Ref No 001/18 Tenure Permanent This post is open to applicants who wish to work on a flexible or part time basis and may be filled on that basis, subject to the preference of the successful candidate and the decision of the employer. Category of Contract Type B Contract Salary Basic salary will be applied in line with approved HSE salary scales. The current salary scale for a new entrant Consultant on a type B contract ranges from 122,753 to 160,893 per annum. Incremental credit will be awarded in accordance with experience and qualifications. St. James s Hospital St. James s Hospital is the largest acute general hospital in the Republic of Ireland. There are 1084 beds, with approximately 3,500 staff. There is a strong academic link with the Trinity Health Sciences Centre on campus. This houses the Medical School, the Institute for Molecular Medicine, a Postgraduate Medical Centre and the Schools of Nursing, Occupational Therapy and Physiotherapy. St. James s provides a comprehensive range of diagnostic and treatment services, many with national and regional status. Current specialties/clinical services at the hospital are as follows: National Specialties/Services: Maxillofacial Surgery Plastics Burns and Limb Reconstructive Surgery, National Centre for Hereditary Coagulation Disorders, Bone Marrow Transplant Unit. Supra-Regional Specialties/Services Infectious Diseases, Oesophageal Surgery, Cardiac and Thoracic Surgery, Vascular Surgery, ENT, Gynaecology, Critical Care. Regional Specialties/Services Cardiology, Respiratory Medicine, Endocrinology, Gastrointestinal Medicine, Genito-urinary medicine, Clinical Haematology, Rheumatology,, Medical Oncology, Laboratory services. Catchment Specialties/Services include - Major Emergency Unit, Major Day Care of the Elderly Centre, Orthopaedics, (Trauma), Palliative Care, Dermatology, Neurology, Nephrology, General Medicine, General Surgery, and Urology Area of Assignment SACC Directorate St. James s Hospital 15hrs / HSE Primary Care (Orthognathic) 16hrs / Our Lady s Children s Hospital Crumlin 8hrs. Primary base: Oral Maxillofacial Speciality, SACC Directorate, St. James s Hospital. The SACC Directorate are responsible for the co ordination, management and delivery of all 1

2 Oral and Maxillofacial Specialty Principal Duties surgical, anaesthetic and critical care services within the hospital for both inpatient and ambulatory care. Specialities under the SACC Directorate include anaesthesia, cardiothoracic and thoracic surgery, colorectal, otolaryngology, orthopaedic, plastics and burns reconstructive surgery, upper GI surgery, vascular and urology surgery.clinical Leads have have been appointed in each specialty with a common structure and data-driven approach to specialty and directorate operations meetings. St James Hospital Oral and Maxillofacial Surgery Department is the National Maxillofacial Surgery Unit. The clinical activities at the Unit include the following services : Head and Neck Oncology and Microvascular Reconstruction, Orthognathic Surgery, Facial Trauma and Craniofacial Trauma (Based at the Neurosciences Centre Beaumont Hospital), and Paediatric Oral and Maxillofacial Surgery (Based at Crumlin Children s Hospital).The OMFS Service also forms an integral aspect of the Multidisciplinary Cleft Lip and Palate Service. This is a new Consultant Post in Oral and Maxillofacial Surgery developed to support the increased demands of the Facial Deformity Service (Orthognathic Surgery), the Head and Neck Oncology Service and also the Paediatric Oral and Maxillofacial Surgery Service at Crumlin Children s Hospital. The successful candidate will be expected to have proven and contemporary clinical expertise in Head and neck Surgical Oncology including Microvascular Surgery, Orthognathic Surgery and Paediatric Oral and Maxillofacial Surgery including Secondary Cleft Lip and Palate Surgery. The on call facial trauma rota will be a 1 in 5 rota shared with Consultant colleagues at the Unit. The post has a shared commitment to St James Hospital, Crumlin Children s Hospital and the Primary Care Orthodontic Services with regard to the provision of Orthognathic Surgery. St James Hospital is the designated National Oral and Maxillofacial Surgery Unit and is recognised as a centre of excellence for Head and Neck Oncology Surgery and Reconstruction, Orthognathic Surgery and Facial Trauma. The Unit is closely linked with Beaumont Hospital (National Neurosurgical Unit), Crumlin Children s Hospital and the Dublin Dental University Hospital and also plays an integral role in the Multidisciplinary Dublin Cleft Centre. The duties for this post are in accordance with the duties as set out in the letter of approval and will incorporate commitments as follows: St. James s Hospital 15 hrs (Oncology / general) Primary Care Services HSE 16hrs (Orthognathic) Our Lady s Children s Hospital 8hrs The appointee will require close collaboration with colleagues in St. James s Hospital, Our Lady s Children s Hospital Crumlin, and the Primary Care Orthodontic Services providing joint consultation Orthognathic Surgery clinics and Orthognathic Surgical care for these patients. The appointee will be involved in the totality of oral and maxillofacial surgery with an emphasis on supporting the development of oral and maxillofacial surgery in St. James s Hospital. The appointee will be expected to possess the requisite knowledge and ability for the proper discharge of the duties of the office. Orthognathic Surgery Service A central aspect of this post is the provision of a multidisciplinary Surgical Orthodontic Service which will involve weekly joint clinics and treatment planning sessions, with dedicated operating time identified for the provision of the service. Our Lady s Children s Hospital Crumlin The Paediatric OMFS Service based at Crumlin Children s Hospital is delivered by a Consultant Oral and Maxillofacial Surgeon supported by a Fellow in Paediatric OMFS. The on call duties are shared by the OMFS Consultants at the National Maxillofacial Surgery Unit at St James Hospital. Crumlin Children s Hospital will integrate with the Paediatric Unit at Tallaght Hospital and Temple Street Children s Hospital as part of the National Paediatric Hospital on the St James campus in the coming years. 2

