All of our co-pay plans have a moneysaving discount for a 90-day supply of maintenance drugs purchased through the home delivery pharmacy. You'll pay only 2.5x the 30-day retail copay for a 90-day supply at the home delivery pharmacy, including drugs on tiers 2 and 3. *The home delivery pharmacy is owned and operated by our pharmacy benefits manager, Express Scripts. 10

Preventive care (like checkups and screenings) is extremely important in keeping you healthy. It s more than just your yearly checkup. Preventive care is getting certain exams, tests and screenings throughout your life to make sure your body s working the way it should. This type of care helps your doctor find medical problems early and treat them before they become serious. Your doctor sends in claims for your exam and flu shot. These claims are processed. When both claims have been processed, we ll send you two letters with directions on how to collect your gift cards. 13 Know your Emergency Room (ER) alternatives When it s not an emergency, but you need care now, your Empire plan may cover these options that can save you a lot of time and money. Selecting an in-network facility can save you money. Urgent Care Center: Doctors at these centers treat conditions that should be looked at right away, but aren t as severe as emergencies. Often, they can do X-rays, lab tests and stitches. Retail Health Clinic: This is a clinic staffed by health care experts who give basic health care services to walk-in patients. Most often it is in a major pharmacy or retail store. Walk-in doctor s office: This type of doctor s office doesn t require you to be an existing patient or have an appointment. It handles routine care and common illnesses. LiveHealth Online: Network doctors are available to see you 24/7 through a computer (with a webcam), a smartphone or mobile device.
