BCBSNC regularly analyzes member claims to identify those who might benefit from Case Management for example, if our data shows a member is taking multiple medications and seeing several doctors, or when we identify gaps in care for a high-risk individual, Case Management helps the member take control. Members with high-risk conditions such as heart disease, diabetes, cancer, or stroke can benefit from the support, navigation, and coordination our Case Managers provide.
