3 PURPOSE OF THE POSITION This position is responsible for providing clinical pharmacy services to designated areas or teams at Counties Manukau Health District Health Board ( CM Health ) within an interdisciplinary setting. The role will ensure safe, effective and evidence based individualised pharmaceutical intervention is provided to patients. The position will also support other clinicians in providing safe and optimal pharmaceutical care through provision of medicines information and education and where appropriate ensuring care is delivered in accordance with any CM Health policies and procedures. The role will support the provision of the overall pharmacy services by participating in dispensary tasks and quality improvement initiatives PLACE IN THE ORGANISATION Position description: Counties Manukau Health 3

4 NATURE AND SCOPE OF RESPONSIBILITIES Key Accountabilities Standards / Achievements Clinical Team To provide comprehensive clinical pharmacy services to patients at CM Health, including medication history and reconciliation; review and monitoring of drug therapy, drug interactions, and adverse drug events; drug or dosage modification where needed. To educate patients on managing medications or on a specific medication. Provide a medication card when deemed appropriate. To participate in the patient discharge process where appropriate To participate in Medicines Information provision To participate in team and clinical pharmacy meetings. To participate in ward rounds and interdisciplinary meetings where these are relevant To participate in clinical audits and quality use of medicines audits as required. To plan and deliver education to pharmacy staff and to other clinicians. To answer queries from clinicians utilising advanced knowledge and resources To participate in all clinical service rosters and extended services To record intervention and activity data To report against performance measures and KPIs on a monthly basis. To raise any medication errors/incidences through the appropriate departmental processes and/or complete incident reports To contribute to team work by providing support to colleagues within the clinical pharmacy team and to all of pharmacy services To foster and maintain professional relationships with colleagues and other staff at CM Health To assist the Team Leaders and Clinical Position description: Counties Manukau Health 4

7 COMMUNICATION AND INTERPERSONAL SKILLS Will be required to interact on a regular basis with a range of Counties Manukau Health staff members including: Patients and their families Pharmacy staff - Team Leader, Clinical Pharmacy Manager, Pharmacy Services Manager, Dispensary Manager, Pharmacy Systems Manager, Pharmacists and Technicians Other clinical and non clinical staff Externally there will be contact with: Other hospital staff and pharmacy organisations (Pharmacy Council, PSNZ, NZHPA) Community pharmacists and health care professionals Situations may often call for tact, diplomacy and will require information to be handled in a discreet and sensitive manner. In conflict situations will be required to exercise sound judgement, negotiation and persuasiveness skills, toward facilitating a workable outcome. PROBLEM COMPLEXITY The Clinical Pharmacist will be regularly challenged by a wide range of medication, funding and education issues. There will be a requirement to be able to prioritise issues and negotiate time frames, while still providing a quality customer service. The range of problems will be diverse and require solutions customised to meet the circumstances of the client. Therefore opportunities will arise to provide innovative options to the client based on proven outcomes. There will be demands to meet deadlines, maintain accuracy and quality of information. Failure to do so could impact on the organisation in terms of overall quality of patient care. The Clinical Pharmacist will refer to the Clinical Pharmacy Manager and the Pharmacy Services Manager for advice or second opinion for non clinical issues. For clinical issues refer to team leader or senior pharmacists. SCOPE FOR ACTION The Clinical Pharmacist is empowered to make decisions or recommendations relating to safe and effective use of medications for the patient. The Clinical Pharmacist is encouraged to use initiative and problem solving skills to develop innovative approaches to issues. Personal Qualities Ability to contribute positively toward the team Professionalism Motivated and have a can do attitude Postgraduate qualification or be working towards a postgraduate qualification At least 1-2 years experience as hospital pharmacist. 10 APPENDIX 1: Minimum Criteria for Appointment at Level 1 to 4 Clinical Pharmacist, Middlemore Hospital The table below outlines the minimum entry criteria for each level.
