Am I eligible to participate? Employees who are eligible for the In-Area Employee AHS PPO Plan or Out-of-Area Employee AHS PPO Plan by 8/15/2017 can participate. You must enroll in either Atlantic Health System medical plan by 1/1/18, remain enrolled and actively employed to receive the 2018 reward. COBRA, retirement and PD plans are not eligible. 4 Wellness Visit Reward: $200 Partner with your health care provider to maintain and improve your health! By completing your wellness visit, and submitting your Wellness Form by December 15, you are eligible to earn the $200 wellness visit reward. Get started now! Register and download your Wellness Form at or call Bravo at for assistance. Complete your wellness visit, lab work and Wellness Form with your healthcare provider. BIOMETRIC MEASURES: Height, weight, waist circumference and blood pressure. LAB MEASURES: Full cholesterol panel, fasting glucose & Hemoglobin A1c and cotinine (nicotine). Note: If you are on the AHS PPO medical plan, you may complete your wellness visit at any time during the calendar year. It does not have to be one year from your last wellness visit. See the Compass for a list of Atlantic Health System labs. Submit your completed Wellness Form to Bravo by December 15. Ensure your form is complete all fields are required. Your form may be faxed to Bravo at , ed to forms@bravowell.com, or uploaded to your Bravo account at bravowell.com/atlantichealthsystem. Confirm Bravo has received your Wellness Form. It is your responsibility to ensure that Bravo receives your form by the deadline. You will receive an within 48 hours of form submission indicating they have received your form for processing. If you do not receive an response from Bravo within 48 hours, please contact them to confirm they received your form. You must complete a wellness visit and submit your form to Bravo by December 15 in order to be eligible to earn any rewards. Partner with your health care provider! 5 Health Improvement Reward: $100 Your health matters! Bravo will review your Wellness Form, and you will automatically earn the $100 Health Improvement Reward if: You meet all standard health goals. 7 Bravo Online Health University Courses HOW DOES IT WORK? The online interactive programs simulate a live, oneon-one session with an expert instructor. View the online programs where and when convenient to you, along with optional educational materials and a weekly internet radio support group. Courses typically take six to eight weeks to complete and must be finished by December 15, How to access the Online Health University 1. Login to your Bravo account at bravowell.com/ atlantichealthsystem 2. Wellness Tools >Access Online Health University >Get Started. This will take you to the program page. Click to agree to the terms, and then click Start Now. 3. Enroll in any course by clicking Click Here to get started, and schedule your first session. IMPORTANT DEADLINE! Courses can be started at ANYTIME and take six to eight weeks each, so start in enough time to complete them by December 15, If you have questions, contact Bravo for more information at AHS Wellness Program Challenges HOW DOES IT WORK? AHS Employee Wellness is offering six health engagement challenges (listed above) throughout the year. Registration details and deadlines will be provided at the beginning of each quarter to all employees via and on the AHS Compass. You must sign up for each challenge individually during designated registration periods. Once registration is closed you cannot join the challenge. If you register for a challenge, but do not complete all challenge requirements, you will not earn credit towards earning the reward. 8 Results and Rewards It s up to you how much you earn in rewards. Take charge of your health and earn up to $500! How do I know what rewards I ve earned? After submitting your completed Wellness Form to Bravo, you will receive a results letter in the mail in approximately 2 to 4 weeks. This letter will come from Bravo and will be marked time sensitive on the envelope. Be sure to open and review this letter as it contains important information about your reward status, and there may be additional action required on your part to earn the rewards. The letter will also be available on your Bravo portal. You may select the paperless option on the portal. Your Results Letter will include: 2017 wellness visit results, as