Day to day supervision of technical services provided to the hospice by our local pharmacist. 4. Dimensions and limits of Authority This post will hold no budgetary responsibility but will be expected to contribute to the control of cost and expenditure within the scope of the role. 2 5. Qualifications Masters qualification in Pharmacy (four years), or equivalent qualification, plus one year pre-registration training. Member of the General Pharmaceutical Council(GPHC) Postgraduate Diploma in Clinical Pharmacy, or equivalent experience or qualification. Minimum of five years post registration experience as a practising clinical pharmacist, including two years as a specialist pharmacist, preferably in areas relevant to the post. Non-medical prescriber status would be useful 6. Experience and Skills Professional attitude. Committed to CPD. Deal with service users and colleagues with honesty and integrity. Highly motivated to ensure that service is continually assessed to ensure improved patient outcomes / experience. Empathetic and sensitive to customer needs (these include both patients and members of NHS staff). Strict adherence to patient confidentiality. 7. Duties and Key Responsibilities Clinical Pharmacy Role Provide expert opinion and advice, both within and outside the hospice, to ensure the safe and effective delivery of drug therapy to palliative care patients across the Manorlands catchment area. This will include prescription review, advising on dosage and side effects, and on safe administration of medicines. The post holder will also perform the role of a lead clinical pharmacist to the hospice, community and day therapy palliative care teams. In this role, the post holder will ensure medicines are used appropriately, safely and cost effectively, in accordance with hospice policy as appropriate, standard operating procedures and medicines legislation. Be responsible and accountable for ensuring that the legal and statutory requirements in relation to medicines are met, and ensure that the delivery of pharmaceutical care to patients in the defined clinical area meets both national and local standards. Identify and interpret relevant national and regional strategies and guidelines related to palliative care and keep appraised of current pharmaceutical issues so that local policies are developed and implemented in a timely manner. Initiate, develop and maintain robust policies and procedures to effectively manage medicines within the hospice environment. 3 ensure pharmaceutical compliance with regional and national peer reviews. 4 Investigate possible role of the pharmacist in supplementary and independent prescribing. Support the accountable officer in their role. Quality Assurance Role The post holder will lead and develop clinical audit and undertake research in pharmacy related aspects of palliative care. Lead pharmaceutical input into clinical trials, both locally and regionally. To participate in appropriate clinical governance and risk management activity within the hospice, in particular be a member of the Pharmacy Group and Quality Improvement Group. Play a key role in the investigation of medicine related incidents and agreement of action plans. (eg. near miss data and error reports, and to ensure that remedial action is taken to correct areas of weak practice) Identify MHRA and / or government information relevant to the Ensure use of drugs in hospice / medicines risk management. Provide pharmacy advice / input for inspections. Education Role To provide advice, education, induction and training, and continuing professional development to nursing, medical and pharmacy staff. Co-ordinate and develop, with other members of the multidisciplinary team, specialist education and training sessions to meet the needs of specialist and nonspecialist providers of palliative care. Patients and carers will be educated, where appropriate, with regard to concordance and medicine administration, both within the hospice and at discharge. (ensuring patient and carers have full understanding (if possible) of why and how to take their drugs, and monitor for side effects and that they have appropriate written information.)
