3 1.0 ABOUT THE AREA Our hospitals are located at the heart of Central Lancashire and serve the three major boroughs of Preston, Chorley and South Ribble. Preston is the administrative capital of Lancashire set against a backdrop of countryside rich in heritage. The town has also become the retail, commercial and cultural centre of Lancashire and exhibits a buoyant and vibrant town centre. Preston s excellent shopping facilities attract people from across the North West. Close at hand, however, lies impressive countryside such as the verdant Ribble Valley and the upland moors of the dramatic and beautiful Forest of Bowland and Beacon Fell Country Park. The Preston Guild Hall presents an extensive programme of events in one of the finest entertainment centres in the country. Throughout the year, there is something for everyone from orchestral to rock concerts and televised sports and theatrical productions of every kind in the Charter Theatre. The redeveloped dock (Riversway) is popular; there is a UCI cinema, restaurants, marina and boats. It is also the site for the annual Maritime Festival. Chorley has been a market town since 1498 and positively bustles with all kinds of market activities. Chorley s history is as deep as England itself and this is no more evident anywhere than in the area s stately homes, parks and gardens. Almost within walking distance of the town centre is Astley Hall, a Tudor mansion set in one hundred acres of parkland. Situated in the picturesque heart of rural Lancashire, Chorley has wide open moorland, parks and woodland making it ideal for enjoying the great outdoors. The 1,900ft high Rivington Pike and Lever Park provide easily accessible outdoor leisure activities. South Ribble provides an attractive living and working environment. It has excellent communications and access, well established local businesses and a widely spread catchment area for recruiting people with the relevant skills. The borough of South Ribble lies between the valley of the River Ribble and the edge of the West Pennines. It contains the main urban communities of Walton-le- Dale, Bamber Bridge, Lostock Hall, Leyland and Penwortham. Away from the main urban concentrations, there are other settlements such as Longton, new Longton, Walmer Bridge, Much Hoole, Hutton and Higher Walton which have expanded to accommodate dormitory development around their historical village origins. Several parishes contain small villages. The rural landscape, providing mixed farming, a variety of market gardens and open countryside is an attractive backdrop to the unobtrusive urban scene. The rich heritage of South Ribble is evident in the many ancient churches and carefully preserved historic building in the villages and settlement across the borough. 2.0 OVERVIEW OF THE TRUST Lancashire Teaching Hospitals NHS Foundation Trust was established on 1 April 2005 as a public benefit corporation authorised under the Health and Social Care (Community Health and Standards) Act We are registered with the Care Quality Commission without conditions to provide the following regulated activities: Diagnostic and screening services Maternity and midwifery services Surgical procedures Termination of pregnancies Treatment of disease, disorder or injury Management of supply of blood and blood-derived products Assessment or medical treatment for persons detained under the Mental Health Act 1983 As the regional Hospital, we serve a local population of around 390,000 and provide a number of specialised services to around 1.5 million people across Lancashire and South Cumbria. Most of our clinical services are provided on our two hospital sites Chorley and South Ribble Hospital and Royal 3 P a g e

4 Preston Hospital. We also have a specialist mobility and rehabilitation service in Preston, the Broadoaks child development centre in Leyland and we provide dialysis units in various locations in Lancashire. We provide the following general hospital services to our local population: 24-hour emergency department facilities Intensive, high dependency and coronary care units General medicine, including elderly care General surgery and urology Child health Ear, nose and throat surgery Orthopaedics Maternity services Gynaecology Anaesthetics Oral and maxilla-facial surgery Ophthalmology Support services for diagnosis and treatment, such as pathology, x-ray, physiotherapy, occupational therapy and specialist nurse Rehabilitation services People in Lancashire and South Cumbria also access the following specialist services: Neurosurgery and Neurology Oncology (radiotherapy and chemotherapy) and complex cancer surgery Renal and plastic surgery Specialist mobility and rehabilitation services Major trauma services In 2013 we became the regional vascular centre for Lancashire and South Cumbria. Elective and emergency vascular surgery is transferring to the vascular centre from University Hospitals Morecambe Bay, Wrightington, Wigan and Leigh and Blackpool Teaching Hospitals on a phased basis, with all surgery for the area being undertaken at the centre by Internally, our clinical services are structured into three divisions: division of medicine division of surgery division of diagnostic and clinical support Each of our divisions is headed by a divisional director, and each has its own divisional medical director and head of nursing. A number of clinical directors, matrons and other managers support the delivery of services. The Trust is host to the regional neurosciences centre for Lancashire and South Cumbria. There have been several major service developments recently. These include accreditation as a major trauma centre and designation as a tertiary vascular centre. The neurosciences centre at Royal Preston Hospital provides neurological services for a population of 1.5 million residents covering the whole of Lancashire and South Cumbria. Tertiary level services are offered to the following hospitals: Burnley General hospital Blackpool Victoria hospital Furness General hospital Blackburn Royal Infirmary Royal Lancaster Infirmary The adult Neurology department is staffed by 12 Consultant neurologists. There are also 2 paediatric neurologists who have links with Central Manchester Children s hospital. 5 P a g e

