3 1. INTRODUCTION TO DEPARTMENT/SPECIALITY The Department of Radiology forms part of the Medical Division of the Trust s Operations Directorate. This comprises all emergency medical, ambulatory medical and specialist medical services together with Diagnostic Services including Radiology and Pathology. The Chief Operating Officer is Paul Bytheway. The Divisional Medical Director is Dr Matthew Banks and the Clinical Director in charge of Diagnostic Services is Dr Alexander Wolinski. The Radiology Department is both physically and operationally at the centre of the Trust. The Medical Service Head for Radiology is Dr. P. S. Oliver, Consultant Radiologist and the Imaging Services Manager is Mrs. J. Whiles, Superintendent Radiographer. There is a Division of Radiology Committee that meets regularly and to which all Consultant Radiologists belong. The current X-Ray Department Budget is approximately 11 million per annum. The approximate annual workload of the department by modality is as follows: Plain film CT MRI Non-obstetric Ultrasound Nuclear Medicine Fluoroscopy Vascular intervention Symptomatic Mammography 135,000 examinations 23,000 examinations 20,000 examinations 45,000 examinations 3,000 examinations 7,000 examinations 1,100 procedures 5,000 examinations Dudley Group of Hospitals are the Hub site for Vascular Surgery in the Black Country. The Vascular Interventional Radiology Service is operated from Russells Hall Hospital jointly with personnel from New Cross Hospital, Wolverhampton. The Royal College Equipment Replacement Guidelines have been accepted in full by Summit Healthcare (PFI Partners). There is a 30 year guaranteed replacement schedule for all major equipment:- every 7 years for CT, MRI and Angiography; every ten years for other machines, ensuring that the department is always technically up to date. All the main equipment is located within a modern department at the heart of the Russell s Hall Hospital site. The Department contains: General Purpose Rooms all of them DR Dedicated Emergency Rooms within the Emergency Department.
5 CT 3. 2 General Screening Rooms Angiography suite, contained in its own Operating Theatre within X- ray department 5. 5 Ultrasound Rooms Philips iu22 and GE Logic EQ scanners 6. Nuclear Medicine 1 Siemens Symbia S Dual Head Camera and 1 Siemens Symbia T6 SPECT/CT Hybrid Camera 7. 2 CT Scanners (1 x Siemens Somatom Definition 64 Slice; 1 x Siemens Somatom Definition 128 slice) 8. 2 MRI Scanners (Philips 1.5T Achieva & Siemens Magnetom Verio 3.0T) 9. Mammography 2 digital Hologic units in a purpose-built Breast Imaging Suite which includes consulting rooms. The CT suite houses 2 scanners in back-to-back configuration, namely a Siemens Somatom Definition AS 64 Slice and a Siemens Somatom Definition AS 128 slice scanner. 3D post processing of CT images is performed on the Siemens Via platform giving all reporters full image processing facilities at their own desktop PACS workstation. MRI A Philips Intera 1.5T MR Scanner has been upgraded to Philips Achieva 1.5T scanner in March A Siemens Magnetom Verio 3.0T Scanner was commissioned in Image post processing is available on the Siemens Via platform. Angiography and Interventional Procedures Angiography is provided by a dedicated suspended Siemens Artis Zee C arm within a full Operating Theatre Status Suite with its own 2-bedded recovery Unit. The Trust is the Hub vascular surgical unit for the whole of the Black Country, so the facilities for vascular interventional imaging are constantly being developed. ULTRASOUND The Radiology Department at Russell s Hall Hospital has 5 Ultrasound Rooms with additional ultrasound facilities being provided in the Obstetric Unit, Surgical Assessment Unit and the Vascular Laboratory. 2 further Ultrasound rooms are located at the Corbett and the Guest Ambulatory Care Centres providing outpatient facilities for the people of Stourbridge and Dudley. The ultrasound rooms are equipped with a mix of high specification Philips and GE scanners. The Radiology and Gastroenterology departments jointly run an Endoscopic Ultrasound service with Pentax Radial and Linear scanning endoscopes operating through Hitachi Ultrasound scanners. This service offers the full
6 range of EUS diagnostic and interventional procedures to support the Upper GI Cancer Unit. The Radiology department also provides a full EndoBronchial Ultrasound service in conjunction with the Respiratory Physicians. All Consultant Radiologists, together with a team of experienced Ultrasonographers are involved with Ultrasound Service provision. Individuals have developed and maintained areas of specialist expertise. It is hoped that the appointee will contribute to and extend the quality of this service. The Vascular Lab is a self contained unit comprising two Philips Ultrasound machines staffed by a team of Vascular Technologists and Sonographers. RADIOGRAPHIC, NURSING & SUPPORT STAFF Mrs J. Whiles, Band 8B Radiology Manager and Superintendent of Ultrasound Mr T. Usher, Band 8A - Superintendent of CT, MRI and Nuclear Medicine. Mrs J. Rothery, Band 8A - Superintendent of Interventional Services and Fluoroscopy. Mrs D. Lowe, Band 8A - Superintendent of General Radiography Mrs J. Dunham, Band 7 - Head of Nuclear Medicine. Mr T. Wicker, Band 7 - Mr I. Wright, Band 7 - Head of MR Scanning. Head of Ultrasound & Vascular Lab There are 72 Radiographers, 13 Sonographers and 3 Nurses within the Department. The Radiographers as a staff group not only acquire the radiographic images, but we also have at least one accredited Reporting Radiographer in every imaging modality. There is one Consultant Radiographer in Mammography and Breast Screening. Radiographers and nurses are trained in Intravenous Injections where appropriate. Secretarial and support staff number 20 WTE. There are also 8 WTE Radiographic helpers. DEPARTMENTAL WORKING ENVIRONMENT The Radiology Department is justly proud of the spirit of co-operation and teamwork that exists both between the Radiologists and with the other professional groups in the department. The successful applicant will be expected to cultivate the team-working ethic to facilitate the amicable, efficient and smooth running of the department.
