3 INTRODUCTION This Annal Report and Qality of Care Report provides an overview of Ballarat Commnity Health operations from 1st Jly 2013 to 30th Jne The report is designed to inform clients, carers, the local commnity, staff and volnteers, and stattory athorities abot the bsiness, performance, qality, safety systems, processes and otcomes of or activities for the year. Also inclded in this report is a financial smmary for the Financial Year. Qality of Care Report The BCH Qality of Care Report is prodced annally, as reqired by the Department of Health for all Health Services in Victoria. The prpose of the Qality of Care Report is to inform people in the commnity abot how the organisation operates responsibly, and to demonstrate how BCH measres and improves the qality of services we provide. Like last year, the Qality of Care Report has been incorporated into the Annal Report. Featred throghot the report are examples and case stdies to highlight some of the ways in which BCH ensres that or services are delivered safely, appropriately and with vigilance to meet the diverse needs of or commnity. 4 Profile BCH is a pblic company limited by garantee, registered nder the Corporations Act 2001, and complies with all relevant legislation pertaining to client privacy inclding the Health Records Act The company is registered with the Victorian Department of Health as a Commnity Health Centre and receives income from a range of sorces inclding State and Federal governments, other grants and donations, and private service income. BCH provides commnity-based health care for residents of the City of Ballarat, Golden Plains and Moorabool Shires, as well as to individals from Hepbrn and Pyrenees Shires. Residents of other mnicipalities may also choose to access BCH programs. Some service types are provided across the Grampians region and beyond, with clients residing in most Victorian mnicipalities, and some interstate. Or services over the past year have been provided from sites at in Central Ballarat, Sebastopol, Wendoree, Ballarat East and Smythesdale, with otreach clinics and services delivered at a range of locations throghot the Grampians region. For the last 2 weeks of Jne services were also delivered from or new facility in the sbrb of Lcas. Service delivery at BCH is focsed on: working collaboratively with the commnity to promote health and wellbeing empowering individals and commnities to be proactive abot health providing holistic and coordinated health care in partnership with other agencies and health services targeting services to people and grops who are particlarly disadvantaged or who have difficlty accessing the health services they need early intervention where health is compromised offering treatment services to spport clients to reach best possible health otcomes Vision, Mission and Vales The Vision and Mission of BCH are absoltely central to or endeavors, and form part of the day to day ethos and condct of or staff. BCH is committed to operating as a vales-based organisation and has adopted the vales shown below as integral to or work. We seek to demonstrate these vales in the way we choose to interact with each other, or clients, partners and the commnity. 6 PRESIDENT AND CHIEF EXECUTIVE OFFICER REPORT All years have highs and lows, sccesses and losses /14 has exemplified this for BCH mch more than any other recent year becase both the sccesses and the losses have been significant and with lasting impacts for staff, the organisation and the commnity. There has also been mch that is bsiness as sal delivering important, in-demand health care, health edcation and health promotion services for or commnity. In this annal report we have tried to give yo a sense of the year: a taste of the work of BCH in service delivery; snippets from or clients and staff; a snapshot of organisational performance and achievements; a smmary of or strategic direction, and most importantly a feel for the people at the centre of BCH - commnity, clients, volnteers, staff and Directors. The organisational vision, mission and vales gide and inform the work of BCH at all times. Strategy and Governance are the work of the Board, incorporating sond financial, risk, qality, and compliance oversight. This work mst be copled with excellent, well-qalified staff delivering the desired operational otcomes according to policy, procedre and professional gidelines. Throghot this report we have highlighted aspects of this ongoing work. At the 2013 Annal General Meeting (AGM) we were sad to farewell Liz Sheedy who has been a valed Director since The Board had made a decision to increase the nmber of Directors from nine to ten, and several nominations for election were received inclding from Robert Thrman, Bryan Crebbin and Jennifer Cobrn whose terms had expired, bt were eligible for re-election. In addition nominations were received from Sndram Sivamalai, Glenda Cairns and Pal Ryan. Glenda sbseqently withdrew meaning that no election was reqired as the nmber of nominations matched the vacancies available. Sndram Sivamalai and Pal Ryan joined the BCH Board from October It shold be noted that Dean Nttall has relocated temporarily to Qeensland, and has been granted 12 months leave of absence from Jne Marie Degrazia to investigate the possibility of establishing a day rehabilitation program for people recovering from alcohol or other drg addiction BCH Conselling Team to investigate and report on the feasibility of staging an international conference on intervention and treatment in circmstances of family violence Congratlations to both award winners on their sccessfl sbmissions and for the work done since those awards to advance the proposals. Sbmissions for fnding have been made to spport the development of the Day Rehabilitation model, whilst an International Family Violence Conference will take place in Ballarat in November Dring the year the Board ndertook a review and refresh of the Strategic Plan, which was a good opportnity to engage new Directors as well as to ensre a clear and shared ftre focs. In Febrary 2014 BCH participated in or external accreditation review by Qality Improvement Performace. Seven aditors observed and reviewed or services and processes over five days and awarded a frther three year s accreditation. They made special mention of the qality of or commnity mental health services, achieving accreditation against the new National Mental Health Standards. A major focs of the year has been the constrction of the new Lcas facility, with the bilding contract awarded to Nicholson Constrction in Jly In spite of many early wet weather set-backs, a very tight program, smmer heat delays and other challenges, we did meet or program deadlines. Staff began moving into the Lcas facility dring Jne, with the first client services offered on-site from 11 Jne. This bilding project has been an amazing effort with congratlations de to DesignInc, Nicholson Constrction, PlanCost, the Project User Grop and Project Control Grop and especially to the BCH Project Manager Jim Rodgers. At the 2013 AGM we were very pleased to annonce the winners of the inagral staff sponsorship awards. De to a generos donation from the Timbek Family Trst, together with the BCH allocation, we were able to spport two staff proposals: Ballarat Commnity Health Annal Report and Qality of Care Report

7 Dring the year we have initiated or spported the development of a range of new programs inclding Partners in Recovery, the Grampians Loddon Mallee Pharmacotherapy Network, Ballarat Health Companion Program and others. In addition the fnding for some existing programs was extended inclding School Focsed Yoth Service, PODS yong parent program and the Yoth Spport Service. We have also faced the decommissioning of APROTCH, the Adlt Psychiatric Rehabilitation of the Central Highlands Program. APROTCH has operated from the Qeen Street site in Ballarat East since The BCH APROTCH staff were informed on 7th March that their service wold cease on 31 st Jly They showed the most incredible, selfless commitment to their clients in spporting the transition to the reformed service system. They will all be missed as valed members of staff. Alcohol and other Drg Services for Adlts in Victoria were also recomissioned in this year. BCH formed a consortim with Grampians Commnity Health, Hepbrn Health, Djerriwarrh Health and Yoth Spport and Advocacy Service to bid to provide services in the Grampians region. We were sccessfl in being a provider nder the reformed service system, however treatment services available in the region have been sbstantially redced as a reslt of the recomissioning process. BCH contines to be spported with wonderfl staff who contribte above and beyond the call of dty and a team of enthsiastic volnteers who assist with a wide range of administrative, transport, delivery and other tasks. We cannot close this report withot thanking them for their invalable contribtion. We trst that yo will enjoy reading abot the work of BCH in these pages, and look forward to yor involvement in the coming year. 8 STRATEGIC PLAN The BCH Board of Directors has responsibility for the development and implementation of the strategic direction of the organisation. The BCH Strategic Plan was developed by the Board, in conjnction with staff and other stakeholders. In early 2014 the Board ndertook a review of the plan to refresh strategies and sccess measres. Some action plans were complete and reqired no frther attention whilst changing circmstances led to the incorporation of additional strategies. The Strategic Plan nderpins all organisational activities, informing the development of team and individal work plans and any bsiness cases for the development of new programs. The Board receives reglar reports on progress against the sccess measres and milestones for each strategy. 