By attending this session, participants will obtain information and tools to assist in establishing, maintaining and evaluating an organizational performance management process. This session is meant to address PHAB Domain 9 to evaluate and continuously improve health department processes, programs and interventions. Continuing education is being sought for CPH, RS and RN. (Jack) Moran, Jr., MBA, PhD, CMC, CQM, Senior Quality Advisor, to the Public Health Foundation. Jack is a Senior Fellow at the University of Minnesota, School of Public Health in the Division of Health Policy and Management. He brings over 30 years expertise in developing quality improvement tools and training programs, implementing and evaluating quality improvement programs, and in writing books and articles on quality improvement methods. For 21 years, Dr. Moran was employed at Polaroid Corporation, working in various senior management capacities in manufacturing, engineering and quality. His last position was as the Director of Worldwide Quality and Systems. Page 2

Plenary Session This second plenary session will share examples of health-related information and/or campaigns using both the traditional visual media and newer social media technologies in both statewide and local perspectives. TUESDAY, MAY 20, 2014 Opening Remarks This session will provide an overview of current initiatives and insights on what might be expected of the Ohio Department of Health. Keynote Address This session will share national and regional strategies and examples of how data systems and technology are being used to improve the public s health and our ability to track changes in health status. Concurrent Sessions A. Student Oral Presentations this session will include presentations selected from among those submitted by students in various public health-related academic programs throughout Ohio. B. Shale Gas Development and Public Health Issues - this session will provide an overview of Utica and Marcellus Shale development with an emphasis on Public Health issues related to potential groundwater, surface water and soil contamination from drilling activities, potential induced seismicity and soil and water contamination issues from Class 2 injection activities. C. Mapping Data Systems Technologies, GIS this session will discuss the types of mapping systems available and how they can be used to monitor a variety of Public Health factors. D. Improving Maternal and Infant Outcomes in Ohio through Preconception Care this session will highlight how maternal and infant outcomes in Ohio can be improved through increased provision of preconception care. An interdisciplinary approach to preconception care will be emphasized, calling attention to professionals often underutilized in preconception care. E. Ohio s 5-Year Chronic Disease Plan LHD and CHIP Integration this session will discuss the role of Ohio s statewide chronic disease plan with local health departments and community health improvement plans. PHAST Public Health Activities and Services Tracking this session will engage participants around findings that depict relationships between local Public health services and specific population health outcomes as well as demonstrate longitudinal data showing variation an change in services for Ohio and other state partners. Opportunities for local Public Health data standardization will also be described. G. Faculty Oral Presentations - this session will include presentations selected from among those submitted by faculty in various public health-related academic programs throughout Ohio. H. Infant Mortality and Ohio Equity Institute, City-Match - this session will describe efforts in place by Ohio s Infant Mortality Coalition to reduce infant mortality rates and to address the inequities among these rates by geographic area and other factors. I. EHDSI System Update this session will describe the work of the group developing the revised EHDSI system and the next steps for its implementation. J. Research in Ohio Local Health Departments: Past and Future this session will provide an overview of recently completed work related to the Direct Observation of Local Public Health (DOLPH) project, and will describe the Cost of Public Health Practice in Ohio project currently ongoing. K. Shared Services this session will provide examples of how various local health departments have found ways to improve efficiency through the use of shared services. L. Using Age Progression Software in Delivering Health Education Messages: Focus on Skin Cancer this session will describe how the April AGE software program can demonstrate age progression with diseases such as skin cancer. General Session Looking at the Dashboard for Sailing your SHIP this session will share examples of different innovative approaches to conduct dashboard assessments in order to document progress in meeting the objectives of Local and State Health Improvement Plans. WEDNESDAY, MAY 21, 2014 Concurrent Sessions M. Ohio s Vector-borne Disease Surveillance and Prevention Plan - this session will describe the elements of Ohio s Plan; and will describe the emergence, clinical manifestations and prevention of tick-borne diseases in Ohio. Navigating Sources of Evidence-Based Prevention to Find What Works to Improve Health this session will help participants gain a common understanding of evidence-based practices and tools to identify effective strategies to prevent high-priority health problems. P. Promoting and Improving the Health of Ohio Adolescents this session will identify characteristics of adolescents that can be implemented to support health behavior and systemic change. Q. Billing Options CPT Code Updates, EHRs, Reimbursement, and More this session will present various solutions found to the challenges of obtaining reimbursement for Public Health services. R. Introducing STEADY U Ohio Preventing Falls One Step at a Time this session will introduce the STEADY U Ohio falls prevention initiative and help participants learn how they can help to reduce falls using evidence-based programs in Ohio. S. Accreditation Update Tools and Templates for Accreditation this session will describe the use of templates for local health department accreditation. T. Social Media in Crisis Communication this session will look at how the private sector has capitalized on the benefits of social media with its customers and will then examine how Public Health and emergency responders have used social media in crisis communication in past disasters. Session will also provide attendees with best practices of social media emergency management, downloadable and customizable templates for their social media accounts and resources and support for their social media efforts. U. Utilizing Social Media to Advance Health Communications and Outcomes this session will provide an overview of the types of commonly used social media in Public Health, with a discussion of the benefits and challenges of the most commonly used types. V. Public Health Law - this session will review current policy efforts in Public Health at the local and State levels. W. Tobacco Old Problem New Technologies - this session will share how current laws, rules and prevention strategies can be used and/or modified to address emerging technologies in tobacco use, and will share information about the Public Health impacts of e-cigarettes. X. Pharmacists Role in Public Health this session will indicate the role of the pharmacist in Public Health, including patient education, prevention of infant death related to over-the-counter medication use; medication adherence and reconciliation, and promotion of vaccination.
