1 A Healthier You Clinical Care Plan Configuration

2 Onboarding Review After entering the A Healthier You portal, you will arrive at the Onboarding feature. Here you will answer questions that will help the portal get to know you and your lifestyle goals. Completing these questions will ensure that you are assigned Care Plans that are tailored specifically to your needs and interests and will provide you with opportunities to earn healthy points for rewards. Just click on the Get Started button to begin the process of answering the questions. The portal will guide you through each section, personalizing the experience to your needs and goals.

3 Personalization Questions As you are guided through the process, you will continue to answer questions about yourself, your routines, and any health conditions you may have. Once complete, you will finish and navigate to the Dashboard, where you will configure Care Plans and earn Healthy Points for chances to win valuable prizes.

4 Dashboard Navigation After completing onboarding, you will be directed to the A Healthier You Dashboard. The My Care Plans section houses all your chronic condition and lifestyle Care Plans. Care Plans are essential to improving your health. Your Care Plans are designed just for you, guided by our clinical team's evidence-based health management plans and provide you with opportunities to earn healthy points by completing certain actions.

5 Understand Your Health Care Plan The General Health Care Plan displays all health actions required to complete what are often referred to as preventive health actions. The Care Plan provides details as to why these health actions are important to you. These actions will vary based on your personal information, and employer specific requirements, such as biometric screenings. Selecting the Understand My Health Care Plan will reveal the actions assigned to you. In some case, such as the HRA, you can take action directly from the Care Plan. Other actions require you to visit your doctor, such as your preventive visit or cancer screenings. To view more details and learn how to earn healthy points, click the more details arrow near the bottom of each health action.

6 Clinical Care Plan Configuration Clinical Care Plans are plans assigned to you based on your individual claim history, or in some cases, self-identified conditions selected during the Onboarding process or in the HRA. These plans are designed to assist you in managing chronic conditions in order for you to lead a healthier life. Examples of clinical plans are: Reduce Metabolic Risks Lower My Blood Sugar Lower My Blood Pressure Prevent Heart Failure Lower My BMI Manage my Coronary Artery Disease (CAD) Lower My Cholesterol Prevent Cardiovascular Disease (CVD) Be Tobacco Free Have a Healthy Pregnancy Manage Asthma Once you click on Configure Now, you begin entering your personal information and preferences. As you complete each step, the bubble on the left will turn green and the menu will drop down to the next section of required information. Configuring a Care Plan should take you less than 5 minutes, and can help you earn valuable points and incentives, and improve your health.

8 Configuring a Clinical Care Plan Continued The first step of configuring a Care Plan is setting a start date for your plan. For Clinical Care Plans, the start date Is automatically selected for you as of the date you configure your plan. The end date is determined by your Wellness Plan Year. The second step is to be aware of the goal for the Care Plan. In this example, the goal is to have a LDL cholesterol level of less than 130 mg/dl. Other Clinical Care Plans may advise you to be below a recommended level, such as blood pressure. Note these levels are simply a recommendation; specific goals should be set by you and your physician.

9 Configuring a Clinical Care Plan Continued The third step of configuring a Care Plan will present you with the clinically recommended actions you should take, such as a doctor s visit and a lipid panel. These actions earn points very quickly, so please contact your physician to schedule an appointment for your clinical condition. The fourth step introduces education to your Care Plan. Education is a very important part of reaching your health goals. You also earn points for completion of items. Examples of education items are: Articles Videos Decision Point Interactive Tool Action Sets

10 Configuring a Clinical Care Plan Continued The fifth step of configuring a Care Plan introduces the concept of Fitness Tracking. Fitness is an important part of improving health. You can select to measure your fitness with a connected device, such as a FitBit or Garmin, or on your own by manually entering information daily. If you set up a device during the Onboarding Process, you will automatically track the data with that device, if the device tracks steps or calories. You can now choose the type of fitness you will track. The options are steps or calories. If you choose steps, you will need to reach the number of steps you select, within the intensity and frequency you select. For calories, you choose to burn a certain amount of calories within a single workout. The intensity choices for steps are 5,000, 7,500, or 10,000 steps. For calories, you can choose 200, 225, or 250. Finally you can choose to meet these goals 3x, 4x, or 5x per week. Healthy points are awarded based on reaching the goal selected.

11 Configuring a Clinical Care Plan Continued Selected Care Plans include nutrition (calorie counting) as a goal as well. In this example, you can choose to measure nutrition with a device or app, such as MyFitnessPal, or on your own by manually entering the data on a daily basis. If you have already connected an eligible device or app for nutrition tracking, the selection will already be made for you. After you have completed configuration of your Care Plan, you will see the green checkbox next to the Create This Care Plan. Choosing to create the plan will navigate you back to the dashboard where you will now begin to interact with your plan on a daily basis in order to improve your health and earn points!