11 PROGRAMS & SERVICES Yoth Services Ballarat Commnity Health assists yong people and their families to make positive changes, connect and contribte to their commnity. Or Yoth Team works holistically with clients to ensre access to safe and affordable hosing, qality healthcare, edcation and employment opportnities and to focs on commnity connectedness. Immigrant and Refgee Spport Services BCH provides a range of services designed to spport refgees and other immigrants to settle in Ballarat. Over the year staff have been involved in a nmber of partnerships with key agencies to enhance collective impact on the development of services, programs and spports that are vital to achieving positive settlement otcomes for new arrivals. Refgee Health Nrse Fnded by the Department of Health, the Refgee Health Nrse role assists with health assessments and spport, working with General Practitioners and other health professionals to address the specific needs of immigrant clients. Assistance for Newly Arrived Hmanitarian Entrants The Hmanitarian Settlement Services program assists people in their early settlement period in Astralia. Changes to federal government policy regarding asylm seekers have meant that referrals to Ballarat for direct settlement have decreased significantly. Collaborative and respectfl partnerships with clients, families, and local commnity will spport, inform and direct service development to meet emerging needs and isses. This framework nderpins the BCH vales of respect, responsibility, corage, integrity, optimism and resilience. The Yoth Services Team provides a range of yoth services to vlnerable yong people in or commnity: Yoth Homelessness Spport working with yong people to provide access to safe, secre and affordable hosing, and to develop their independent living skills Creating Connections linking yong people at risk of homelessness with edcation, employment and training opportnities Innovative Health Services for Homeless Yoth (IHSHY) School Focsed Yoth Service (SFYS) - fnding was renewed in 2014, and SFYS contines to work with schools to spport those yong people most at risk of disengaging from school Providing Opportnities and Developing Services (PODS) for yong parents Yoth Spport Service (YSS) spporting yong people and their families, with a focs on diversion from the yoth jstice system Spport for Refgees and Immigrants The Settlement Grants Program is a free and confidential service for Refgees and Immigrants (depending on visa types) who have been living in Astralia for p to 5 years. The program delivers a range of services inclding case co-ordination and spport, referral, commnity development and advocacy. Steady demand for this service has contined becase of an increase in eligible immigrants arriving in Ballarat. Conselling and Advocacy for Srvivors of Tortre and Trama This specialist conselling program for refgees and asylm seekers aims to help srvivors find a way forward as they strive to live with the memories and conseqences of their experiences with corage and dignity. Paediatric Immigrant Health Clinic - The PIHC provides care to immigrant children for any health concerns and assistance for adlts to achieve adeqate levels of Vitamin D to spport their optimal health and wellbeing. The Clinic also contines to spport local General Practitioners (GPs) who have become accredited prescribers of High Dose Vitamin D by facilitating the retrn of these clients to the GP once their Vitamin D has achieved maintenance stats. The PIHC provided 205 episodes of care in Ballarat Commnity Health Annal Report and Qality of Care Report 2014

12 Doctors Clinics BCH Doctors Clinics are located at Wendoree, Sebastopol and Smythesdale. The Clinics crrently serve arond 8,000 patients, with an aim to improve access to healthcare for people living in regional and rral commnities in Ballarat and northern Golden Plains Shire. The Clinics have expanded throghot and we have increased or nmbers to six GPs. We now have two GPs working in each Clinic, while an increase in Practice Nrsing over the last 12 months has also allowed the Clinics to offer greater access to services sch as: Women s and Men s Health Screening Care Planning Immnisations Wond Management Blood collection Adolescent Health The GP Clinics will expand frther in the second half of 2014 with the commencement of a forth clinic located at the new BCH Lcas facility. Nmber of registered patients of the BCH GP Clinics Sexal Health Clinic The Sexal Health Clinic contines to provide otstanding services in sexal and adolescent health, and remains the only dedicated Victorian sexal health clinic otside metropolitan Melborne. The key services of the SHC inclde: Contraception Family planning advice Pregnancy testing Pap screening Sexally transmitted infections testing and treatment Sexal health conselling IUD insertion The SHC has contined to exceed its targets for Blood Borne Virs and Sexally Transmitted Infections (BBV/STI) services and Commnity Women s Health Program services. It also contines to provide valable experience in the development and edcation of medical stdents. Throghot this year the SHC has been working with three secondary schools in developing a trial to place Adolescent Health Clinics in those schools in the later part of 2014 to assist the stdents in accessing adolescent specific health care. I was so impressed with the warmth and time taken to explain the sitation and test reslts to (my daghter). In this day and age of pblic health services where yo are given a nmber and rshed throgh the system it was amazing to me to see how well my daghter was treated from the nrse throgh to the doctors. 13 PROGRAMS & SERVICES Ballarat Primary Care Ballarat Primary Care (BPC) provides allied health services to BCH clients throgh an extended access clinic. The clinic operates from several BCH locations with evening and weekend consltations available. BPC crrently has staff providing the following allied health services: Podiatry Speech Pathology Diabetes Edcation Dietetics Clinical Psychology ballarat primary care consltations Exercise Physiology BPC contines to be sed as an example in the se of the Medicare Benefits Schedle (MBS) for provision of allied health services. 14 Health Management The Health Management team provides a range of clinical services which spport clients to achieve their best possible health otcomes. Early Intervention in Chronic Disease Management program provides a range of grop and individal services to spport people and their families and carers to manage chronic illness. This year a social worker has been appointed to assist people in dealing with the emotional impact of having a chronic disease as well as providing case management. Diabetes Nrse Edcators provide services to people with type 1 and type 2 diabetes. The referrals to diabetes nrse edcators contine to increase as the nmber of people being diagnosed with diabetes increases. Ftre work will focs on piloting ways to address this increasing demand. Dietetics provides information and spport to people on a variety of isses and for people of all ages. This service is provided at a nmber of locations inclding BCH sites, with otreach work to Smythesdale, Ballarat And District Aboriginal Co-operative and, more recently, headspace (a mental health service for yong people). Podiatry provides assessments and review for people of all ages, screenings at SRSs and edcation to parents at Maternal and Child Health Centres. Respiratory Nrse provides services to people with asthma and Chronic Obstrctive Plmonary Disease as well as providing smoking cessation spport. A wide range of grop programs are provided inclding: Caring for Diabetes (for people with Type 2 Diabetes) DAFNE - Dose Adjstment for Normal Eating (for people with Type 1 Diabetes) Spermarket Tors and Label Reading Workshops Life! (Diabetes and Heart Disease Prevention) MEND 2-4 and MEND 7-13 (healthy eating and activity program for fssy eaters or overweight children and their families) Exercise Physiology provides a range of services and programs inclding assessments, home exercise programs and a wide range of exercise programs. There are grop exercise programs at some Spported Residential Services (SRS), Smythesdale and BCH sites. Physiotherapy spports Physio Gym for Stroke grop and provides individal assessments and reviews. Physical activity programs for people of all abilities Falls Prevention (this has been extended to inclde a new program that started in Smythesdale this year) Living Longer Living Stronger (fitness and strength training for over 50 s) Ballarat Commnity Health Annal Report and Qality of Care Report