6 The Neurosurgery department is staffed by 10 Consultant neurosurgeons providing a comprehensive adult neurosurgical services. Two of them provide neurovascular surgery and participate in the neurovascular clinics/mdts There are specialist neurology and neurosurgery trainees from North West Deanery rotating to Royal Preston hospital regularly as part of their training. The post holder will be expected to provide a comprehensive neuro-interventional service, including neuro-diagnostic commitments, and will provide support for the four existing neuro-radiologists, two of whom perform neuro-intervention. In addition, a recently appointed neuro-radiologist is expected to start in post in February 2017 supporting diagnostic/pet imaging. The new post will be expected to participate in a neuro-radiology on call rota with 1:6 frequency. The postholder will: - Adopt, in conjunction with colleagues, an active role in the planning, development and delivery of the neuro-interventional radiology service for the Directorate and associated Network. The successful candidate will be proficient in performing a full range of procedures including angiography, embolisation and stroke intervention. There is also an ongoing commitment to teaching SPRs who rotate through attachments within The Trust. Support diagnostic neuroradiology colleagues in both IP and OP MRI/CT reporting. Also close working with colleagues in the wider neurosciences and radiology department is expected as part of the daily work. Provide support for the local and Network neuroradiology MDTs. Participate in existing 1:6 specialist neuroradiology on call. This is done as a week of on call every 6 weeks. As neurovascular Services develop within the region, there may be an opportunity/ requirement to support stroke intervention, and the successful candidate will be expected to embrace and support these developments. The successful candidate will provide imaging support for research trials ongoing in The Trust, and would be expected to take an active role in audit. The successful applicant will be a Fellow of the Royal College of Radiologists, or equivalent, and should have Royal College accreditation and/or the Certificate of Completion of Specialist Training in Radiology. Assume a continuous commitment for the care of patients, even if employed less than full-time and ensure that adequate arrangements are made for leave and off duty periods. Promote high professional standards of clinical care for patients, both through direct patient care and by the maintenance of continuing professional development. Develop close links with General Practitioners operating in the area. Exercise the freedom to develop specialist elements of the services to reflect perceived priorities within available resources. The job plan is based on the new consultant contract and job planning process. It consists of a basic 10 programme of activities (40 hours) contract comprising 8.0 direct clinical care and 2.0 supporting programmed activities. An indicative model job plan is enclosed. The job plan is a guide, which will be discussed with the Clinical Director and may be amended depending on the departmental needs complexity of case mix. 6 P a g e

7 5.0 THE RADIOLOGY DIRECTORATE The Radiology service is managed within the Clinical Directorate structure. The Radiology Directorate comprises Radiology, Neuroradiology, Ultrasound, Vascular (general) Interventional Radiology, Chest Radiology, Nuclear Medicine, including SPECT scanning and Oncological Radiology. Mammography is performed at the Chorley and South Ribble Hospital site. The Clinical Director (currently Dr Teik Chooi Oh since April 2016) is in post and represents Imaging on the hospital management board. There is a full-time Clinical Business Manager, Mr Russell Dineley and a full-time Specialty Business Manager, Mrs Mags Barrow. The successful candidate will assist in the smooth running of the Directorate and with colleagues in the provision of an effective and efficient service. He/she will be expected to take part in the management of the service and attend meetings of the Diagnostic Imaging Directorate, the Medical Staffing Committee and such other committees he/she may be appointed to by clinical colleagues. The post holder may be expected to attend meetings convened by management, which directly affect the work of his/her department. All Consultants meet monthly for a Directorate meeting and Audit meetings, which are chaired by the Clinical Director and Clinical Governance Lead respectively. The Medical Staffing Committee meets alternate months. The successful candidate will be an employee of the Lancashire Teaching Hospitals NHS Foundation Trust. An image transmission system is used for on-call CT and MRI scans with a departmental laptop machine available for the on-call Radiologist. PACS was implemented across the Trust in May The diagnostic department is well equipped, with a range of modern imaging equipment across two sites. There are four multislice CT scanners. Further facilities include a modern bi-planar neurointerventional suite and two interventional cardiology suites. There are three 1.5T MRI scanners and a specialised extremity MRI scanner. A 3T MRI scanner has been procured and will be installed in A well-developed nuclear medicine department is on site, with two gamma cameras. There is a trust wide GE PACS. PET-CT is provided on site via a partnership with Alliance Medical. NEURORADIOLOGY At present there are 2 diagnostic and 2 interventional neuro-radiologists in the department based at Royal Preston hospital. A third diagnostic Neuro-radiologist has been appointed and is expected to be in post by February The department caters to all aspects of neuro-imaging required in a tertiary neurosciences centre. At present radiology cover is provided for the following MDT s: Neurosurgical meeting every weekday Neurology - weekly Neuro-oncology weekly Low grade glioma 1:4 Pituitary 1:2 Skull base 1:2 Specialist spine 1:2 Head and neck cancer - weekly Paediatric neurology - weekly Neurovascular - weekly There is a dedicated neuro-intervention suite with a Philips Allura FD2010 biplane angiography machine. Approximately 200 interventions (coiling, stenting, embolisation etc.) and 200 angiograms (cerebral and spinal) are performed in the department. 7 P a g e

