4 Monthly Reminders: OneCity Health Partner Webinar Series Logistics 4 Today is the fifth webinar in our monthly OneCity Health Partner Webinar series (referred to as Participation metric P_3 ) o Held the second Tuesday of every month, beginning at 2 p.m. o To meet the Comprehensive Schedule B performance metric, you need to attend 6 of 8 webinars To ensure you receive credit for participating today, please share your organization by chatting to OneCity Health Questions now Over the next three months we will be using these webinars as a venue for important updates relating to contracting and flow of funds for DSRIP Year 3, which begins April 1, 2017 Participation and engagement during these webinars will be especially important to ensure you are informed and prepared as we move into DSRIP Year 3 To help guide who from your organization should attend, we publish the webinar agenda on our website by the Friday prior to each webinar

8 OneCity Health Partner Reporting Manual 2.1 Now Available 8 On December 1, 2016, we issued Version 2.1 of the OneCity Health Partner Reporting Manual Version 2.1 replaces Version 2.0; we distributed it via and it is available to download on our website The latest manual includes the following updates: o Additional guidance for all Comprehensive Schedule B metrics due December 31, 2016 o New guidelines on two distinct Schedules B, including Health Home At-Risk (HHAR) and Care Transitions Amendment o New general reporting guidance; you can now answer questions directly through the OneCity Health Partner Portal rather than downloading excel templates for some metrics o Updated guidance on three Participation metrics from the Comprehensive Schedule B o Updated information on how to resubmit reports Page 1 of the manual includes a summary of the updates included in Version 2.1, as well as specific page numbers of the updated content

9 Use the New Manual for the Next Round of Metrics 9 Five metrics are due December 31, 2016; Version 2.1 of the OneCity Health Partner Reporting Manual has updated guidance for each metric: o Operations & Outcomes IDS (OO_3): Page 30 in the OneCity Health Partner Reporting Manual o Operations & Outcomes PCBH (OO_10): Page 43 in the OneCity Health Partner Reporting Manual o Operations & Outcomes Palliative Care (OO_18): Page 59 in the OneCity Health Partner Reporting Manual o Data & Tracking IDS (DT_1): Page 69 in the OneCity Health Partner Reporting Manual o Data & Tracking IDS (DT_6): Page 77 in the OneCity Health Partner Reporting Manual As a reminder, you are only responsible for completing the metrics that are included in your Comprehensive Schedule B

10 Reminder: Patient Engagement Metrics Due on January If you have Patient Engagement (PE) metrics in your Comprehensive Schedule B, five are also due to OneCity Health on January 15, 2017 These are the same metrics that were due on October 15 (they are due three times over the duration of the Comprehensive Schedule B) Version 2.1 of the OneCity Health Partner Reporting Manual includes information on how to report Patient Engagement (PE) metrics o PE_1 (ED Care Triage) - Reporting Manual, page 79 o PE_2 (Care Transitions) - Reporting Manual, page 80 o PE_3 (Integration of Primary Care and Behavioral Health) - Reporting Manual, page 81 o PE_4 (Cardiovascular Disease Management) - Reporting Manual, page 83 o PE_5 (Integration of Palliative Care into the PCMH Model) - Reporting Manual, page 84

11 For Some Metrics, You No Longer Need to Download Templates 11 In an effort to make reporting easier for you, for some metrics you will now log into the OneCity Health Partner Portal and enter required information, rather than downloading templates For these metrics, you will select Submit Report to view the page where you will answer questions, enter required information and upload any necessary attachments, such as a workflow to accompany the information you have entered into the Portal Please continue to refer to the specific metric instructions for guidance on how to complete the metrics The OneCity Health Partner Portal is most compatible with Google Chrome as your web browser; the OneCity Health Partner Reporting Manual includes a link to download and install this browser

12 If You Need to Complete Metrics, We Are Here to Answer Questions 12 As you work to complete this next round of metrics, please let us know if you have questions or need additional information well in advance of deadlines by calling our support desk or speaking with your hub executive o If the question pertains to all partners, we will the information Our support desk team will begin to reach out to you to remind you of metrics that are due, and help answer any questions If you hear from our support team that you need to resubmit information, the process for resubmission is identical to that for the original submission o To resubmit a report, log into the Portal, enter the required information and upload any documentation for that specific metric o When you are required to resubmit a report for a performance metric, you must resubmit all attachments you initially submitted, even if changes were required to only one

13 Final Reminders on Upcoming Metrics 13 You can submit metrics in advance of the deadline. Once you submit them, we immediately begin processing them. If they are approved, you can invoice us for payment o If you did not complete past metrics, that does not preclude you from completing this round and future metrics, and earning payment o When you complete reports for each metric, you earn the payment that is listed in your Comprehensive Schedule B. Please don t forget to invoice us to receive payment! As of the beginning of this month, you can log onto the Portal and submit information for all metrics with a December 31, 2016 deadline, as well as patient engagement metrics with a January 15, 2017 deadline

14 Invoicing for GSI Training Attendance 14 As described on previous webinars, there are two Comprehensive Schedule B metrics associated with attending training in GSIHealthCoordinator (GSI), a care management software: Metric ID P_4 P_5 Participant Responsibilities Attend required GSI training for the Care Transitions project that will be offered by OneCity Health Services within the deadline for implementation Attend required GSI training for the Asthma project that will be offered by OneCity Health Services within the deadline for implementation Last month, we kicked off trainings in GSI for partners participating in both the Care Transitions and Asthma clinical projects o So far, a total of approx. 160 people have attended GSI trainings for these two projects If someone from your organization has attended a GSI training, you can now log onto the Portal and report on completion of this metric. To achieve these metrics, at least one person trained in GSI is sufficient to trigger payment We will be collecting reports for this metric on a rolling basis throughout the contracting period