15 PROGRAMS & SERVICES Healthy Lifestyle & Commnity Services The Healthy Lifestyle Team has delivered a broad range of health promotion activities and programs in and completed a major research project to inform health promotion activities that address gambling isses in teenagers. Workplace Health Worksafe s five year WorkHealth Program ended in December BCH was the local provider across the Grampians Region and condcted 4,711 health checks throghot the life of the program. The data collected and the research reslts provide valable insight into the health of the region s workforce. Following the sccess of this program, BCH has developed a site of programs for workplace health and wellbeing inclding a workplace immnisation program and the contination of workplace health checks. Health Promotion in Schools BCH has been actively working with 45+ workplaces delivering these programs which also inclde health information presentations on Mental Health, Women and Men s Health, QUIT smoking and Healthy Eating cooking demonstrations. The BCH sexality edcation program was delivered by the sexal health nrse in local primary and secondary schools. A nmber of sessions abot body image were delivered by health promotion officers in primary and secondary schools to increase stdents knowledge of self-esteem and body image and the factors that may inflence these. BCH condcts the Dental Health Service s Smiles for Miles Program, working with local organisations to improve the oral health of preschool aged children and their families. BCH staff have spported 23 early childhood services to implement the program, which involves developing a ntrition policy, healthy messages and activities with the children and engaging parents. Congratlations to Phoenix P-12 Commnity College one of three primary schools across Victoria that have been recognised for their work towards creating a Healthy School. Phoenix P-12 Commnity College is working throgh the Achievement Program to become a health promoting school with spport from Ballarat Commnity Health. The College has engaged with staff, children and families, and has developed a new health and wellbeing team to spport the implementation of the Achievement Program. The information the team has gathered throgh exploring their crrent practice and needs has informed the selection of their health priorities to work on, identified areas of good practice and gaps in practice. Health priority areas selected were Sn Protection and Physical Activity. As a SnSmart school, they are able to apply for advanced standing for sn protection which recognises their pre-existing achievements in this area. For physical activity, they are in the process of finalising a plan of action to work towards meeting each of the benchmarks. We believe that creating a healthy school will improve health and wellbeing otcomes for all members of the school commnity and that will impact positively on the learning and development of or stdents. The Achievement Program provides a framework for s to coordinate health and wellbeing practice at Phoenix P-12 Commnity College. 16 Exercise Programs Exercise is an important component of the improvement and maintenance of health and wellbeing. There are a range of exercise programs at BCH designed to meet specific needs, inclding Pilates, Zmba Gold, Tai Chi and Yoga. The exercise physiologists at BCH have rn the Living Longer Living Stronger (LLLS) strength training program for older adlts for a nmber of years and participants have consistently shown significant improvements in their physical activity levels. Many positive comments demonstrate that the program is not only effective bt fn and a great way to meet people. Exercising at the gym makes me more aware of also being more active on other days of the week and keeping mscle strength p. I cannot speak too highly of the program, and the instrctors. Commnity Arts and Wellbeing Program The Commnity Arts and Wellbeing Program has expanded over the past 12 months to inclde art, drama and msic workshops and creative learning programs with schools and other organisations. The Arts Otreach Program incldes creative workshops for a grop of yong girls who sffer from chronic illness and social development workshops and drama sessions for people with disabilities. Life Story Writing has also been a part of the commnity arts program for several years and participants in the eight week corse enjoy the opportnity to get their story on paper, socialise with like-minded others and share their stories. Health Companion Service The health centre is very impor tant to s. We love the friendly atmosphere and helpfl trainers who know their job. We also love the after-exercise cppa and socialising with others. Every visit is wor thwhile and enjoyable. Hepatitis C Program The Health Companion Program is a new transport assistance program for people travelling to Ballarat for a health appointment. Ballarat Commnity Health provides specially trained volnteer Health Companions to meet clients at the Ballarat Railway Station and accompany them to and from their health appointment by bs or taxi. So far, nine volnteers have been trained for the program. The Integrated Hepatitis C Program is now into its forth year, contining to provide edcation and spport to clients with Hepatitis C. Or nrse-rn clinics are held throghot the Grampians region in Ballarat, Stawell, Horsham and Daylesford. Working closely with GPs and Ballarat Health Services Liver Clinic, these clinics assist clients to gain access to treatment and spport clients throgh this process. Over the past 12 months, there have been 40 clients in varios stages of treatment, with 14 clients sccessflly clearing the virs. Hepatitis C sessions have also been condcted in a nmber of schools to increase yong people s knowledge and awareness of Hepatitis C, particlarly in regard to transmission and prevention. 17 PROGRAMS & SERVICES Mental Health Services Grampians Partners in Recovery Grampians Partners in Recovery was established in 2013, with staff employed throghot the Medicare Local region, as planned with or consortim partners who are Grampians Commnity Health, Wimmera Uniting Care, St Lke s Anglicare, Centacare, MIND and Grampians Medicare Local, ensring good access to services. At BCH we have three Facilitators and one Spport Liaison Information Officer. The staff contine to develop and provide a more collaborative, coordinated service in an integrated way, for people with complex needs who experience severe and persistent mental illness and reqire spport from mltiple agencies. and general food handling/food safety reqirements. The evalation shows sbstantially improved confidence for the majority of participants in their skills arond sch kitchen tasks as knife handling, preparing basic cooked meals and general kitchen safety. In March we were informed that or bid to contine to provide commnity mental health services had been nsccessfl, and that nder the mental health reform process new providers wold commence service delivery from 1 Agst The BCH APROTCH staff response to this otcome was an incredible credit to them, and shold be acknowledged. Faced with loss of their employment their immediate concern was for clients and they devoted their efforts to ensring the best possible transition for clients to the new service system. Significant resorces over the last few months have been devoted to commnicating with clients, carers and other commnity agencies in planning for the transition to the new system. Their focs and attention to client needs was evidenced right throgh ntil they finished work on 30 Jly Alcohol & Other Drg Treatment and Spport APROTCH In Febrary APROTCH (Adlt Psychiatric Rehabilitation of the Central Highlands) received special mention dring or external accreditation review for the extremely high qality of service provided by the BCH Mental Health team, and they were one of the first Victorian Services to gain accreditation against the new National Mental Health Standards. Preparation for the recommissioning of Victorian commnity based Mental Health Services began in the early part of the financial year. In spite of the ncertain circmstances staff contined to focs on the development of innovative new programs to spport the recovery jorney of people experiencing serios mental illness with excellent consmer/ carer feedback and some significant positive otcomes in edcational and employment opportnities. One of the programs ndertaken by the Mental Health Team for its client base was the Back to Basics class designed to assist clients with living skills, particlarly arond cooking The BCH Alcohol and Other Drg (AOD) Team provides a range of treatment and spport services for individals and families who are facing alcohol and drg related isses. The services are also designed to meet the needs of individals, families and significant others. Key services within this program inclde: Conselling Yoth Otreach (11-25 years of age) Mobile Drg Safety Withdrawal Needle and Syringe Program (NSP) Health Promotion Highlights of this financial year inclde: Recommissioning of AOD services the Grampians Consortim was formed between BCH (lead agency), Grampians Commnity Health, Hepbrn Health Service, Djerriwarrh Health Service and YSAS. Extensive consltations were condcted with staff, consmers and commnity of interest in preparation for or sccessfl stage one tender application. Commnity Rehabilitation Program with spport from the staff sponsorship grant the AOD Team commenced a project to explore the feasibility of a Commnity 16 Ballarat Commnity Health Annal Report and Qality of Care Report 2014

