4 Welcome to My Wellness at Work The Hartford s My Wellness at Work program makes it easy for you to create and maintain a healthy lifestyle. These programs are designed to empower you to make smarter decisions about your health and to connect you with the information and resources you need to maximize your health and quality of life. To learn more and get started, register on the My Wellness at Work portal ( 4

5 YOUR GOAL: EARN A TOTAL OF 40 POINTS BETWEEN OCTOBER 1, 2017 AND SEPTEMBER 30, 2018 (10 EACH QUARTER) When you participate in wellness activities, you will earn wellness points. If you are enrolled in one of The Hartford s health plan and earn at least ten new points during a calendar quarter, you will receive a $150 healthy lifestyle credit toward your health premiums the following calendar quarter throughout 2018, for a total premium credit opportunity of $600. "Unused" wellness points carryover from one quarter to the next. For example, if you earn 14 points one quarter, 10 will be used toward premium credit the next quarter and 4 will carryover. To receive premium credits, you must complete the well-being assessment which is available on the My Wellness at Work portal ( This is the only required activity; you select which other activities you want to do to build your pathway to wellness. 6 HERE S HOW IT WORKS Each activity shows the number of wellness points that you can earn. Upon completion of the activity, your wellness points are awarded. You can track your progress on the My Wellness at Work health portal. You can log your activity online using your computer or personal smartphone or tablet on the mobile-friendly portal. 8 Welcome Get Started to My by Completing Wellness at Work Your Well-being Assessment! Because we care about your health and well-being, we are offering an assessment to help you (and your dependents if they are covered by The Hartford s health plan) learn about the factors that affect your overall health and well-being. The well-being assessment (WBA) which is available beginning in January 2018 on the My Wellness at Work portal is administered by Optum, a leader in the delivery of healthcare management programs. The results of your WBA will help you learn more about ways you can improve your physical, emotional, social, spiritual and financial well-being. Your results will also allow Optum to tailor your activities on the portal to your individual needs. 8

9 COMPLETE YOUR WBA Beginning in January, 2018 log on and register on the My Wellness at Work portal ( You will see the WBA module on the home page. The WBA is confidential; your answers will not be shared with The Hartford. Please respond to the questions as candidly as possible to ensure your results accurately reflect your well-being. It should only take 15 to 20 minutes and, once you re done, you will immediately see your results. Please note: Completing your WBA is a required activity to earn premium credits. Even if you earn ten wellness points by participating in other activities, you will not receive premium credits after the first quarter of 2018 until you complete the WBA. The WBA will be available on the My Wellness at Work portal until you complete it. You will receive a premium credit the quarter after you complete the WBA. Retroactive credits will not be provided. Remember, to receive any credits toward your 2018 medical premiums after the first quarter, you will need to complete the WBA. 9

11 THIS YEAR up to 15 wellness points can be earned based on your health screening results. If you participate in the company-sponsored health screening program and have your screening by the Nov. 3, 2017 deadline*, you can earn three wellness points for each of the five measures below that fall within the healthy range (up to a total of 15 additional wellness points). 16 Welcome Health to My Coaching Wellness at Work Qualified participants can work with a personal coach! Successful people from athletes to actors use coaches to help them reach their goals. So why wouldn t you use one too? 16

17 Whether you want to fit into your skinny jeans, play with your kids without getting winded or boost your confidence and get healthier enrolling in Health Coaching can help make it happen. No goal is too small or too big. YOUR PERSONAL COACH CAN HELP YOU: 1 Lose weight 2 Eat healthier foods 3 Be more active 4 Gain energy 5 Ease stress If the results of your WBA indicate you would benefit from working with a Health Coach, you will have unlimited access to your personal coach by phone or for 12 months. 19 IMPROVE YOUR HEALTH WITH A VIRTUAL COACHING PROGRAM Reach your health goals, big or small, working at your own pace with a virtual coach online! Choose your focus area like improving your diet, managing stress, getting more physically active, and more. Set your goal and get started with healthy steps you can take to reach it. You ll find all sorts of fun tools such as healthy tips, meal planners, recipes, and quizzes. Log in to the My Wellness at Work portal and complete at least one healthy action each week in six different weeks and a check-in assessment to reach a milestone. The tracker on your home page helps you manage your progress. You will get four wellness points when you finish a milestone, and you can complete up to three for a total of 12 points. The action plan helps you transition to and from phone based coaching programs by maintaining a consistent action plan and completed progress. 19

