1 MID CHESHIRE HOSPITALS NHS FOUNDATION TRUST WOMEN S AND CHILDREN S DIVISION JOB DESCRIPTION Post: Responsible to: Accountable to: Base: LAS ST3+ Doctor (Fixed Term) in Obstetrics & Gynaecology (x 2.4 WTE) Ms Lynda Coughlin, Clinical Lead Dr David Butterworth, Divisional Clinical Director Chief Executive Leighton Hospital, Crewe THE POST Applications are invited for an LAS ST3+ Doctor in Obstetrics and Gynaecology There are 2 full-time vacancies (2 WTE), and 1 part-time vacancy (.4 WTE). These are fixed term vacancies until 1 st August These are ST3+ Level positions to support service commitments, with excellent opportunity to develop training. The posts will incorporate both general Obstetrics and Gynaecology commitment with 1 in 8 on call rota. THE APPOINTMENT The successful candidates will be based at Leighton Hospital, Crewe in the Obstetrics and Gynaecology Department, Women and Children s Division. The successful applicants must have full registration with the GMC and continue to hold a Licence to Practise. THE OBSTETRICS & GYNAECOLOGY DEPARTMENT AND WOMEN AND CHILDREN S DIVISION There are 9 Consultants within the Obstetrics and Gynaecology Department. The current consultant body is: Mr M J M Luckas Lead Gynae Oncology, Associate Medical Director Mr J W Meekins Lead Urogynaecology Miss S Pinto Lead Clinician for Fetal Medicine, RCOG Tutor Miss Lynda Coughlin Lead Clinician for Department Miss R Sawyer Lead Colposcopist and CNST Miss K McIntyre Lead Clinician for Labour Ward and Risk Lead 1

2 Mr A Swaminathan Miss R Thompson Miss S Arutchelvam Lead Minimal Access Surgery Consultant Gynaecologist and Obstetrician Locum Consultant Obstetrics and Gynaecology The Department is supported by 6 ST3-7 trainees and 5 advanced Midwifery Practitioners (AMPs). We have one Gynaecology Nurse Practitioner. The AMP s take part in the Junior Doctors (ST1-2) on call rota. FACILITIES Obstetrics: Mid Cheshire Hospital NHS Trust currently undertakes approximately 3000 deliveries per annum. All Consultant led maternity services are on the Leighton Hospital Site. The trust has a Neonatal Intensive Care Unit (NICU), which is a level two unit. The Department regularly receives in-utero transfers from other units unable to provide NICU facilities. The Department is committed to expanding the role of the Midwife and has Advanced Midwifery Practitioners working alongside trainees on the first on call rota. General Obstetrics is currently provided by 8 Consultants. The recently refurbished Labour Ward has dedicated Labour and Delivery rooms, Induction Suite, Triage facilities and two Maternity Theatres on the ward. The brand new Midwifery led unit consists of three rooms with birthing pool. There are regular case review meetings in labour ward. The maternity outpatients houses the Antenatal Clinic, Fetal Medicine Clinic, Day Assessment Unit for high risk pregnancies, pre-operative assessments, Glucose Tolerance Tests (GTT) and External Cephalic Version (ECV). The Maternity Unit has recently been awarded CNST Level 3 and Midwifery Service of the year at the RCM annual awards Fetal Medicine services are currently provided by 3 Consultants with a specialist twin clinic. Regular joint meetings are held with the Neonatologists and close links are maintained with paediatric surgical services and genetics services at Alder Hey Hospital. Maternal medicine is currently provided by a joint Endocrine Clinic held on a weekly basis with a Consultant Obstetrician, Endocrinologist, Specialist Diabetes Nurse and Midwife. Preconception clinic service is provided by the Consultant Obstetrician and Endocrinologist once a month. Gynaecology: General Gynaecology is currently provided by all 9 Consultants. The dedicated Gynaecological Out-patient Department provides one stop menstrual dysfunction clinics with trans-vaginal scanning, out-patient hysteroscopy, colposcopy, fertility clinic and cystometry. The Department has an early pregnancy assessment unit (EPAU), which is coordinated by a specialist nurse and in addition has a specialist nurse for both subfertility and uro-gynaecology. All in-patient work is on the Leighton site in a female surgical ward. The unit is part of the South Manchester Gynaecological Cancer Network. All in-patient work is on the Leighton site in a dedicated women s surgical ward with in patient operating in the main theatre suite and day case lists in the purpose built day case unit (see below). 2

