We also run GP hubs and walk in services across London. Founded by six GPs in 2004, our mission is Improving Healthcare, Improving Lives, and our services are characterised by great access, high quality, real innovation and a real understanding of individual patient need. Our innovations have been recognised at National level. The Directors are keen to develop primary care teams. We want to manage medicines more effectively. We need pharmacy technicians. These are new posts and as such the successful candidates will help develop this role. You will be a self-motivated forward thinking. You will be a pharmacy technician with a good knowledge of primary care. You will possess the ability to clearly describe and provide the rationale for changes to prescribing to GP and patients and liaise with colleagues in the Clinical Commissioning Groups (CCG) on the development of area wide prescribing changes whilst protecting the best business interests of A T Medics.

3 Main Responsibilities To be part of the practice prescribing service in managing prescribing repeat, medicines queries, acute medicine requests, letters and discharge reconciliation and other services to AT Medics practice. The post holder will work closely with the practice Doctors, Nurses, other clinicians and the practice team.. It is important that all visitors to the practice, including ill, worried or angry patients are dealt sensitively, both face to the face and over the telephone. You will work to ensure an effective and efficient service, helpful to patients and staff, in line with the practice policies, protocols and procedures. Duties and Responsibilities To process all relevant prescriptions as authorised by GPs in accordance with the principles of good practice To be patient focussed, positive and support in transferring work from GPs Respond to all medication queries including acute medication requests, patient medication queries and any medication related issue triaged by the practice team. Respond to medication re-authorisation requests. To deal with patient queries via telephone or face to face, or . Speaking to patients and members of the practice team including doctors, nurses and receptionists. Liaising with local pharmacies as required. Undertake medicine reconciliation to patients recently discharged from hospital and ensure that their medical record is updated accordingly. To participate in medication audits to improve safety, quality and cost effectiveness To advise on medicine sourcing and safe management. To assist in submitting claims for personally administered drugs as required.

4 To support the practice team in achieving QOF and other contractual targets. To manage prescribing budgets and assist with the appropriate monitoring. Working with the practice team to review and develop systems/sops for repeat prescribing. To help develop, support and plan the introduction of new working processes within the practices to improve the quality of service and cost effective prescribing. To take prompt action in response to any drug alert bulletins that may be received from time to time. To participate in team meeting and regular update/training sessions Strategic Direction and Business Development To review existing prescribing practice and make recommendations to the prescribers To develop new prescribing policies in line with best practice and local care pathways Ensure the practices are compliance with the Medicines Act 1968 and the Misuse of Drugs Act 1971 To provide a link function between the hospital and the local community pharmacy in the use of Medicine Use Reviews and New Medicine Service To make recommendations to the clinical staff regarding new and innovative ways of providing review of medication for patients with Long Term Conditions and Co-Morbidities To support the development of practice services that reduces the workload within the practice Review and make recommendations to GPs of patients medication who have Long term conditions as part of the patients care plan Engage with the Quality, Innovation, Productivity and Prevention (QIPP) agenda and support the Quality Outcomes Framework (QOF)

5 To provide updates to the Board on clinical pathways, new medicines and NICE recommendations and guidelines. Helping practices prepare for CQC visits To develop systems and processes which reduce drug wastage and expenditure Review and report on the usage of Electronic Prescription Service To develop control systems for patients requesting repeat prescriptions by post, at the practice or on line Leadership To undertake risk assessments in line with practice or clinical priorities Review, maintain and update clinical records, as required To be part of medicine safety and ensure compliance with National Patient Safety Agency requirements To action patient safety alerts and report to the GP Directors Agree clinical audit of prescribed drugs with GP Directors Contribute to Significant Event investigations and lead where these are drug related To oversee the management of drugs purchase and storage to ensure there are processes in place to minimise errors or omissions, which could compromise Care Quality Commission Standards. To regularly audit against QOF domains, improve risk stratification of patients and improve service targeting and delivery. Develop cycles for prescribing audits, monitor and ensure change to practice where this is indicated To ensure that the practice has robust governance in respect of prescribing all drugs on prescription

6 Budgetary Control To set up systems in practices that enables reconciliation of stock with FP10 claims and payment/reimbursement received Raise queries with NHS England or the NHS Business services authority (NHSBSA) regarding payment/reimbursement queries To set targets, monitor and report to the Board on systems and processes that reduce prescribing costs and improve patient outcomes Communication Provide medicines information to ensure all practice staff have access to up-to-date medicines information in a format that supports patient engagement and education. To provide regular updating on clinical pathways, new medicines and NICE recommendations and guidelines Internal and External relationships To be part of a small network of practice based pharmacy technicians To be a valuable and integrated member of the practice team Be part of regular practice based pharmacy network meetings with peers with a view to sharing best practice and consolidating processes across AT Medic To liaise directly with local community based pharmacist To work with both the CCG and the Local Authorities and Public Health to develop and support the delivery of services that improves the health of the local community To liaise with Acute and other care providers

7 Personal Specification Primary Care Pharmacist Candidate Profile: You will be an experienced Pharmacist. Your experience will have been gained within primary care or another healthcare setting. You will have experience of developing the need for strategic changes to prescribing practices and have a have a track record of succeeding in implementing these changes engaging with clinicians and patients internally and externally. You will have the can do attitude where anything is possible and know that you can lead this and engage with the key players and team members in order to effect planned changes. Ideally you will have piloted and developed completely new reviews of prescribing services from your conception working across and with various stakeholder groups. Your communication and presenting skills will be exemplary and you will have had direct experience of presenting to external stakeholders where you have been able to enhance organisational reputation and profile. Essential Qualifications Registration as a Pharmacy Technician with the General Pharmaceutical Council Evidence of CPD. Experience Negotiation skills obtained through delivery prescribing change management programmes Understanding of prescribing and Medicines Management issues Ability to work effectively within multidisciplinary groups Skills, knowledge and capabilities Good knowledge of the requirements of primary care prescribing Good broad knowledge of primary care. Ability to prioritise against conflicting timescales and Desirable Any post graduate qualification Qualified pharmacist Post qualification experience within primary or secondary care Previous experience of working with GP practices in primary care Knowledge of SystmOne/Emis clinical system Experienced gain in community pharmacy Interpreting and understanding prescribing budgets
