1 Opening Day for the School Year Tuesday, August 15, 2017 for all schools in Lompoc Unified School District GENERAL REGISTRATION INFORMATION ALL STUDENTS - GRADES K-12 PARENTS OR GUARDIANS MUST ACCOMPANY STUDENTS WHEN REGISTERING IMMUNIZATION INFORMATION California law requires that all school children be immunized against Polio, Diphtheria, Pertussis and Tetanus (DPT), Measles, Mumps and Rubella (MMR), and Varicella (Varicella is required if enrolled for the first time after July 1, 2001). Additionally, Kindergarten students are required to have Hepatitis B immunizations. Varicella is also required for all out of state entrants in grades 1-12 or to show physician documentation of having the disease. All 7 th - 12 th grade students receive a whooping cough booster shot, called Tdap, before entering school. Parents should call their doctor s office to schedule the student=s appointment if we have not yet received required documentation for the student. New Law (SB 277) effective in 2016: Parents or guardians of students in any California school, private or public, will no longer be able to submit a personal belief exemption to currently required vaccines. Only a medical exemption from or by a Doctor will be allowed. Please go to for more information. First grade students must have had health screening within the prior 18 months or less as specified by the State of California, Department of Health Services, on their form entitled Report of Health Examination for School Entry that is available at school sites and in your doctor s office. Students must submit an up-to-date immunization record. New legislation for a Kindergarten Oral Health Check began January 1, This requires that children have a dental check-up by May 31 of their first year in public school, at Transitional Kindergarten or first grade. Dental evaluations that have happened within the 12 months prior to school entry also meet this requirement. Students whose immunization records have not been verified by the school nurse WILL NOT BE ENROLLED OR READMITTED. They will not be allowed to attend school for the school year until their immunization records are in compliance with the law. KINDERGARTEN / TRANSITIONAL KINDERGARTEN (TK) Children entering Kindergarten must be five years of age on or before September 1, The District will offer Transitional Kindergarten at all school sites for children who turn 5 years old between September 2, 2017 and December 2, Children who turn 5 years old between December 3, 2017 and June 30, 2018 may apply for enrollment by submitting a transitional kindergarten application to the District Office. Applications will be placed on a waiting list that will be prioritized by birth date. Page 1 of 6
2 ALL KINDERGARTEN AND TK STUDENTS Kindergarten students who registered in March, April, May or June of 2017 do not need to re-register. Parents registering students for Kindergarten should bring verification of the child s age and immunization records. CHILDREN NEED NOT BE PRESENT DURING KINDERGARTEN REGISTRATION. Vision and hearing screening of all Kindergarten children will be completed during the school year. Please refer to the IMMUNIZATION INFORMATION at the beginning. The first three weeks of school at all elementary schools will have a Kindergarten and Transitional Kindergarten Jump Start. All Kindergarten and Transitional Kindergarten students will be on a minimum day schedule during the three weeks of Jump Start. Beginning September 5, 2017 Kindergarteners and Transitional Kindergarteners will be on a full day schedule. TRANSPORTATION A complete bus schedule will be posted on the Lompoc Unified School District web site and at the schools just prior to the opening of school. Transportation for School of Choice students will NOT be provided by Lompoc Unified School District. Transportation to and from the School of Choice is the sole responsibility of the parent. 3 LOMPOC VALLEY MIDDLE SCHOOL Registration for students NEW to the Lompoc Unified School District will begin on Wednesday, August 2, Please call for an appointment. Parents must accompany new students to registration. Please bring address verification documents (rent receipt, utility bill), immunization records, and last report card (if available). IMPORTANT: PLEASE REFER TO THE IMMUNIZATION AND NEW STUDENT SECTIONS (above). All students attending LVMS for the school year may pick up their packets in the office beginning Tuesday, August 8, 2017 from 9:00 to 11:30 a.m. and 1:00 to 3:30 p.m. The forms enclosed in the packet must be completed before students pick up their schedules. Parents new to LVMS are invited to an information night on Wednesday, August 9, 2017, at 5:30 p.m. in the school gym. New students will return their signed packets to the LVMS gym on Thursday, August 10, 2017 and attend a WEB Welcome Day from 9:30 to 11:30 a.m. (lunch will be provided from 11:30 a.m. - Noon). New students will receive their schedules during WEB Day. Schedule pick up for returning students will be Thursday, August 10, 2017, from 5:00 to 7:00 p.m., Friday, August 11 and Monday, August 14 from 9:00 to 11:30 a.m. and 1:00 to 3:30 p.m. in the LVMS office. The forms enclosed in the packet must be completed before students pick up their schedules. P.E. uniforms will be on sale in the office starting Tuesday, August 8, Uniforms will also be available during the first week of school. The cost is $15 per set (shorts and T-shirt) or $7 for a T-shirt and $8 for shorts. Please make checks payable to Lompoc Valley Middle School. (LVMS cannot accept bills over $20.) The first day of school is Tuesday, August 15, 2017, and will be a regular full