Agency Updates- Erica offered additional information regarding Laura s report (via ) that select FACS supervisors have been invited to develop a coordinated school crisis response team and protocol which may overlap with other programs/departments; may invite other key participants (including School-Based Services director as this develops) Meichle provided update regarding Diocese-facilitated Health & Human Services committee In process of developing 2017 Catholic schools strategic plan which will continue to target securing social workers in every school and strengthening the partnership and enhancing the collaboration with CCB Gateway-Longview and Catholic schools have developed an MOU to support a MH counselor at OLBR for a dedicated to school for specified number of hours; this provides opportunity for CCB social worker at OLBR to promote CCB services/programs with Gateway-Longview MH counselor Meichle shared update s regarding BCSD- Community Schools & Parent Centers (see accompanying handouts) Jackie Richter- has accepted the ISSWP supervisor position, replacing Kathleen Hall & Laura Greene as supervisors of the Catholic schools; Consequently, Kathleen will no longer participate in this committee; Jackie will attend the next CCSSCT meeting Meichle provided brief overview of CCB Encompass Children s Health Homes that are scheduled to begin ; Meichle shared the materials (brochure & referral form) that MCI Rich St. has been sharing with families to help acquaint committee members with the essence of health homes, noting that committee members will soon receive updated forms and guidance directly from their supervisors Meichle Latham confirmed that she has accepted the CCB ECHH- WNY Regional Director; TSFP Supervisor- pending. Meichle noted importance of continued collaboration with committee members as health homes evolve- Meichle plans to attend meeting to share more about health homes Meichle shared that Dennis and Meichle recently met with Erie 1 Boces regarding opportunities to collaborate, including Foster Grandparents program and Cheektowaga SIS; Meichle will connect with program supervisors as collaboration continues 1 P a g e

Funding/Partnership opportunities- opportunities for program development, retention and expansion Committee members reflected on value of sustaining positive and effective relationships internally and externally as experiences foster future funding opportunities TSFP UWBEC/DOE: BPS #93 + partial BPS #206- secure through ECDoMH: BPS #67, BPS #72 & BPS #206- renewed for 2017 BPS: BPS #94- due for renewal Program updates, Achievements & Celebrations A Gathering Place- Heidi- N/A; Excused- no updates provided Catholic Charities Service Corps (CCSC)- Lauren Lauren shared that through partnership with CC USA & AmeriCorps funding, CCB currently provides 6 Service Corps members with placements in various parts of the city, as well as various programs targeting those impacted by poverty Lauren invited committee members and program supervisors to consider hosting a Service Corps member during the upcoming placement year (Sept- Aug) Lauren noted that recruitment applications will open mid-january/february As Lauren receives many requests for student intern placements (particularly those with a school-related interest), she will forward to the committee at ccssct@ccwny.org Communications & PR- Rose (per ) Rose reminded committee that PR/Comm is available and interested in promoting program events, news, etc. Kaitlin Jackson recently joined the PR/Comm Department and in 3 short weeks, has jumped in to covering various CCB-related events, manages the social media forums and has been updating the intranet Comprehensive School Services Program (CSS)- Laura (per ) contract renewed Allison Duffy began in August & is doing well 2 P a g e

3 Employee Assistance Program (EAP) collaborations- Laura (per ) Grand Island School District o Laura noted no updates Sweet Home School District o Laura noted no updates Family Support Center (FSC) Collaborations- Laura (per ) KenTon FSC o A KenTon staff person provides services 1 evening (2 hrs.)/week o Liz Mayer (MST) provides services 1 evening (2 hrs.)/week Sweet Home FSC o Laura noted that the relationship with SHFSC is good o Laura reported that CCB is now providing services at 2 Sweet Home grade schools- both half day/week o CCB counselor is planning a 4 part presentation on divorce/separation for parents at Maplemere; targeted for January-February Foster Grandparent Program (FGP)- Susan Susan shared that the FGP program is always recruiting FGPs and placements at public & parochial schools, including in the 1 st ring suburbs Susan noted that the current target is to secure a FGP at Blessed Trinity, OLBR, and St. John the Baptist (Alden), Susan shared that little is required with FGP placements-fgp s receive training & the assigned site liaison receives training Susan noted that the FGP at BPS #91 will be featured in the 2017 Appeal videoalready under production; Meichle noted this as a great recruiting tool for giving potential placement coordinators a greater sense of the impact of FGPs Susan expressed disappointment that she has not received any interest in a FGP from Catholic school principals Meichle sahredthat principal meeting is scheduled at the Diocese- Meichle will ask Sr. Carol (Superintendent) if she d like Susan to attend to further discuss the FGP to principals with hopes of securing placements (Susan has ISSWP chart to consider prioritized schools) Susan shared that Tish will be hiring as Susan transitions out of the leadership role of the Foster Grandparent Program to become the Coordinator of Preventive Services Program Operations- Congrats, Susan! 3 P a g e

4 In-School Social Work Program (ISSWP)- Kathleen & Laura (per ) Contract now runs July 1 st -June 30 th to align with school year ISSWP team is reviewing deliverables and reevaluating what they re doing compared to what they re reporting All schools going well New staff and new schools on track ISSWP team respond to school-related crises as requested o Erica reported that about 10 people responded from CCB September crisis at St. John the Baptist (Tonawanda), providing support & resources to students, teachers and parents ISSPW school assignments provided (see ISSWP FTE chart)- note Jackie Richter as Supervisor MCI- School-Based Behavioral Health Counseling (SBBHC) - Lori Lori noted that MCI s SBBHC services continue in 4 schoolso BPS#37 o BPS #43 o BPS #93 o Elmwood Village Charter school Lori reported that the principal who d begun Elmwood Village Charter school resigned & accepted a leadership position at South Buffalo Charter School Lori shared that the new principal is acclimating to the school and has begun to make some operational changes Lori continues to work with some students at 20 Rich St. as well MST- Jenelle- N/A; Excused (no updates provided) School Intervention Service- Erica, Laura Laura (Amherst & Kenmore) o City of Tonawanda (KenTon) - no update provided o Amherst (Amherst) - no update provided FAST referrals Sweet Home FSC referrals Erica (Cheektowaga)- o Erica provided a summary of the Cheektowaga SIS program o Erica reported that the program is on track for achieving the targeted output of serving 300 Cheektowaga youth 4 P a g e

CCSSCT committee meeting structure discussion Committee members noted appreciation for quarterly meetings with dedicated time (~30-45 min.) to exchange information/resources, as well as to explore opportunities for enhanced intra-agency collaborations Committee members offered mutual interest in utilizing CCB s intranet as a mechanism for accessing the SBS directory, shared references (e.g. Georgia's Family-Friendly Partnership School Initiative Improvement/Federal-Programs/Partnerships/Pages/Georgia-Family-Friendly.aspx), and resources (e.g. Department of Health & Human Services- Administration for Children and Families (ACF) & Family and Provider/Teacher Relationship Quality Measures: Updated User s Manual 5_508_0.pdf) Committee members expressed trust in fact that through routine discussion, topics identified within 2017 Targeted Focus topics (See VII.) will be achieved VII Targeted Focus of School Services Collaboration Team IMPACT!
