1 Wessex Deanery Poole Hospital NHS Foundation Trust Individual Placement (Job) Descriptions for Foundation Year 1 Placement F1 - Diabetes and Endocrinology comprises 3 diabetes and endocrinology consultants. Most days will generally be ward during the normal working day with ward rounds at the beginning of each day. Whilst on-call with the team the F1 will be involved with the generic clerking of patients being admitted. The F1 also has to work with their team on nights to cover the medical wards. The overall educational objectives of the F1 year are to provide the trainee with the knowledge, skills and attitudes to be able to Take a history and examine a patient Identify and synthesise problems Prescribe safely Keep an accurate and relevant medical record Manage time and clinical priorities effectively Communicate effectively with patients, relatives and colleagues Use evidence, guidelines and audit to benefit patient care Act in a professional manner at all times Cope with ethical and legal issues which occur during the management of patients with general medical problems Educate patients effectively Become life-long learners and teachers. Where the is Clinical Supervisor(s) for the Poole General Hospital Dr Masding, Dr Coppini and Dr Poole The F1 s main duty is for care of medical inpatients as part of a multidisciplinary team. It is the F1 s duty to ensure they know all the patients current medical condition, where they are, to contribute to ward round decisions, to order and act upon appropriate investigations, to respond to changes in patients medical conditions, and to complete necessary patient correspondence. Whilst on call it is the F1 s responsibility to work with their team to clerk new medical conditions and to present on the post take ward round. At night it is the F1s responsibility to manage medical ward cover. 3 F1 - Cardiology Cardiology Department in Poole Hospital. This department consists of two cardiology teams. One is comprised of two consultants, a registrar, a ST grade and two F1s. The other consists of two consultants, one registrar, a ST and one F1 (this is the post outlined in the following job description). Work involves: Accurate medical note keeping on ward rounds Practical procedures within the FY1s competencies Assessing in-patients and devising a management plan - ensuring one asks for help/advice if needed History taking and examination of both elective and, when on call, acute admissions Interpretation of investigations such as blood tests and ECGs Writing referrals to a wide range of specialties Telephone communication skills Liaising with the multidisciplinary team to enable effective patient care Learning : A weekly journal club meeting on Monday at 1pm where a topic specific to cardiology is presented and discussed. The ECHO meeting on Wednesday at 1pm provides an ideal opportunity to learn the basics in the interpretation of ECHOs by listening to the discussion of interesting cases and their outcomes. Additionally, on Wednesday afternoons one of the cardiology consultants (Dr Blakemore) carries out teaching for the junior members of the team which is very useful in learning about the fundamentals of cardiology. The radiology meeting on Fridays at 12.00h provides exposure to various imaging modalities used in cardiology and helps to appreciate the application of imaging in cardiology Where the is Clinical Supervisor(s) for the Predominantly on the coronary care unit (accommodates 8 patients) and Cranborne (a 30 bed ward) but also entails working on other wards throughout the hospital depending on where patients are located Drs Blakemore/Boos/Bruce/Greaves Main duties involve involvement in the ward rounds of the cardiology in-patients and subsequently carrying out the jobs that are generated from this. Such jobs range from writing angiogram referrals to various practical procedures. Activities also include clerking in elective admissions and seeing daycase patients on the medical investigation unit. Working in cardiology also means that the FY1 is placed on the on call rota which involves helping your senior colleagues to clerk in the acute medical admissions. You are also required to come in for the post-take ward round the following morning and
4 present any patients you clerked in on the preceding day. 7 F1 General Surgery There are 7 consultants that F1s are attached to on general surgery, covering colorectal, upper GI and breast surgery. All consultants except the breast surgeon have an acute surgical on-call. F1 doctors all work as a team and cross-cover work from consultants during busy phases. expect and learning A normal day of work comprises care of your ward patients, assisting in theatre and assessing patients pre-op. There will be days when you are on call and take referrals from A+E and GPs. This is the greatest opportunity for learning as you are the first doctor to assess the patient (ages ranging 5 years and older) and initiate a management plan before senior review. If medical students are attached to surgery this is a good opportunity to teach by discussion around history, examination and management plan. 12 F1 - Trauma & Orthopaedics Department of Trauma & Orthopaedics, Poole Hospital When on a ward the FY1 will take responsibility for the daily medical management of ward patients with supervision from a SHO. This will include optimising patients for surgery. Whilst on-call there will be many to clerk orthopaedic presentations and consent for the appropriate operation with supervision from the SHO. Whilst on-call the main responsibility is to aid the SHO clerking, consenting and marking patients that are to be admitted. Ensuring that the patient is optimised for any surgery that may be required, together with their ongoing medical management. Post-take responsibilities are to attend the daily trauma meeting and complete the post-take ward round with a SHO and a consultant to ensure their management plan for the previous day/night admitted patients is completed. Together with assisting in theatre as required. Whilst on the wards for a week main responsibilities are to ensure completion of the surgeon s post operative plan.
