Today s organizations are living in a world where constant change is the only constant. New advances in biotechnology, nanotechnology and information technology are bringing forth exciting and unexpected discoveries every day, while the expansive and growing power of the Internet, social networking and the open-source movement are fueling the fi res which threaten to consume much of today s existing business landscape. Jack Uldrich Life-long learning will obviously be more essential than ever in this chaotic and churning environment; but often lost in this new emerging reality of exponential change is the fact that before an organization can seize tomorrow s opportunities it must fi rst unlearn old, obsolete knowledge as well as the old ways of doing business. 3 11:15 am Breakout Sessions 1A 1B 1C 1D Readmissions Track Iowa Health System s Readmissions Strategy Gail Nielsen, Director of Learning and Innovation, Iowa Health System, Des Moines Identifying and preventing readmissions is a prominent national focus today. Nielsen serves on the faculty of the Institute for Healthcare Improvement and in the Performance Improvement Department for Iowa Health System. In her roles, she works with many hospitals across the country to improve care. With a high level perspective, Nielsen will frame the readmissions topic and provide direction for where to start. Identify ways to put a human face on the problem of preventable readmissions. Explore the impact of readmissions and the issues contributing to the problem. Describe innovative national initiatives to reduce readmissions. Leadership Track Applying the Lessons of Unlearning Jack Uldrich, Author and Futurist, The School of Unlearning, Minneapolis, MN Building off of the morning s keynote session on unlearning, Uldrich will walk participants through a series of exercises designed to apply the concepts of unlearning to their own businesses and organizations. Identify a series of areas where unlearning is directly applicable to an organization. Discover how to communicate the need for unlearning to others within an organization. Develop some tangible skills designed to help with organizational unlearning. LEAN Track Sustainability of a Culture of Improvement Dan Varnum, Vice President Performance Excellence and Professional Services, Mercy Medical Center, Des Moines In health care we are constantly faced with change and the need for ongoing improvement. Employing the use of LEAN/Six Sigma tools with discipline can lead to changing everyday behavior from fi refi ghting to being focused and proactive. During this session you will learn about Mercy s LEAN/Six Sigma journey and how this journey has led to a sustainable culture of improvement. Discover how Mercy Medical Center has built an organizational culture of improvement. Recognize how LEAN and Six Sigma tools can work together to reduce waste and variation in health care. Transitions Track Heart Failure Care: Reducing Readmissions by Improving Transitions William Wickemeyer, MD, CEO, Mercy-Iowa Heart Center, Des Moines This session will provide an overview of heart failure and discuss the complexity of the care continuum. Strategies to monitor and reduce hospital readmissions will be reviewed along with discussion of overcoming common barriers. Improving discharge processes and managing transitions of care between acute care and primary care will be highlighted. Discuss evidence-based strategies to reduce heart failure hospital readmissions. Develop a consistent discharge process to facilitate transitions of care. Discuss methods to improve transitions of care between acute care and primary care providers. 12:05 pm Lunch 12:50 pm Breakout Sessions 2A Readmissions Track IHC Statewide Readmission Strategy Tom Evans, MD, President/CEO, Iowa Healthcare Collaborative, Des Moines The national Partnership for Patients campaign creates a new imperative today to reduce preventable readmissions. Iowa was the fi rst state in the country to engage 100 per cent of community hospitals toward this goal. Now that everyone is engaged what next? Dr. Evans will discuss statewide strategies to promote care coordination and prevent inappropriate readmissions. Discuss the forces driving the focus on readmissions in Iowa today. Describe the IHC statewide strategy for reducing preventable readmissions.