documented on your Wellness Form. You must complete a wellness visit and submit your Wellness Form to Bravo by 12/15/17 to be eligible to earn any rewards. Rewards you ve earned to date, which may include: --$200 Wellness Visit Reward for a complete Wellness Form submission. You can submit your Wellness Form at any time, as long as it is received by December 15, $100 Health Improvement Reward if your Wellness Form shows you met all standard health goals OR showed improvement in one category. New participants who don t meet the standard health goals on their Wellness Form may complete a Health University course to earn the reward. Contact Bravo for more information. --$200 Health Engagement Reward if you completed 2 Online Health University courses and 2 AHS Wellness Program Challenges, and also submitted your completed Wellness Form by December 15, You can get started on the Health Engagement Reward (the online courses and AHS challenges) anytime, before and/or after you submit your Wellness Form. All program requirements must be completed by 12/15/17. An updated results letter will be mailed to your home upon completion, indicating you have earned the $200 Health Engagement Reward. --Appeal information and deadline, if you would like to dispute or re-submit your results. 9 FAQs PRIVACY CAN MY EMPLOYER REWARD ME BASED ON MY HEALTH STATUS? Yes. Bravo administers programs in compliance with the Affordable Care Act s (ACA) Incentives for Non-Discriminatory Wellness Programs in Group Health Plans as well as recent rules added to the Americans with Disabilities Act (ADA). This allows employers to give cash incentives to those employees who complete participation in wellness incentive programs. IS MY PRIVACY PROTECTED? WILL ATLANTIC HEALTH SYSTEM SEE MY RESULTS? Bravo Wellness administers the program on behalf of Atlantic Health System to protect your privacy. Bravo will never share your individual results with Atlantic Health System. Atlantic Health System will only know if you earned rewards for next year (2018). Bravo adheres to the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and Protected Health Information (PHI) protocols. This ensures that your personal information, including your results, will not be shared with any unauthorized parties, including your employer. WHAT ARE MY RESULTS USED FOR? Your specific results are important information for you and your health care provider to review so you can track your health and develop a wellness plan. Although Atlantic Health System does not receive individual results, Bravo provides a group report of results each year to help guide the programs and resources needed for employees to improve and/or maintain their health. Your individual results will not be shared with Atlantic Health System. ELIGIBILITY AND PARTICIPATION AM I ELIGIBLE TO PARTICIPATE? If you are eligible for Atlantic Health System medical benefits ( In-Area Employee AHS PPO Plan or Out-of-Area Employee AHS PPO Plan ) on or before August 15, 2017, and plan to enroll in the medical benefits for 1/1/2018, you are eligible to participate. DOES EVERYONE HAVE TO PARTICIPATE? No. Participation in this program is voluntary. If you do not participate, you will not receive the wellness reward. HOW OFTEN WILL I NEED TO PARTICIPATE IN THE PROGRAM? This is an annual program, so if you choose to participate in order to receive the reward, you must complete the program requirements each year. SHOULD FAMILY MEMBERS ON MY ATLANTIC HEALTH SYSTEM MEDICAL INSURANCE PARTICIPATE IN THE PROGRAM? No. This program is currently for employees only who are eligible for Atlantic Health System s medical benefits, and plan to enroll in CAN I PARTICIPATE IN THE PROGRAM IF I AM ON LEAVE OF ABSENCE? Yes. If you are eligible for Atlantic Health System medical benefits and plan to enroll for 2018, you are eligible to participate in the program. WHAT IF I HAVE A MEDICAL CONDITION AND CANNOT PARTICIPATE IN THE PROGRAM? If participation in this program is unreasonable or medically unadvisable, you might qualify to earn the same reward by different means. Bravo will work with you, and if you wish, your physician, on an alternative way to qualify. If you have any other questions, please contact Bravo Wellness Customer Service at I AM PREGNANT. CAN I PARTICIPATE IN THE PROGRAM? Yes. You are still eligible to participate if you are pregnant. There is a check box on your Provider Form to indicate that you are currently pregnant if you wish to disclose that. It is likely that your values may be abnormal due to pregnancy, and should be discussed with your health care provider. If your pregnancy affects your wellness visit results, you may work with Bravo and your physician on an appeal to earn the same reward through different means. 9
