1 Job Description Title: Community Specialist Practitioner Band: 7 Reports to: Sector anager (Professionally accountable to the Area Consultant Paramedic) Job Purpose The Community Specialist Practitioner (CSP) will be an experienced clinician, aligned to CCGs where there is an identified need to improve the equity of NWAS service provision and/or improve the management of demand due to poor population health and higher deprivation levels. The CSP will report into their Sector anager, but will have clinical and professional accountability to the Area Consultant Paramedic. Urgent Care guidance will be provided by the Regional Urgent Care Development anager. The CSP will provide clinical leadership within their agreed community, to help deliver the NWAS principle of early clinical intervention. Aligned to primary and community care providers, the CSP will develop a Local Integrated ealth Response Plan, which is designed in partnership with patients, the community and other local organisations including the voluntary sector. The CSP will be responsible for the assessment, care, treatment, or referral of patients, operating across primary, pre-hospital and secondary care. They will provide a critical 999 response for the community approx. 40% of their time, and as part of an integrated service model, promote the delivery of safe care closer to home and support other clinicians to do so. The post holder will also be responsible for leading the development of locally agreed services and care pathways by maximising the use of existing community assets. They will assist in developing community capacity to improve population health by providing clinical learning updates and other educational/awareness sessions to a wide range of stakeholders. Key Duties and Responsibilities Clinical Responsibilities The Community Specialist Practitioner will be responsible for: Providing assessment and treatment of a wide spectrum of patients utilising relevant evidenced based practice, local guidelines and protocols.

Work across an agreed range of care settings to provide an advanced clinical assessment of a wide range of minor illness and minor injuries and critically ill patients and be willing to develop skills in line with service requirements. Provide remote or face to face assistance for decisions relating to End of Life Care. Empower others and demonstrate an ability to make effective clinical decisions based on recent events and evidenced based practice. Know their limits in relation scope of practice and recognise when a requirement exists to refer patient to, or seek advice from, a Consultant Practitioner or any other relevant senior healthcare professional including a medical professional. Lead on the development of practice and become an expert resource in a specialist area of care which will be agreed with the post holder. Participate and where appropriate lead local service improvement teams to ensure clinical quality remains the focus for all staff. Ensure vehicles and equipment meet agreed serviceability ensuring that they are kept in a state of cleanliness and any defects are recorded and reported for action to be taken.

3 In accordance with Trust policies and procedures ensure that own personal practice in infection prevention and control meet the requirements of hygiene code and health and safety at work; this will include cleaning of ambulance vehicles and equipment, maintaining cleanliness of vehicles and equipment, own personal hygiene and promotion of good infection prevention control standards for the protection of self, colleagues and patients from healthcare acquired infections. Within are of responsibility ensure the effective maintenance of infection prevention and control standards of all staff and the decontamination and cleaning of ambulance vehicles and reusable equipment. Undertake audits of vehicles, equipment and premises and ensure that there are associated cleaning schedules that are accessible for audit and public inspection Clinical anagement and Performance Take a holistic approach to managing and developing patient care systems; bringing together their medical, social and psychological needs. In partnership with other providers, support the development and delivery of health promotion/education within the community. In partnership with other providers, Identify and support patients with complex needs, long term conditions, frailty and co-morbidities to self-care and care planning to support their ongoing needs. Work with Care ome staff to reduce emergency demand by education and utilisation of the NRTT. Work with patients and patient groups to develop self-triage and self-care guidance to enable improved access to the right services at the right time. Empowering patients to take responsibility for managing their own care and treatment where it is safe and appropriate to do so. Lead the integration and liaison with complementary NWAS resources such as; Community First Responder, Frequent Caller and Safeguarding Teams to improve the delivery of local services. With the community and other providers, lead on the development and implementation of a Local Integrated ealth Response Plan. Lead on the establishment of a local community forum to enable ongoing reviews of the effectiveness of services, patient experience, outcomes and to support the development of new initiatives and innovations.

4 Working with the community, other provider services and commissioners, develop a local evaluation process to measure and provide ongoing assurance of the effectiveness of any services provided. Develop and implement robust clinical governance systems to underpin all innovative clinical activity, including partnership and handover arrangements with other service providers. Assist with the development of other systems and pathways within NWAS to provide alternatives to 999 (such as direct access to UCD or 111 Clinical Advisors). Contribute to evaluation, audit, and research and be personally responsible for self-directed learning. Provide information to inform the evaluation and efficiency of the role including activity data and audit. Complete the skills and competencies of the Community Practitioner through reciprocal training and development opportunities linked to a pre-determined competency framework and participate in the coaching, training, development, and mentoring sessions in clinical practice situations facilitating own Continual Professional Development. Develop links with local clinical networks to ensure Trust representation when considering changes to care pathways and service re-design and change in practice. Clinical Education, entorship and Support The CSP will: Assist in the production of self-directed learning packs and other learning resources. Support the local mentorship and supervision of trainees and student clinical placements for staff progressing to Senior and Advanced roles. Work at a local level to develop multi-professional education and training opportunities to support local care pathway and practice development. Where necessary, provide education and training to Senior Practitioners, Practitioners, Technicians and any other clinical roles as part of the EI and in-house education CPD process. Also, where appropriate support external stakeholders with education and training, including Care ome staff on the Nursing & Residential ome Triage Tool. To then support and review the training and its impact.

5 Clinical Audit & Research Lead on identifying and undertaking clinical audits in line with the Trusts annual clinical audit programme and lead on the dissemination of results from the audit. Undertake research activities appropriate to supporting development of specialist areas of practice and wider ambulance/urgent care practice. Interpret and evaluate information from diverse sources and make informed judgements about its quality and appropriateness to disseminate to others Critically appraise and synthesise the outcomes of relevant research, evaluations and audits and apply them to improve practices. Undertake any other duties commensurate with the grading of the post and to ensure the necessary further development of the post as required.