4 2B 2C 2D Leadership Track The Iowa Experience: Excellence Every Time Jean Robillard, MD, Vice President for Medical Affairs and Ken Kates, CEO, University of Iowa Hospitals and Clinics, Iowa City UI Health Care is working in partnership with the Disney Institute to develop and implement a comprehensive service excellence initiative. The Iowa Experience is designed to engage people who work, learn or volunteer at UI Health Care in understanding, committing to and practicing service excellence. This session will provide an overview of this approach to fostering a culture of service excellence from establishing a Service Ambassadors program and training thousands of employees to promoting leadership rounding and disseminating tool kits, all toward ensuring an excellent experience for every patient, every time. Review what is meant by service excellence and why it is important to the patient experience. Discuss the resources and commitment needed to foster an organizational culture of service excellence. Define the step-wise approach to rolling out a comprehensive initiative to thousands of employees. LEAN Track Catalyzing Change: Lessons Learned From LAS (panel) Dean Bliss, Senior Lean Coach, Altarum Institute, Cedar Rapids The Lean Application Series (LAS) held in the spring of 2011 brought together health care professionals from across the state to work on today s health care problems using LEAN techniques. Participants focused LEAN work on medication reconciliation, care transitions and patient fl ow. Bliss will lead a panel discussion of LAS participants to share identifi ed best practices and opportunities. Review the purpose of the Lean Application Series. Discuss the issues surrounding the four areas of study. Discover how applying the LEAN concepts and tools can improve processes in the four areas of study. Transitions Track Aligning Public Health and the Medical Community Mariannette Miller-Meeks, MD, Director, Iowa Department of Public Health, Des Moines As a physician, Mariannette Miller-Meeks knows the challenges of aligning public health work with the acute care medical community. Dr. Miller-Meeks will provide an overview of the Department of Public Health, exploring areas of oversight and opportunities for better alignment with providers to improve public health in Iowa. List the major divisions within the Department of Public Health. Identify authority for planning the health benefit exchange directed by the Affordable Care Act. Identify resource areas and individuals within the department that can help facilitate health promotion. 1:40 pm Break 1:50 am Breakout Sessions 3A 3B 3C Readmissions Track Mercy Cedar Rapids Readmissions Strategy Mark Valliere, MD, Senior Vice President, Medical Affairs, Mercy Medical Center, Cedar Rapids The Commonwealth Fund has recognized Mercy Medical Center in Cedar Rapids as a national top performer in preventing readmissions. Mark Valliere, MD, will describe Mercy s strategy to drive down the readmission rate and the future directions of the program. Identify existing programs that have been successful at reducing readmissions. Identify future initiatives and the multidisciplinary cooperation required to enact them. Recognize the importance of readmission management in an era of value-based purchasing. Leadership Track Implementing Across Culture Bill Leaver, President/CEO, Iowa Health System, Des Moines This session will focus on transforming culture across multiple business units and managing change. It will focus on the leadership requirements necessary to implement change in an organization. Define key leadership requirements to manage change. Identify key elements that must be managed to transform culture. Review practical examples of applying these variables. LEAN Track CEO Perspective - How Do You Launch a LEAN Strategy Brian Dieter, President/CEO, Mary Greeley Medical Center, Ames Health care trends, particularly declines in reimbursement and growth in labor costs, cannot sustain current processes. Pressures on margins plague our industry, yet our typical solution is to re-educate, not reinvent. Using the unique perspective of a former CFO who was promoted to follow a much-loved CEO, Dieter will share how his search for an on-ramp to acceleration of quality led to LEAN, including the assessment of other options and tips on how to get started.

5 Compare LEAN as a business improvement model with other choices. Define an incremental approach to LEAN implementation. Recognize how standard work can transform your culture. 3D Transitions Track A Comprehensive Approach to Improving Patient Safety Jim Lehman, MD, Vice President of Quality, Genesis Health System, Davenport This session will overview the Safety Essentials Program implemented in a regional health system with the goal of reducing and eliminating serious safety events. Included in the discussion will be a description of the internal patient safety fellowship developed to create expertise in patient safety tools and techniques across the health system. Review the seven safety essentials and how they were implemented. Describe the components of a successful patient safety fellowship program. Describe a serious safety event and how to approach an event of harm when it occurs. 2:40 pm Break with Vendors 3:00 pm Breakout Sessions 4A 4B 4C 4D Readmissions Track Alegent s Readmissions Story Rick Miller, DO, Senior Vice President and Chief Quality Offi cer, Alegent Health, Omaha, NE This session will describe how Alegent Health structured its care management process across the continuum to improve the continuity of patient care and reduce readmissions. The elements of this process include inpatient care managers, home health care nurses and ambulatory clinic health coaches working together with an information technology enabled patient care summary to coordinate care for patients during care transitions. Identify the elements of care management across the hospital clinic continuum. Discuss how information technology can be used to facilitate coordination of care. Describe specific patient support services to reduce treatment failures during transitions in care. Leadership Track Building a Culture of Change Jim FitzPatrick, President/CEO, Mercy Medical Center-North Iowa, Mason City This session will review one organization s journey with LEAN. The organization s LEAN path, results and lessons learned will be covered. Hint: The Role of the Leader is Key. Cite one example of organizational cadence for introducing LEAN. Discuss and select LEAN tools. Identify the role of the leader in LEAN transformation. LEAN Track University of Iowa Value Award Sabi Singh, Assistant Vice President, University of Iowa Healthcare, Iowa City With a mission entailing both care delivery and education, University of Iowa Hospitals and Clinics (UIHC) is one of the most complex clinical environments in the state. But even with that complexity, UIHC has become a champion of LEAN techniques applied to health care. Sabi Singh will discuss the UIHC approach and processes that were recognized by the Iowa Business Council with its Iowa Effi ciency Award. Identify the successes and challenges of the University of Iowa Hospitals and Clinics LEAN journey. Review the Iowa Efficiency Award. Transitions Track Physician Engagement Mark Purtle, MD, Vice President of Medical Affairs, Iowa Health - Des Moines Physician engagement is critical in the wake of the Affordable Care Act and the ramifi cations to the operation, fi nance, viability of hospitals and physician practice and to respond to the new requirements for reporting and demonstrating value and improved outcomes. Physician engagement comes in various forms; Dr. Purtle will discuss how Iowa Health - Des Moines has approached this issue. Recognize the importance of physician engagement.