18 Rehabilitation Program at BCH. Crrently clients from this region who wold benefit from a rehabilitation program are restricted to residential programs located in other parts of the state. They can experience lengthy wait times for admission, isolation from family and spports de to distance, financial strain de to program costs, and lack of opportnity to link sccessflly with local spports. The project commenced in March 2014 with consmer consltation and research in the development of the model. Ftre partnerships and fnding avenes are now being explored. In-hose NSP Training Harm Redction Victoria provided training for BCH staff and consmers in relation to the Needle and Syringe Exchange Program (NSP) to redce risks associated with injecting drg se. International Overdose Awareness Day was highlighted by staff and clients with a walk arond Lake Wendoree, and importantly the distribtion of resorces to increase overdose awareness in the commnity. Of all opioid related deaths in Astralia between , medical prescription opioids contribted to 71.1% while illicit opioids were involved in 28.9% of cases. (National Coronial Information System 07-09) Workforce development we have contined to place strong emphasis on professional development and training leading p to the recommissioning of AOD adlt services. Training opportnities for staff have inclded: trama informed practice, resoltion of tramatic stress, consmer participation training, pharmacetical misse, harm redction, GLBTI, change management, and working with clients with amphetamine type isses (Cert IV Unit). Partnerships contining relationships with the broader welfare sector throgh representation on Grampians Integrated Family Violence Committee, Family Services Alliance, Yoth Sbstance Abse Service, High Risk Yoth Panel, Yoth Dal Diagnosis Network and CYMHS Redesign Demonstration Project Practitioner Grop and Headspace SSMART ASSK (Srviving Sbstance Misse and Alcohol Risk Taking - Alcohol and Sbstance Srvival Knowledge) this alcohol and drg awareness edcation program is designed to provide Year 9 stdents with information and skills to identify risks and minimise alcohol and drg related harm in a party environment. The program is collaboratively facilitated by Ballarat Commnity Health, Victoria Police and local health & welfare organisations. In , twelve secondary schools participated in the program, with approximately 1600 stdents attending. As part of the ongoing and long term evalation of the program, Federation University Astralia have been engaged to research the impact of the SSMART ASSK alcohol and other drgs health promotion program on the knowledge, attitdes and behaviors of secondary school stdents. I sed to go to Ballarat Commnity Health in Str t St when I was in the depths of despair and immediately I had access to the most wonderfl drg and alcohol consellors who treated me with total non-jdgement and respect. Even if I had to wait for treatment, I had access to being able to talk to in person or on the phone ntil I got that treatment. At times when I have been desperate and in a non-coherent state, all I wanted was to see someone in person and that is why I went to Commnity Health. 19 PROGRAMS & SERVICES Conselling Services The conselling program provides professional conselling services to individals, coples and families. Often clients attend conselling to deal with a range of concerns inclding relationship isses, stress, trama, depression, anxiety and self-esteem. Refgees and other newly arrived migrants also access the service to work throgh a range of crrent and past isses that may be impacting on their efforts to adjst to their new sitation. Key services inclde individal conselling, relationship conselling, advocacy and conselling with refgees and new migrants, grop work and commnity edcation. Family Violence Program The Family Violence Conselling program provides professional conselling services for women who have experienced abse in their intimate or family relationships. This is achieved by fostering professional links with family violence and related services. The Family Violence Program offers: Individal conselling Grop work Single session work Secondary consltation Commnity edcation The program this year assisted women from a range of age grops, diverse cltres, and disabilities. There has been an increase in self-referrals, bt agency referrals contine to be prominent. The demand for this service exceeds its capacity. To address demand for spport with family violence clients and to ensre a timely and appropriate response to the large nmber of referrals, a priority will be given to: self-referrals, to enable a comprehensive safety assessment clients where the referring agency has recommended priority single session work This year has also seen BCH participate in a working grop to develop police protocols for responding to Family Violence in the region. BCH contines to have an active role in the Grampians Integrated Family Violence Committee which aims to work collaboratively to address broader regional isses. International Family Violence Conference Dring the year all staff participated in ongoing professional development relevant to their roles. The team has also been bsy working with and responding to: clients wanting to address isses to do with self-esteem throgh a grop program known as Branching Ot. This eight-week program assists people to develop a stronger sense of self and to make positive, meaningfl changes in their lives. In addition to or ongoing responses to family violence, the conselling team has been able to explore the possibility of presenting an International Conference on Family Violence. This exciting event has been made possible throgh the Staff Sponsorship Program and throgh the generos spport of the Timbek Family Trst. We have sccessflly contracted Dr Steven Stosny and Dr Allan Wade, from North America and Canada respectively, along with a nmber of nationally renowned speakers on this very important topic. 20 Opportnities for expanded services The conselling team has received several reqests in the past 12 months to provide Employee Assistance Programs (EAP) for other organisations or companies. In response to this we are developing a range of packages to present to companies. We are also working closely with the Healthy Lifestyle team to inclde opportnities to provide Work Health Checks for any company that enters into an EAP contract with s. Where sccessfl this will provide an invalable opportnity to promote Ballarat Commnity Health to an even broader sector of the commnity. Pharmacotherapy Network In December 2013 the Victorian Department of Health selected Ballarat Commnity Health to lead a clinical network which will improve access to pharmacotherapy services. Pharmacotherapy is an effective medical treatment for people who are dependent on opioid drgs sch as heroin, Oxycontin and Fentanyl. Opioid dependence is a complex health condition that affects people from all walks of life not only injecting drg sers bt also people who become addicted to pain relievers throgh a range of different circmstances. Long-term pharmacotherapy treatment has been proven to benefit both the patient and the commnity, throgh redcing crime, preventing the spread of blood-borne diseases, redcing early death, illness and the brden on the health system. The Network is a partnership of Commnity Health Services and Medicare Locals which covers a large geographic area from Bacchs Marsh throgh to the Wimmera, from Mildra to Bendigo and the Macedon Ranges. In or early establishment phase we spent time bilding extensive relationships across or area, compiled or base-line dataset and designed a strategic action plan to gide or work. Since then we have: Spported nmeros regional doctors and pharmacists to access pharmacotherapy training in Ballarat and Bendigo. A nmber of doctors have since gone on to treat pharmacotherapy patients Developed a draft gide for the se of MBS items to better spport the care of drg-dependent patients with complex needs Developed or own visal branding, website and e-news to be lanched in September 2014 In the coming months we look forward to introdcing addiction specialist services to or catchment area. These specialists will provide primary treatment, secondary consltation and GP edcation. We will also set p a nmber of local special interest networks to spport working relationships, bild referral pathways and offer professional development opportnities. This involves: Celebrating or diverse commnity Forming strong partnerships with other organisations and agencies Working with others to identify, nderstand and address commnity needs Training and developing capacity of others to promote health and deliver care Improving access to services and targeting gaps Providing holistic care across mltiple services Empowering individals to manage their own health and be involved in decisions abot their care Spporting carers and families of people strggling with their health Providing commnity-friendly spaces Celebrating or Diverse Commnity To provide qality healthcare, BCH needs to nderstand and cater for the diverse needs of those in or commnity. BCH has a Diversity Plan which is a reqirement of or HACC (Home and Commnity Care) fnding throgh the Department of Health. The plan otlines specific priorities for improving access to services for those in or commnity, inclding people with dementia and their families, indigenos clients, people from cltrally and lingistically diverse (CALD) backgronds, financially disadvantaged individals, rral and remote commnities, and people who identify as GLBTI. GLBTI Commnity Diversity training for BCH staff incldes specific training to promote awareness and to help create a lesbian, gay, bisexal, transgender and intersex inclsive service. This objective is spported with rainbow posters arond sites and gay & lesbian welcoming posters from Gay and Lesbian Health Victoria. CALD Commnity The indction program for new staff at BCH incldes information on cltral awareness and access isses. The availability of interpreters is actively promoted with posters in place at all sites to spport CALD clients in accessing services. The client management software in se at BCH has been developed to enable data on the contry of birth and preferred langage of existing clients to be extracted for reporting to help provide a clearer pictre of the CALD needs of or clients. BCH contines to actively spport and promote mlticltral activities and initiatives within the commnity, with representation in a nmber of grops sch as the Ballarat Regional Mlticltral Concil (BRMC) Cltrally Inclsive Aged Care Committee (CIACC). Members from the BRMC visited or Sebastopol site to become more familiar with the organisation and what we have to offer. BCH was also involved in the preparation for a sccessfl week of events to mark Refgee Week BCH has commenced work towards achieving The Rainbow Tick to become a GLBTI accredited organisation. 