1 POSITION DESCRIPTION Title: Social Support Worker Business Unit: Uniting John Macrae Centre Location: 603 Toorak Road Toorak 3142 Employment Type: Part Time (46 hours per fortnight) Reports to: Centre Manager ABOUT UNITING Uniting works alongside people of all ages in local communities in Victoria and Tasmania. Our services reach to Albury-Wodonga in the north, Mallacoota in East Gippsland, the Wimmera region in the west, and across Tasmania. Uniting is the services and advocacy arm of the Uniting Church. We ve been supporting people and families for over 100 years. We are 7000 skilled, passionate and creative people providing over 770 programs and services. We empower children, young people and families to learn and thrive. We re there for people experiencing homelessness, drug and alcohol addiction or mental illness. We support people with disability to live the life they choose. We assist older people to maintain their independence and enjoy life. We provide opportunities to access training and meaningful employment. We re proud to welcome and support asylum seekers to our community. 2 POSITION PURPOSE The role supports a socially inclusive approach to active and healthy ageing through the delivery of effective assessment and support which enhances the person s quality of life and wellbeing within the community. Each person s capabilities are developed to enable independence in daily living and be able to participate in individual and/or group activities. SCOPE Budget: People: nil nil Relationships: Internal: All aged care staff All support and foundation staff All volunteers and students External: Clients Government funding bodies Families, carers and advocates KEY RESPONSIBILITY AREAS Service delivery To assist in the preparation of the day s activities, following the program plan and staff duty roster To facilitate activities both independently and in co-operation and in consultation with other staff To facilitate conversation and positive interaction between participants within the members. To be familiar with member s history, care plans and goals. To be aware of the physical, psychological, social, emotional, cultural, religious needs of members before providing care and documenting any changes. Complete Member Care Plans and review care plans yearly or when required in consultation with members, their carers, staff and Manager Page 2 of 6

3 To encourage independence and participation in activities within member s goals and needs To encourage member involvement in the program and document any changes accordingly. Transport- As per Staff duty roster Rotate between Jockey and driver -To safely transfer members to and from their residence and into the John Macrae Centre, Complete Bus log and fill with petrol on a weekly basis. To support and assist in the personal care needs of members including assisting with meals, transfers, toileting, mobility, hygiene and continence aids. To assist with preparing and/or serving morning or after tea and assist with serving and clearing lunch, and from time to time assist with preparing lunch Cleaning duties as per duty roster- and general tidy up as required To perform other work related tasks as requested by the Program Director consistent with the needs of the John Macrae Centre. To cover extra shifts when staff are absent or on holidays Leadership and Teamwork Maintain enthusiasm and understand own role in achieving organisational mission Openly share information, participate and contribute to team discussions Consider the views of others and aim for group cohesion Communication Interact with members (clients) who may have limited verbal communication skills Demonstrate appropriate interpersonal skills Quality and risk To ensure safety of members and self while participating in the program To report to the Manager any incidences, challenging behavioural problems, or changes that need to be identified and documented. Personal accountability Compliance with Uniting s values, code of conduct, policies and procedures and relevant government legislation and standards where relevant. Ensure appropriate use of resources. Work collaboratively with Uniting (Victoria Tasmania) employees, founding agency employees and external stakeholders in accordance with Uniting s values and professional standards of behavior... 4 Actively participate in initiatives to maintain, build upon and promote a positive and collaborative workplace. Identify opportunities to integrate and work collaboratively across teams. Take reasonable care for your own health and safety, and health and safety of others (to the extent required). Promote a positive safety culture by contributing to health and safety consultation and communication. Promptly respond to and report health and safety hazards, incidents and near misses to General Manager People and Culture. Attend mandatory training sessions (i.e. equal employment opportunity, health and safety) and mandatory training specific to position. PERFORMANCE INDICATORS Performance monitoring will be undertaken every twelve months. Criteria for appraisal will be based on this Job Description and the policies of the John Macrae Centre. This Job Description will be reviewed annually or as necessary. PERSON SPECIFICATION Qualifications/Licenses Essential: Certificate 3 in HACC and/or Aged Care or equivalent Desirable: Certificate 4 in HACC and/or Aged Care or equivalent Essential: First Aid Level 2, CPR & Safe Food Handling Essential: Assistance with Medication module. Essential: Current valid driver s license Desirable: Cert IV Lifestyle and Health CHC4341 Desirable: Essentials in Dementia CHCAGE005 Experience Page 4 of 6
