3 Registra ration Information Each applicant must complete a separate registration form. Early registration is encouraged! Registrations received after March 1 will be accepted on a space-available basis. Registration and payment must be received by March 1 to avoid the late registration fee. Cancellations received prior to March 1 will receive a refund, less a $15.00 processing fee. Cancellations received prior to March 10 will receive 50% credit toward future OKAHSA seminars. Substitute attendees are welcome. Notify OKAHSA by March 1 of any substitutions. Cost: see registration form. Registration fee includes Continental Breakfast, Lunch Buffet, and Afternoon Refreshments. CEU Information Concurrent Seminars - Attend the designated seminar to receive appropriate CEU credit. Nursing Home Administrators The Long Term Care Administration Seminars on March 13 and 14 have been reviewed by the National Continuing Education Review Service (NCERS) of the National Association of Boards of Examiners of Long Term Care Administrators (NAB) and approved for 12 clock hours and 12 participant hours. Program approval number: Home Care Administration The Long Term Care Administration Seminar on March 13 and the Nursing Seminar on March 14 have been submitted for review by the Oklahoma State Department of Health for CEU credit for Home Care Administration. Dietary Seminar has been submitted for 6 Hours Continuing Education by the Certifying Board for Dietary Managers. Assisted Living/Residential Care The Long Term Care Administration Seminars have been submitted for 12 hours Residential Care Administrators Continuing Education Credit through the Downtown College Consortium, in conjunction with the University of Central Oklahoma. If you prefer to have your CEU s catalogued by the credentialing institution, there is an additional fee. This is optional. All attendees will receive the necessary certificate for CEU verification. The CEU credentialing fee is $20/6hours, $30/12 hours which is charged by the credentialing institution. Nursing Seminar has been submitted for approval for continuing eduation by the Ohio Nurses Association, an accredited approver by the American Nurses Credentialing Center s Commission on Accreditation. 9 Special Events MARCH 13 OKAHSA Annual Business Meeting Association Architecture A brief business meeting will be held to elect officers and recognize OKAHSA leadership. Dinner tickets for the OKAHSA Membership Dinner will be distributed at the close of the Annual Business Meeting. OKAHSA Membership Dinner Exterior Plans All OKAHSA members are invited to attend the OKAHSA Membership Dinner at Abuelo s in Bricktown at 6:30 p.m. You must reserve your tickets in advance.