21 If the results of your WBA indicate you would benefit from an additional level of support, the Weight Talk program is built just for you. A dedicated coach will work with you one-on-one to guide you every step of the way. You ll learn how small changes in daily food choices and eating habits can make a big difference when it comes to dropping extra pounds. Throughout the six-month program, our coaches will help you lose weight and keep it off. You will schedule regular calls with your coach. Together you will set realistic goals and make a plan to reach them. You ll also have access to an online program and workbook. The Weight Talk program includes the following strategies for longterm weight loss: Personalized eating plans Weight, food and exercise trackers Tips for a healthier environment Quick wins for addressing barriers Ideas for managing stress With Weight Talk, you ll never be alone. Your coach will be there whenever you need support. You will also earn 10 wellness points when you complete the coach check-in calls along with a status assessment. 22 The Quit For Life Program Take the first step toward quitting today. The Quit For Life Program is the nation s leading tobacco cessation program. It employs an evidencebased combination of physical, psychological and behavioral strategies to help you overcome your addiction to tobacco. Together you and your coach will create an easy-to-follow Quitting Plan to help you get ready, take action and live the rest of your life tobacco free. As an alternative to the Quit For Life Program, all employees and covered dependents can enroll in the Live Tobacco Free online health coaching program. It provides online support and motivation to quit smoking through It is confidential and can be accessed anytime 24/7. To get started: 1. Log on and select the Coaching link then choose the Live Tobacco Free focus area and get started. 2. Work through your To-Do list each week. 3. Complete the final assessment to receive 4 wellness points (up to a max of 12 for online coaching programs). 22

23 EMPLOYEES AND THEIR COVERED DEPENDENTS ENROLLED IN THE HARTFORD S HEALTH PLAN HAVE ACCESS TO THE QUIT FOR LIFE. WHEN ENROLLED, YOU WILL BENEFIT FROM THESE PROGRAM FEATURES: Quit Coach Staff Coaches will support you over the phone whenever you need it. They are experienced in helping to overcome the struggles people face to beat tobacco for good. You can call your Quit Coach staff member as much as you want for 12 months. Quit Guide. You will receive an easy-to-use printed workbook you can reference in any situation to help you stick with your Quitting Plan. Web Coach Website. Enjoy lifetime access to a private online community where you can complete activities, watch videos, track your progress and join in discussions with other program participants. 10 Wellness Points. Earn credit when you complete at least five designated calls with a Quit Coach staff member and a six-month survey. Quitting Aids. Your coach will help you determine which type, dose and duration of nicotine substitute or medication is right for you, and teach you how to use it so it really works. Plus, you can get up to 8 weeks of gum or patch therapy sent directly to your home. Text2Quit SM. In addition to calls with your Quit Coach staff member, you will receive supportive text messages on your mobile phone to help you prepare to quit, use medications correctly, manage urges and avoid relapse. With healthy eating, it can help prevent a range of chronic diseases, including heart disease, cancer and stroke. Physical activity helps to control weight, builds lean muscle, reduces fat, promotes strong bones, muscle and joint development, and decreases the risk of obesity. 26

27 MAKE FITNESS FUN Virgin Pulse is a program designed to get you moving by providing an easy and fun way to encourage, measure and reward physical activity. If you enroll in the program you will: Receive a subsidy to purchase a Virgin Max pedometer at no cost or to use towards the purchase of another compatible device Be able to easily upload and track your daily steps and other activities on the Virgin Pulse website Have the chance to participate in challenges Earn from two to eight wellness points each quarter The program includes the tools, information and tracking methods you need to begin or maintain a program of regular physical activity. Most importantly, it makes fitness fun. You ll even have the opportunity to challenge your co-workers in a little friendly competition. HOW IT WORKS You can participate if you are a benefits-eligible employee. You don t have to be enrolled in The Hartford s health plan. Here s how you get started: Sign up by logging on to You will receive a subsidy to order a free Max pedometer or to use towards the purchase of another compatible activity tracker. Activate your pedometer. Then get moving. Go for a run, walk the dog, play with the kids, dance whatever activity you enjoy. Upload your steps as often as you like, but at least every two weeks. You can also selfenter other activity in your online Activity Journal on the site. Earn Virgin Pulse points. As you upload your steps and your activity accumulates, you ll earn Virgin Pulse points. The more active you are, the more Virgin Pulse points you will earn. You can also earn Virgin Pulse points by participating in individual, team and company-wide challenges. Track your progress. On the site, you can set goals, watch your points add up and move through different activity levels as you become more and more active. Sign up by logging on to 27