7 Secretarial support and office accommodation will be provided. Radiologist has a named Secretary. Each There is, by and large, an excellent working relationship between the Radiology Department and other clinical departments. Regular Multi- Disciplinary Team meetings take place in most disciplines; these are well attended and are considered most valuable by all. There are also numerous clinico-radiological meetings within the Trust. The Department and its development are at the heart of the PFI Programme. The PFI Project has provided all major Radiological Facilities centrally within a single main Department at Russell s Hall Hospital. All inpatient services are at Russell s Hall. Satellite Ambulatory Care Centres have been built at the Guest Hospital in Dudley and at the Corbett Hospital in Stourbridge. The Corbett Radiology department has 2 General DR X-ray rooms, 1 Ultrasound room and 1 DEXA scanner. The Guest Radiology department has 1 General DR X-ray room and 1 Ultrasound room. Examinations performed in satellite units are reported at Russell s Hall via Trust-wide PACS. Radiologist presence will not normally be required at the satellite units. The Radiology Department and the wider Trust has invested heavily in IT systems to facilitate our work. The Radiology Information System (CRIS) is regularly upgraded to the latest version. Radiological reports are dictated directly onto the RIS using Dragon v11 voice recognition software. A Siemens Plaza PACS system fully integrated with the RIS is available throughout the Radiology department. This was the first installation of this PACS system in the UK. Each Radiologist has a PACS workstation in their office. The Trust has been completely filmless for more than ten years. PACS images are available on any networked PC throughout the Trust and on personal laptops at home for on-call duties. The PACS and RIS are also fully integrated with the Trust s Electronic Patient Record system to enable electronic radiology requesting and instant access for clinicians to radiology reports and images. Local General Practitioners surgeries are also linked to our PACS network. Computers linked to digital projectors are used for the display of radiological images at multidisciplinary team meetings in the radiology seminar room and in all the meeting rooms in the main Education Centre. Some MDTs use video-conferencing facilities to hold unified meetings with other hospitals in the Black Country Cancer Network. 2. JOB SUMMARY MAIN PURPOSE & KEY OBJECTIVES OF THE POST To provide a comprehensive Radiology service as part of an experienced and flexible team to patients and clinicians within the local area as part of the Imaging Department. The post holder will work in a manner that complies with the principles of the Trust and with particular regard to Risk Management, Clinical Governance and IR (ME) R 2000 Cross Sectional imaging with CT, MRI and Ultrasound is the cornerstone of modern diagnostic radiology.