22 Indigenos Commnity Ballarat Commnity Health s partnership with the Ballarat And District Aboriginal Co-operative (BADAC) contines to deliver services sch as dietary conselling throgh BADAC. BCH staff have also provided come and try opportnities for volnteers at BADAC to become familiar with BCH services, and BCH participated in a BADAC Open Day for the commnity. Several sggestions have been pt forward by staff and implemented to address the need for greater connection and awareness with the indigenos commnity. These have inclded revisions to BCH policies to reflect the importance of acknowledging indigenos land on which BCH sites are located, and the inclsion of a standard acknowledgement on staff templates. Frther work has been ndertaken to enable s to extract and analyse data from or client management software in regard to Aboriginal and Torres Straight Islander stats of clients. Shadowlands Performance International Disability Day Working with clients from Scope and McCallm, local commnity and gest artists, the BCH Commnity Arts Program prodced a stage show focsing on stories told by the clients. The performance project encoraged people to be active in all areas of the prodction: performing, back stage, props and costme design, sond and lighting, msic, front of hose, ticketing and pblicity. Over a twelve week rehearsal period the Shadowlands prodction was developed and advertised to the pblic before the final showing on December 6th and 7th, coinciding with International Disability Day. The event was an exciting opportnity to encorage inclsion and promote opportnities for people with disabilities to be part of the commnity. 23 BCH AND THE COMMUNITY Improving Access to Healthcare BCH has a particlar focs on targeting services to people and grops who are particlarly disadvantaged or who have difficlty accessing the health services they need. There are a broad range of factors that impact on the accessibility of services inclding financial disadvantage, disability, complex needs, fragmented services, waiting lists and lack of services to remote areas. Feet First - Podiatry Screening for People living in Spported Residential Services Residents of SRSs in Ballarat are being offered annal foot screens at the SRS by podiatrists with the valable assistance of an allied health assistant. The screen involves the podiatrist assessing the lower limbs/feet and being able to prioritise the need for podiatry appointment. Any follow p treatment that is reqired is provided at Ballarat Commnity Health or Ballarat District Nrsing & Healthcare with transport being provided from Uniting Care. This service has reslted in a marginalised, high risk grop within the commnity having access to qality podiatry services. Over a 6 month period a total of 287 people had a podiatrist screening of which 143 (50%) were SRS residents. Of the 179 people living in the six SRSs, 147 (80%) have had an annal podiatry screening. Managing Waiting Lists Clients go throgh the BCH intake system to access allied health services (within the Health Management team). Throgh intake a client is given a priority rating depending on their isses and those given a high priority will be seen sooner. The podiatrists have screening appointments so all clients are screened and the podiatrist rates their level of priority. Bilding Commnity Capacity BCH cannot achieve or mission and vision in isolation. We have contined to focs on bilding strong partnerships with other organisations and agencies and to work with others to identify, nderstand and address commnity needs. BCH is also committed to training and developing the capacity of others to promote health and deliver qality care. Conference Presentations BCH staff delivered five presentations at the inagral Barwon-Soth Western and Grampian Regions Allied Health Conference Better Placed a collaborative approach to clinical learning, described how BCH is implementing the Best Practice in Clinical Learning Environments (BPCLE) tool to assess BCH s crrent capacity and capabilities in the provision of stdent placements and how we are prioritising areas for improvement so as to provide better stdent placements for both staff and stdents. Trama informed practice and yoth homelessness provided recommendations to spport the effective se of trama informed practice in the yoth hosing and homelessness sector at the systemic, agency and individal program and worker levels. A clinical adit of podiatry infection control procedres and post-operative infection rates following nail srgery showed excellent reslts for BCH in this area with zero post-operative infection rates and the importance of contining to monitor throgh adits. Implementation of best practice podiatry nerovasclar assessment for clients with diabetes compared evidencebased best practice with crrent BCH podiatry practices which identified areas being done well and some other practices that cold be improved. Management of Clients with Chronic Disease: a best practice implementation project highlighted how an evidence-based clinical adit of client files, along with involving health professional staff in the discssions of findings of the adit, lead to improvement in practice. Ftre work in regard to waiting lists will involve exploring the potential of small grops for initial appointments with a diabetes edcator and other ways in which more clients with diabetes can be edcated and spported. 22 Ballarat Commnity Health Annal Report and Qality of Care Report 2014

24 Responding to the needs of the Ballarat Commnity Dementia It is estimated in 2010 there are almost 1,300 people with dementia living in Ballarat and this is projected to rise to over 5,200 people by This represents over a 300% increase between 2010 and To manage this increasing nmber of people with dementia, Ballarat Commnity Health focsed on staff training and ensring people with dementia and their families have access to valable information sch as Grey Matters A gide for living well with Memory Loss and Dementia in the Grampians. Staff from the Health Management and Conselling team participated in a workshop rn by Alzheimers Astralia, Victoria Branch which inclded: identifying the main cases, risk factors and symptoms of dementia implementing effective strategies for commnicating with people living with dementia all reported that there was little information available to them abot gambling and problem gambling and few schools address it in their crriclm. If teenagers had a problem with gambling, the majority indicated that they wold most likely seek help from a family member first followed by the school conselor. These findings have been disseminated to local schools and organisations to inform edcation programs abot gambling and problem gambling delivered to yong people in Ballarat. Food access and availability The most recent data show 7.3% of Ballarat residents experienced food insecrity over the past 12 month period. This compares to 5.6% for Victoria. To assist Ballarat Commnity Health and City of Ballarat to better nderstand the reasons for this high level of food insecrity we reqired frther information. As part of their stdent placement for dietetic stdents from Monash University ndertook a stdy to map locations of food otlets and completed the Victorian Healthy Food Basket Srveys on 25 otlets to gain information on the affordability of a ntritionally adeqate diet. nderstanding the changes in behavior in the person living with dementia sing the person centred care approach to improve the qality of life for the person living with dementia sing the Problem Solving Approach to spporting people living with dementia. Staff also have access to e-learning dementia training and this is now part of orientation of new staff working with HACC clients. Online gambling in adolescents The research project titled Investigation of the incidence of online gambling in adolescents in Ballarat, and attitdes to and awareness of problem gambling in adolescents was ndertaken by the Healthy Lifestyle Team and fnded by Central Highlands Primary Care Partnership (CHPCP). It involved srveying teenagers in local secondary schools and their parents and teachers. The findings showed that very few teenagers had gambled online bt jst over half had participated in some form of gambling. Althogh most parents, teachers and stdents were aware of the risks of gambling, the majority did not consider gambling to be as serios as other risky teen behaviors sch as drinking and driving, alcohol and drg se, bllying and nsafe sexal activity etc. Teachers, parents and teens Key findings from the mapping and the