3 Consultant in Orthodontics 1. The post 1.1 Job Description and proposed job plan This recruitment pack contains information relating to the post of Consultant Orthodontist and outlines the job description, other responsibilities and a proposed job plan. This post is based at the Royal Bournemouth Hospital. The successful applicant will agree a job plan with the Clinical Director and Directorate Manager for Specialist Services, which is subsequently reviewed on a yearly basis. This is a permanent clinical post which may include additional responsibilities such as clinical/governance lead. Full-time (10PA), part-time and job share applications will be considered. The role is based at the Royal Bournemouth Hospital. The successful applicant will join two consultant colleagues (1 wte) in providing a comprehensive, consultant orthodontic service for the population of East Dorset. Changes may be made in line with the needs of the service and with agreement of colleagues. 1.2 Duties and responsibilities of the consultant The postholder will work closely with colleagues to provide a comprehensive consultant orthodontic service for the population of East Dorset. The Department is working closely with specialist practitioners in primary care and general dental practitioners to ensure that orthodontic treatment is provided in the most appropriate location depending upon complexity and need. The postholder will be expected to play a full role in the delivery of performance targets (including 18 weeks) and contribute to the continued development of the orthodontic service. Flexibility will be required in the delivery of service. The postholder will share with colleagues the teaching and supervision of general dental practitioners and the orthodontic therapist working in the Orthodontic Department. It is essential that the postholder participates in clinical audit and continuing professional development The postholder will be expected to work with local managers and professional colleagues in the efficient running of services and will share with consultant colleagues in management responsibilities. The postholder is expected to make sure that there are adequate arrangements for hospital staff involved in the care of their patients to be able to contact them when necessary. The postholder will continue personal education and development through opportunities for internal and external continuing medical education. 4

Foundation status was achieved in April The bed complement is 609 in-patient beds, plus 12 ITU, 10 CCU and 139 day-case beds. A district-wide strategic review has led to concentration of specialised services within particular provider units. Among the specialties at Bournemouth are: The Dorset Heart Centre provides first class specialised adult cardiac services for a population of one million, including much of Dorset and parts of Wiltshire and Hampshire as well as general cardiology for the local population. Here at the Royal Bournemouth Hospital, we are the only district hospital in the UK to provide comprehensive high volume coronary angioplasty and electrophysiology services. Elective Orthopaedics As well as joint replacements and revisions, the directorate carries out a comprehensive range of orthopaedic procedures, encompassing treatments for hands, wrists, elbows, shoulders, hips, knees, 6

7 ankles and feet with majority of patients treated as day cases at the Sandbourne Suite opened January This one of the largest hip and knee replacement centres in Europe, focusing exclusively on elective surgery, providing high quality comprehensive care for patients with painful bone and joint conditions. Eye Unit This purpose built unit opened in April 2000 and includes 16 bed inpatient ward (including three paediatric beds) together with eye theatres, Outpatient Department which includes one stop cataract clinic and glaucoma clinics. Almost 34,000 patients per year are seen in eye clinics and a further 10,500 benefited from urgent or emergency treatment. Surgeons in the Eye Unit perform around 3500 cataract operations every year, which comprises around 75% of the surgical workload. Interventional Radiology The hospital is well equipped and has a new CT / MRI suite (four scanners) two dedicated digital imaging labs for interventional radiology and state-of-the-art PACs implementation. In addition to the main Radiology Department sited at the Royal Bournemouth hospital, there is also a busy imaging service located at Christchurch Hospital. Stroke Services Previously located on two sites, a new Stroke Unit, purpose built to combine the care of both acute and rehabilitation patients under one roof opened in November The unit has capacity for up to 36 patients at any one time and around 700 stroke patients are expected to benefit from the new unit over the next 12 months. Emergency Department There is a 24 hour Emergency Department with 75,000 attendances annually facilities include emergency helicopter landing area. The main Trauma centre for the area is based at Poole Hospital. Other specialties at Royal Bournemouth Hospital include vascular surgery, laparoscopic surgery, urology, endocrinology and metabolic medicine, thoracic medicine, GU/HIV medicine, haematology, histopathology, medical oncology and the Women s Health Unit including breast surgery, gynaecology and a Midwife lead maternity unit. 2.4 Christchurch Hospital Two miles from the Royal Bournemouth Hospital, Christchurch Hospital is set in attractive grounds, which are listed as a conservation area, providing a pleasant environment for rehabilitation and a range of outpatient services. An all-age rehabilitation service has been developed, particularly in the award-winning Day Hospital. Most patients are elderly, reflecting the local population. There is an excellent infrastructure to support rehabilitation with superb physiotherapy and occupational therapy facilities. Outpatient clinics have expanded over recent years and include gastroenterology, breast, oncology, plastic surgery, ophthalmology, podiatry and medicine for the elderly. Dermatology and rheumatology services are also provided at Christchurch Hospital together with phlebotomy (blood taking) services, diagnostic services and palliative care (the Macmillan Unit). The hospital is currently undergoing a multi-million pound investment which will secure NHS services on site and ensure the hospital remains a key part of the community for years to come. Many patient services are being improved, a new entrance and X-ray Department will be built and a new GP surgery, a pharmacy and community clinics will be brought on site. A quality nursing home and senior living accommodation are also being built as part of the project. 7