8 Dudley radiologists are encouraged to develop expertise in one or more clinical subspecialities. We aim to have strength in depth with at least two radiologists with expertise in each clinical speciality. Although potentially any subspeciality interest can be accommodated, we are particularly looking for successful candidates to possess or develop skills in Cardiac Imaging, Paediatrics, Gastroenterology and Breast Imaging. Participation in multidisciplinary team and clinico-radiological meetings with other members of the radiology team will be a key part of the post. The successful candidate will be expected to attend at least one MDT meeting regularly. The job-pan will include preparation time prior to the MDT meeting. Consultant Radiologists are also expected to take an active role in the training and supervision of Radiology SpRs allocated to the Department from the West Midlands Radiology Training Scheme. We normally host 4-5 radiology SpRs at a time. Registrars in the department do specialist training in Musculoskeletal and Interventional Radiology. As an undergraduate Teaching Hospital, radiologists are frequently called on to provide clinical teaching for our students. There are ample opportunities to develop a role in undergraduate and postgraduate teaching if desired. 3. KEY RESPONSIBILITIES 3.1 Activities This is a General Post with a Specialist Interest in Cross-Sectional Imaging. The post holder is expected to: - 1. Provide, and develop further with clinical colleagues, integrated Imaging services for the Trust in collaboration with colleagues in all disciplines. 2. Take responsibility for patients whilst under his/her care. 3. Provide a Consulting and Advisory Service to other clinical colleagues. 4. Participate in the development of the imaging service, protocols and guidelines and the greater knowledge supporting the provision of service. 5. Develop and maintain collaborative relationships with medical colleagues in other specialities and participate in regular Multi- Disciplinary Team Meetings. 6. Participate in the On-Call Rota on an equal basis with Consultant colleagues to ensure appropriate emergency services are provided outside normal working hours. 7. Take responsibility for professional supervision and development of trainees within the speciality and other post-graduate activity. 8. Maintain and develop good communications with General Practitioners and other outside agencies. 9. Demonstrate a firm involvement in Clinical Governance, Risk Management and Clinical Audit. 10. Ensure that his/her own practice is up-to-date including responsibility for Clinical Professional Development. 11. Comply with the General Medical Council guidelines on Good Medical Practice 12. Share responsibility for Data Protection arising out of the use of
9 computers. 13. Comply with all relevant Trust Policies and Procedures. The Trust has a duty of care to employees and will ensure that, as far as is reasonably practicable, adequate training, facilities and arrangements for risk avoidance are in place. All Trust employees are required to comply with relevant Health & Safety legislation and the Trust s policies relating to Health & Safety and Risk Management. In particular, they have a duty to: Take reasonable care of both their own and others health & safety. Report any adverse incident (including near misses ) in accordance with the Trust s Policy. Report any concerns regarding both general health and safety and the well being of patients and staff, in accordance with Trust Policies. Undertake or participate in risk assessments, as appropriate. Support the Trust in complying with relevant legislation and/or directives from relevant external organisations, such as the Health & Safety Executive. To use facilities and equipment provided in the interests of health and safety in as appropriate manner, in order to minimise risk and avoid compromising health and safety. This will include not intentionally or recklessly misusing facilities and equipment. 3.2 On Call Requirements The post holder will be required to participate in the General Radiology oncall. Currently the general rota is on a 1 in 12 basis with prospective cover and is paid at category [A] level. The frequency level will be subject to change, in accordance with schedule 16, but it currently attracts a supplement of 3%. Remote access to PACS and CRIS is available to enable the on-call radiologist to report examinations without necessarily returning to the hospital. The on-call radiologist provides cover from 5:00pm to 10:00pm Mondays to Fridays and from 8:00am to 6:00pm on Saturdays and Sundays. Outside of these hours the on-call CT service is outsourced to a teleradiology company based in London and Australia. This enables the duty radiologist to have an uninterrupted night s sleep in order to be fresh for the following day. 3.3 Productivity Radiology room time is allocated in sessions or Programmed Activities of
10 Direct Clinical Care and consultants are expected to ensure optimum utilisation of the Programmed Activities allocated to them, in terms of time and productivity. Post holder will be expected to provide 8.5 Programmed Activities per week of cross-sectional general radiology reporting and related activities e.g. Multidisciplinary Team meetings in accordance with an agreed Job Plan Post holder will comply with the department s policy for minimising the number of dropped clinical lists. Specific details of these arrangements are given below: Where direct clinical care (DCC) time is dropped to attend supporting professional activities, e.g. audit meetings, the displaced clinical work should be reallocated to a time normally allocated to SpA work. These are subject to change, as required by the needs of the service. 4. WORK SCHEDULE 4.1 Indicative Timetable The post holder would initially be contracted for 10 Programmed Activities (PAs) per week comprising 8.5 Direct Clinical Care (DCC) and 1.5 Supporting Professional Activities (SPAs). 1.5 SPAs are automatically granted to each consultant. A further discretionary one SPA may be available by negotiation at Job Planning, provided the post holder can demonstrate the need for the additional SPA time. Additional PAs of Direct Clinical Care may also be available as the service needs of the department require. The Trust is also a family friendly employer and would consider application from the successful candidate to work part time by negotiation and mutual agreement. The timetable below is indicative of the number of sessions allocated to each modality in the post holder's job plan. The actual times of the sessions may be subject to change to fit in with the timetables of the other consultant members of the department. There is the intention to move to 7 day working in the near future. The post holder may be required to work some of their sessions at evenings or weekends, although the post holder would not be required to work every evening or every weekend. The post holder will be required to be present on site for the duration of their Direct Clinical Care PAs. Where clinically appropriate, on-call work may be performed off-site via a teleradiology link.