Food Basket Srveys inclded: More takeaway food otlets than fresh food otlets Higher density of takeaway otlets in Ballarat Central Frit and Vegetables more expensive in spermarkets than green grocers Food Basket srvey showed foods being more expensive in some low socio-economic areas sch as Wendoree and Sebastapol. Into the ftre BCH will work with City of Ballarat and other agencies to gain a better nderstanding of food insecrity in Ballarat, what is crrently in place and what evidence based-practices cold be pt in place to address this isse. 25 BCH AND THE COMMUNITY Empowering the Commnity One of the most significant things we can do to create a healthy commnity is to empower individals to nderstand and manage their own health, to be actively involved in decisions abot their healthcare, and to make healthy lifestyle changes. As is the focs across all BCH programs, the work of the Health Management Team is bilt arond the philosophy of promoting self management and client-centred care, with clients directing clinicians as to what they feel is important to them. Care planning has been introdced across the team to spport client centred care and to encorage the engagement of clients in the management of their condition. BCH is also committed to spporting carers and families. We acknowledge the important and vital work of carers in or commnity. The Carer Relationship Principles otlined in the Carers Recognition Act 2012 are sed as a benchmark in providing services to clients and to those who care for them. Fragmented services can have a significant negative impact on health in the commnity. BCH offers a broad range of services from health promotion and edcation throgh to treatment, rehabilitation and chronic illness management. This allows s to provide a holistic approach by catering for all aspects of a client s health and providing smooth and easy referrals throgh the range of services and programs delivered at BCH. Donna Donna, age 51, was referred to the withdrawal program by her GP de to concern abot her prescribed medication doses. She was an overweight single mother with two children. The oldest child was living independently and the yongest, who lived with Donna, was still at school. Donna had sffered past childhood trama isses as well as a recent loss of a family member. The withdrawal nrse and the GP have worked together to develop a spported commnity redction plan which involved changing and stepping down her medication. Plans to provide additional spport by admissions to detox at staggered redction times were scrapped as Donna fond this was too difficlt. Donna s redction was complicated by chronic pain and mental health isses. As a reslt, constant setbacks needed to be managed and overcome. This involved service coordination, case management planning and conferencing facilitated by the withdrawal nrse. Two years on, Donna s medication is stabilised and she has reported improvements in her mood and mental health. She has lost a significant amont of weight and her self esteem has improved. She is more otgoing and beginning to socialise and show healthier family relationships. Alison and Sam Alison came to conselling becase she felt depressed and worried abot her relationship with Sam. After a cople of individal conselling sessions, Alison and Sam attended relationship conselling to work on some concerns they had and to improve their relationship. After several weeks of relationship conselling they reported feeling more connected, more confident abot their relationship into the ftre and less stressed. In addition to the conselling, both Alison and Sam have contacted or exercise physiologist for an assessment to attend programs to regain some fitness and to manage chronic pain isses. Alison has also been attending podiatry, seeing a dietician and is crrently attending or Branching Ot grop to improve self-esteem. 26 quality & safety QIP Accreditation In early 2014 BCH participated in the external accreditation process throgh QIP (Qality Improvement Performance) and is pleased to confirm that we met all of the formal standards applicable to BCH services. BCH is very prod to continally and consistently demonstrate or ongoing commitment to the provision of qality services in a safe environment. In addition the organisation was jdged to have exceeded the reqirements for the standard relating to Planning and Assessment - an otcome illstrative of the organisation s ability to effectively develop programs and services and to reglarly consider the views of participants, sing the information to continosly improve those services. Following the formal report, we are already identifying relevant areas for frther improvement, and will be developing a detailed three year plan. Clinical Governance In 2013/2014, Ballarat Commnity Health developed a process for ensring that the programs and services we condct are of the highest qality. This is to ensre that new programs: Are a response to a need identified in the commnity Are based on the latest research evidence Have a program plan developed. Program content and intended evalation that mst be all approved by management prior to commencement Are evalated reglarly Are modified in response to the findings from the evalation to ensre the highest qality program is offered on an ongoing basis Program development and evalation process Program Plan developed & / or modified Program Content A schedle of clinical adits has been developed for implementation in the coming year. This schedle will maintain a rolling smmary of adits as they are planned and then completed and will otline the objectives, timelines, otcomes and recommendations for reporting to Managers and the Board. Clinical Adit: Implementation of best practice podiatry nerovasclar assessment for clients with diabetes Podiatrists at Ballarat Commnity Health (BCH) treat a significant nmber of patients with diabetes. In order to prevent foot complications in diabetes patients, a detailed nerovasclar assessment is reqired. The aim of this adit was to determine whether the podiatrists at BCH were meeting best practice standards for assessments and whether they were consistent in the assessment process. The reslts indicated high compliance with the adit criteria for both nerological and vasclar assessment. Podiatrists at BCH considered this to be a valable qality improvement activity that allowed them to focs on the qality of their assessment and practice, particlarly in relation to foot complications in patients with diabetes. Ongoing annal clinical adit is planned to ensre that best practice standards are maintained. Risk Management BCH has maintained its commitment to effective risk management organisation-wide. Dring the last year the organisation has: provided targeted and specific training to each operational team; enhanced the involvement of the Board of Directors in risk assessment throgh the development of measrable Risk Tolerance Indicators and a formal Statement of Risk Appetite; bilt pon the existing Bsiness Continity Framework to encompass crrent developments in pandemic planning and bsiness impact analysis; and contined to thoroghly review and enhance the content of the organisational Risk Register. This work has enabled BCH to better nderstand the risk environment in which it operates inclding the financial, reptational, environmental and compliance risks that it faces and to develop appropriate mitigation and response strategies. Legislative Compliance Ballarat Commnity Health has identified legislative and Need for a reglatory compliance as a key element of its corporate new program governance response. In order to respond appropriately identified and efficiently, a new BCH Legislative Compliance Register Evalation Program completed Evalation Plan is being developed to encompass all reporting obligations associated with a health agency inclding those linked to important fnding acqittal reqirements. Several Directors of the BCH Board have ndertaken specific training with Program the Astralian Institte of Company Directors in corporate condcted governance, to increase their knowledge of the obligations associated with their roles. 27 quality & safety Client Feedback Feedback from the commnity and or clients is essential to improving the qality of the services we deliver and ensring that or service delivery stays relevant and aligned with the needs of the commnity. BCH receives feedback throgh a variety of avenes. We condct a range of evalation and consltation activities and we also collect and record incidental feedback and inpt throgh or Compliments and Complaints process and Client Feedback Policy. Feedback and sggestions are encoraged and Client Feedback forms are available at all sites for this prpose. Formal complaints are registered and responded to by the CEO or relevant Team Leader. All BCH clinical teams condct reglar evalation and adit programs of their services in order to ascertain their effectiveness and find means for improving those services. BCH condcts an annal client srvey of all clients who attend its services, sally over a period of one month. Other program-specific client srveys are also condcted reglarly. HACC Client Srvey Reslts A client satisfaction srvey of individals accessing HACC services (services delivered throgh the Department of Health s Home and Commnity Care fnding) is ndertaken each year. Fifty for srveys were retrned from HACC eligible clients. It is estimated that 184 clients attended service appointments over the srvey period which eqates to almost 30% of these clients completing a srvey. The following is a smmary of the reslts in relation to service delivery and qality. 