2 Purpose : Identify clinical events which meet the ACC Treatment Injury claim criteria. Complete paperwork to lodge the required ACC paperwork [45 and 2152] Identify patients who are non-wdhb domicile and coordinate with the Charge Nurse Manager and Clinical Nurse Specialists to support their transfer to their home DHB when clinically ready KEY ACCOUNTABILITIES (GENERIC) Domain One Professional responsibility Includes accountabilities for professional, legal, ethical and culturally safe practice. Self-directed in achieving learning and development plan, including PDRP responsibilities/ current portfolio Domain Two of nursing care Includes accountability related to patient/client assessment and management of advanced nursing care that is supported by evidence and alsos assessment, developing differential diagnoses, ordering and interpreting diagnostic tests, planning and providing treatments in order to manage the health of individual patients Clinical Care/Care Work Stream Treatment Injuries Coordination Identify inpatients with potential and actual treatment injuries in discussion with clinical teams and the quality leads Leads and manages an Obtains patient consent and or liaises with family and whanau. Maintains database and provides weekly reports tracking progress ACC are provided with all necessary information to make an informed decision on cover. 3 Work Stream Inter District Flow Patient Identifies non-dhb domicile inpatients Liaises with Charge Nurse Managers and Clinical Nurse Specialists to ensure that patients are prepared for transferred back to their home DHB as soon as clinically stable. Facilitate planning so that out of domicile patients only remain at WDHB as long as is clinically required. Liaises and consults with all disciplines regarding the patient care management plan as needed to ensure seamless communication with team and patient. Identify patients with potential ongoing health care needs and facilitate appropriate referrals as required, in consultation with the Charge Nurse Managers and Clinical Nurse Specialists. Determine the need for interdisciplinary referral, and coordinate relevant appointments. In conjunction with the patient and family initiates discharge planning processes Domain Three Interpersonal relationships and enhancing the patient experience Includes accountability for interpersonal and therapeutic communication with clients/patients and members of the health care team. Interpersonal relationships with clients and members of the health care team Maintain a professional and collaborative manner when liaising with patients and families, colleagues and health care professionals (inclusive of surgeons, anesthetists, booking clerks and hospital managers). Strong relationship management skills that result in professional credibility, good patient outcomes and integrated care. Communicates effectively, regularly and clearly in both oral and written messages. Maintains open and effective communication with the entire patient care team. 6 PERSON SPECIFICATION POSITION TITLE: Registered Nurse ACC Clinical Case Education and Qualifications Registered Nurse - RCpN, RGON, BHSC - with current Annual Practising Certificate with current portfolio/equivalent Experience 3 years post registration in contemporary acute care nursing practice preferably in surgical specialties Achieved RN Level 3 or equivalent Understands ACC preferred but can be taught Understands IDF [Inter District Flow] processes preferred but can be taught Skills/Knowledge/ Specialty knowledge and knowledge of the care pathways Behaviour Demonstrated operational / care coordination skills Clinical professional role modelling Understands the importance of good relationships. Ability to work as a team member Excellent verbal and written English Excellent written and verbal communication.
