Wide exposure to general and Geriatric medicine with comprehensive geriatric assessment. Exposure to chronic disease as well as sub-acute and acute illnesses. Multidisciplinary input (OT/PT/SS/ DIETICIAN &SALT). Chance to attend weekly MDT meetings. Exposure to medicines use and excellent learning in that respect. Communication skills with the patients and their relatives as well as the staff. Some exposure to procedures. Will be able to interpret blood results, x-rays and ECGs and be able to act on abnormal results. Understanding what disease is and what is ageing. Ward 14 QEH Medical SHO, Medical SpR and a consultant Day to day care of the patients. Presenting cases to the team/ consultant WR. Carrying out jobs related to disease management. Writing out discharge notification/letter to GPs. Talking to relatives. Interacting with multidisciplinary team members. Chance to do audits, case presentation. Chance to attend teaching at lunchtime and Mandatory FY2 teaching Start at 9 Am. Attending Board meeting with the rest of MD. Then going round patients with SHO / Registrar writing in the clinical notes and twice weekly with the consultant. Chancing up results, interacting with relatives and other members of the team. The Bexley population mix is around 88% white and 12% from other ethnic group. There is an Intake and Liaison team (ILT) providing tele-triage for all new referrals, a rapid assessment new patient clinic, a Short Term team (SIT) working with patients for 6-12 months primarily with non-psychosis and a recovery team working with predominantly patients with psychosis and complex needs for up to 2 years. This is all supported by an Early Intervention (EI) team, Home Treatment/Crisis team and the rehab/assertive outreach teams. The Woodlands Unit at Queen Mary s Hospital, Sidcup provides 24 hour inpatient services for adults and older adults. There are 2 acute adult mental health wards Millbrook and Lesney each with 20 beds; and 2 wards for the elderly mentally ill Leyton and Camden. In addition there is an ECT suite and a day unit which provides occupational and activities for patients on the acute wards. The working age adult wards have dedicated in-patient consultants (a functional model) and the 2 older adult wards have been reconfigured to follow a similar model. Intensive care facilities are available by referral to The Tarn Unit on our Greenwich site. We also have access within the Trust to a challenging behaviour unit, medium security and many residential rehab placements. Bexley s Early Intervention in Psychosis Service is based at the Bexleyheath Centre and provides a borough wide service for young people experiencing a first episode of psychosis.
5 The Bexley Crisis Resolution Home Treatment team is based at Woodlands Unit, Queen Mary s, and provides a borough wide mental health crisis services including a limited out-of-hours service. The team works in close conjunction with the StR in Liaison Psychiatry and the nurse led Psychiatric Emergency Assessment Team at the Woodlands Unit, Queen Mary s Hospital. The Bexley Rehabilitation team is based at the Goldie Leigh site and provides support to people with severe/enduring mental health problems either on the rehabilitation inpatient unit or in their own homes or in the various residential units within the service. The service also includes a Dispersed Intensive Supportive Housing (DISH) scheme. The Bexley Assertive Outreach service is based at the Erith Centre and works with people with complex needs who are difficult to engage in other services. Bexley Older Adult services are based at the Upton Centre which includes a Day Hospital providing therapeutic services. Inpatient services for older adults are based at the Woodlands Unit, Queen Mary s Hospital. Bexley Psychotherapy services (BPS) are based at Bexleyheath Centre and provide group and individual psychotherapy services. The Bexley Learning Disability team, based in Sidcup, provides services to adults with moderate to severe learning disabilities and includes a low secure unit at Goldie Leigh. Type of work to expect and learning opportunities This post may provide the trainee s first introduction to psychiatry training and it offers a wide range of learning opportunities with sound support and supervision arrangements. This post is based across two wards at the Woodlands Unit, Queen Mary s Hospital Sidcup which provide for the assessment and treatment of older people with dementia. The trainee will gain experience in managing people with advanced dementia, their medical conditions and detailed understanding of social networks and care. There is opportunity to observe the complex interplay between physical ill-health and mental illness offering a broader experience of general psychiatry.. At least one session weekly will be spent in the role of Woodlands duty psychiatrist. On the in-patient wards, the trainee will ensure
6 that all new patients admitted under their team s care are fully assessed, including an assessment of their physical state, so that a preliminary formulation of their problems is made, at the latest within two working days of their admission. All in patients should be reassessed as necessary and no less frequently than once per week, so that an update is made before each main ward round. Although this is a largely ward based post there are opportunities for exposure to community psychiatry. The trainee will liaise closely with a community based multidisciplinary team within which there is representation from a wide range of other mental health professionals including nurses, mental health social workers, psychologists and occupational therapists. The F2 will conduct a weekly Thursday am outpatient clinic at the Older Adults CMHT at the Upton Centre under the supervision of Dr Owen Box, Consultant. There is also the opportunity for the trainee to visit the CMHT and see community patients jointly with Community Psychiatry Nurses and other multidisciplinary team members. Trainees gain valuable experience of assessing older adults social care needs and identifying those people who require placements in residential and nursing care. The trainee will join the consultant psychiatrist in reviewing NHS Continuing care patients in Dementia Nursing Homes. The caseload is shared with a core trainee. Senior medical input is provided by the consultants Dr Adrian Treloar (ward) and Dr Owen Box (community) and one Higher Specialty doctor. They will provide clinical supervision to assist the trainee with clinical casework and decision making. Training in communication and interview skills as appropriate to the practice of psychiatry is available and will be tailored to the needs of the post holder. On Monday afternoons, twice a month, the F2 will attend the dedicated Psychiatry teaching for F2s and GPVTS doctors. This teaching covers topics such as the Recovery Approach; Mental Health Act; understanding personality disorders; psychiatric medications and safe prescribing; introduction to child and adolescent psychiatry; leadership development; case presentations;

Shrewsbury Ward, Oxleas House, QEH Dr Ehab Morgan, Consultant Psychiatrist Clerking under supervision of new inpatient admissions and completion of relevant assessment sections on Rio including Risk assessments Liaising with families, GPs and other sources for collateral information Completing physical examination, admission bloods and ECGs and ensuring physical needs to patients are assessed and managed appropriately. The ward does have phlebotomy and ECG technician support. However, F2 would be expected to take bloods and ECGs when situation is urgent or support is not available. F2 would ensure that essential admission investigations including physical observations, Urinary Drug Screen, plasma drug levels, confirmation of allergy status, medication reconciliation and request for GP summary records are completed in a timely manner. The F2 would also make sure that any investigations results are acted upon and fed back to MDT. F2 will be expected to participate in daily MDT handover in morning and would contribute to MDT management planning. F2 will be expected to contribute to MDT discussion in weekly review meetings and keeping accurate records. You will be expected to familiarize yourself with all new admissions and their progress prior to their weekly review. One afternoon a week the F2 will be expected to go to
18 A & E and shadow the Mental Health Liaison team to get A & E experience. When based at the in-patient ward, you will ensure that all new patients admitted under your team s care are fully assessed, including an assessment of their physical health, so that a preliminary formulation of their problems is made, at the latest within two working days of their admission.