8 Two new 1.5T MR scans have recently been installed with capability to perform all aspects of advanced neuro-imaging. There will be opportunity for the post holder to develop these advanced techniques and incorporate them into clinical management of selected patients. All Consultants are expected to support the Directorate and Division in managerial roles as required. All Consultants are eligible for consideration of appointment by the Chief Executive to the office of Clinical Director. Cox Weekly 6.0 JOB PLAN/ON CALL REQUIREMENTS The job plan will be based on the new consultant contract and job planning process. It is expected to be a basic 10 programmed activities (40 hours) contract likely to comprise 8.0 direct clinical care and 2.0 supporting programmed activities. 1.5 SPAs are allocated for revalidation related activities with a further 0.5 dependent on additional contribution to the trust based on educational, management or governance roles. An indicative model job plan is enclosed. All consultants are expected to work productively for 42 weeks a year and will be required to undertake annual appraisal and job planning within their allocated SPA time. 10 P a g e

ON-CALL ROTA The post holder will be expected to participate in the Interventional radiology rota 1 in 6. There is currently a general on call rota of 1 in 14 and a registrar on-call rota between 1 in 7. There is also a separate vascular/ir on call rota. The successful applicant will be a Fellow of the Royal College of Radiologists, or equivalent, and should have Royal College accreditation and/or the Certificate of Completion of Specialist Training in Radiology. The trust accepts that the primary means of on-call provision is via teleradiology. The Trust provides a secure laptop with access to PACS web browser. This is a full time post. The contract may be supplemented or amended by local collective agreements reached with trade unions recognised by the Trust and the rules and resolutions of the Trust Board. A consultant is required to reside within a distance of 30 minutes or ten miles by road from their principal place of work unless an employing organisation agrees that they may reside at a greater distance. Should a new substantive consultant wish to claim for relocation expenses they must ensure they request this intention in writing at interview/offer stage otherwise relocation expenses will not be paid. All relocation expenses are in line with the Trust relocation policy. In line with the National Terms and conditions (Consultants (2003) you and your clinical manager are required to agree a prospective Job Plan that sets out your typical schedule of activities, and your main duties and responsibilities. This job plan must be reviewed annually in line with the provisions in Schedule 3 of the Terms and Conditions. All substantive consultants must have Specialist Registration with the GMC and hold a valid license to practice. 8.0 EDUCATION AND TRAINING Modern Education Centres are based at both Royal Preston and Chorley and South Ribble Hospital sites and offer excellent library and study facilities, including PC/Internet support for staff and students. As a teaching hospital, we encourage all our staff to participate in teaching and training at both undergraduate and postgraduate levels. We are committed to developing our teaching and academic activities and can facilitate meeting any training needs for these roles. It is the expectation that every member of the consultant body will undertake teaching of undergraduate medical students, other learners and trainees alongside providing clinical care during DCC activities. We support the requirements for Continuing Medical Education as laid down by the Royal Colleges and are committed to providing time and financial support for these activities. You will receive the full support of the Trust and the Directorate to develop specialist areas of interest in line with Directorate business plans and the development of services generally within the Trust. All newly appointed substantive consultants are enrolled on the award-winning Consultant Leadership Programme and as part of that, mentorship is available. The Consultant will be expected to assume an important teaching role within the Directorate. Clinical Audit is organised on a rolling monthly basis when time is committed to the presentation and discussion of the Medical Imaging Audit. All clinical members of the Directorate are expected to attend and the Consultants and Registrars as well as non-medical staff arrange presentations. The Trust supports audit projects through the Department of Clinical Audit.