12 0.5 PA of Direct Clinical Care is allocated to on-call work. 4.2 SPA details Following discussion in the Trust Joint Local Negotiating Committee, the Trust has clarified activities that fall within SPAs and agreed that consultants will receive 1.5 SPAs as of right and a further 1 SPA by negotiation provided the consultant can justify the use of SPA time (excepting part time consultants who will receive the amounts specified in the national terms and conditions). The following work is therefore to be included within the 1.5 to 2.5 SPAs in the post holder s job plan. The distribution of work will be agreed at a speciality level, as it is not the intention of the Trust that any one individual would take responsibility for all of the above listed activities, within the allocated 1.5 to 2.5 SPAs. The Trust Joint Local Negotiating Committee have agreed that up to 1 SPA per week may be worked off-site. 5. GENERAL RESPONSIBILITIES All staff are expected to adhere to policies and procedures in the Trust, which establish standards of good practice and follow any codes of conduct, which are relevant to their own profession. 5.1 Mentoring The Trust operates a mentoring process for all newly appointed consultants to enable them to become a fully productive member of the DGNHSFT team as quickly as possible both in terms of what they deliver (outputs) and the way in which they deliver (behaviour and attitudes). All newly appointed consultants are required to participate in the process.
13 5.2 Performance Management The Trust operates a structured performance management approach for all newly appointed consultants to ensure that there is an appropriate fit between the individual and the organisation. This includes: Performance reviews with the Medical Service Head at 3, 6 and 12 months after appointment Performance reviewed against capability, productivity and behavioural standards All newly appointed consultants are required to participate in the process. 5.3 Trust Policies & Procedures Infection Control The post holder will ensure that they and their teams fully comply with the policies/procedures in place for infection control No Smoking The Trust has a Smoke Free policy. SUPPORTING INFORMATION 7.1 Dudley Group The Dudley Group NHS Foundation Trust is the main provider of Hospital services to the populations of Dudley, significant parts of the Sandwell Borough and smaller, but growing, communities in South Staffordshire and Wyre Forest. Currently we serve a population of around 400,000 people from three sites at Russells Hall, Guest Outpatient Centre in Dudley and Corbett Outpatient Centre in Stourbridge, providing the full range of secondary care services and some specialist services for the wider populations of the Black Country and West Midlands regions. The Trust was authorised by Monitor, the independent regulator of NHS foundation trusts, to commence operation as an NHS Foundation Trust from 1 st October The Trust s hospitals form part of a Private Finance Initiative (PFI) with Summit Healthcare and its service providers: Interserve Facilities Management and Siemens Healthcare. With effect from 1 st April 2011 the adult community services section of the Dudley PCT Provider Services function has transferred under the management of the Foundation Trust. With the addition of Adult Community Services the Trust employs over 5,000 members of staff providing over a million patient contacts per year across all Trust sites. The Trust has invested heavily in medical equipment over recent years and during 2010 commissioned a 7m new multi-tiered staff car park at Russells Hall Hospital. Local state schools and universities (Birmingham, Aston and Wolverhampton) are excellent and have high success rates for students continuously. 18 Dudley Group NHS Foundation Trust Management Structure The Dudley Group NHS Foundation Trust operates with the emphasis on Clinicians being responsible for the management of individual clinical units, which make up the Operations Directorate. Within this structure management of each clinical unit is the responsibility of the Medical Service Head and the Modern Matron. Reporting immediately to a Clinical Director. The Operations Directorate along with the Directorates of Finance and Information, Nursing and Human Resources make up the components of the Trust Main Board. The members of the Executive team are: Chief Executive Chief Operating Officer Director of Finance Director of Strategy and Performance Chief Nurse (interim) Medical Director The Trust has two Deputy Medical Directors Diane Wake Paul Bytheway Paul Taylor Anne Baines Siobhan Jordan Paul Harrison Roger Callender Paul Stonelake Matthew Banks Teaching Hospital Status As of 2002 Dudley Group NHS Foundation Trust achieved teaching hospital status as part of the Birmingham Medical School. Third, fourth and fifth year medical students are taught within the Trust. Since then Dudley has begun the introduction of an Undergraduate Teaching Academy status under the leadership of Mr Atiq Rehman (Head of Teaching Academy). A dedicated Undergraduate Educational Block supports learning of undergraduate medical students. Consultant staff, Registrars and other junior medical staff are expected to participate in the teaching of medical students. Medical Education A modern and fully equipped Postgraduate Medical Education Centre is situated at Russells Hall Hospital. There is a modern and fully equipped library, which is linked with the Barnes Library at the Medical School in Birmingham together with the national lending library networks. There are also excellent facilities for lectures and meetings. Dudley and the Surrounding Area Set in the heart of the West Midlands access to both to major cities and rural countryside is close.