20% of the clients srveyed accessed more than one service at BCH. This highlights the benefits of offering HACC allied health services within a commnity health centre that also provides a range of other allied health services as well as diabetes edcators, general practitioners, drg and alcohol and mental health services. Infection Control Infection control procedres aim to prevent or minimise the spread of infection by limiting the exposre of ssceptible people to micro organisms that may case infection. This is an important area in all healthcare environments and BCH is committed to minimising the risk of infection for clients and staff. The BCH Infection Control Committee has contined to refine and improve infection control processes, and to monitor compliance. The BCH Infection Control Manal addresses: Sharps and needle-stick injry management Blood and body flid spills management Hand hygiene A reglar cleaning schedle describing the freqency of cleaning, prodcts and procedres in clinical and non-clinical areas of or practice Sterilisation processes Waste management inclding the safe storage and disposal of clinical waste Prevention of disease in the workplace Employees of Ballarat Commnity Health are familiarised with the Infection Control Policy as part of their orientation to the organisation. The members of the committee act as a resorce for other staff and provide pdates and information to BCH staff meetings and the OHS Team. Hand hygiene has been a specific area of emphasis with the introdction of an online training modle for staff to complete. 28 OHS and Incident Reporting Dring the corse of BCH has maintained its commitment to workplace safety, ensring the wellbeing of staff, clients and visitors at all times in accordance with legislative obligations and best practice. All identifiable hazards, near misses and incidents are docmented in the Victorian Health Incident Management System. A dedicated Occpational Health and Safety Team then reviews and analyses all sch reports for process and general safety improvements, recommending actions to preserve safety for all. As in previos years, nmbers of incidents involving staff and/or clients has contined to be very low and this is a reflection of the organisation s commitment in this area. BCH has worked to improve client and staff safety throgh the refinement of Safe Work Method Statements, general safety practices and procedres. 29 LUCAS OUR NEW FACILITY After 20 months of consltation, designing and constrction the new Ballarat Commnity Health Lcas facility opened its doors in Jne Fnded jointly by the Commonwealth Government of Astralia, throgh the Health and Hospitals Fnd Regional Priorities Rond 3, and Ballarat Commnity Health, the new $13.6m BCH Lcas facility creates a new approach to delivering qality and affordable healthcare to the broader Ballarat and District commnity. BCH otgrew or existing facilities some time ago. The state of the art facilities and eqipment at Lcas will improve or capacity to deliver qality services, and will spport and extend the existing services available at or Wendoree, Sebastopol and Smythesdale sites. The new facility is home to a fll site of services inclding General Practice, Sexal Health and Podiatry clinics, mlti-fnctional consltation rooms accommodating a fll range of health and wellbeing services, as well as a gymnasim, conference and meeting rooms, an independent café and office facilities for over 100 staff. The vision for the new facility was to create a flexible space that caters holistically for the wellbeing of individals and also the commnity. It has been designed as a commnity focsed bilding where people can gather in the central landscaped atrim to access a fll range of services in a comfortable environment. The se of internal gardens, natral light and fresh air promotes a whole of life health approach. It is a place where people can access health services and advice, join the gym, visit the café for a bite to eat or a coffee, browse the internet on the pblic compters, and enjoy the art exhibitions on display in the Lcas Commnity Arts Space. Or Project team led by Jim Rodgers and spported by Melborne based Architects DesignInc, local bilder Nicholson Constrction and Qantity Srveyor PlanCost Astalia, have worked tirelessly to deliver this magnificent new bilding on time and on bdget. Location Located jst six kilometres west of the centre of Ballarat on the Avene of Honor, Lcas is the new master planned sbrb located in the Ballarat West Growth Area. 30 Commnity consltation in the Lcas Project The Lcas site is a commnity facility, and it was important that the commnity was involved in the development of the project. Initially we held two pblic consltation sessions, advertised in The Corier, seeking feedback on the schematic design and frther inpt into the design development. Each design stage was docmented and made available to the commnity on the BCH website where feedback was encoraged throgh the project@bchc.org.a address. We also presented the project to services and client grops inclding the Wendoree Rotary, Parkinson s Spport Grop and the Wendoree Ladies Probs. Finally, we issed press releases at varios stages throghot the project which were covered by The Corier, The Miner, WIN News and the ABC again reqesting feedback to the design proposals. A series of for flyers was distribted locally and via the varios newsletters inclding volnteers and members periodicals. Environmental and Sstainable Design (ESD) Featres The design team was focsed on creating an excellent indoor environment qality in the bilding with a strong commitment to sstainability. Featres inclde: High daylight penetration High efficiency lighting High efficiency doble glazing Natral gas-powered heating & cooling systems 80,000 litre water storage tanks for toilet flshing and irrigation Solar hot water heating 25kw photovoltaic solar energy system Onsite bilding waste recycling Locally sorced recycled timber incorporated into floor, wall, joinery and frnitre finishes Use of low maintenance materials where possible The bilding project inclded over 35,000 hors of labor in the 10 month bild program. The largest amont of workers on site at any one time was 62. There were 21 constrction days lost de to bad weather. 93 % of sb-contractors and sppliers were from Ballarat or Regional Victoria. 31 EXECUTIVE SERVICES The Exective Services Team incldes Hman Resorces, Governance and Qality, Cstomer Service, Volnteers & Stdents and Database Spport. BCH Staff Staff professional development has remained an important focs and we have contined to provide staff with training in a variety of areas sch as GLBTI, Cltral Awareness, First Aid and or annal All Staff Day. BCH has also been implementing a variety of online learning packages for staff and volnteers. This has enabled a larger grop of people to participate in a greater variety of programs, adding vale to staff, volnteers and BCH. BCH s Staff Health and Wellbeing Committee has again been focsed on providing opportnities and the right environment for staff to increase their own personal wellbeing and health otcomes. In collaboration with HR, Team Leaders and Senior Management, the committee has provided the following staff programs and activities in the financial year: Act@Work Initiative International Women s Day Breakfast and Fashion parade action (fnds raised were donated to WRISC) Work, an organisation wide training program, was delivered in collaboration with Women s Health Grampians. Over 80% of staff participated in this program. This new program is designed to edcate employees on how to prevent violence against women and children both at work and in the commnity. Dring this process BCH developed and implemented a policy to specifically spport staff experiencing family violence, and frther progress the work already being done to enhance staff health and wellbeing. Ride to Work challenge Fl vaccinations for approximately 70+ staff On yor Feet Day where staff were sent s every half hor encoraging them to get p from their desk and move to ballet, hip hop and aerobic moves. When I heard that Ballarat Commnity Health was introdcing a policy to sppor t any member of or staff who may be experiencing family violence, I was so prod to work for this organisation. It s fantastic to think that sch a policy exists now, given that not too long ago the isse of family violence was not even okay to mention in most work places. This policy and or workplace s commitment to the Act@ Work project has given me some real hope that we can make a difference to the incidence of family violence and in par ticlar to the families whose lives are so deeply affected by this experience. 