school day. Classes begin at 8:20 a.m. and end at 2:45 p.m. Passing bell rings at 8:15 a.m. Back to School Night is Thursday, August 17, 2017 from 6:00 to 7:30 p.m. A PTA meeting will be held from 5:30 to 6:00 p.m. Picture Day will be Tuesday, August 22, VANDENBERG MIDDLE SCHOOL Registration for new students to Vandenberg Middle School will begin on Wednesday, August 2, Please call the counseling office at for an appointment. Parents/Guardians must accompany new students to registration. Please bring birth certificate, current immunization records, address verification documents and last report card (if available). IMPORTANT: Please refer to the IMMUNIZATION and NEW STUDENT SECTIONS above. NO SCHEDULE WILL BE RELEASED WITHOUT A COMPLETED EMERGENCY CARE CARD AND A SIGNED CONSENT FORM and PROOF OF IMMUNIZATIONS IF NOT ON FILE. SCHEDULE PICK UP An Emergency Care Card, completed and signed by parent/guardian, PLUS an acknowledgement and consent form signed by the parent and student, are required for students to pick up their schedules. This includes Jump Start students who will receive their schedules at Jump Start on Thursday, August 10 th. Returning students and new 8 th grade students may pick up class schedules on Page 3 of 6
4 Friday, August 11 th from 11:00 a.m. 4:00 p.m. in the VMS Multi-Purpose Room (MPR). Campus will be open for student tours. If needed, schedules may also be picked up on Monday, August 15 th from 9:00-11:00 a.m. and 1:00 3:00 p.m. in front of the VMS Administration office. Make-up day for schedule pick-up only will be on Monday, August 14 th from 9:00 a.m. to 12:00 p.m. and 1:00 p.m. to 4:00 p.m. in the patio area just outside the Administration Office. The campus will be closed on Monday for student tours. REMINDER Both parents and student should read the handbook BEFORE they sign the acknowledgement and consent forms. ORIENTATION There will be a Jump Start Orientation Night on Wednesday, August 9, 2017 beginning at 6:00 p.m. in the VMS MPR. Jump Start for 7 th grade students and any new 8 th grade students will be held on Thursday, August 10, 2017 from 9:15-11:30 a.m. Students are encouraged to ride the bus to Jump Start. Jumpstart bus schedule and further information will be mailed separately in late July in post card form. P.E. UNIFORMS will be on sale the first week of school in the PE classes. Uniforms are $20 per set. Sweats are optional and cost $15 for sweat pants and $15 for a crew neck sweat shirt. Please make checks payable to VMS. FIRST DAY OF SCHOOL The first day of school is Tuesday, August 15, School will begin at 8:40 a.m. and will dismiss at 3:15 p.m. VANDENBERG SOLAR PROJECT SUNPOWER, a renewable energy company, is building a Solar Energy Farm which will be located in the acreage between Timber Lane and Mountain View Blvd. near Vandenberg Middle School. Construction began in early 2017 and will continue through the sumer months with a targeted completion date of December Drivers should watch for large equipment, trucks and other vehicles working in the area. Please adhere to any posted signage and/or traffic control in place when you enter from Timber Lane onto Mountain View. Your cooperation and patience during this time of construction is appreciated. CABRILLO HIGH SCHOOL New students need to call the counseling office at starting July 26, 2017 to set up a registration appointment. Office hours are 7:30 a.m. -12:00 p.m. and 1:00 p.m. - 3:30 p.m. Registration appointments require both the student and parent/guardian to attend. No class assignments will be made unless the registration packet is complete including up-to-date immunization records, birth certificate and address verification. A welcome meeting, orientation and introduction to concurrent enrollment (CCR) for parents and new students will be held in Cabrillo s Little Theater on Thursday, August 10, 2017, from 9:30 10:30 a.m. New students are invited to attend a campus tour on Monday, August 14, 2017, starting at 2:00 p.m., in front of the cafeteria. There will be a Conq. Camp (Freshmen Orientation) on August 10, 2017, from 9:30 a.m. 1:00 p.m. All pre-registered students (which includes incoming 9 th graders from VMS) may pick up information packets starting on August 4, 2017, from 8:00 a.m. - 3:00 p.m., in the Attendance Office at Cabrillo High School (4350 Constellation Road) or at the LUSD District Office (1301 North A Street). 5 To pick up their schedules, pre-registered students must submit the parent-signed documents (received in the information packets). Students must be present to pick up their schedules. All students will have ID pictures taken before schedules are given. Students who wish to purchase school pictures must bring the picture packet with payment when they pick up their schedules. Students who qualify for District transportation will not need a bus pass until the third week of school. At least one parent or guardian should accompany the student to the appointment and should bring immunization records, birth certificate, address verification and an unofficial transcript from the previous school (if new to the District). If newly enrolling students and their parents fail to attend, they must wait for the next available opening to enroll their student. All students must return their registration paperwork, completed, with parent signatures, at their Orientation/Registration. Maple High School classes begin Tuesday, August 15, 2017 at 8:40 a.m. DR. BOB FORINASH COMMUNITY DAY SCHOOL Dr. Bob Forinash Community Day School is a referral only school. Any new students will be referred by their home school and contacted individually for an orientation.
