2 Job Title: Paediatric Rapid Assessment Staff Nurse Reports to: Location: Key Working Relationships: Lead Nurse (Clinically) Service Manager (operationally) Ealing Hospital Urgent Care Centre, Ealing Hospital Lead Nurse, Service Manager, Lead GP, Clinical Director, Shift lead, nursing and doctor teams Job Summary: The post holder will be responsible for supporting the multi-disciplinary team (MDT) in the delivery of the highest standard of urgent health care to the patients within the Urgent Care Centre. The post holder will have the ability to identify deteriorating patients of all ages from birth upwards. This primarily comprises of providing prompt assessment and on-going monitoring of higher acuity children that present to the UCC. Working in conjunction with the GPs, this consists of recording vital signs and commencing early treatment prescribed by the GPs. During quiet times, the post holder will support the UCC team in treating adult patients, within their scope of competence. Children remain the priority for the Paediatric Rapid Assessment Staff Nurse. The post holder will work as part of the UCC team, reporting to the Shift Lead on a day to day basis, strictly within the professional boundaries defined for the role. The post holder will be expected to be self-motivated and use their initiative when necessary. The post holder will be a key member of the MDT being responsible for the assessment, monitoring and supporting the transfer of patients under their care, working closely with the senior clinical teams, comprising of GP s and Emergency Nurse/Care Practitioners. The post holder will work flexibly, including a shift pattern that covers 7 days of the week, to meet the needs of the service. The post holder will contribute to meeting the identified Key Performance Indicators within the time frames identified. To ensure a high quality patient centred service.

Key Responsibilities: 1. Manage patients appropriately within specific clinical areas of the UCC such as the paediatric monitoring bay and consulting rooms 2. Full triage assessment of all patients identified by the streaming nurse at streaming, of patients placed under their care 3. Maintaining continued monitoring and assessment of patients under their care and escalating immediately any patient that has red flags 4. Delivering and/or administering individualised patient care and medical/ pharmaceutical treatments as instructed by GP s, non-medical prescribers such as analgesia, nebulisers, fluid challenges etc. 5. To have an understanding of the different pharmaceutical requirements of patients presenting with urgent /unexplained/traumatically acquired pain 6. Conveying information to clinicians as required 7. To continuously act as the patients advocate in the interest of their health and medical well-being
4 Clinical responsibilities 1. Supporting patient care and carrying out clinical procedures delegated by Senior Practitioners, within their own competency such as Carrying out basic observations as instructed for initial triage assessment and on-going monitoring Carrying out urinalysis, record results and report any abnormalities Dressings as instructed by Senior clinicians ECG s as requested Applying plaster of paris 2. To assist clinical staff as requested by them in the management of patient care 3. To administer medication under PSDs (Patient Specific Directions) as requested by medical and non-medical prescribers 4. Report any relevant information or changes in the patients condition promptly to the Shift Lead or GP 5. To perform duties in accordance with policies, procedures and standards 6. To liaise with and support all members of the MDT working in the department 7. To ensure clinical areas are clean and tidy at all times and comply with infection control guidelines and signing the cleaning logs as per local policy Service Delivery responsibilities: 1. Work within local and Greenbrook protocols, guidelines and policies 2. To work flexibly, including a shift pattern that covers 24 hours a day, 7 days a week, with the Service Manager and UCC Lead Nurse /GP in delivering and promoting the needs of the service 3. To support the gathering of patient feedback following local systems in place 4. To ensure that Service Manager, Lead Nurse / GP are kept informed on service issues and potential risks 5. To work with the Service Manager and UCC Lead Nurse /GP in delivering and promoting the needs of the service 6. To work with the UCC Lead Nurse /GP and Service Manager to meet the Clinical Governance needs of the UCC; including reporting and assisting with investigation of incidents and complaints where required 7. To provide clinical supervision/mentorship (where appropriately trained) to junior nurses/practitioners 8. To remain updated in the use all equipment used in the UCC and to report any technical problems or issues with that equipment 9. To participate in nurse / team meetings to discuss day to day operational issues 10. To collect data, and provide service information to the Service Manager as required
5 11. To support the Lead Nurse in maintaining the stock control and ordering systems to ensure stock is replenished as requested 12. Check oxygen and suction, ensuring there is a clean and bagged nebuliser available in each cubicle 13. To unpack deliveries of clinical consumables including stock rotation 14. To ensure clinical storeroom is tidy and stock placed in the appropriate areas 15. Chaperone duties in line with Greenbrook policy 16. To ensure drug/cold chain storage, monitoring and recording is carried out in line with Greenbrook/ local policy Personal & professional development 1. To maintain and work within the NMC Code of Professional standards of practice and behaviour for nurses and midwives at all times 2. To recognise and work within own competence 3. To keep abreast of developments in nursing practice and topical issues ensuring that skills are maintained and enhanced through continued professional development 4. Show self-motivation toward expanding practice and developing advanced skills and knowledge as required for the role 5. To be an effective role model and expert resource for nursing staff and other members of the MDT as appropriate 6. To maintain clinical excellence within the department in line with the role 7. To initiate and/ or participate in research if required Patient information and records: 1. To ensure that patient records and documentation are maintained in line with NMC standards, Greenbrook Healthcare policies and use of the electronic patient records system. This includes: Documenting full and accurate records of assessments, investigations, procedures, treatments and aftercare Encouraging colleagues to maintain good and complete records Working closely with the reception team and the administrator, ensuring all records pertinent to patients are scanned and filed as appropriate 2. Contributing to the Read-Coding of patient data Policies & Guidelines 1. Awareness of and compliance with all relevant company policies/guidelines, e.g. medicines management, confidentiality, data protection, health and safety, safeguarding 2. To participate and contribute to operational policy making, attending appropriate meetings and representing the UCC as required. 3. Ensure service development and delivery follows local and national guidelines, for example and where applicable Enhanced Service agreements and contract key performance indicators. 4. Ensure that the UCC complies with NHS / CCG / NHS England contractual obligations in relation to patient care.

Uniform and Dress Code All nursing and Emergency Care Practitioners should adhere to infection control principles and must wear a uniform and identity badge when on duty. Smoking Smoking is not allowed on any part of Greenbrook Healthcare premises. Policies & Procedures The post holder is expected to comply with Greenbrook Healthcare s policies and procedures.
