1 Salisbury NHS Foundation Trust Individual Placement (Job) Description Placement The department The type of work to expect and learning opportunities F1 ACUTE MEDICINE There are currently 3 Acute Physicians at Salisbury Hospital who work on the Acute Medical Unit (Whiteparish Ward). Direct admissions from Primary Care are accepted on the Unit as are transfers from the Emergency Department who have been referred for medical admission. The 3 Acute Physicians work from every day and also offer advice via a direct telephone line to local general practitioners. The Acute Medical Unit has a catchment population of over 200,00 and is closely linked to the Emergency Department and all other areas of the hospital. The F1 trainees will gain experience in assessing medical patients with a wide spectrum of acute illnesses. They will learn to carry out the acute assessment with rapid senior review of up to 20 patients admitted between the hours of 0900 and They will also continue the management of patients who are admitted under the Acute Physicians and remain on Whiteparish Ward or are transferred to one of the escalation areas where care continues under the admitting consultant. The overall objectives of the F1 year are to provide the trainees with the knowledge and skills to take history and examine a patient identify and summarise problems prescribe safely document appropriate and accurate medical records manage time and prioritise efficiently communicate effectively with patients, relatives and colleagues use evidence and guidelines to ensure that patients presenting with all forms of medical illness are managed in the most appropriate and effective way understand and recognise the acutely sick patient requiring prioritization above others on the Unit. 2 Main duties of the Typical working pattern in this The F1 trainee is expected to assess new patients admitted to the ward between the hours of under the care of Dr Finneran and Dr Henderson. They are expected to undertake history and examination of the patients being admitted, formulate a diagnosis and initiate management. They will provide a summary of the diagnosis to the Acute Physician and from there management will continue with appropriate investigations being reviewed. They will share the responsibility of arranging investigations and reviewing the results and documenting management plans with other members of the Acute Medical Team. They will also continue the management of those patients remaining on Whiteparish Ward or transferring to an escalation area where they continue under the care of either Dr Henderson or Dr Finneran. In addition they will undertake day time and out of hours work in the Medical Admissions Unit as part of the on-call rota. Outside of the hours of 0900 to 1900 all patients will be reviewed by either an Acute Physician or Physician on-call within 12 hours of being assessed. 3 Placement The department The type of work to expect and learning opportunities Where the is based Clinical Supervisor(s) for the Main duties of the F1 CARDIOLOGY The Department of Cardiology comprises 4 Consultant Cardiologists who do a 1 in 4 on call for Cardiology. There is a 9 bedded CCU adjacent to the Cardiac Catheter Suite and Medical Assessment Unit. The ward can accept non-elective admissions transferred from the Medical Admissions Unit, HDU, ED and the Cardiac Suite with an average turn over of 5 8 admissions every 24 hours. The department serves a catchment population of nearing 200,000 and is closely linked to the tertiary service at Southampton General Hospital. There are also close links with the cardiology service at Bournemouth District Hospital. The F1 trainees will gain experience in the management of patients with all forms of cardiac disease including acute coronary syndrome, dysrhythmias, cardiac failure and the pre assessment and post procedure care of patients undergoing pace maker insertion, coronary angiography, percutaneous coronary intervention and transoesophageal echocardiography. Trainees will develop close links in communication with CCU, Cardiac Rehabilitation Nurse, Heart Failure Specialist Nurse, echocardiography and cardiac suite. Dr Manas Sinha The F1 trainee is expected to review all new patients admitted to the ward in the previous 24 hours or over the weekend and be able to provide a summary of diagnosis and initial management for their senior colleagues. They provide prospective cover for other colleagues who may be absent from the ward. They will share the responsibility for arranging investigations and reviewing the results and documenting management plans with the

