1 Salisbury NHS Foundation Trust Individual Placement (Job) Descriptions for Foundation Year 1 Placement The department The type of work to expect and learning opportunities F1 Cardiology The Department of Cardiology comprises 4 Consultant Cardiologists who do a 1 in 4 on call for Cardiology. There is a 9 bedded CCU adjacent to the Cardiac Catheter Suite and Medical Assessment Unit. The ward can accept non-elective admissions transferred from the Medical Admissions Unit, HDU, ED and the Cardiac Suite with an average turn over of 5 8 admissions every 24 hours. The department serves a catchment population of nearing 200,000 and is closely linked to the tertiary service at Southampton General Hospital. There are also close links with the cardiology service at Bournemouth District Hospital. The F1 trainees will gain experience in the management of patients with all forms of cardiac disease including acute coronary syndrome, dysrhythmias, cardiac failure and the pre assessment and post procedure care of patients undergoing pace maker insertion, coronary angiography, percutaneous coronary intervention and transoesophageal echocardiography. Trainees will develop close links in communication with CCU, Cardiac Rehabilitation Nurse, Heart Failure Specialist Nurse, echocardiography and cardiac suite. 2 management for their senior colleagues. They provide prospective cover for other colleagues who may be absent from the ward. They will share the responsibility for arranging investigations and reviewing the results and documenting management plans with the other members of the ward medical staff. They will also undertake day time and out of hours work on the Medical Admissions Unit, admitting and treating unexpected acute medical emergencies under appropriate supervision. The F1 doctor will also take part in the General Medicine on call working on the Medical Admissions Unit (Whiteparish Ward) clerking patients being admitted and initiating management plans. All patients will be reviewed by the Acute Physician or Physician on-call within 12 hours of being assessed by the F1 doctor. 3 Placement The department The type of work to expect and learning opportunities F1 Colorectal Surgery The Department of Surgery is comprised of 4 Specialty areas. There are 4 Consultants whose interest is Colorectal Surgery. They take part in General Surgical on call and have an interest in minimal access surgery. The department serves the Salisbury area with a catchment population of around 200,000. The department does have links with the tertiary services at Southampton General Hospital and is the Regional Centre for Lower Rectal and Anal Surgery. All F1 doctors will take part in daily ward rounds as well as the day to day care of in patients on the firm. This will be a mixture of general surgical and colorectal patients. The firm undertakes a full colorectal practice as well as taking part in the general surgical on call every other week. There is a daytime commitment to the on call rota which involves clerking emergency admissions and preparation for senior review. The surgical on call rota is 1 in 9 for nights with Hospital at Night Team duties. There is also a particular interest in diabetes with specialist foot clinics and microbiological foot ward rounds with input from dedicated diabetic specialty nurses. The department serves a catchment population of nearing 200,000. The F1 trainees in Endocrinology will be ward based on Pitton and Redlych template and will be expected to deliver medical care of all the patients under the care of the 2 consultants on the ward. There is a weekly microbiological foot ward round which the F1 doctors should attend. They will work with the consultants and seniors to ensure that multiple pathologies are addressed and complex discharge planning is carried out in a multi disciplinary and effective manner. The doctors in these posts will also look after acute medical outliers on one of the surgical wards. In addition they will be expected to attend the white board round which builds on decisions made during the multi disciplinary meetings. 15 Placement F1 Respiratory / Oncology The department The department of Respiratory Medicine comprises 2 Consultants who work to cover Oncology patients on the Pembroke Unit. There are 3 Haematology Consultants who look after haematological patients also on this Unit. In addition there are 5 visiting Oncology Consultants from Southampton Hospital who will review their patients. This department serves the Salisbury area with a catchment of about 200,000. The department is closely linked with tertiary services at Southampton General Hospital particularly for those patients requiring radiotherapy. The type of work to expect and learning opportunities The F1 doctor in this post will be ward based looking after haematological patients under the care of the haematological consultants and oncology and general medicine patients also based on this Unit. These patients are under the care of the Respiratory consultants with input from the visiting Oncology consultants. Dr Effie Grand Dr Anthony Warley The F1 doctor is responsible with more senior members of the team to care for the patients based on this ward. These will be a mixture of haematological, oncological and general medical patients. The F1 doctor will also be asked to help on the Pembroke Unit at times where patients come for the day for chemotherapy and other interventions. The F1 doctor will be expected to take part on the general medical on call rota. They will admit and initiate management of patients on the Acute Medical Admissions Unit (Whiteparish Ward). All patients on this ward will be reviewed by one of the acute physicians within 12 hours of being assessed by the F1 doctor. 