28 EARN WELLNESS POINTS As you accumulate points each quarter, you will move up the defined activity levels earning more wellness points. For example, if you accumulate 6,000 points at the end of a calendar quarter, you will receive four wellness points on the My Wellness at Work portal toward the ten needed to receive the next quarter s premium credit 1. 1 To be eligible to receive a premium credit, you must be enrolled in The Hartford s Health plan and complete the online Well-being Assessment. 28

29 FITNESS REIMBURSEMENT PROGRAM The Fitness Reimbursement Program allows employees of The Hartford to receive reimbursement of up to $90 per quarter towards the cost of certain fitness-related activities including a membership in a fitness centers, health clubs or aquatic centers, as well as session fees for such activities as group/personal training, Pilates, yoga instruction, etc. when offered by a certified instructor. HOW IT WORKS: To receive fitness center membership reimbursement, you must follow all of these steps: If you are not already participating in a fitness activity, get started. Enroll in Virgin Pulse by logging on to join.virginpulse.com/thehartford Earn at least 3,000 points (Level 1) by midnight ET on the last day of the each calendar quarter. Complete the Online Fitness Reimbursement Form, which is available on the Virgin Pulse member website ( by the 5th calendar day after the close of the calendar quarter for which you are requesting reimbursement. Submit your completed online form along with proof that you have paid your program fees for the preceding three months. You will receive a confirmation from Virgin Pulse. Be sure to retain this for your records. Within 30 days of the end of the quarter, you will receive an letting you know your reimbursement is available to redeem on the Virgin Pulse member website. Log in to the member website and redeem your reimbursement in the form of a gift card or as a direct deposit into a bank account. 30 Employee Assistance and Work/Life Program Solutions for balancing work and family life. Employees and their dependents don t have to wait until problems turn into crises to call the Employee Assistance and Work/Life Program (EAP) for help. This program offers a variety of tools and services to help you manage life s every day challenges. 30

31 1 2 THE PROGRAM HAS TWO COMPONENTS. The EAP program offers confidential* counseling by a licensed clinician to you and your family members on a wide range of topics. Employees and dependents receive five free counseling visits per problem per year. The Work/Life component offers a number of resources to help you deal with child and eldercare issues, and includes both legal and financial counseling services as well. Services are provided by OptumHealth. In Connecticut, onsite EAP services are available and provided by Wheeler Clinic. Optum s website, liveandworkwell.com, provides extensive and valuable information, an online library of work/life articles, interactive discussions, and a host of prevention and wellness resources. 33 Get More Sleep, Get More Out of Life Like eating a healthy diet and exercising regularly, getting enough sleep is essential to your well-being. Lack of sleep can affect how much energy you have, your appetite, your physical and emotional health and your relationships at work and at home. Sleepio can help. The first step is to discover your personal Sleep Score by taking a short online quiz. It only takes a minute and based on your score, you will get a personalized sleep report and sleep tips. If you need additional help to improve your sleep, you can sign up for the six-week, online sleep improvement program to learn ways to change your daily schedule, bedroom, lifestyle and thoughts to improve your sleep. Complete the online sleep quiz and earn two wellness points. Complete all six weekly sessions and you will earn six wellness points. You may only complete the Sleepio program one time. Because the quality of your sleep can impact or be impacted by your partner, the program is also available to employees spouses or domestic partners. Stress Less, Live More will help you learn mindfulness techniques to calm your mind and reduce your stress. You may also enjoy the benefits of increased energy, creativity, focus and concentration. Delivered by emindful, this evidence-based program has helped thousands of participants learn sustainable skills and successfully reverse stress-related health problems, improving their quality of life. What is Mindfulness? It s bringing complete attention to your present experience on a moment-to-moment basis. Doing so empowers you to recognize your behavior and make positive choices for improved health and happiness. Through 10 weekly, 5 biweekly, or 12 monthly sessions, led by an instructor in an online classroom, you ll learn how to train your mind to be present in the moment. You ll also learn how to improve your interpersonal relationships (both at home and work), your mood and your immune system function while potentially lowering health risks, such as high blood pressure and overeating. 35 CONSUMERMEDICAL ConsumerMedical provides employees and their dependents with reliable, current and personalized information and support with any health-related concern or issue. During difficult times, spending hours on the Internet sifting through conflicting and outdated information can be overwhelming, confusing and not very productive. ConsumerMedical provides the latest research, available treatment options and things to consider for any medical diagnosis or health topic, so you feel confident in the medical decisions you make for yourself and your family. If you or one of your dependents is considering a certain significant surgery, the Surgery Decision Support program can help you learn about treatment options, the benefits and risks, and how to choose a surgeon and hospital to get the best outcome. Employees and their covered dependents are required to participate in the Surgery Decision Support program if they are having any of the following surgeries on an elective basis: hip replacement, knee replacement, bariatric (weight loss) surgery, back surgery, or hysterectomy. Failure to complete the program will result in a $1,000 penalty at the time the claim(s) is processed. The program is designed to help participants lose weight and decrease their risk factors for metabolic syndrome. First time participants enroll in the Foundations class and those who have completed Foundations can enroll in the skills maintenance program called NS4You and NS4 Life. Designed and administered by experts in the field of nutrition, medicine and psychology, the Naturally Slim curriculum teaches participants how the habits they have formed over the course of their lives can be reversed. The program consists of a series of ten, weekly online informative videos with each video building on the previous one. Employees can earn ten wellness points for meeting the completion criteria of each class, for a maximum of 20 wellness points a year. 40 Frequently Asked Questions WELL-BEING ASSESSMENT Learn more about the WBA and My Wellness at Work health management programs. WHAT IS THE WELL-BEING ASSESSMENT? To help you identify and understand your personal health risks, The Hartford is offering an annual online well-being assessment (WBA) through Optum a leader in the delivery of healthcare management programs. The WBA is a series of health and lifestyle related questions that will take approximately 15 minutes to answer. The results of your WBA will help you learn more about your physical and emotional health as well as actions you can take to improve your overall well-being. Your results will also allow Optum to tailor your activities on the My Wellness at Work portal to your individual needs. WHY SHOULD I PARTICIPATE? Here are five good reasons to complete the My Wellness at Work well-being assessment (WBA): 1. It s quick. It should only take about 15 minutes to complete the online questionnaire. 2. It delivers immediate results. Within seconds of completing the questionnaire, you ll have your results. 3. It provides personalized information. Your WBA results will give you information about your current health status and identify your risk factors. These risk factors often have no symptoms. You can proactively identify risk factors which can lead to chronic illness. Obtaining this information will show you the areas where you can make a lifestyle change or seek treatment to avoid serious illness. 4. It is your gateway to new wellness programs. By completing the My Wellness at Work WBA you will be eligible to participate in no-cost lifestyle and disease management programs. 5. Last, but not least, you ll save money! If you are enrolled in The Hartford s health plan, you will want to complete the WBA so you can earn health premium credits in