8 3. The District The Royal Bournemouth and Christchurch Hospitals NHS Foundation Trust is within the NHS South West area. The Trust provides general health care for the residents of Bournemouth, Christchurch, East Dorset and part of the New Forest with a total population of over 550,000. This is a major tourist area and during the summer months over one million holidaymakers visit Bournemouth including substantial numbers of foreign language students. The fluctuating population, three time s greater growth than the average for England and Wales, and the high proportion of elderly present special problems for health care. The major acute hospitals in Dorset are: East Dorset The Royal Bournemouth Hospital - the other hospital forming part of the Royal Bournemouth and Christchurch Hospitals NHS Trust is Christchurch Hospital. Poole Hospital NHS Foundation Trust - serves as the main trauma centre for East Dorset West Dorset Dorset County Hospital, Dorchester Poole Hospital NHS Foundation Trust The hospital provides a wide range of acute services to people in Poole, East Dorset and Purbeck with 789 beds, including 74 for day cases and serves as the major trauma centre for East Dorset, this means that more than 85% of inpatients are admitted as emergencies. The hospital is a modern district general built in 1970 and all the principal specialties are represented providing services for all medical, elderly, surgical and child health emergency admissions to include general surgery, trauma, ENT, oral and maxillofacial surgery, obstetrics, gynaecology, paediatrics, general medicine, neurology, rheumatology and dermatology. Poole Hospital (with some services at Bournemouth) has recently been designated as the Cancer Centre for Dorset and provides all radiotherapy services plus the central base for medical oncology. The Dorset Breast Screening Centre is also based here. Forest Holme hospice opened in 1994 to offer palliative care and is set in its own secluded location close to the rear of the main hospital site. There is a large new hospital for the elderly on the site and there has recently been a major development of the Obstetric Unit. The Poole Maternity Unit which lies opposite the main hospital in St. Mary s Road now also delivers those high risk patients who reside in the Bournemouth end of the district and contains all Consultant obstetric beds for East Dorset where approximately 4,000 deliveries occur per annum. There is a new day surgery unit and an open access endoscopy service. Dorset HealthCare University NHS Foundation Trust (non-acute) Dorset HealthCare provides a wide range of mental health, and disability services. The Trust primarily covers the areas of East Dorset comprising the local councils of Bournemouth, Poole, Christchurch, Wimborne and Purbeck and provides some specialist services (e.g. forensic and Eating Disorders) for the whole county of Dorset. The Trust provides a comprehensive community based mental health service covering the full range of recognised mental illnesses and psychological problems. The services have been highly commended by the Royal College of Psychiatrists, Mental Health Act Commission and Department of Health Mental Health Task Force Leader. 8