32 Cstomer Service Team The year has had a focs on increasing the skills and knowledge of or expanding Cstomer Service Team, to ensre that we contine to provide excellent and consistent service to or commnity. As the initial point of contact for the commnity or staff strive to always be a welcoming face to clients and assist all visitors with any enqiry or isse. BCH Cstomer Service staff see hndreds of clients and visitors a week and have played a significant role in making the move to Lcas a positive and sccessfl one by reglarly informing the commnity abot the new facilities. They have played a pivotal role in assisting clients with transport qestions and concerns and have assisted with ensring that clients have easy access to services following the official closre of Strt Street. The committee contines to formally recognise employees who are positive role models throgh the Employee Recognition Program each month with 12 staff awarded throghot the year. Together with the Healthy Lifestyle Team the committee has also been working towards becoming acknowledged/accredited nder the Workplace Achievement Program. Dring this process BCH has conslted with staff in a nmber of ways to frther nderstand their health and wellbeing needs and the committee is crrently designing the programs for year. BCH has contined to have a strong focs on the importance of the agreed BCH vales and ensring that these are well anticipated, nderstood and displayed by all BCH commnity members. The staff commitment to living these vales has been evident particlarly dring the recent times of ncertainty srronding the Mental Health & Alcohol & Other Drg recommissioning and BCH is very prod of the way the BCH staff have managed the changes with respect, integrity and compassion for the clients. The last six months has seen a hge focs on the implementation of a new payroll and HR system, and we are looking forward to finalising this project in the next few months. With increasing services and program changes the Cstomer Service Team aims to keep abreast of everything that is happening across all sites and we thank them sincerely for their spport for clients and commnity members. Information Technology BCH Client As the position of TCM Spport Officer ( The Care Manager - BCH s client management software) remained vacant for some months staff have relied on the spport of each other and their managers to maintain accrate records and prodce timely reports for the relevant fnding bodies. Dring this time BCH staff also participated in a significant review of the crrent IT environment and assisted external consltants to prepare a thorogh and informative report on or existing arrangements and proposals for an IT Strategic Plan. The staff played a significant role in providing vital information and BCH now has a new and detailed plan to work with over the next 3-5 years. 33 EXECUTIVE SERVICES BCH Volnteers The Volnteer and Stdent program has grown significantly in the last 12 months since BCH has employed a fll time coordinator to oversee these programs. The dedicated resorce for these areas has enabled BCH to provide a more targeted approach to assisting the volnteers and stdents in their placements and to make sre their experiences are flfilling, satisfying and achieve great otcomes for themselves and others. BCH now has 55 active volnteers, which is an increase of almost 50%. These volnteers have participated in the newly designed and implemented orientation and training program which has been rn a nmber of times in the last 12 months. The opportnity that is provided for them to meet with other volnteers, staff and management, together with the pdated handbook, has enabled new volnteers to integrate more effectively into their role at BCH. 34 John Harrison - Volnteer A volnteer for 18 years at or APROTCH site, John has assisted with grop activities inclding the gardening grop, woodwork, frnitre restoration grops and a cooking grop. He passes on his knowledge and expertise to assist people with a serios mental illness to gain positive otcomes. John displays an empathy and nderstanding for people with a major mental illness and has a passion for learning and passing on his knowledge. Having a mentor is a valable asset to a person s recovery. John was instrmental in setting p and now rns a Bi-polar spport grop on a monthly basis. He is a member of the Consmer Only grop that provides a form for clients to be heard by management as well as an active member of the APROTCH Representative Committee (ARC). ARC provides a partnership and working relationship between consmers, carers, Board of Directors, staff and volnteers to ensre that the services and programs remain responsive to the needs of the consmer or are developed to express crrent consmer needs, wants and desires. Stdent Placements Stdent placements at BCH have increased both in nmbers and qality. BCH has recently completed the initial stages of the Best Practice in Clinical Learning Environments (BPCLE) tool, being one of the first agencies to significantly progress throgh the program. We have commenced the implementation of the agreed objectives and are crrently doing some terrific work with staff with regard to their professional development and increased capacity to spervise stdents. De to or contined work in this area BCH presented at the Allied Health conference in November 2013 on the sccess of the implementation of the BPCLE tool. BCH was also a finalist in the recent 2014 People in Health awards for or commitment to improved promotion, spport and resorces for stdent placements in Expanded Settings sch as Commnity Health organisations. John has assisted so many people on their personal jorney that it is difficlt to calclate the impact of his work, except to say that people who have met John or worked alongside him appreciate what he does and the help he gives. When asked what his experience as a volnteer has taght him, John says, Each of s is a store of knowledge, and love for other people which we do not own bt are given to share. Dring , BCH has worked with 42 stdents for a total of 266 days. I have thoroghly enjoyed my placement at Ballarat Commnity Health. It has been sch an invalable experience, and I have learnt so mch dring this placement. It has been a pleasre being able to work with sch a range of people in different professions... 36 FINANCIAL SERVICES The Finance Team has contined to maintain robst financial internal control throgh a very bsy financial year. Two sccessfl internal adits were ndertaken in the areas of Employment Taxes & Fringe Benefits Tax and Fleet Management, with no isses of concern identified. Finance staff have also been heavily involved in investigation and implementation of a new HR & Payroll system. The move to the new Lcas site reqired a significant amont of preparation for the transition of IT systems, payroll and financial service delivery. The team has commenced work on setting p the Lcas Asset Register in line with Commonwealth reqirements. 38 Corporate Governance defines the prescribed framework, systems and processes for financial accontability, risk management and social responsibility. BCH is governed by a Board of Directors drawn from the membership of BCH, who are elected for three-year terms with three positions becoming de for election each year. The Board does have the capacity to appoint Directors from time to time, particlarly where there is need to recrit for specificskills sets. All voting members of BCH not otherwise exclded by law are eligible to nominate for election to the Board of Directors. BCH operates nder a Constittion, adopted by members. As a living docment the Constittion is reviewed on a reglar basis to ensre progress is being made to meet targets and that it maintains its viability. BCH also has in place a governance framework and policies to gide the work of the organisation. 39 Celebrating past achievements Looking forward to new directions The end of the Financial Year marks a pivotal point for Ballarat Commnity Health. It has been an eventfl year and it is an opportnity to reflect on or past achievements with pride and look ahead to exciting new developments. BCH services have been impacted in several areas by changes to fnding arrangements inclding mental health services and alcohol and other drg services. This, along with the emergence of new partnerships and fnding opportnities, has reqired innovation and focs on the ways in which we can meet the continally changing needs of or clients and the commnity. APROTCH holds a very significant and special place in the story of this organisation. The service reflects the mission and vision of BCH as clients, volnteers, staff and the commnity work together with a holistic approach to wellbeing, creating a niqe sense of ownership and belonging. That commnity spirit has been particlarly evident dring the preparation to transition to a new mental health service provider in Agst There is sadness and ncertainty bt also a sense of celebration and pride in everything that has been achieved over the years. The Alcohol and Other Drg Services Team at BCH has assisted individals and families who are dealing with alcohol or other drg related isses for many years. New fnding arrangements will change the way we deliver services in this area bt or commitment to spporting people to receive the services they need remains the same. The closre of or Strt St and Grevillea Road sites also represents the end of an era. BCH otgrew or existing facilities long ago and the beatifl Victorian bilding on Strt St has strggled to hose the necessary services and eqipment. The constrction of or new facility in Lcas opens so many opportnities for new programs and commnity participation. BCH Lcas will allow s to spport and expand the services available at or Wendoree, Sebastopol and Smythesdale sites. As the new financial year gets nderway we have celebrated the official opening of the new facility and the lanch of the Lcas Commnity Arts Space. The commnity is taking fll advantage of the conference and meeting room facilities, and or new gym. New programs and partnerships are anticipated across all sites and the new financial year will also see the commencement of a significant refrbishment of or Sebastopol site.
