You will find within the information package: An overview of Murrumbidgee Primary Health Network How to apply Position description About us Murrumbidgee Primary Health Network (MPHN) is one of 31 primary health care organisations that have been established to achieve the following key objectives: Increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and Improving coordination of care to ensure patients receive the right care in the right place at the right time. The Department of Health has set PHNs six (6) key priority areas for targeted work; Mental health, Aboriginal and Torres Strait Islander health, Population health, Health workforce, ehealth and Aged care. The MPHN is informed by our local health professionals and communities through four regionally-based clinical councils, and 33 Local Health Advisory Committees informing a single Community Advisory Council. We meet our objectives by undertaking population health planning and priority setting. Our decision making in the region is based on reliable health data and evidence, and on criteria that is open to public scrutiny. 3 Region Map MPHN covers a land area of 124,412 square kilometres and services a population of 240,755 covered by 28 Local Government areas. The organisation works across fours sectors within the MPHN region, they include the Riverina Sector, Wagga Wagga Sector, Border Sector, and the Western Centre. 4 Role Requirements Objective: General practice and hospitals are the major local sources of health care. Their role in the care of individuals is closely linked health/service problems which demand a coordinated response. This role will be key to improving local service planning, timely and appropriate exchange of health information, and integration of care for consumers, families and communities. The four main themes of collaboration between general practice and hospitals are: Establishing better working relationship to improve problem solving Improving transitions of care for people entering and exiting inpatient facilities Providing care in the most appropriate setting Reducing the episodes of hospitalisation through better prevention and coordination Key Requirements: Consumer Transitions Provide a primary care perspective on consumer care Contribute to hospital policy in regard to consumer care e.g. hospital admission, discharge planning, edischarge summaries, ereferrals and emergency presentations Promote the integration of primary care services with hospital based services at the point of admission, throughout treatment and on discharge Consult with key stakeholders and Clinical Councils to inform priorities Strategic Development and Quality Improvement Contribute to the development of health policy and the planning of health services Participate in Hospital Committees and working groups Identify and develop opportunities for GPs to contribute to the operation of the hospital Provide early and effective consultation with general practitioners regarding hospital based quality improvement initiatives Communication between hospital personnel and general practitioners Foster positive attitudes of hospital staff toward GPs Provide an advocacy role to represent GPs and consumers within the hospital and Local Health District environment Act as conduit for communication between the hospital and the general practice community Advise on communication mechanisms between the hospital and general practitioners e.g. admission and discharge planning Consult with the MPHN on issues relevant to general practice Liaise with local general practitioners Monitoring, evaluation and reporting Assist the hospital s personnel in designing and collecting data to evaluate the integration of the hospital with community care providers including general practitioners Engage in quality improvement processes at the hospital and the MPHN to develop appropriate evaluation tools. Undertake Needs Analysis of local GP s in regard to: Their current level of satisfaction with services offered by the hospital, Reporting mechanisms between the hospital and GPs, GP involvement in the hospital, and Other issues as required
5 Meet with the nominated representative from the MPHN on a monthly basis. Provide a brief written report every six months to the Board of Directors and the nominated representative of the Hospital. Provide an annual report, which will include an evaluation of the position to the Board of Directors of MPHN and the hospital. Meet with MPHN Clinical Councils on a regular basis. General Practice Support Work with the MPHN Provider Support Team to build capacity within general practice and associated primary healthcare providers to provide high quality care to people experiencing problematic substance misuse or addiction Provide clinical support to MPHN AOD commissioned services Provide liaison support and consultation to individual GPs requiring advice to support patients with detoxification, ambulatory withdrawal management, opioid prescribing, relapse prevention and other general practice presentations Contribute to the development of AOD specific health pathway Consult with key stakeholders and Clinical Councils to inform priorities Contribute to the development of health policy and the planning of health services. Participate in Hospital Committees and working groups. Qualifications and Experience 1. Current medical board registration 2. Vocationally Registered (RACGP/ACRRM) medical practitioner with recent or active general practice experience 3. Medical Professional Indemnity 4. Good oral and written communication skills 5. Highly developed organisational skills 6. Knowledge of general practice, hospital and specialist practices, primarily in the geographic area of Murrumbidgee PHN 7. Current Driver s licence and availability of personal private vehicle