9 St Ann's Hospital St Ann s Hospital, Canford Cliffs, Poole is set amongst secluded cliff top grounds, and is the centre of a comprehensive psychiatric service for East Dorset and provides acute admission facilities. The hospital provides specialist services for mother and baby, people with eating disorders and mentally disordered offenders. Community care plays an important part in the service, with multi-disciplinary community teams operating in adult, elderly and rehabilitation services. St Leonard s Hospital This hospital is in the north of the sector with beds for long stay care, slow stream rehabilitation, respite care and terminal care. These facilities are shared by the Bournemouth and Poole Consultants. 4. The local area Bournemouth and the surrounding area is a major tourist attraction with outstanding beaches, large gardens and extensive shopping areas. The New Forest, Poole Bay and Purbeck (the Jurassic Coast) are all within easy reach and there are excellent recreational facilities. Both Bournemouth and Poole have a number of theatres and concert halls, among them the Bournemouth International Centre and The Lighthouse theatre which is the home of the Bournemouth Symphony Orchestra. There are also numerous restaurants, pubs, night-clubs and cinemas in the area. Adjacent to the hospital complex are two leisure/fitness centres both with indoor swimming pool, gym/fitness room, squash courts etc. Bournemouth has its own University as well as college sites and there are many excellent schools in the area. The travel time by rail to London (Waterloo) is approximately 1 hour 40 minutes and by road (M27, M3) approximately two hours. Bournemouth has its own recently expanded airport with internal and international flights. 5. Orthodontic Department The Orthodontic Department at the Royal Bournemouth Hospital provides a comprehensive orthodontic service predominantly for patients in East Dorset. On average, the Department sees around 1,000 new patients each year and there are approximately 11,000 out patient attendances. A referral and advice service is provided for general dental practitioners who want to undertake orthodontic treatment for their patients, where this is appropriate. There is a close working relationship with the Department of Oral and Maxillofacial Surgery for the treatment of patients requiring multi-disciplinary management. The treatment of facial disproportion is a particular local interest. The department is a spoke of the Spires cleft lip and palate service at Salisbury which is now the cleft lip and palate centre for Dorset. 5.1 Who we are Consultant Staff: Vacant post (10 PAs) Miss Susan Power (5.0 PAs) 9

10 Mr Earl Read-Ward (5.0 PAs) Other staff: St1: Approved by the Specialist Advisory Committee (SAC) in Orthodontics and Paediatric Dentistry of the Joint Committee for Specialist Training in Dentistry. The current incumbent is on a 3+ 2 training pathway (One of the first in the country) The training programme is linked to Guys, Kings and St Thomas hospital. Hospital Practitioners: One post: 1PA, One post: 3PA (currently vacant) Orthodontic Therapist: One part time (0.8 wte) Clinical Attachments: Occasional clinical attachments are arranged for general Dental practitioners and community dental officers. Admin: One full-time service manager, two part-time secretaries (1 wte) and four part-time receptionists (2 wte). Technicians: One laboratory manager, one chief orthodontic technician and one plaster technician. Dental nurses: One senior dental nurse, part-time deputy (0.8 wte), one fulltime and three part-time dental nurses. 5.2 Facilities Three orthodontic surgeries (access to an additional surgery 1 or 2 sessions per week - shared with oral surgery) Well-equipped laboratory Radiography room with Planmeca digital imaging system and Vista scan unit for intra oral imaging 3D digital study models, model storage room Consultants offices and part-time secretarial support are provided at Royal Bournemouth Hospital Specialist Registrar office/computer room (Dolphin imaging system). Reception area with patient record storage (shared with the Oral Surgery Department) Waiting area (shared with the Oral Surgery Department). 6. Specialist Services Directorate The Orthodontic Department is part of the Specialist Services Directorate which is led by a Clinical Director, Directorate Manager and Matron. Our purpose is to work together to drive innovation and quality care for our patients. We aim to provide excellent care for every patient, every day, everywhere. In addition to Orthodontics, the directorate includes the Department of Department of Sexual Health, Dermatology, Pharmacy, the Dorset Prosthetic Service and Rheumatology, located across both the Royal Bournemouth and Christchurch hospitals. Our purpose is to work together to drive innovation and quality care for our patients. We will achieve this through the following objectives: 10

