2 Table of Contents July 2013 Dear Colleague: I invite you to participate in the University s wellness program, Hoo s Well. Welcome to Hoo s Well 5 Hoo s Well Steps Dimensions of Wellness 10 Wellness Programs Physician Screening Form 14 Hoo s Well is a comprehensive program designed to improve your health and wellbeing. There are numerous benefits to participating in the program increased energy and improved sleep habits, for example that can help you be more engaged and more productive at home and at work. I urge you to make physical activity, healthy eating, and work-life balance priorities through this program. 3 Keeping your good health in mind, we encourage you to get a biometric screening and complete the Aetna online health assessment. In 2014, your rewards for participating will be different than in past years, when we gave you a one-time cash payment. This year, if you complete your screening and assessment by Oct. 31, 2013, you will see a monthly health plan savings. UVa remains dedicated to your health. All employees and spouses enrolled in the UVa Health Plan are eligible to participate in Hoo s Well. Thanks Hoo s Well! 4 5 Welcome to Hoo s well The University of Virginia cares about your good health. When you re healthy, you have the energy and focus to do your best work. We want to help you stay healthy. That s why we ve invested in comprehensive wellness programming that helps you know more about your current health status and how to make improvements. Hoo s Well provides strategies to support better health and well-being through every stage of life. Hoo s Eligible? All Academic and Medical Center employees (spouses too!) who are enrolled in the UVa Health Plan can participate. Retirees, Cobra enrollees, and their spouses enrolled in the UVa Health Plan may participate in a number of programs including the biometric screening, Aetna wellness programs, and Quit For Life Tobacco Cessation Program, but are not eligible for rewards. hooswell@virginia.edu for questions about rewards eligibility. Get Rewarded As an employee, you can earn rewards by taking these first two steps to improve your health: Step one Have a biometric screening AND Step two Complete an Aetna online health assessment New this year: If you participate in a biometric screening and an Aetna online health assessment this fall, you will see $40 per month in health plan savings. Look for a mailing with new health plan option descriptions in a few weeks. Important: Complete both items by Oct. 31, The CDC recommends that healthy adults get 150 minutes of moderateintensity aerobic activity every week along with at least two days of muscle strengthening activity. You all are doing a great job with so many things! Several months ago, I d asked for quit smoking information to give an employee who asked about it you replied immediately, I passed along the info and when I saw the employee the other week, he said he had been cigarette free for five weeks. You are making a GREAT difference in our employees quality of life!

4 Hoo s Well Steps Step one Have a biometric screening to know your numbers. I would never have come into the gym without the Hoo s Well program. Screening Options When How OPTION 1 Biometric Screening UVA-WorkMed will conduct screenings at various locations on Grounds. OPTION 2 Physician Screening Form You may elect to use lab results from Jan. 1 to Oct. 31, 2013 instead of attending a biometric screening. OPTION 3 NEW My Chart My Chart is the UVA Health System s medical record system you use to manage information about your health. For access go to virginia.edu/mychart/ or call New this year, you may elect to use lab results in My Chart from this calendar year instead of attending a biometric screening. Attach the results from My Chart to your completed Physician s Screening Form. (See Option 2). No physician s signature needed. Have your screening between Aug. 19 and Oct. 11, Sign & submit the completed Physician Screening Form by Oct. 31, Forms must be received at UVA- WorkMed by Oct. 31, 2013 in order to qualify for monthly health plan savings. I even discovered I like yoga! Sallys Well. It was just the motivation I needed to make exercise a part of my daily routine once again. Thank you Hoo s Well and Intramural- Recreational Sports. Your Health Information is Private All information obtained from or provided by you as a result of your participation in the Hoo s Well program is kept private and secure, and is not shared with the University of Virginia as your employer. Consider placing reminder notes on the refrigerator or images of healthy people around your house or office for inspiration.

