THE POST 1.1 Post Description To be a highly specialised clinical nurse specialist in the field of diabetes. An opportunity has arisen to work within the Diabetes Specialist Team in CHFT. The Diabetes Centre at CHFT deliver specialist diabetes care to people with complex needs. We are at the forefront of advances in diabetes care and provide number of specialist services. We work very closely with all the members of the multidisciplinary team which includes Diabetes consultants, diabetes dietitians and our diabetes foot team. We also provide care and support to our in-patient team and work very closely to implement BPT and CQUIN. We are a well-established DAFNE and Insulin Pump Centre. During this year and the coming years our whole system will be undergoing a transformational change in providing integrated diabetes services. This is an exciting time to work within this team. The diabetes in-patient service continues to expand in response to the increasing number of people with
2 diabetes. Experience with in-patient diabetes care and management is desirable for this post. We are looking for an enthusiastic nurse to join our innovative team. The successful candidate must be passionate about working together with people with diabetes and their families. The successful candidate will have the responsibility of managing a mixed caseload of type 1 and complex type 2 diabetes. You will be delivering all the Level 4 and level 5 Diabetes Services. You will also provide comprehensive, evidence based diabetes care for people with diabetes and support health care professionals both within the community and General practice within the Calderdale site. You will be required to work as a highly skilled practitioner implementing treatment based on expert clinical practice for both people with diabetes and other professionals providing care, in a range of settings. It would be benefit to have an interest in antenatal, adolescents and insulin pump therapy. It is essential to have experience on the initiation of injectable therapies. As a specialist nurse you will have a minimum of 5 years post registration (RN) experience. Proven clinical expertise, clinical leadership and management are essential. You must be able to support the implementation of changes in practice and have the ability to work collaboratively at all levels and across disciplines and agencies. 2. MAIN TASKS REQUIRED OF THE POST 2.1 Main Tasks As a highly specialised nurse working as part of the team to deliver the nursing service for patients with diabetes you will demonstrate highly specialist knowledge and skills in the specialty of diabetes. To hold and manage an active case load and maintain responsibility for a defined patient caseload and to Undertake patient assessment, develop, implement and evaluate plans of care for patients with diabetes Provide telephone support, assessment and advice to people with diabetes on their management in accordance with national and local guidelines and practice. To participate in nurse led clinics, outpatients and to do monthly joint community clinics with P/N or GP s and to undertake home visits as required Undertake clinical procedures and interventions commensurate with role including adjustment of medication dosage as indicated and in accordance with national and local guidelines/practice To act as a resource for people with diabetes and their families and ensuring your own actions are consistent with clinical governance. To access clinical supervision individually and as part of the Team. Provide the nursing lead within the context of the multidisciplinary team throughout the patient journey. To be the primary link in the coordinating of patients with diabetes between hospital services including surgery, medicine, paediatrics and community services. To be the point of direct contact providing assessment, advice and clinical judgment on future patient management. Participate in development within the service and contribute to achieving performance targets, meeting capacity and demand required.
3 Play an active role in research and evaluation of the diabetes service and ensure best use of evidence based health care. Contribute to a wider national agenda within the field of Diabetes. 3. DUTIES AND RESPONSIBILITIES OF THE POST 3.1 Clinical Duties Deliver direct expert nursing care in diabetes for both outpatients and inpatients across both acute sites. Demonstrate highly specialist knowledge and skills through nursing practice. Maintain own expert levels of clinical competence. Promote and facilitate research and evidence based practice in Diabetes Nursing. To lead in establishing, implementation and monitoring of nursing practice guidelines, standards, policies and protocols/care pathways. Maintain a high profile in the clinical area, acting as a role model for other health care professionals. Be aware of ethical and legal issues. To use advanced clinical skill and knowledge, including nurse prescribing, to meet the complex needs for patients requiring level 5 care. To act as an autonomous, registered practitioner who is legally and professionally accountable for own unsupervised actions guided by the professional code of conduct and Trust guidelines and protocols. To ensure practice is supported by research, evidence based practice, literature and peer review. 3.2 Professional Duties Act as a role model for quality nursing care at all times, promoting the need for effective evidence based interventions. Act as patient advocate at all points of the care pathway. Work within the NMC Code of Conduct at all times. Adhere to national, local and professional standards, guidance and directives relevant to their sphere of practice. 3.3 Clinical Governance/ Quality Ensure compliance with policies, procedures and clinical guidelines for self and others and participate in the development of the same. Continually monitor standards of care and contribute in the improvement of care through benchmarking, Clinical Audit and Research. Support and facilitate research evidence- based practice in the specialist area. Disseminate good practice by pursuing opportunities to present work at national conferences, MDT meetings and journal publications. Promote patient and public involvement activities in the specialist area leading to service improvement. Promote people s equality, diversity and rights. Identify clinical issues and incidents within the Trust that reduce the quality of care within the specialist services. Take part in activities to counter these to ensure the delivery of safe and effective care.
