3 Ecclesiology and Spiritual Renewal Program FOR HEALTH CARE LEADERS APRIL 19 24, 2015 Please join us in an important leadership formation experience: a week-long learning and renewal program in Rome for leaders of Catholic health care. I encourage you to consider this program for yourself and for the executive and governance leaders of your organization. This program offers an opportunity for us to experience the broader perspective of our health care ministry and to realize the worldwide nature of the Church s work an important perspective as we as leaders are at times in communication with various congregations of the Holy See. OUR PROGRAM INCLUDES: v Meetings with Vatican leaders at V The Congregation for the Doctrine of the Faith V The Congregation for Consecrated Life and Societies of Apostolic Life V The Pontifical Council for Health Pastoral Care v A general Papal Audience in St. Peter s Square v A one-day pilgrimage to nearby Assisi, the home of Saints Francis and Clare v Formative presentations by CHA staff and others highlighting the response to a call to lead in Catholic health care, personal spirituality, the workings of the Holy See and more v Celebrations of the Eucharist and personal reflection exercises v A commissioning ceremony for participants. Please be sure to read the remainder of this invitation for additional details, and share it with all those whom you will be inviting to participate. Also, please note that our deadline for registration is December 15 and that the number of participants is generally limited to about 40. 8 REGISTRATION FEES AND PROCESS Program Costs for Participants and Guests PARTICIPANTS: For participants, the cost of this program is $8,750. The complete package for participants includes: v Six nights at the Marriott Grand Hotel Flora (including American buffet breakfast, service charges and tax). Those six nights are: April 19, 20, 21, 22, 23 & 24, v Three dinners and three lunches. v Motor coach transportation in Rome to Sant Egidio community, dicastery visits, papal audience and St. Peter's Basilica. v Motor coach transportation to/from Assisi. v Transport to/from airport or train station. v All workshop materials. REGISTERED GUESTS: For registered guests, the cost of the program is $2,250. The package for a registered guest includes: Please Note: Should you select to travel with a guest, and not register him or her, your guest will be on his or her own. This includes getting themselves to/from the airport and all meals. Your guest will not be registered to attend any off-site events (i.e., papal audience, St. Peter's Basilica, day in Assisi). CANCELLATION FROM THE PROGRAM CHA is required to pre-pay all program costs by March 23, As a result, no refunds will be offered for any program cancellations made after March 23. There is no cost to send a substitute. Please contact Adele Gianino at (314) or agianino@chausa.org to register a substitute. CANCELLATION OF SLEEP ROOMS Many participants choose to arrive early or depart several days after the program. CHA is happy to make reservations for additional nights of stay at the program hotel. However, once those reservations are made, should you need to make any modifications, please do so on/before April 3, Any rooms that are cancelled after April 3 will be charged to the registrant at the CHA contracted room rate. CHA will bill the registrant directly for these cancelled room nights approximately six weeks after the program concludes. v Daily (5) American buffet breakfasts, two lunches and three dinners. v Motor coach transportation in Rome to papal audience and St. Peter's Basilica. v Motor coach transportation to/from Assisi. v Transport to/from airport or train station.
9 2015 Registration Process for Participants and Registered Guests Registrations are now being accepted for the 2015 program. To register, you must call CHA as we will only accept phone calls to register individuals and groups. Please call Adele Gianino, CHA director of meetings and travel, to register; she can be reached at (314) Please leave a voic if she is unavailable. IMPORTANT REGISTRATION NOTICES: v Space is limited to approximately 40 participants. v Those 40 persons and their guests will be registered on a first-come, first-served basis by calling Adele Gianino. v The deadline to register is December 15, 2014, however, because of the popularity of this program, we suggest you make your plans and register as soon as possible. v Once the 40 spots have been filled, CHA will compile a wait-list. PLEASE HAVE THE FOLLOWING INFORMATION AVAILABLE WHEN YOU CONTACT ADELE GIANINO: v Contact Person if more than one person attending from the organization V Name and contact information v For the Participant(s): V Name, with all relevant prefixes and suffixes V Preferred name for badge V Title in health care (example: Trustee, CEO, etc.) V System or facility name and city/state/zip V Personal addresses. v For a Guest for Each Participant: V Name, with all relevant prefixes and/or suffixes V Preferred name for badge IMPORTANT PASSPORT INFORMATION While not required at the time of registration, a passport number and exact name included on the passport, or a copy of the application for a passport for those who do not yet have one, will be required soon after registration. For those who do not yet have a passport or need to renew participants or guests please note that there is a week wait for application and receipt. 10 WHAT TO EXPECT AFTER REGISTRATION February 2015 v Registration Fees January 2015 v Registration Invoices Mail FIRST WEEK IN JANUARY The first week of January 2015, CHA will mail each contact person an invoice. It will include a listing of all attendees participants and registered guests and the payment amounts for each individual. Payment should be received by February 13, 2015, in order to confirm registration. v Request for Participant and Registered Guest Items SECOND WEEK OF JANUARY The second week of January, CHA will request by several required items that are due to CHA by February 20. They include: v A (head and shoulders) photo (jpeg) of each participant, as well as final contact information and titles for all registrants. v A copy of each participant and registered guest s passport the opening page with name, and passport number. v A copy of travel itineraries for each participant and registered guest. v Pre-Read Preference. One of the books used in the program is available as an e-book. We need to know participants preference for e-readers. DUE ON FEBRUARY 13 Registration will be forfeited if payment is not received by February 13 unless prior arrangements with CHA have been made. v Participant Photos and Participant and Registered Guest Passport Information and Itineraries DUE ON FEBRUARY 20 March 2015 v Mailing of Program Materials MARCH 9 CHA will mail each registrant her or his materials (electronic copies of some materials are also provided at this time) 6 weeks ahead of the program, on or about March 9. v Webinar: Know Before You Go MARCH 19, 2015 NOON 1 P.M. ET This webinar will offer information about the program that will help all attending better understand and prepare for the week of activities. It is very important that everyone who is traveling to Rome attend. 11 LOGISTICS Hotel The program is being held at the Marriott Grand Hotel Flora right on top of the Via Veneto. The hotel is: v Located in the heart of Rome within a few minutes walking distance from the Spanish Steps. v Equipped with high-speed Internet access and a spacious work desk for the business traveler. A CHA master account has been established to cover room and tax for the nights of April 19, 20, 21, 22, 23 & 24. You will be responsible for any additional nights and all incidentals, which must be settled upon checkout.