4 other members of the ward medical staff. They will also undertake day time and out of hours work on the Medical Admissions Unit, admitting and treating unexpected acute medical emergencies under appropriate supervision. 5 Placement F1 ENDOCRINOLOGY The department The department of Endocrinology comprises 2 Consultants who take part in the Acute General Medical on call rota in addition to looking after general medical and general endocrinological problems. There is also a particular interest in diabetes with specialist foot clinics and microbiological foot ward rounds with input from dedicated diabetic specialty nurses. The department serves a catchment population of nearing 200,000. There is a weekly microbiological foot ward round which the F1 doctors should attend. The F1 doctor will also take part in the general medicine on call working on the Medical Admissions Unit (Whiteparish Ward) clerking patients being admitted and initiating management plans. There are interests in Hepatology, Inflammatory Bowel Disease, Nutrition, gut motility and all forms of gastroenterological malignancy. The Department serves the Salisbury catchment population of nearing 200,000. The Department also has links with tertiary services at Southampton General Hospital and some investigatory service at Bournemouth Hospital. All F1 doctors in Gastroenterology will be based on Redlynch and Pitton Wards and they are expected to deliver the daily medical care of general medical and gastroenterological patients on their ward. There is also involvement with patients requiring nutrition particularly those on the surgical wards following major abdominal surgery. There are interests of Orthogeriatrics, falls, Parkinson s disease, dementia and stroke in the department. The department serves the Salisbury catchment population of nearing 200,000. The type of work to expect and learning opportunities All F1 doctors in Elderly Care will be based on Winterslow Ward covering the day to day medical care of patients on the ward. This is a mixture of complex elderly patients requiring discharge planning, patients requiring rehabilitation and some patients with more acute pathology. They will work with the consultants and seniors to ensure that multiple pathologies are addressed and complex discharge planning is carried out in a multi-disciplinary and effective manner. The doctors in these posts will also look after acute medical outliers on one of the surgical wards. In addition they will be expected to attend the white board round which builds on decisions made during the multi-disciplinary meetings. The F1 doctor will also take part in the general medicine on call working on the Medical Admissions Unit (Whiteparish Ward) clerking patients being admitted and initiating management plans. 11 Placement The department The type of work to expect and learning opportunities F1 PSYCHIATRY = Amblescroft Ward The Foundation Doctor will work on Amblescroft North Ward, a 20 bed unit for functionally ill patients, taking admissions from the whole of Wiltshire and from other areas of AWP Trust when the need arises. The Doctor will also work on Amblescroft South Ward, a 20 bedded organic psychiatry ward with the same catchment area. They will work closely with another trainee who will be either a psychiatry core trainee or a GP trainee, in addition to their Consultant. There is a second Consultant team working across the two wards and the patients are split between the two teams therefore approximately 20 patients each in total. This post is with an over 65 population. The Foundation Doctor will be involved in a variety of tasks on the ward including: clerking in of new admissions to the ward with full psychiatric history and mental state examination, ordering appropriate investigations, formulating differential diagnosis and management plans and also risk assessments. They will assess physical health problems of patients on the ward and will take advice from colleagues at Fountain Way and also where appropriate from medical colleagues at Salisbury District Hospital. They will review the mental states of patients on the ward and ensure that the electronic patient record is appropriately completed and up to date. They will attend ward rounds with the multidisciplinary team where they will present cases of new admissions, update on patient progress and examine patient mental states on the ward round. They will be involved in preparation of discharge summaries. There will be opportunity to gain experience in other departments including Child and Adolescence Mental Health, Forensic Psychiatry, Drug and Alcohol Services, General Adult Psychiatry, Crisis Team and the Primary Care Liaison Service. Foundation Doctors working with this team will gain plenty of experience in a variety of presentations of the mental health issues of old age. In addition to gaining experience of the management of acutely ill psychiatric patients, the post will offer opportunity to gain experience in multi-disciplinary community orientated psychiatry with the community mental health team and the memory service. The post also includes opportunities to gain experience in ECT training and delivery (Green Lane Hospital and Salisbury District Hospital).

