1 Poole Hospital NHS Foundation Trust Individual Placement (Job) Descriptions for Foundation year 2 Placement F2 - Cardiology Cardiology Department in Poole Hospital. This department consists of two cardiology teams. One is comprised of two consultants, a registrar, a ST grade and two F1s. The other consists of two consultants, one registrar, an ST, an FY2 and one FY1. Work involves: Taking referrals on-take and working with Acute Physicians on Medical Admissions Unit when on-call Assessing in-patients and devising a management plan - ensuring one asks for help/advice if needed History taking and examination of both elective and, when on call, acute admissions Interpretation of investigations such as blood tests and ECGs Writing referrals to a wide range of specialties Telephone communication skills Liaising with the multidisciplinary team to enable effective patient care Practical procedures within the FY2s competencies. Learning : A weekly journal club meeting on Monday at 1pm where a topic specific to cardiology is presented and discussed. The ECHO meeting on Wednesday at 1pm provides an ideal opportunity to learn the basics in the interpretation of ECHOs by listening to the discussion of interesting cases and their outcomes. Additionally, on Wednesday afternoons one of the cardiology consultants (Dr Blakemore) carries out teaching for the junior members of the team which is very useful in learning about the fundamentals of cardiology. The radiology meeting on Fridays at 12.00h provides exposure to various imaging modalities used in cardiology and helps to appreciate the application of imaging in cardiology. for the Predominantly on the coronary care unit (accommodates 8 patients) and Cranborne (a 30 bed ward) but also entails working on other wards throughout the hospital depending on where patients are located. Drs Blakemore/Boos/Bruce/Greaves Involvement in the ward rounds of the cardiology inpatients and supervising the FY1 doctor in carrying out the jobs that are generated from this. Activities also include clerking in elective admissions and seeing day-case patients on the medical
2 investigation unit. Working in cardiology also means that the FY2 is placed on the on call rota which involves helping your senior colleagues to clerk in the acute medical admissions. On this ward there are a wide range of disciplines working very closely together, coming together for weekly MDT meetings and neuroradiology meetings. 5 for the in this F2 - Rheumatology & Dermatology Dermatology: comprises 4 consultants, 1 associate specialist, a number of GPSI s, 1 SpR and a multitude of specialist nurses. Mainly outpatient led with services including simple to complex skin surgery in 3 designated theatres, UVB light therapy, photodynamic therapy, leg ulcer team, and other services. Inpatient dermatology normally between 1-5 patients with complex skin management issues, plus referrals and shared care with other teams. Rheumatology: 4 consultants and 1 SpR with RFU nurses and extended scope physiotherapists. Mainly outpatient. Non-cauda-equina back pain on the wards under care of Rheumatology. Otherwise, referrals seen on the ward, with 1-2 inpatients at any time. The main priority of this job is towards your inpatients. When the SpR s are unavailable you are expected to see referrals. Clinic attendance is not mandatory but is expected at some point. You are also encouraged to learn certain minor surgical procedures in dermatology including punch biopsy. 7 F2 - Medical Microbiology & Virology The Microbiology Department at Poole Hospital consists of 2 consultant microbiologists, the F2, laboratory staff and 3 infection control nurses. for the in this Work entails daily ward rounds of inpatients referred to the microbiologists by clinicians as well as regular visits to ITU and the medical assessment unit. There are multiple learning including tutorials by the consultants on a range of microbiological topics. The F2 also attends some clinics (e.g. cystic fibrosis, GUM and tuberculosis outpatient clinics) and spends time in the laboratory learning about the various microbiological techniques. The majority of the attachment is in the Microbiology Department at Poole Hospital although there are 2.5 days spent at Bournemouth Hospital at the GUM clinic. Dr Flanagan (Lead) and Dr Hill The F2 attends the daily ward round(s) with the consultant and is expected to ensure an accurate list of the microbiology results of ITU patients is prepared. He/she takes referrals from clinicians and liaises with the microbiology consultants to provide appropriate advice. The F2 is required to follow up blood culture results and complete audit forms for all C. difficile, E-coli and MSSA patients. In addition the F2 undertakes both an audit and a number of literature reviews with formal presentations during their attachment. 9 for the in this F2 - ENT (Otolaryngology) The ENT department comprises of 5 Consultants. The ENT patients are cared for within the dedicated ENT/OMF 23 bedded ward. 5 of these beds are allocated for short stay surgery which is closed on Friday evening for the weekend. There are 24 day case trolleys and 13 reclining chairs for use by all surgical and some medical specialties in an adjacent Day Case Unit. There are 3 dedicated Day Case Theatres with a recovery area. The Day Case Unit including theatres is the management responsibility of the Surgical Clinical Care Group. There is also a Paediatric Unit with access to ENT beds. Ward during a normal working day. 11 F2 - Acute Paediatrics of Paediatrics comprises 2 inpatient wards and an acute admissions unit. On average we have inpatients at any one time on the inpatient wards with an additional patients (per 24 hours) on the assessment unit. There is a large body of Consultants representing all specialties. operates with a Consultant of the week system who is responsible for the care of all patients seen on the ward for that week. serves Bournemouth, Poole and Swanage. for the F2s are on the SHO rota. They split their time between the inpatient wards and the acute assessment unit. There is lunchtime teaching most days of the week. If working on the assessment unit the F2 carries the on call bleep and takes referrals from A&E/GP/midwives. The F2 then clerks the patients and initiates appropriate management. Occasionally you are called down to A&E resus (with the paeds registrar) to assist with an emergency admission. 