1 JOB DESCRIPTION JOB TITLE: Clinical Pharmacist CLINICAL UNIT: Pharmacy BASE: The Portland Hospital for Women and Children MANAGED BY: Pharmacy Manager ACCOUNTABLE TO: Pharmacy Manager HOSPITAL PROFILE Internationally renowned for clinical excellence, The Portland Hospital is the only independent hospital in the UK entirely dedicated to women and children. Our inpatient and outpatient specialties reflect this, providing clinical expertise in Paediatrics, Obstetrics and Women`s Health. The hospital has been consistently growing over the past few years, almost doubling since Our Paediatric wards hold 56 inpatient beds + 11 day case and are equipped with the latest technology to provide advanced medical and surgical care, including a Paediatric Intensive Care Unit which reflects the acuity and complex nature of the children who are admitted to our hospital. The Portland Hospital has a large number of international Paediatric patients, often funded by relevant embassies. The Portland Hospital is the only fully private maternity hospital in the UK. The hospital provides for both consultant-led and midwifery-led deliveries, having an 18 bedded ward to facilitate this. Alongside this there is a 12 bedded Gynecology and Women s Health in-patient ward. The Portland Hospital also has a large outpatient department for all areas of its clinical expertise, to facilitate continuous care or simpler procedures. To be accountable for the continuous improvement/quality of the pharmaceutical services, safeguarding high standards of care. Page 1 of 6

2 To undertake the professional and legal duties associated with the responsible pharmacist role in compliance with the requirements of the Medicines Act 1968 and the regulations made under the Act, The Medicines (Pharmacies) (Responsible Pharmacist) Regulations MAIN DUTIES AND RESPONSIBILITIES To contribute to HCAI Medicines Management framework ensuring the prescribing, procurement, receipt, stock control, security, supply, use of, safety and disposal of medicines are of the highest standard and are compliant with relevant policies, regulations and legislation. To participate in the dispensing, labelling and supply of medication for all Portland hospital patient types (Obstetrics, Gynaecology and Paediatric patients) for all hospital pharmaceutical requirements including Controlled Drugs in accordance with HCA Medicines Management and controlled drug policies, Medicines Act 1968 and Misuse of Drugs Act 1971, GPhC Code of Ethics and Good Dispensing Practices. To participate in the responsible pharmacist duties/rota, ensuring safe and effective running of the pharmacy in relation to the sale and supply of medicines. To actively participate as a member of the pharmacy team. To ensure that stock levels of medicines, medical gases and intravenous fluids are adequate to cover the hospital requirements and that stock is stored in accordance with the manufacturer s recommendations. To provide clinical pharmacy services to inpatient wards including the paediatric intensive care unit providing relevant skills and knowledge to ensure the provision of an appropriate level of care to the patients which is safe and delivered in a confident and professional manner. To provide patient counselling to all patients or their representatives receiving medication from the pharmacy department in accordance with the GPhC Code of Ethics. To liaise with reconstitution service provider units for TPN and other aseptically prepared pharmacy products. To liaise with doctors, nurses and other staff to ensure a satisfactory level of patient care within the hospital. This may involve participating in various working parties as required. To participate in the provision of an impartial pro-active/reactive medicines information service to the hospital staff, consultants, patients and the general public. To assist the Pharmacy Manager in the care and maintenance of equipment ensuring that a sufficient level and standard is maintained. Also to bring to the attention of the proper authority any defects. To participate in the training of other disciplines/members of staff including nursing staff on pharmacy related subjects. 3 To ensure high level of competency in carrying out the tasks required and self-development professionally and personally through routine Continuing Professional Development activity with facilitation and training. To participate in audit activities, which will improve the quality of the services, provided by the department including medication incident reporting. To plan and prioritise own work schedule to fit in with the departments needs. To assist the manager in maintaining a tidy place of work. To actively participate in team meetings. To actively contribute to the review and update of departmental procedures in line with statutory requirements. To assist in the capture of revenue from all sources available to the department. To maintain all prescription documentation in line with hospital, departmental and statutory requirements. To effectively communicate relevant information to staff using the available resources. To participate in hospital and pharmacy department staff induction. To be familiar and comply with hospital and the pharmacy department s guidelines for Health and Safety conditions at work, as well as the fire precautions and emergency procedures. To participate and complete against agreed deadlines objectives which have been set by the Pharmacy Manager and/or the Lead Clinical Pharmacist. To participate in departmental rotas including rota including late/early shifts, weekends and bank holidays. To undertake other duties as delegated by the Pharmacy Manager commensurate with the grade. Interim appraisal to be completed within first 3-6 months of employment. Full appraisal to be completed biannually thereafter. This job description (including locum posts) does not attempt to detail every duty and is produced as a guideline only. 4 KNOWLEDGE AND UNDERSTANDING M.Pharm or equivalent Must be registered with the General Pharmaceutical Council (GPhC) Additional post graduate clinical qualifications e.g. Postgraduate Diploma in General Pharmacy Practice (desirable) Minimum 1 year post registration hospital experience Completion of 1 year hospital pharmacy rotation. Clinical pharmacy knowledge Knowledge of relevant hospital policies and procedures PROBLEM SOLVING ABILITY Early recognition of problems and to seek advice as appropriate Dealing with enquiries from consultants, staff and patients and refer as necessary Dealing with discrepancies on supply issues Solving clinical pharmacy issues in patient care SPHERE OF INFLUENCE Liaison with consultants, managers, staff and patients To demonstrate care and dedication Influence over consultant s prescribing To be efficient and professional in representing the organisation RISK AND RESPONSIBILITY Maintain a high level of clinical practice in accordance with legal and professional requirements To take responsibility for own practice, in accordance with professional and hospital guidelines/ protocols To prevent risk Supervision of technicians dispensing Maintain confidentiality To maintain equipment To keep up to date with current procedures/knowledge To maintain accurate records Mandatory training SKILLS AND EFFORT Good verbal and non verbal communication skills Arabic language (desirable) Supervisory skills Problem solving skills Accuracy and attention to detail Good team player Ability to work under pressure Evidence of updating clinical skills/knowledge Customer care skills Manual Handling skills PC skills WORKING CONDITIONS Need for protective clothing on occasions Provision of on-call service Page 4 of 6
