2 Purpose : To provide specialist nursing care and expertise in direct care delivery for the management of urology patients. Advanced practice role, working within clinical pathway/guidelines/protocols and standing orders for patient groups providing innovative, evidence based nurse-led solutions to meet patient demand for urology Clinical Nurse Specialist: Definition under DHBNZ/NZNO titles Focus on care delivery Providing specialist nursing care and expertise, both in direct care delivery and in support to other staff in the management of a defined patient group/area of specialty practice Researching, evaluating, developing and implementing standards of nursing practice in the specific area of practice Leads the development of pathways, protocols and guidelines in the specific area of practice Care assumed according to the CNS level of practice, speciality competencies, care bundles described in a credentialing document, standing orders and delegated authority. 3 Domain Two Management of nursing care at advanced practice level Includes accountability related to patient/client assessment and management of advanced nursing care that is supported by evidence Clinical Care/Care Coordination Education and clinical teaching Provides specialist nursing care and expertise both in direct care delivery and in support to other staff in the management of a defined patient group/urology practice. Demonstrates advanced comprehensive health assessment skills and diagnostic decision making relevant to patients with urology issues Has been credentialed for certain procedures at an advanced practice level Role models expert clinical practice, excellent communication skills and manages clinical workload to meet service targets and requirements Determine the need for interdisciplinary referral, and coordinate relevant appointments. Ensure relevant pre-admission investigations and referrals are executed as per agreed protocols with Anaesthetic Department and Surgical Service. Triage to the appropriate person, ensuring that the results of investigations and referrals is available to the surgical unit and broader multidisciplinary team as per agreed protocols and readily accessible on the day of appointment or day of surgery Initiates ordering of diagnostic tests in order to reach differential diagnoses according to best practice guidelines, clinical pathways and evaluates outcomes Implements appropriate interventions based on a systematic decision making process. Implements agreed care plan and treatments, prescribing and managing care to discharge from the service according to approved service protocols Provides specialist nursing care in the delivery of nurse-led clinics In conjunction with the patient and family initiate discharge planning process and liaise and consult with all disciplines regarding the patient care management plan, ensuring seamless communication with the admitting ward nurse manager (or delegate) on patient s admission. Liaise with the multidisciplinary services for the provision of services and ambulatory supports required by the patient. Identify and initiate remedial action in consultation with the Operations Manager and Clinical Director (if necessary) to system impediments that negatively impact on a patient s progress and communicate issues appropriately. Identify patients with potential ongoing health care needs and facilitate appropriate referrals as required. Works closely and completes delegated work from the Clinical team e.g. SMO Specialist or Nurse Practitioner as appropriate. Follows up outcomes and liaises with the interdisciplinary team, primary and tertiary services Provides patient education to the patient group to improve patient outcomes and gain participation with their treatment Supports other staff in the management of a defined patient group/areas of specialty practice Acts as a nursing resource providing expert advice and education to nursing staff and health care professionals / family-whanau within specialty scope of practice, demonstrating skilled mentoring/coaching, teaching and supervision. May teach at post registration level to enhance evidence based practice Continues to update and produce new and appropriate education materials for staff.
