2 PURPOSE OF THE POSITION This position is responsible for maintaining a database which records all patients in the Counties Manukau District who are diagnosed with cancer and receiving treatment. This is to enable an accurate base of information to be kept to enable patient and population data analysis and reporting of national health targets for provision of Cancer Services. 3 PLACE IN ORGANISATION General Manager Medicine CMDHB FCT Team Regional FCT Project Manager Clinical Lead Cancer Service Manager Decision Support lead Service Manager Medicine Clinical Cancer Pathway Tracker NATURE AND SCOPE OF RESPONSIBILITIES Key Accountability Identify and record key points in cancer pathway Standards / Achievements Confirm that diagnosis of cancer for patients identified by screening process Determine the applicable tumour stream for these patients Refer on to the clinical lead for the tumour stream, or an assigned clinical nurse specialist/ nurse coordinator (or other clinician), if the stream has established roles. Use patient information systems and liaison with services to identify key information such as first suspicion of cancer, decision to treat and date of first treatment, according to Ministry of Health rules Record key information in electronic form Prepare monthly reports Ensure all data is collected to report national cancer treatment targets on time With Decision Support Services assistance, generate monthly report, with narrative if necessary Recommend process changes to data collection and entry practices where necessary Initiate improved care coordination processes Identify opportunities to improve data collection or entry, and specify recommendations to Faster Times to Cancer Treatment (FCT) Steering Group Provide information on how cancer treatment time targets data collection can be automated Together with cancer services, design improvements to care coordination processes within each tumour group so to ensure that Counties meets any current and future
4 Key Accountability HEALTH & SAFETY Recognises individual responsibility for workplace Health & Safety under the Health and Safety Act 1992 Standards / Achievements treatment time targets, and addresses ethnic and other inequalities in cancer outcomes Toward end of year in position, design system(s) to allow ongoing collection of cancer treatment target data by the cancer services. COMMUNICATION AND INTERPERSONAL SKILLS Will be required to interact on a regular basis with a range of CMDHB staff members including: Clinical Services treating patients with cancer through Clinical Nurse Coordinators and clinical teams Service Manager Medicine Clinical Lead Cancer Pathology Cancer Control Governance Group
5 Decision Support Service analysts Funding and Planning Team Public Health Physicians Externally there will be contact with: Northern Region Cancer Network Regional Oncology Operational Group (ROOG) Laboratories Cancer Trackers for Auckland, Waitemata and Northland DHBs Ministry of Health analysts Situations may often call for tact, diplomacy and will require information to be handled in a discreet and sensitive manner. In conflict situations will be required to exercise sound judgement, negotiation and persuasiveness skills, toward facilitating a workable outcome. PROBLEM COMPLEXITY The Cancer tracker will be regularly challenged by a wide range of operational issues from a number of areas. There will be a requirement to be able to prioritise issues and negotiate time frames. The range of problems will require solutions customised to meet the circumstances of the service need. Therefore opportunities will arise to provide innovative options based on proven outcomes. There will be demands to meet deadlines, maintain accuracy and quality of information. SCOPE FOR ACTION The Cancer tracker will be required to make decisions or recommendations relating to the gathering of information for the cancer pathway. This position is encouraged to use initiative and problem solving skills to develop innovative approaches to issues. Discretion is required to be exercised in releasing confidential information to the appropriate parties. DIMENSION OF THE POSITION Will be responsible to the Service Manager Medicine for the achievement of agreed objectives and operates within the confines of Counties Manukau District Health Board. 7 PERSON SPECIFICATIONS Minimum Qualification New Zealand Registered Nurse Experience Skills/Knowledge/ Behaviour Current Annual Practicing Certificate Experience working with multidisciplinary groups and ability to gain cooperation. 5 years + in health care Ability to navigate CMDHB clinical systems (Concerto, PiMS, Éclair) Intermediate to Advanced user of Microsoft Office. Reputation for delivering on target with reliability Excellent communicator, highly articulate and excellent listening skills Self-directed and able to achieve results. Confidence with learning new information systems Demonstrated use of initiative, with confident skill in negotiation and consultation.