In addition the post holder will be expected to attend the weekly educational programme at Fountain Way Hospital consisting of Journal Club Presentation, Case Conferences and Speakers from 12pm-2pm. 15 Placement F1 RESPIRATORY The department The Department of Respiratory Medicine comprises 3 Consultants who take part in the General Medical on call rota on a 1 in 12 basis each. There are interests in all aspects of Respiratory Medicine as well as support from Respiratory Nurse Specialists. The Department serves the Salisbury catchment population of nearing 200,000 and has links with tertiary services at Southampton General Hospital The type of work to expect and learning opportunities All F1 doctors in Respiratory Medicine will generally be ward based during the normal working day and will be expected to deliver the daily medical care of patients on the ward under the care of the Respiratory team. They will join the 3 consultants, 2 STs, 1 Core Medical Trainee and care for respiratory and general medical inpatients. In addition there are weekly Lung Multi-Disciplinary Team Meetings and monthly respiratory educational sessions. 17 Placement The department The type of work to expect and learning opportunities F1 STROKE The Stroke Medicine Team are part of the Medicine for Elderly Care Department and comprises 2 Consultants both who take part in the General Medical rota but also take part in the Local Network Stroke on call rota. The department serves the Salisbury area with a catchment population of around 200,000. The department has links with Avon, Wiltshire and Gloucester Stroke Network and also tertiary services at Southampton General Hospital. The F1 doctors in the Stroke Team will participate in the medical care of patients on the Stroke Unit. The Stroke Unit cares for patients in acute and rehabilitation phases of recovery from stroke together with a variable number of patients with general medical problems. Whilst on the Stroke Team, the educational objectives for the F1 trainee will be to take history and examine a patient identify and summarise problems prescribe safely document appropriate and accurate medical records manage time and prioritise appropriately communicate effectively with patients, relatives and colleagues understand and benefit from involvement in Multi- Disciplinary Team Meetings use evidence, guidelines and local protocols to ensure the best care for patients presenting with a stroke. They are expected to attend the white board meeting every day where discharge of the patients is coordinated in an appropriate manner. They will also be expected to be involved in the Multi-Disciplinary Team Meetings, family meetings and the assessment of direct admissions to Farley Stroke Unit. They will become familiar with NIHSS assessment of patients with stroke and will have daily consultant review of any acute admissions admitted to the Unit. The F1 doctor will also take part in the General Medicine on call working on the Medical Admissions Unit

The consultants take part in an on call for Urology which is separate from the General Surgical on call. The department serves the Salisbury area with a catchment population of around 200,000. There are links with the tertiary services at Southampton General Hospital. The F1 doctor in Urology is primarily responsible for the day to day running of the Urological wards including clerking new and emergency admissions and organising their initial investigations. Direct contact with the on call consultant in encouraged as registrar cover is not always available. The Urology on call is part of the General Surgical / Hospital at Night Team rota. Mr James Brewin Mr Mohammed Elsaghir The F1 trainee is responsible for the day to day running of patients on the Urology ward. They will be supervised by more experienced members of the team. Ward rounds are held daily and there is a separate ward round on a Tuesday afternoon. In general the F1 doctor works closely with the Registrar in resolving problems that arise on the wards and with acute admissions. 23 Placement The Department F1 Post in Old Age Psychiatry South Wiltshire Complex Intervention Team and Memory Service. This post is with an over 65 population. The post holder will work with two Consultants as part of a multi-disciplinary team providing mental health services for people over the age of 65 living in Salisbury and Warminster and the surrounding villages of South Wiltshire, serving a catchment area of thirty six thousand population. The medical staffing team includes a Specialty Doctor, a Psychiatry core trainee and a GP trainee The type of work to expect and learning opportunities Foundation doctors working with this team will gain plenty of experience in a variety of presentations of the mental health issues of old age. In addition to gaining experience of the management of acutely ill psychiatric patients, the post will offer opportunity to participate in multi-disciplinary community orientated psychiatry. The community team consists of a Senior Practitioner, 5 CPNs, 1 Band 4, and 1 Band 3. The team has access to the Therapies Team comprising Psychologist, OT s and Physiotherapist. The post holder will also work within the memory services, and its constituent members; two band 6 nurses, three band 3 therapists, a band 5 therapist and a team manager. Accordingly the post holder will assess and treat patients within the memory service. The post holder will provide medical backup work for the CIT assessing and treating patients in the community, and in the outpatient clinic under consultant supervision. The post holder will attend weekly CIT meetings to discuss complex cases and have the opportunity to work closely within the MDT and liaise and delegate accordingly. The post also includes acute hospital liaison/deliberate selfharm work alongside more senior trainees and the Crisis Team and consists of adults of working age as well as older persons, which is under consultant supervision. The post also includes opportunities to gain experience in ECT training and delivery (Green Lane Hospital and Salisbury District Hospital). Other learning opportunities will be: Taking part in the teaching of medical students with consultant supervision. Weekly supervision with a more senior doctor or senior mental health practitioner. Participate in psychological assessments. Join the case based discussion groups with the other trainee doctors.