10 I AM RETIRING. SHOULD I PARTICIPATE? No. The retirement medical plan is managed separately from the Atlantic Health System medical benefit plan. The Bravo program does not apply to those on the Retirement plan. I AM A NEW HIRE. CAN I PARTICIPATE IN THE PROGRAM? Yes, if you are actively employed and eligible for the In-Area Employee AHS PPO Plan or Out-of-Area Employee AHS PPO Plan on or before August 15, 2017, you are eligible to participate. I AM ON COBRA. SHOULD I PARTICIPATE? No. The COBRA medical plan is managed separately from the Atlantic Health System medical benefit plan. The Bravo program does not apply to those on the COBRA plan. I AM ON THE PER DIEM MEDICAL PLAN. SHOULD I PARTICIPATE? No. The Per Diem Medical Plan is not eligible to participate. FORM SUBMISSION HOW DOES BRAVO WELLNESS RECEIVE MY WELLNESS FORM? Completed and signed Wellness Forms can be uploaded to your Bravo account at bravowell.com/atlantichealthsystem, or mailed, faxed or ed to: Bravo Wellness One International Place Emerald Parkway Dr. SW, Suite 400 Cleveland, Ohio Fax: forms@bravowell.com It is your responsibility to confirm receipt of your form. SHOULD I SUBMIT MY WELLNESS FORM OR SHOULD MY HEALTH CARE PROVIDER? It is recommended that YOU submit the form to Bravo Wellness. It is your responsibility to make sure the form is complete, signed, and that Bravo Wellness receives the form no later than December 15, Do not rely on your health care provider s office to submit information on your behalf. It is ultimately your responsibility to ensure Bravo receives your form on time. It is also recommended that you follow up with Bravo Wellness after submitting your form to confirm receipt. If you or fax in your form, please retain the record that you sent it in. WILL I GET CONFIRMATION THAT BRAVO WELLNESS RECEIVED MY WELLNESS FORM? Yes. If you provided an address during Bravo registration, Bravo will automatically you when your form has been received within 48 hours. You may also call Bravo to ask if they received it. If you do not receive communication from Bravo, call them right away. It is your responsibility to ensure Bravo receives your form on time. Confirmation of receipt does not confirm that you will receive the rewards, just that your form has been RECEIVED. If you have not received confirmation of receipt via within 48 hours of submitting your form, or you are concerned that your form was not received, please call Bravo at DOES SUBMITTING MY FORM MEAN I WILL EARN THE FULL $500 REWARD? Submission of a completed Wellness Form will earn $200. You may choose to complete additional program requirements to earn up to $500 total in DO I HAVE TO BE CURRENTLY ENROLLED IN THE ATLANTIC HEALTH SYSTEM MEDICAL BENEFITS PLAN TO SUBMIT A FORM? No. If you are eligible for medical benefits, and think you may enroll for 1/1/2018, you may submit a form to Bravo. You would utilize your current medical plan, lab, and physician to complete the form. Your wellness visit and any associated lab work will be subject to any costs as outlined by your current medical insurance carrier. Personal expense may be incurred. HOW WILL I KNOW IF I COMPLETED THE PROGRAM AND WILL RECEIVE THE REWARD? A results letter will be mailed to your home within 2 to 4 weeks of submitting your form to Bravo Wellness. The letter will contain your results and current reward status. You may receive more than one results letter if you complete components of the program at different times throughout the year.* If you haven t received your results letter within that time frame, please contact Bravo Wellness. By completing the 2017 program requirements, you will earn up to $500 in 2018 wellness rewards. *You may also select the paperless option on the portal. 10