15 We Will Publish Version 3.0 in 2017 We will publish Version 3.0 of the OneCity Health Partner Reporting Manual in early 2017; it will provide you with specific instructions on: o How to report on the remaining Comprehensive Schedule B metrics due in the fourth quarter of DSRIP Year 2 (through March 31, 2016) o Reporting requirements for two additional distinct Schedules B (Asthma Community Health Workers and Cultural Competence and Health Literacy [CCHL] Organizational Site Self-Assessment) o Additional reporting instructions for both the Health Home At-Risk and Care Transitions Amendment Schedules B, which are included in this version of the manual 15

18 What is the DSRIP Midpoint Assessment? 18 On November 29, the New York State Department of Health (NYS DOH) released Midpoint Assessments for each of the 25 Performing Provider Systems (PPSs) in the state These assessments are used by NYS DOH to review compliance with program requirements and determine modifications necessary for future PPS success through the remainder of the DSRIP program The Mid-Point Assessment was conducted by the Independent Assessor (represented by Public Consulting Group) with key findings and recommendations in the following areas: o 360 Partner Survey (August 2016) thank you to those of you who, if asked, provided feedback o State Implementation Plan quarterly reports and narratives (July 2016) containing: Organizational (areas such as governance, IT and financial stability) Recommendations Project Recommendations In the next few slides, we ll discuss some of the results, recommendations and impact for our PPS. We encourage you to read the full report

19 Where Can You See the OneCity Health Report? 19 On November 28, we ed to you a letter from Dr. Christina Jenkins, CEO, OneCity Health, which summarized our report and included our initial feedback We also posted the letter on our website. Included in that post is a link to the NYS DOH website to download the full report and companion materials NYS DOH also provides instructions on how to submit comments on the Midpoint Assessments

20 What Were the Key Findings from the 360 Partner Survey? OneCity Health survey results ranked 7 th highest of the 25 PPSs Per the Independent Assessor, the most relevant data point is that selected partners have a generally positive attitude about the PPS OneCity Health scored higher than the PPS average in overall score and across four organizational domains: 20 o Governance o Performance management o Information systems o Contracting funds/flow The data from the 360 Survey did not itself result in any specific Independent Assessor recommendations, but serves as a key component of the overall assessment

21 One Organizational Recommendation was to Accelerate Funds Flow 21 Our DSRIP Year 1 focus was building infrastructure within our network to ensure that our redesign efforts would be sustainable during DSRIP Years 2 through 5. The NYS DOH encouraged all PPSs to invest appropriately in infrastructure In DSRIP Year 2 (April, 2016 March, 2017), we have allocated $55 million for direct payments to you covering 11 DSRIP projects We have also invested $7 million to date in strengthening our partner network o $1.8M to assist our community primary care providers in achieving PCMH certification o $4.45M to design and implement cultural competence and health literacy programs o Nearly $1M for care management software platform across the network

22 A Second Organizational Recommendation was to Increase Partner Engagement 22 We agree completely that strong engagement with our partner network is essential to health improvement for the patient population We are actively planning to better amplify our engagement efforts across the entire service area To that end, we believe the considerable financial and educational investments we ve made in our partner network ensure you are well positioned to engage and support patients within an integrated delivery system We are excited to continue our work together to achieve lasting transformation for the benefit of New Yorkers who need us

24 Looking to 2017: Where We Are Today with Contracts 24 This is our last webinar of While our contracts run through next March, we want to update you on how the next round of contracts will evolve OneCity Health s current partner contracts include the Comprehensive Schedule B and Distinct Schedules B In total, partners are eligible for $55 million during this contracting period Current Comprehensive and Distinct Schedules B continue through March 31, 2017

25 If Contracts Expire in March, What Will be Next? 25 Our overall contracting objectives remain consistent with the current approach However, for the next phase of contracting we are also focusing on: o Streamlining metrics and reporting requirements to reduce administrative burden o Aligning metrics to focus on outcomes and the processes that will help us collectively achieve them o Supporting high-value programs that serve our members and promote Triple Aim performance improving patient care, improving population health, and reducing health care costs o Promoting referrals and reporting to support the transformation of the PPS network into a patient-centered integrated delivery system

26 Looking Ahead to the Next Phase of Contracting 26 We are currently developing process and outcomes contract metrics We plan to review these proposed metrics in our first two partner webinars in 2017 Our goal is to distribute contracts in March 2017, with a start date of April 1, 2017

28 Expansion of Asthma Home-Based Self-Management Program: PACE Training Update 28 Training description: The Physician Asthma Care Education (PACE) program: o A proven interactive, multi-media educational seminar to improve physician awareness, ability, and use of communication and therapeutic techniques for reducing the health effects of asthma on children and their families o Provides supplemental asthma clinical practice instruction for clinicians and their office staff on how to efficiently and consistently enhance asthma quality improvement measures Target audience: Pediatric clinical providers (physicians, registered nurses, nurse practitioners) participating in the Asthma project Program requirement: All pediatric providers to complete PACE training during Phase 1 and 2 of project implementation Next Steps: o Contact the OneCity Health Support Desk: ochsupportdesk@nychhc.org OR o Please let us know which training option you prefer: 1.
