3 CEU Information Concurrent Seminars Attend the designated full-day seminar to receive appropriate CEU credit. The CEU Key indicates the continuing education available at each seminar. Seminar titles represent the specific discipline referenced. For those seminars that encompass multiple disciplines, please refer to the CEU Key. All NAB-approved programs will require three verifications of attendance that are required to receive your CEU Verification Form: registration check-in, check-in after the lunch break, and check-in prior to dismissal. 9 Conference Information REGISTRATION Each applicant must complete a separate registration form. Two-day attendees must register for one seminar each day. Prices listed are per-day rates. Early registration is encouraged! Registrations received after February 13 will be accepted on a space-available basis. Registration and payment must be received by February 13 to receive the early registration fee. Each seminar is a full-day program. Mail check payable to LeadingAge Oklahoma to PO Box 1383, El Reno, OK Registration fee includes Continental Breakfast, Lunch Buffet, and Afternoon Refreshments. Credit Card Payments: Credit card payments are accepted online only. Online registration is available at Passport Registration: This entitles the attendee access to any of the seminars for both days. Simply mark the Passport registration and select all seminars you would like to attend. You will receive all applicable seminar materials for the seminars you have selected. To request CEU credit, mark the seminar you plan to attend each day and the corresponding CEU credit. This registration will ensure seating availability at all indicated seminars.