5 Important: In order to earn My Rewards, you must complete a biometric screening and the Aetna online health assessment in the fall of Step two Complete the Aetna online health assessment between Aug. 1, 2013 and Oct. 31, (Assessments must be completed annually.) MY Rewards Activity Wahoo Fitness Challenge Visit On the left side of the page, select the Hoo s Well Portal. Login to the Portal and select Wahoo Fitness Challenge. Register and start tracking your activity between Jan. 20 and Mar. 3, The challenge ends on June 30, reward Having the biometric screenings at different times and locations gave me no excuse. My results made me realize just how important it is to remain aware of one s health. Activity Weight Watchers Sign up today. Meet payment and attendance requirements. Call to get your UVa discount. Employer ID Passcode: WW Earn $100 (taxable income). My Rewards for the Wahoo Fitness Challenge will be included in an August 2014 subscriber s paycheck. reward Receive 50% of your monthly membership fees in your paycheck (taxable income). Because of this program, I am more health conscious, have a healthier diet, and a regular exercise regimen. As a former athlete, sometimes you just need a gentle nudge to get back on track. Activity Beginning Right Maternity Program Enroll in the Beginning Right Maternity Program by your 16th week of pregnancy and meet the program s requirements. Call to enroll. reward Earn a $100 reward in your paycheck (taxable income). Your newborn co-pay or deductible may be waived depending on your health plan enrollment. 6 Exhibit OO13.e Take action Hoo s Fit Fridays We are excited to announce Hoo s Fit Fridays! On the last Friday of each month be sure to bring your tennis shoes so you can easily add extra steps to your day. Together, we will make a powerful impact on improving the health of our University community. THE six DIMENSIONS OF WELLNESS Wellness is much more than exercise or nutrition. It is about the health of the whole person. The Hoo s Well model features emotional, social, career, activity, nutrition, and spiritual components. Activity Spiritual Emotional 10 Social Nutrition Career To improve your overall wellness, try working on one dimension at a time. We invite you to pick an icon on the wheel and find a Hoo s Well program to help you on your journey to good health. Choose your adventure! I worked in private industry for 13 years prior to starting my career at UVa and did not experience this level of care for my well-being. 11 Faculty and Employee Assistance Program (FEAP) FEAP provides free and confidential counseling, problem solving, and referral services for employees and their families. Hoo s Well-related programming includes worklife seminars, stress management and consultation on depression, anxiety, family concerns and other issues. Seminars include Wired for Happiness, Intro to Meditation, and Sleep Health. Visit or call for appointments. FEAP is also available for after-hours emergencies at Ask for the EAP counselor on-call. UVa Nutrition Counseling Center UVa Nutrition Counseling offers one-onone nutrition appointments and group classes (including Emotional Brain Training or EBT). Nutrition coaching is available. Specialized nutrition therapy is also available for obesity, polycystic ovary syndrome (PCOS), diabetes, hypertension, and metabolic syndrome. Intramural-Recreational Sports (IM-Rec Sports) IM-Rec Sports is committed to the health of our employees. They work with individuals of varied fitness levels and interests to support a stronger, healthier lifestyle. Hoo s Well programming includes group fitness classes, small group training sessions, one-on-one fitness consults, and more! Visit or call UVA-WorkMed UVA-WorkMed provides biometric screening and health coaching services, as well as a variety of wellness activities on Grounds. Hoo s Well programming includes Steps@ UVA, Hoo s Well Eats Well and tailored Lunch and Learns. Call or visit for current program offerings and information on registration. UHR Employee Development Personal Enrichment courses and programs enhance your work-life balance. Hoo s Well partnered classes include Insufficient Sleep: Causes and Cures, Wellness Navigation Station, and Stress Management. Career Services offers workshops that help develop personal and social networks and teach career development and revitalization strategies. Quit For Life Tobacco Cessation Program Quit for Life features telephonic coaching, nicotine replacement therapy (NRT), a personalized quit plan, tobacco cessation support medication, and more! To enroll, call or visit Adult dependents, retirees, and Cobra enrollees enrolled in the UVa Health Plan are also eligible to enroll in this program. Weight Watchers Did you know that UVa employees receive a discount for Weight Watchers? Choose local meetings, At Work meetings, or an online subscription. Aetna Online Wellness Log in to At the top, choose Health Programs. On the right, select Simple Steps to a Healthier Life. Under online wellness programs click Go to My HealthMedia Programs. Hoo s Losin It is a two-prong approach for individuals who need to make lifestyle changes to correct obesity and blood sugar problems. Programs Include: Balance, weight management, nutrition education Nourish Relax, stress management Breathe, tobacco cessation Overcoming Depression Overcoming Insomnia Aetna Disease Management Program This program provides one-on-one support and advice from trained disease management nurses for over 30 health conditions, including diabetes, asthma, osteoporosis, and heart disease. The Disease Management Program does not compete with your doctor, but helps you better understand and follow your doctor s advice and treatment plan. Garths Well. It was enlightening to learn that proper nutrition is more appropriate than dieting. Working with a peer group facing the same dietary and physical challenges helped make the time commitment easier. We can do this became the chorus and I can t, a retired tune. 12 An Aetna Disease Management Nurse may call to: Discuss taking your medication safely and to see if you qualify for the HealthyRx program (reduced cost sharing based on participation in the Disease Management Program). For personal weight loss tips, schedule an appointment with UVa Nutrition for your complimentary one-onone consultation.

8 Physician Screening Results You may elect to use lab results from your annual physical from the current calendar year instead of attending a biometric screening. Important: You and your physician need to complete this form in full and sign. Fax the completed form to UVA-WorkMed s virtual fax at Dial the full number. This is not a UVa internal fax. Only fully completed forms are accepted. Due by Oct. 31, As the participant, you are responsible for confirming receipt of this form. Labs sent without this form will not be accepted. Save your fax confirmation. If your physician is sending this form, we recommend obtaining a copy of the form and fax receipt for your records. NEW: You may use lab results from My Chart in lieu of a physician s signature. Go to for specific instructions. Employee Release: By submitting this form, I am requesting that my physician report my biometric screening results to UVA-WorkMed so they can be included as part of Hoo s Well, the UVa Health Plan sponsored employee wellness program. I agree that my personally identifiable information and screening results may be shared with health improvement programs offered to UVa Health Plan participants and that I may be contacted by program counselors if my assessment results point to this as an appropriate action. We started Weight Watchers at Work at Fontaine in May 2009 and by March 2010 I reached goal. I feel great and now really enjoy all kinds of activities with my four wonderful grandchildren.