11 7. Postgraduate Medical Education There is a Postgraduate Centre at the Royal Bournemouth Hospital and Poole Hospital. The successful candidate will be expected to provide teaching for the dentists in training in their department. He/she will be expected to share with Consultant colleagues the role of Educational Supervisor. This will involve the provision of supportive formal appraisals to dentists in training at regular intervals, induction at the beginning of their contracts and career advice as appropriate. Advice on the role of Education Supervisor can be obtained from the Clinical Tutor. There are exceptional libraries based in the Postgraduate Centres of both Bournemouth and Poole hospitals and meetings of general and specialist medical content are held regularly during the week. There is an active Medical Audit Committee supervising Medical Audit within the Hospital. The Dorset Research and Development Unit (RDSU) is based within Poole Hospital. Both trusts are fully committed to supporting continual medical education for consultants, and study leave will be available according to statutory guidelines. Consultant appraisal All consultants are required to have a job plan setting out their main duties and responsibilities, including a work programme specifying their weekly timetable. To support the GMC revalidation process the job plan is subject to review once a year by the post-holder with the clinical director and directorate manager. It is acknowledged that the post holder, as a General Dental Practitioner, will be governed by the General Dental Council but the appraisal process will be conducted in the same manner as for all consultants in the trust. For this purpose, both the postholder and the Medical Director will have a copy of the current job plan, including an up-to-date work programme and relevant departmental statistical information and proposed service or other developments. The appraisal process aims to be a constructive and developmental one, largely concerned with the continuing development of the consultant s skill base and career. A number of physicians across the trust have been formally trained for consultant appraisal, allowing individuals a choice in the identity of their appraiser. 8. Conditions of Appointment This post will be subject to the Terms and Conditions Consultants (England) 2003 and will comprise of a main contract of employment for ten programmed activities and a supplementary annual contract for any temporary additional programmed activities. Temporary additional programmed activities will be reviewed at the same time as the job plan. 11

12 Criminal Records Disclosure As part of our recruitment procedure this post will be subject to a Criminal Record Disclosure. A Disclosure is a document containing information held by the police and government departments. Disclosures provide details of a person s criminal record including convictions, cautions, reprimands and warnings held on the Police National Computer. Where the position involves working with children, Disclosures will also contain details from lists held by the Department of Health (DOH) and the Department for Education and Skills (DfES) of those considered unsuitable for this type of work. Medical Clearance Appointment to this post is subject to satisfactory medical clearance. If shortlisted for interview, applicants will be required to complete a medical questionnaire and attend a health interview. Clinical Governance The hospital has an active programme of clinical governance, led by Dr Alyson O Donnell, the Medical Director. Consultants are encouraged to participate in CME/CPD, both locally and nationally. Rehabilitation of Offenders This post is exempt from the provisions of Section 4(2) of the Rehabilitation of Offenders Act Applicants are, therefore, not entitled to withhold information about convictions and, in the event of employment, any failure to disclose such convictions could result in dismissal or disciplinary action by the Trust. Any information given will be completely confidential and will be considered only in relation to an application for positions to which the exemption order is applied. Residence Residence within 10 miles or 30 minutes by road of the postholder's base is usually required unless alternative arrangements are agreed with local management. The post holder must have a current driver s license and his/her private residence must be maintained in contact with the public telephone service. Commencement of duties The post is available as soon as possible and the post holder will be required to take up the post no longer than three months from the date of the offer of an appointment, unless special agreement is reached between the post holder and the Trust. If an applicant considers it unlikely that he/she will be able to take up the appointment within such a period, he/she is advised to point this out at the time of application. Contravention of the 1998 Act is an offence punishable by heavy fines.
