3 Introduction Wellness is an important part of the culture at BBV Compass, and we encourage healthy diet, exercise and lifestyle choices for all team members. We are proud to offer programs such as Wellthy for Life and Virgin Pulse that reward you for making your health a priority, and we continue to look for ways to make wellness part of the BBV Compass culture. Participation in health and wellness programs increases when access is easy and affordable, which is why I m excited that we re bringing health and wellness to you. The Houston near-site BBV Compass Employee Health Center is open to all team members and eligible dependents covered under the company s health plan, providing a wide variety of health and wellness services delivered by trained medical staff. These services are offered free or at a minimum cost to you as part of your health benefits from BBV Compass, and we hope that your family s utilization of the Center will help control future healthcare costs. Staffed and operated by Concentra, the BBV Compass Employee Health Center serves eligible spouses and children ages 5 and over. 4 Location and Operating Hours The BBV Compass Employee Health Center is conveniently located about half a mile from BBV Compass Plaza in the Galleria area of Houston Sage Road Suite 260 Houston, Texas Hours of Operation Monday-Friday, 7 a.m. to 4 p.m. Closed from 1 to 2 p.m. every day 4

5 Patient Eligibility The near-site BBV Compass Employee Health Center is open to all employees and their eligible spouses and children ages 5 and over who are covered under the company s health plan. This includes all employees who work in the Houston area, as well as employees who travel there. Please have your Blue Cross Blue Shield ID card and a photo ID with you at the time of your appointment. Spouses and other dependents who are not covered under the company s health plan are not eligible to visit the BBV Compass Employee Health Center. BBV Compass Employee Health Center 5

6 Our Health Professionals The BBV Compass Employee Health Center is staffed by a caring team of bilingual medical professionals including two full-time medical assistants, one full-time physician assistant and one part-time oversight physician. Chris Lu, M.D., Clinical Medical Director Dr. Chris Lu graduated with a B.. in the Plan II Liberal rts Honors Program at the University of Texas at ustin in He earned his M.D. at the University of Texas- Houston Medical School in Dr. Lu completed the Memorial Family Medicine Residency Program in Houston in He worked as a locum tenens physician until he joined Concentra full-time in He provides physician oversight to several Concentra Onsite centers in the Houston area and is also a physician float to several of the Concentra Urgent Care centers. Tarah lvarez, Physician ssistant-certified Tarah graduated from University of Texas Medical Branch in 2009 with a Master of Physician ssistant studies and began working in the urology department at M.D. nderson where she treated cancers of the urinary tract. Prior to attending P school, Tarah completed a B.. in biology with minors in chemistry and Spanish. Tarah has been working in urgent care settings over the last few years and also has previously worked in general surgery where she gained experience in treatment of benign breast disease, breast cancer, colorectal disease, colon cancer, and gallbladder disease. Patient education is what Tarah enjoys most -- empowering patients to make healthy lifestyle decisions. Cynthia Saldana, Certified Medical ssistant Cynthia Saldana chose an education in healthcare at Westwood College in Houston. Once she completed her Medical ssistant Program, she began her student externship at Latin merican Clinic. She has a variety of medical experience with family practice, women s health, family planning and urgent care. Verenice Torres, Certified Medical ssistant t a young age, Verenice Torres desired to find a career in healthcare, and she is passionate about helping people. She is a graduate of Everest Institute in Houston and began her medical career with a student externship with Medical ssociates, also in Houston. 6

7 ppointments The BBV Compass Employee Health Center will treat patients on an urgent care and a walk-in basis. If your situation isn t urgent, you can reduce your wait time by scheduling an appointment online via the appointment scheduler on WellthyForLife.com or by phone at (713) When you schedule an appointment for an adult visit, you will choose one of three appointment types: Primary Care, Urgent Care and Wellness. Primary Care: Preventive services such as annual checkups and managing chronic conditions such as high blood pressure or high cholesterol Urgent Care: For acute, episodic conditions such as flu, cold, sore throat, allergies or headache Wellness: Biometrics, immunizations, blood pressure checks or consultations with a nurse When you schedule an appointment for a child s visit, you will choose Urgent Care. For acute, episodic conditions such as flu, cold, sore throat, headache, ear ache, sprains/strains, sinus infections, skin rashes, stomach ailments and cuts and scratches Prior to scheduling an appointment online, you must create an account with a User Name and a Password. Once you re a registered user, you can log in to schedule an appointment. To Schedule an ppointment 1. Go to and click on the Health Center ppointment Scheduler. 2. Click Log On. 3. Click Request an ppointment. 4. Reason Use the drop-down box to select the reason for your visit: primary care, urgent care or wellness (as defined above). This is a required field. 5. ppt Date/Time Only available times will be visible. Use the drop-down arrow to choose an appointment time. 6. Click Save. The system will send out an with a calendar invitation. 7. Click Return to Home Page. 8. Click Pre-Register for an Upcoming ppointment. (Pre-registration recommended but not required.) 9. 8 Health Services Offered The BBV Compass Employee Health Center is dedicated to providing episodic, preventive health and long-term health care for eligible employees and their eligible spouses and urgent care visits for children ages 5 and over. Complimentary Services Regular office visits, preventive screenings, rapid tests and labs related to chronic-care management are offered at no charge to you:* Office visits Blood glucose test Hemoglobin 1c (blood sugar) Influenza +B test Strep test Low-cost Services dditional labs will be billed through LabCorp and subjected to your normal coinsurance (after your deductible is met), depending on which BBV Compass health plan coverage you selected. Examples include Complete Blood Count (CBC), Comprehensive Metabolic Panel, Cholesterol Panel and Urinalysis. Prescriptions The on-site physician assistant may prescribe medication within the normal scope of patient care. Patients are responsible for obtaining any prescribed medications at their local pharmacy. *s of Jan. 1, 2016, team members participating in the High Deductible Health Plan are subject to a $40 copay when visiting the Employee Health Center, until their plan-year deductible has been met. 8

