was provided in a less intensive setting. This is also true for continued hospitalization, when the patient s condition warrants discharge. Delays in discharge, when the patient is appropriate for discharge, are subject to medical necessity denials. Managing Denials Simply put, a denial is a lack of expected payment from a payer. Insurers have implemented a number of utilization management programs in an attempt to remain profitable. Two strategies they use are complete denial of reimbursement for hospital days and downgrading of reimbursement. There are more than 250 remittance codes that denote different types of denials % of all claims are denied or rejected Typically, about 50% of denied claim amounts are not recovered. Hospitals typically lose 3-4% of their net revenue each year from denials. Source: Advisory Board Denials management success comes from aggregating data, identifying trends, and focusing efforts appropriately. By promoting crossdepartmental cooperation, focusing on the breakdowns in various steps in the claims life cycle, denial percentages can be reduced. Some of our hospitals have revenue cycle/denials management meetings with the multidisciplinary team, where process breakdowns are identified and addressed. We will be looking forward to expansion of this practice in One of the denial types that Care Management can impact is the no authorization denials. Patient Financial Services (PFS) provides us with monthly data for the regional hospitals; main campus will be included for Approximately 50% of the no auth denials are attributed to Care Management. They can be categorized as follows: Change in level of care 12% Did not fax clinicals or complete 9% Did not precert entire LOS 12% Did not start case 26% Retro authorization obtained by UR 2% Started, but case denied by payer 40% With the average cost of a denial ranging from $11,000 - $28,200 depending upon the hospital, we have much to gain by reducing denials.