11 WHAT SHOULD I BRING WITH ME TO THE APPOINTMENT? Do not forget to bring your Wellness Form with you to your appointment. You and your health care provider are required to fill out this form in full and sign it. Be sure your form is 100% complete before you submit to Bravo Wellness. This form can be obtained through your Bravo web portal at bravowell.com/ atlantichealthsystem. LABWORK, VISITS, AND PHYSICIANS WHO CAN COMPLETE MY PROVIDER FORM FOR ME? Your health care provider should complete and sign the form during your annual wellness visit. Your health care provider must be an MD, DO, PA, NP, or APN. WHAT TYPE OF APPOINTMENT AM I SCHEDULING? When calling your health care provider s office to schedule your appointment, tell them you would like to schedule an annual wellness visit. This is the frequently used industry term and your health care provider s office should be able to easily accommodate your request. Under the Atlantic Health System medical plan, this visit can be scheduled once per calendar year. It does not have to be one year to date from your last wellness visit. WHAT TYPE OF LAB WORK SHOULD I REQUEST? Your health care provider s office should provide you with a prescription for all the required tests: a standard fasting lipid profile, fasting glucose, hemoglobin A1c, and a cotinine (nicotine) test. For the most accurate results, fasting 10 to 12 hours prior to your health screening lab work is required. Note that all required lab work may be completed at no cost to you at any Atlantic Health System lab if you are currently covered under the Atlantic Health System medical plan. The Atlantic Health System labs have a special code that includes all required Bravo lab tests; BRV16. WHERE SHOULD I GET MY LAB WORK DONE? If you are currently on the Atlantic Health System medical plan, you can complete your lab work at any of the Atlantic Health System labs free of charge to you. CAN MY POST-HIRE PHYSICAL BE USED TO MEET THE REQUIREMENTS OF THE PROGRAM? As part of your post-hire physical, OMS will order your lab work including nicotine, however, OMS will not fill out your Wellness Form. The results of your lab work can be forwarded to your identified health care provider upon request, to be reviewed during your annual wellness visit. If you need assistance finding a health care provider, see page 4. DEADLINES, MISSING INFORMATION AND APPEALS WHAT IS THE DEADLINE TO PARTICIPATE? You must complete all program components by December 15, 2017 to earn up to $500 in wellness rewards in Please note: Wait times for scheduling a wellness visit with a health care provider can be lengthy. If you are planning on participating, do not wait to schedule an appointment and risk missing the submission deadline. LATE FORMS WILL NOT BE ACCEPTED. WHAT IF MY FORM WAS MISSING REQUIRED INFORMATION? If your Wellness Form is incomplete, you may provide the missing information by the appeal deadline indicated on your results letter. You must provide the missing information using the appeal form, and include any lab documentation, by that appeal deadline. WHAT IF I DO NOT AGREE WITH THE RESULTS ON MY FORM? Appeal forms may be submitted with supporting documentation by the appeal deadline. Any retesting may be at the expense of the participant. Call Bravo at for help with the appeals process. 11
12 HEALTH ENGAGEMENT WHEN IS THE LAST DATE I CAN START AN ONLINE HEALTH UNIVERSITY COURSE IN ORDER TO COMPLETE IT BY DECEMBER 15, 2017? Each course takes approximately six to nine weeks to complete. It is suggested you start all courses no later than 10/13/17 to ensure you meet the December 15, 2017, deadline. HEALTH IMPROVEMENT WHAT IF I DO NOT MEET ALL FIVE STANDARD HEALTH GOALS, OR SHOW A DEFINED IMPROVEMENT IN ONE CATEGORY AT MY WELLNESS VISIT, OVER MY 2016 BRAVO RESULTS? You will not earn the $100 Health Improvement Reward. You will still earn the $200 Wellness Visit Reward, and may opt to complete the health engagement activities for the $200 Health Engagement Reward. WHAT IF I DIDN T PARTICIPATE IN 2016 AND, THEREFORE, DON T HAVE 2016 RESULTS FOR BRAVO TO COMPARE FOR HEALTH IMPROVEMENT? If you don t have 2016 Bravo results, you may complete a Health University course to earn the same $100 Health Improvement Reward. Contact Bravo at for more information. The Bravo Online University is accessible from computers, tablets, and mobile devices. The AHS challenges will require simple reporting and survey completions to document participation. Each challenge will provide instructions on what to track and how to report it. If you need help, please call Employee Wellness at HOW WILL I KNOW I COMPLETED WHAT IS REQUIRED TO EARN THE $200 HEALTH ENGAGEMENT REWARD?