13 F2 - Emergency Medicine (A&E) Emergency department in Poole Hospital. On average the department caters for 175 patients a day although this can vary especially on the bank holidays as the area is a popular holiday destination. FY2 doctors see patients in minors, majors and resus. Learning are thus ever present as most cases are discussed with a senior to begin with until experience grows and this is a steep learning curve. These discussions also lend themselves towards eportfolio assessments and Supervised Learning Events. Formal teaching is every Tuesday afternoon from 13:30-16:00 to all FY2s & core trainees. Within the department there is also the opportunity for audit, poster presentation and teaching to the department. for the in this You are placed within the department for the whole four months. 14 for the in this F2 - Obstetrics and Gynaecology The maternity hospital at Poole manages 5,500 deliveries/year. Poole manages normal and high risk pregnancies from the Poole and Bournemouth area, with antenatal clinics, ante-natal day assessment (mid-wife led), ante and post natal wards. Poole has acute and elective gynaecological admissions and takes referrals for Gynae oncology. The FY2 post is 6 months. There are 8 SHOs consisting of GP trainees and ST1/ST2 career grades. 16 for the in this F2 - Old Age Psychiatry comprises of one main (St Brelades) ward for people with dementia with challenging behavior, and a smaller ward (ITU) for the 5 most challenging females with dementia in the area in which Dorset Healthcare University Trust covers. Three consultants lead the care of the patients, one of which leads the care of the ITU patients. is closely linked with the Community Mental Health Teams in the area of Poole, Swanage and Ferndown. The work is entirely ward. As the only doctor on the ward, the majority of tasks are diagnosing and treating minor medical problems, organising investigations, making appropriate referrals to other specialties, prescribing all medications and communicating with patients and their relatives. There is also an element of educating and teaching other members of staff as they are usually only trained in mental health. Since the average length of inpatient stay is longer than that of general hospital there is an opportunity to co-ordinate the care of patients and following them up longer term. There is also the opportunity to learn more about intermediate care and permanent s with complex social needs which is not as fully available in a general hospital. There is also an opportunity to gain experience of legal and ethical dilemmas and gain a better understanding of the Mental Health Act and Mental Capacity Act. 17 F2 - Psychiatry Psychiatry : Acute Assessment Unit (AAU) and CRISIS Team The AAU is the 17 bedded general adult admission ward in St Anns Hospital Poole, with an average length of stay of between 2-4 weeks. The CRISIS Team is a large multi-disciplinary team providing a crisis home treatment service 24 hours a day as an alternative to Inpatient care. This team acts as the gatekeeper for all Inpatient admissions to St Ann s Hospital, as a single point of contact for all queries/ referrals from professional agencies as well as calls from users & carers( via an Out Of Office Hours nurse service) and the general public (via a Helpline). The service covers all adult residents in the East Dorset area (approx 400,000 over 18). The Team is in Central Bournemouth at Hahnemann House, Hahnemann Road. The FY2 Doctor will be primarily on the ward and will have two sessions of working with the CRISIS Team. The post holder will provide medical backup along with the CT1-3 trainees, ST4-6 doctor and the Consultants in the teams. The overall educational objectives are to provide the trainee with the knowledge, skills and attitudes to deal with patients presenting with psychological or behavioral problems associated with severe mental disorders. They would be expected to: Assess new admissions by taking direct/collateral history, carry out mental state examinations and clerk in new admissions, Undertake physical examination of the inpatients and request appropriate investigations (inc. arranging scans, doing ECGs, liaising with GPs, managing straightforward physical conditions, arranging ECT etc.). Prescribe safely, particularly psychotropic drugs Communicate effectively with the patients, relatives and colleagues within the multidisciplinary team and the Community Mental Health Teams. Keep an accurate and relevant medical record electronically Manage time and clinical priorities effectively Use evidence treatment and/or guidelines when treating patients Understand legal and ethical issues in relation to the management of detained patients. 18 The F2 Trainee will provide medical backup to the CRISIS Team assessing & re-assessing the patients under the team s care. There are extensive to manage acutely ill patients, formulate & execute multi-disciplinary care plans. The F2 Doctor, if interested, can perform home visits and carry out assessments in the community with CRISIS Team staff. The Trainee will not be on the out of hour on call rota. The F2 Doctor will be on the daytime Duty Bleep rota providing crosscover for the other wards in St Anns Hospital. The Trainee is expected to attend and contribute to the Education programme held every Tuesday at St Ann s Hospital.