9 Your Privacy and Security Concentra complies with all HIP (the Health Insurance Portability and ccountability ct of 1996) privacy regulations. Disclosing your medical history is voluntary, but it is common practice at all doctors offices to obtain information of your medical conditions and provide appropriate care. s such, you will be required to sign a Privacy Notice before services are rendered. Information relating to your health care history, diagnosis, condition, treatment or evaluation is considered personal health information and, as such, is protected data and completely confidential. BBV Compass will never see your personal health information in connection with any visits to the BBV Compass Employee Health Center. BBV Compass Employee Health Center 9

10 Frequently sked uestions Why does BBV Compass operate a health center for employes and their eligible family members? Employees health and the health care benefits that BBV Compass is able to offer employees and their families consistently rise to the top of every list. We encourage the good health of every employee, and because we want to make health and wellness accessible to all employees, we re investing in this centrally located employee health and wellness center, allowing you to access quality, confidential health care when you need it. Why is the BBV Compass Employee Health Center only available to employees and their family members who participate in the company health plan? The BBV Compass Employee Health Center is an enhancement to the company health plan. It s funded by the Health Plan Trust, which in turn is funded in part by the premiums that plan participants pay each pay period. Since the services delivered through the center are health plan benefits, we may only allow health plan participants to use the center s services. Open Enrollment serves as an opportunity for employees to enroll themselves and their dependents in the health plan if they would like to use the BBV Compass Employee Health Center s services beginning January 1 of the following plan year. What are the BBV Compass Employee Health Center s operating hours? The BBV Compass Employee Health Center is open Monday through Friday from 7 a.m. to 4 p.m. It is closed daily from 1:30 to 2 p.m. for lunch. Will the BBV Compass Employee Health Center be closed on bank holidays? Yes. The BBV Compass Employee Health Center will adhere to the BBV Compass holiday schedule. Where is the center located? 11 Who is managing the BBV Compass Employee Health Center? Concentra has been selected as the provider of choice for the BBV Compass Employee Health Center. Concentra has provided medical care to more than 6 million patients over its 30-year history, and its primary focus is to improve merica s health, one patient at a time. The BBV Compass Employee Health Center will be staffed with a full-time licensed Nurse Practitioner and two trained Medical ssistants, with oversight provided by a licensed physician. Can family members come to the center for treatment? Yes. BBV Compass employees eligible spouses and children ages 5 and over who participate in the company s health plan may be treated at the BBV Compass Employee Health Center. Spouses and other dependents who are not covered under the company s health plan are not eligible to visit the BBV Compass Employee Health Center. Do I need to make an appointment? The BBV Compass Employee Health Center will treat patients on a walk-in basis, but appointments are recommended to reduce wait times. If I visit the BBV Compass Employee Health Center, will BBV Compass have access to my personal health information? 12 . Is there a limit to the number of times I can visit the BBV Compass Employee Health Center?. No, it operates like your doctor s office, and you may visit it as often as your health needs dictate.. Does the BBV Compass Employee Health Center have access to my medical records?. No. Medical records are created when you visit a provider (doctor or hospital). The BBV Compass Employee Health Center does not have access to your medical records from other doctors. If you would like them to obtain those records, please complete a Release of Information form and send it to your doctor so they may coordinate care.. If I went to the BBV Compass Employee Health Center and was diagnosed with something contagious (like strep throat), would the Center contact my manager to provide documentation directly to him/her or Human Resources?. No. BBV Compass will never be provided your personal health information in connection with any visits to the BBV Compass Employee Health Center. However, upon your request they will provide you with a Return to Work slip that you may give to your manager.. If I need a referral, will the BBV Compass Employee Health Center refer me to Blue Cross Blue Shield In-Network providers?. Yes. Referrals to both specialists and facilities will be made within Blue Cross Blue Shield s network.. Can I get my biometrics done at the BBV Compass Employee Health Center?. Yes. Biometrics can be done at the BBV Compass Employee Health Center, but those appointments will not be available during the time that a Wellthy for Life Health Fair is scheduled in your area. 12 Revised January 2016