4 3.4 Education/Training Takes responsibility for own continuing professional development and performance, including identifying own development needs and maintain own portfolio. Participates in supervision and the teaching of pre-post registration students, new staff members and junior medical staff. Provides education and training related to the speciality Supports the ward/development sister in ensuring the clinical environment is conducive to effective learning. Complete all the mandatory training as per the trust requirements. 3.3 Managerial Duties To deputise when required in the team managers absence and delegate appropriately Ensure adherence to all Trust-wide protocols, policies and directive within service delivery. Participate in service review and development alongside diabetes service leads and management team. To work closely with members of the Trust s management team and other appropriate health and social professionals to co-ordinate, influence and develop diabetes services within and across the primary, secondary care settings, in line with local, network and national guidance and standards. Work with other appropriate professionals to ensure holistic, comprehensive nursing assessment, care planning, care delivery and referral mechanisms are in place to support patients at appropriate points of their care pathway, ensuring advice and information is available to both patients and carers. Working closely with all diabetes service multi-disciplinary team members (and wider team members), to ensure effective communication with other appropriate services, patients and carers takes place in a timely and effective way. To act as a role model for other nurses caring for patients with diabetes, establishing positive and credible professional leadership and be involved in mentoring and education of staff. Support the Trust s aims and objectives around service delivery and achieving targets. Work closely with the appropriate ward sisters/charge nurses and matron to influence and promote the nursing service and care provision for patients accessing diabetes services. Attend regular relevant meetings (internal and external) and provide effective channels of communication within the team. Maintain a flexible approach and develop the role according to needs. Participate in and promote patient and public involvement in diabetes services. Ensure all documentation is produced and completed within Trust-wide policy/guidelines. 3.4 Financial Duties None 3.5 Managing Self Participate in regular supervision. Attend all mandatory training. Participate annually identifying, developing and agreeing your own development plan with your Line Manager using the Trust Appraisal. Comply with all Trust policies, procedures and protocols.

HEALTH AND SAFETY 4.1 Healthcare Associated Infection Healthcare workers have an overriding duty of care to patients and are expected to comply fully with best practice standards. You have a responsibility to comply with Trust policies for personal and patient safety and for prevention of healthcareassociated infection (HCAI); this includes a requirement for rigorous and consistent compliance with Trust policies for hand hygiene including the naked below the elbow approach, use of personal protective equipment and safe disposal of sharps. Knowledge, skills and behaviour in the workplace should reflect this; at annual appraisal you will be asked about the application of practical measures known to be effective in reducing HCAI. The Trust has the responsibility of ensuring that adequate resources are available for you to discharge your responsibilities. 4.2 Safeguarding The Trust has in place both a Safeguarding Children Policy and a Safeguarding Adults Policy in line with national legislation. The Safeguarding Policies place a duty upon every employee who has contact with children, families and adults in their everyday work to safeguard and promote their welfare. In the event that you have concerns about possible harm to any child or adult you should contact your line manager immediately or in their absence your Assistant Divisional Director. Out of hours contact should be made with the on-call manager through switchboard. The Trust has nominated Safeguarding Leads who act as contact points for support and advice if concerns are raised about a child or adults welfare. These individuals can be reached through switchboard during office hours by asking for the Named Professionals for Safeguarding Children or Adults respectively. The policies and procedures described below are located on the intranet and internet site and you should ensure you are aware of, understand and comply with these. In addition the Trust will publicise and raise awareness of its arrangements and provide appropriate resources and training. PROBATIONARY PERIODS All staff new to the Trust are required to undergo a 6 month probationary period (except medical staff and staff on temporary or fixed term contracts for 6 months or less who are subject to separate arrangements). All existing Trust staff transferring internally to a new post within the Trust are required to undergo a modified probationary period of 3 months. 6. Evidence of maintaining high level skills in assessment and management of patients with diabetes.