41 HOW DO I ACCESS THE WBA? The WBA is available beginning in January, 2018 on the My Wellness at Work portal, which you can access from iconnect or directly at If you previously registered on the site, you will use the same login ID and password you created when you registered. If you do not remember the login credentials, use the links in the Sign In box on the homepage. Once you are logged in, you will see the WBA in the center of the page. If you have not yet registered on the site, follow the steps to do so. Once you are registered, you can log in to access the WBA. WHAT IF I CHOOSE NOT TO TAKE THE WBA? Completing the WBA is voluntary; however, taking the WBA is required to earn any premium credits after the first quarter of Even if you earn ten wellness points by participating in other activities, you will not receive any premium credits until you complete the WBA. The WBA will be available on the My Wellness at Work portal until you complete it. You will receive a premium credit the quarter after you complete the WBA. Retroactive premium credits will not be provided. WILL MY PERSONAL HEALTH INFORMATION IN THE WBA BE SHARED WITH THE HARTFORD? No personal health information is ever shared with The Hartford. The Hartford may receive aggregate de-identified data in the form of reports. Using these reports, The Hartford can develop health and insurance initiatives that are most helpful to its employees. HOW IS MY WBA DATA USED AND PROTECTED? Your WBA responses are used to provide you with a customized feedback report based on your specific health risks and needs. This data may be used to identify optional health and wellness programs relevant to your health needs. Certain information you provide becomes protected health information under the Health Insurance Portability and Accountability Act (HIPAA) and may only be used or disclosed without your authorization for purposes of treatment, payment or health care operations. For example, a use or disclosure for purposes of health care operations would include use of the information by a disease management vendor, to provide services to employees of The Hartford. WHY AM I NOT ABLE TO FILL IN MY HEALTH RESULTS ON THE WBA? If you participate in The Hartford s health screening program during the fall, your results will automatically be populated in your WBA. If you chose not to participate in the company-sponsored program, no results will appear and you will not be able to enter any values. To ensure the integrity of the information, self-reported screening results will not be accepted. However, even without biometric values, you can still complete the WBA, receive your results (though they will not be as comprehensive), participate in the wellness programs, and earn a premium credit. 41