3 Principal Duties and Responsibilities The appointee will be responsible for Provision of services including outpatient, inpatient, day care services for the oral and maxillofacial patient cohort and provide a diagnosis, treatment or consultant service as may be appropriate the medical care and treatment of patients admitted under his/her charge and round accordingly on patients in SJH, OLCH, and HSE Primary Care services. Participation in joint clinics for orthognathic surgery patients Provision for and attendance for consultation on patients if requested to do so Supervision and responsibility for the clinical work of all NCHD staff attached to the Hospitals in consultation with Consultant colleagues attached to the specialty. Participation on the Oral & Maxillofacial surgery on-call rota Participation in multi-disciplinary team (MDT) Conferences and take responsibility for the maintenance of appropriate liaison arrangements between Oral and Maxillofacial Surgery and other specialties Undertaking teaching duties and partake in undergraduate and postgraduate training as required. Engagement in clinical audit, risk management and quality assurance programmes in the Hospital s. Maintainance patient records and to supply such reports and other information as may be required by the Hospitals with regard to specialty activity. Participation in integrated audit and teaching activities as required. Support and develop research in the specialty Support the continued expansion and development of the Oral and Maxillofacial service. Provision of support to the Director of Surgery in the development of the specialty, in line with the strategic objectives for the Hospital, to assist in delivering patient care Education Participate in Continuous Professional Development in line with professional competency requirements and in accordance with the requirements of the Medical Practitioners Act, 2007 Maintain registration on the Register of Medical Specialists of the Medical Council of Ireland. Participate in and facilitate the Continuous Professional Development of the Oral Maxillofacial Team Participate in Hospital educational programmes for all related groups of staff Audit Plan, develop and maintain responsibility for clinical audit programmes within the Oral Maxillofacial Department in conjunction with consultant colleagues and in accordance with the Hospital s clinical governance structure. Participate in Medical Audit. Assist in the collation of all data/information required in accordance with regulatory, statutory and corporate policies and procedures Teaching Contribute to the training of Oral Maxillofacial trainees (SpRs, Registrars and SHOs) in the department Participate in teaching, including undergraduate/postgraduate teaching Management and Administration Work within the framework of the hospital s service plan and/or levels of service (volume, types etc.) as determined by St James s Hospital. Participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being 3

4 developed. Participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery. To collaborate with Hospital Management, Director of Nursing, Medical and Surgical Staff of the hospital in all matters pertaining to the general efficiency of the hospital. The candidate will also demonstrate the ability to lead, supervise and be responsible for the clinical work of his / her multi-professional team. Organisational Awareness: Provide evidence of knowledge and understanding of the structure and governance of this and similar organisations Teaching: Demonstrate skills required for educational training to medical students, NCHDs and allied healthcare staff. Research: Demonstrate a proven ability and track record of engagement in research relating to the speciality. Continuing Education: Demonstrate a proven record of undertaking continuing medical education and professional development. Audit: The appointee will be required to provide evidence of undertaking and initiating audit and quality projects. Eligibility Criteria Qualifications and/ or experience Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Oral and Maxillofacial surgery. Applicants must be registered as a Specialist in the relevant Specialty on the Specialist Division of the Register of Medical Practitioners maintained by the Medical Council of Ireland within 180 days of the day of interview.