17 Placement The department The type of work to expect and learning opportunities F1 Stroke The Stroke Medicine Team are part of the Medicine for Older People Department and comprises 2 Consultants both who take part in the General Medical rota but also take part in the Local Network Stroke on call rota. The department serves the Salisbury area with a catchment population of around 200,000. The department has links with Avon, Wiltshire and Gloucester Stroke Network and also tertiary services at Southampton General Hospital. The F1 doctors in the Stroke Team will participate in the medical care of patients on the Stroke Unit. The Stroke Unit cares for patients in acute and rehabilitation phases of recovery from stroke together with a variable number of patients with general medical problems. Dr David Walters Dr Toby Black The F1 trainee is responsible along with the other staff on the ward for the care of patients presenting with acute stroke and other general medical problems. They are expected to attend the white board meeting every day where discharge of the patients is coordinated in an appropriate manner. They will also be expected to be involved in the Multi Disciplinary Team Meetings, family meetings and the assessment of direct admissions to Farley Stroke Unit. They will become familiar with NIHSS assessment of patients with stroke and will have daily consultant review of any acute admissions admitted to the Unit. The F1 doctor will also take part in the General Medicine on call working on the Medical Admissions Unit (Whiteparish Ward) clerking patients being admitted and initiating management plans. 19 Placement F1 Urology The department The Department of Urology comprises 5 Consultants (4 substantive and 1 locum). The consultants take part in a on call for Urology which is separate from the General Surgical on call. The department serves the Salisbury area with a catchment population of around 200,000. They will be supervised by more experienced members of the team. Ward rounds are held daily and there is a separate ward round on a Tuesday afternoon. In general the F1 doctor works closely with the Registrar in resolving problems that arise on the wards and with acute admissions. Pre admission clinics are held on a Monday and a Thursday and although Specialist Nurse led, drug charts etc need to be completed by the F1. Attendance at the Surgical M&M meeting is expected. Typical working pattern in this placement: Daily: 0800 Consultant or SpR Ward round Mon: 1500 Check off nursing pre admission clerking

21 Placement The department The type of work to expect and learning opportunities F1 Vascular and Breast Surgery The Department of Surgery comprises 4 specialty areas of which there are 3 Consultants undertaking Vascular Surgery and 3 Consultants in Breast Surgery. The Vascular consultants and 1 Breast surgeon are part of the General Surgical on call rota and therefore have General Surgery in patients. The Breast Surgeons also undertake Endocrinological Surgery. All Surgeons practice minimal access surgery. The department serves the Salisbury area with a catchment population of around 200,000 and there are links with the tertiary referral services at Southampton General Hospital. All F1 doctors in this post will look after in patients under the Vascular and Breast surgeons. This will include a mixture of General Surgical in patients plus those with specialist Vascular and Breast pathology. They will also be expected to undertake pre admission clinic clerking and be involved in the General Surgical on call rota during the day and be part of the Hospital at Night Team at night. The overall objectives for the F1 post are to take history and examine a patient identify and summarise problems prescribe safely document accurate, appropriate and relevant entries in the medical record manage time and be able to prioritise appropriately communicate effectively with patients, relatives and colleagues use evidence, guidelines and local protocols to ensure optimum management of general surgical in patients Where the placement is based Clinical Supervisor(s) for the placement Main duties of the placement Typical working pattern in this placement Salisbury District Hospital, Britford and Downton Wards. They are expected to take part in the Multi Disciplinary Team meeting for the Vascular Team and also the General Surgical Morbidity and Mortality Meeting on a Friday morning. The on call is a 1 in 9 commitment which involves clerking surgical admissions in preparation for senior review. There is also a 1 in 9 rota for nights with Hospital at Night Team duties.