3 CLINICAL DUTIES Gynaecology The successful applicants will carry out daily ward rounds and cover other ward duties such as the review of blood results, VTE risk assessment and completion of electronic discharge. The post holder will see patients in General Gynaecology Outpatient clinics and specialist clinics such as Infertility. The post holder will also have the opportunity to attend Colposcopy, Postmenopausal Bleeding, Out-patient Hysteroscopy and Urodynamics clinics. The post holder will have the opportunity to attend main theatre and day case lists. On call duties include review and assessment of all emergency admissions on the ward and Early pregnancy assessment unit. Obstetrics: The post holders will perform daily antenatal and postnatal ward rounds and review patients admitted under their team. They will be expected to see patients in antenatal clinics, joint multidisciplinary diabetes clinics and the pre-birth assessment unit. They will also have the opportunity to attend fetal medicine clinics. The successful applicants will be covering the labour ward (2 nd on call) and review women attending triage. They will be expected to be able to interpret CTG (Cardio Toco Graphy), perform fetal blood sampling, non-rotational instrumental deliveries and caesarean sections independently. The labour ward has a 60 hour consultant cover which provides extra support and supervision. These posts are a full shift with prospective cover for leave and currently attract remuneration at Band 1A. All doctors within a specialty are expected to cross-cover each other during periods of absence. QUALITY IMPROVEMENT The Trust has a monthly rolling half day for quality improvement. The post holder will be expected to participate in the Departmental Quality Improvement Programme and time and facilities will be made available as required EDUCATION AND TRAINING The post holders will be required to undertake Continued Professional Development (CPD). The Trust supports the requirements for continuing education as laid down by the Royal College, and is committed to providing time and financial support for these activities. In addition the post holders will be required to attend mandatory and essential training in line with the Trust policy. There is also a purpose built and very active Post Graduate Medical Centre at Leighton Hospital. The Centre has a well-equipped lecture theatre with internet facilities and a library. INTEGRATED GOVERNANCE 3

4 The post holders will be expected to: Contribute to the management of clinical risk by reporting clinical incidents and near misses Contribute to achieving CNST standards where appropriate Ensure that record-keeping of medical staff meets both the Trust s and the Division s standards Ensure that the post holder's practice complies with health and safety guidelines and that any deficiency in health and safety systems or documentation are reported to the manager of the area concerned. HEALTH & SAFETY The post holders will be expected to: Attend Occupational Health for health surveillance/vaccination following accidents as laid down by Trust policies. Attend health and safety training as required Contribute to the delivery of the organisation s health and safety responsibilities to staff, patients and others where appropriate. APPRAISAL The Trust has arrangements in place to ensure that all doctors have an annual appraisal with a trained appraiser and are supported through the revalidation process. The post holder is responsible for ensuring that he / she has an annual appraisal in accordance with the Trust Policy. GENERAL MANAGEMENT / ADMINISTRATION The post holders will be expected to: Deliver a team approach to care through the establishment of explicit lines of communication, ensuring that the roles and responsibilities of doctors and nursing staff are clearly defined and recorded to ensure clarity. Participate in multidisciplinary forums to ensure that patients receive care appropriate to their needs. 4

TERMS AND CONDITIONS Basic Salary The salary scale is as per the Junior Doctors Terms & Conditions of Service. The current salary scale is 30,302 rising to 40,090 per annum pro rata. Starting salary will be determined in accordance with the Junior Doctors Terms and Conditions of Service. Where a candidate has service in or outside the NHS which he / she feels should be taken into account in determining the starting salary, this will be considered following the offer of appointment. Registration You have to have continuous full registration with the GMC and to continue to hold a Licence to Practice. Tenure This is a fixed term until 1 st August Annual Leave Your annual leave entitlement is 27 days per annum pro rata. Annual leave will be sought in writing providing at least 6 weeks notice before the first day of proposed annual leave. Residence Removal expenses shall be re-imbursed in accordance with the Trust Policy. Successful candidates are advised not to enter into contractual arrangements for the removal of their home until such time as the formal approval of the Trust is confirmed in writing.
